    Against the temporalities and spatialities of progress and modernization, Sérgio Ferro and the “Arquitetura Nova” group carried out a critique of design, technique, and the construction site, proposing a space-time rupture through a return – miserabilism, the economy, manufacturing, the memory of the gesture – in a way that reverberates Benjamin's sense of ruin. Despite its undeniable historical importance, the economic boundaries of his discourse curb the potential for deviating from the prevailing teleological rationality.Contra as temporalidades e espacialidades do progresso e da modernização, Sérgio Ferro e o grupo Arquitetura Nova realizaram uma crítica ao desenho, à técnica e ao canteiro de obras, propondo uma ruptura espaço-temporal por meio de um retorno – o miserabilismo, a economia, a manufatura, a memória do gesto – de modo a reverberar os sentidos benjaminianos de ruína.     La noción de mito, parte del programa ambiental del artista Hélio Oiticica, llamó la atención por romper tanto con las representaciones lineales y progresivas del tiempo como con cierta nostalgia reaccionaria propias de la década de 1960, tomando a contrapelo la utopía del futuro, rescatando el sentido dialéctico y poético de obra. Este artículo pretende retomar y, así, desatar nuevos sentidos de esa potencia creadora hacia lo posible.The notion of myth, part of the artist Hélio Oiticica's environmental program, drew attention for breaking both with linear and progressive representations of time and with a certain reactionary nostalgia typical of the 1960s, taking the utopia of the future against the grain, by rescuing the dialectical and poetic sense of work.     What Behaviors Are Important for Successful Weight Maintenance?

    Purpose. To examine behavioral factors related to successful weight maintenance. Methods. Subjects were 90 middle-aged participants who attended a weight loss program and were followed for one year. The subjects were classified into either successful weight maintainers (maintained a weight loss of 5% or more from their initial weight for one year) (SWM) or unsuccessful weight maintainers (USWM), and weight control practice, stress, obstacles, support, and self-efficacy during the program and follow-up period were compared. Results. SWM had mean loss of 12% from their initial weight during the program. They showed a greater improvement in their regularity of eating, walked more, and felt less stress regarding their increased physical activity than the USWM. During the follow-up period, significantly more SWM participants had self-efficacy (for measuring weight, practicing dietary objective, and assessing the practice and keeping records), actually kept records and measured weight more than the USWM participants. In contrast, more USWM participants felt stress about measuring weight. Conclusion. In addition to a substantial initial weight loss due to an increased amount of physical activity, having a higher self-efficacy and consistently keeping records of one's activities, as well as regularly weighing themselves, may be important for successful weight maintenance

    Management of Lacerated and Swollen Tongue after Convulsive Seizure with a Mouth Protector: Interprofessional Collaboration Including Dentists in Intensive Care

    We encountered a 74-year-old male patient with tongue laceration after convulsive seizures under intensive care. The tongue showed severe swelling, and the right ventral surface had been lacerated by his isolated and pointed right lower canine. Our university hospital has established a perioperative management center, and is promoting interprofessional collaboration, including dentists, in perioperative management. Dentists collaborating in the perioperative management center took dental impressions, with the support of anesthesiologists who opened the patientʼs jaw under propofol sedation, to produce a mouth protector. By raising the patientʼs bite, the completed mouth protector prevented the isolated tooth from contacting the tongue and protected the lacerated wound. Use of the mouth protector prevented the lacerated tongue from coming into contact with the pointed tooth, and the tongue healed gradually. These findings underscore that interprofessional collaboration including dentists can improve the quality of medical care

    Recognition of the "team medical-care" in medical field.

    チーム医療に対して高い関心が寄せられていることから,チーム医療として認識される具体的な事柄や各職種に対する認識,その利点や職種間の認識の違いなどを明らかにする目的で,岡山県内の病院の看護師,診療放射線技師,臨床検査技師,計652名に対してアンケート調査を行った。有効回答者数470名で,その内訳は看護師207名,診療放射線技師91名,臨床検査技師172名であった。結果はチーム医療と認識される具体的な事柄については,異業種間カンファレンスがチーム医療を促進する事柄として認識されているが,整備・実施されていないことがうかがえた。各職種に対する認識については,職種別・経験年数別にみても,医師,看護師,理学・作業療法士はチーム医療のメンバーとして認識されている割合が高かった。また,経験年数別では,10~14年の人までは統計上ややばらつきがみられるが,経験年数15年以上の人はあらゆる職種をチーム医療のメンバーであると考え,その重要性を認識していることがわかった。チーム医療の利点については,どの職種も共通して「患者中心の医療」と考えている割合が高かった。Recently, the "team medical-care" has been received increasing attention in medical field. Therefore we have to develop a clear understanding of "team medical-care". The purpose of this study is to investigate things regarded as "team medical-care" by co-medical, recognition of other category of license, the advantages of "team medical-care" and recognition against the "team medical-care" by different category of license. Total 652 nurses, clinical radiological technologists and clinical laboratory technologists who are working at 8 hospitals in Okayama prefecture were examined by using questionnaire and 470 members (72.1%) responded to it. They were nurses 207 (44.0%), clinical radiological technologists 91 (19.4%), clinical laboratory technologists 172 (36.6%). Majority of respondents recognized that the medical conference consisted of different category of license promoted the "team medical-care", and ninety percent of participants prompted more frequent medical conference. In both by category of license and years of experience, they regarded doctor, nurse, physical therapist and occupational therapist as the menber of the "team medical-care" at high rate. The respondences which have over 15 years of experience regarded all category of license as the menber of the "team medical-care", while the respondences which have less than 10 to 14 years of experience showed disperision of the recongnition.     肥満解消のための無作為介入研究による脂肪量とアディポサイトカインの変化

    Adipocytokines arebioactive substances synthesizedand secretedbyfat cell. Previous studies have reported an association between weight loss and adipocytokines. However, these studies are inconsistent and they have not clarified the relationship between weight regain and changes in circulating levels of adipocytokines. In this study, we analyzed the relationship between weight and fat volume changes and adipocytokines. The subjects were 235obese people recruited in the Saku Control Obesity Program (SCOP). Participants were randomly assigned to either immediate (Group A) or delayed (Group B, control group). Group A participants were followed for another two years after completion of the one year intervention. As controls, Group B participants received the same intervention as Group A after a delay of one year. Then they were followed up for one year. The intervention consisted in a one-year lifestyle program to induce weight loss, based on a cognitive-behavioral approach. After the first year of the study, body weight, BMI, body fat and abdominal fat areas were significantly lower in group A participants, compared to controls. After the intervention, leptin levels were significantly lowered both in men and women. After one year follow-up, both men and women re-gained about 1.5 kg body weight on average. BMI, waist circumference, fat areas by CT and HBA1c significantly increased during the follow-up period. The change of adipocytokine levels by analysis of the quartile of body weight decrease and regain revealed that increased adiponectin and decreased leptin was noteworthy for weight reduction, while increase of leptin influenced the weight regain. In conclusion, our results suggest that leptin could have broad effects on the distribution off at tissues and on lipid metabolism. Leptin inversely associated with adiponectin, which in turn was necessary to decrease body weight. In particular, leptin decreased remarkably in the process of weight reduction, and its increase seemed to be related in weight regain. The observed increase of adiponectin seemed to be induced by reduction in fat volume

    Detection of Identical Isolates of Enterococcus faecalis from the Blood and Oral Mucosa in a Patient with Infective Endocarditis

    The detection of infective endocarditis (IE) of oral origin has been previously discussed. However, there are few reports confirming this infection using molecular biological techniques. We herein describe the case of a 67-year-old man who developed IE. Blood culture samples and strains obtained from the gingival and buccal mucosa showed 100% identity to Enterococcus faecalis JCM 5803 on sequencing of 16S rRNA gene fragments. A random amplification of polymorphic DNA (RAPD) analysis showed the same pattern for these samples, thus confirming the identity of E. faecalis isolates in the blood and oral mucosa. Our observations provide novel information regarding the level of identity between IE pathogens and oral bacteria

    Safety of Silk-elastin Sponges in Patients with Chronic Skin Ulcers: A Phase I/II, Single-center, Open-label, Single-arm Clinical Trial

    Background: Although traditional wound dressings such as collagen scaffolds promote granulation tissue formation, the efficacy of these dressings in chronic wounds is limited because of high susceptibility to bacterial growth. Biomaterials that can be applied to chronic wounds should have an anti-bacterial function. We previously reported that administering a silk-elastin solution that forms moisturizing hydrogels to wound surfaces of diabetic mice reduced bacterial growth and promoted granulation tissue formation compared with control or carboxymethyl cellulose hydrogels. We hypothesized that silk-elastin promotes wound healing in human chronic wounds by suppressing bacterial growth. Methods: An open-label, clinical case series was conducted with a prospective, single-arm design at Kyoto University Hospital in Kyoto, Japan. In this study, 6 patients with chronic skin ulcers of any origin (2 < ulcer area (cm2) < 25) on their lower extremities were included; patients with critical ischemia were excluded. Silk-elastin sponges were applied and covered with a polyurethane film without changing the dressing for 14 days. Inflammation triggered treatment discontinuation due to fear of infection. The primary study endpoint was adverse events, including inflammation and infection. Results: Poor hydrogel formation, possibly due to continuous exudation, was observed. No serious adverse events were noted. Two patients discontinued treatment on day 6 and day 7, respectively, due to inflammation, but they were not infected. The other 4 patients completed the 14-day silk-elastin sponge treatment without infection. Conclusion: Silk-elastin sponge is safe for chronic skin ulcers, and its ability to promote wound healing should be determined by confirmatory clinical trials

    Default mode network in young male adults with autism spectrum disorder: Relationship with autism spectrum traits

    Background: Autism spectrum traits are postulated to lie on a continuum that extends between individuals with autism and individuals with typical development (TD). Social cognition properties that are deeply associated with autism spectrum traits have been linked to functional connectivity between regions within the brain\u27s default mode network (DMN). Previous studies have shown that the resting-state functional connectivities (rs-FCs) of DMN are low and show negative correlation with the level of autism spectrum traits in individuals with autism spectrum disorder (ASD). However, it is unclear whether individual differences of autism spectrum traits are associated with the strength of rs-FCs of DMN in participants including the general population. Methods. Using the seed-based approach, we investigated the rs-FCs of DMN, particularly including the following two core regions of DMN: the anterior medial prefrontal cortex (aMPFC) and posterior cingulate cortex (PCC) in 19 young male adults with high-functioning ASD (mean age = 25.3 ± 6.9 years; autism-spectrum quotient (AQ) = 33.4 ± 4.2; full scale IQ (F-IQ) = 109.7 ± 12.4) compared with 21 age- and IQ-matched young male adults from the TD group (mean age = 24.8 ± 4.3 years; AQ = 18.6 ± 5.7; F-IQ = 109.5 ± 8.7). We also analyzed the correlation between the strength of rs-FCs and autism spectrum traits measured using AQ score. Results: The strengths of rs-FCs from core regions of DMN were significantly lower in ASD participants than TD participants. Under multiple regression analysis, the strengths of rs-FCs in brain areas from aMPFC seed showed negative correlation with AQ scores in ASD participants and TD participants. Conclusions: Our findings suggest that the strength of rs-FCs in DMN is associated with autism spectrum traits in the TD population as well as patients with ASD, supporting the continuum view. The rs-FCs of DMN may be useful biomarkers for the objective identification of autism spectrum traits, regardless of ASD diagnosis. © 2014 Jung et al.; licensee BioMed Central Ltd

    A region-based palliative care intervention trial using the mixed-method approach: Japan OPTIM study

    <p>Abstract</p> <p>Background</p> <p>Disseminating palliative care is a critical task throughout the world. Several outcome studies explored the effects of regional palliative care programs on a variety of end-points, and some qualitative studies investigated the process of developing community palliative care networks. These studies provide important insights into the potential benefits of regional palliative care programs, but the clinical implications are still limited, because: 1) many interventions included fundamental changes in the structure of the health care system, and, thus, the results would not be applicable for many regions where structural changes are difficult or unfeasible; 2) patient-oriented outcomes were not measured or explored only in a small number of populations, and interpretation of the results from a patient's view is difficult; and 3) no studies adopted a mixed-method approach using both quantitative and qualitative methodologies to interpret the complex phenomenon from multidimensional perspectives.</p> <p>Methods/designs</p> <p>This is a mixed-method regional intervention trial, consisting of a pre-post outcome study and qualitative process studies. The primary aim of the pre-post outcome study is to evaluate the change in the number of home deaths, use of specialized palliative care services, patient-reported quality of palliative care, and family-reported quality of palliative care after regional palliative care intervention. The secondary aim is to explore the changes in a variety of outcomes, including patients' quality of life, pain intensity, family care burden, and physicians' and nurses' knowledge, difficulties, and self-perceived practice. Outcome measurements used in this study include the Care Evaluation Scale, Good Death Inventory, Brief pain Inventory, Caregiving Consequence Inventory, Sense of Security Scale, Palliative Care Knowledge test, Palliative Care Difficulties Scale, and Palliative Care Self-reported Practice Scale. Study populations are a nearly representative sample of advanced cancer patients, bereaved family members, physicians, and nurses in the region.</p> <p>Qualitative process studies consist of 3 studies with each aim: 1) to describe the process in developing regional palliative care in each local context, 2) to understand how and why the regional palliative care program led to changes in the region and to propose a model for shaping regional palliative care, and 3) to systemically collect the barriers of palliative care at a regional level and potential resolutions. The study methodology is a case descriptive study, a grounded theory approach based on interviews, and a content analysis based on systemically collected data, respectively.</p> <p>Discussion</p> <p>This study is, to our knowledge, one of the most comprehensive evaluations of a region-based palliative care intervention program. This study has 3 unique aspects: 1) it measures a wide range of outcomes, including quality of care and quality of life measures specifically designed for palliative care populations, whether patients died where they actually preferred, the changes in physicians and nurses at a regional level; 2) adopts qualitative studies along with quantitative evaluations; and 3) the intervention is without a fundamental change in health care systems. A comprehensive understanding of the findings in this study will contribute to a deeper insight into how to develop community palliative care.</p> <p>Trial Registration</p> <p>UMIN Clinical Trials Registry (UMIN-CTR), Japan, UMIN000001274.</p
