    During the needs assessment for Langley Park, a Latino Promise Neighborhood outside Washington, DC, the Urban Institute went into the community expecting to find a significant proportion of young people out of school and unemployed but instead found something else (Scott et al. 2014). The rates of disconnected youth were on par with national averages, but nearly 40 percent of young people between the ages of 16 and 19 were working and not in school. This raises several questions. Is this trend specific to this neighborhood, Latinos, or first- and second-generation immigrants? Or is it a clue to a larger trend

    Neuropsychological Outcomes of U.S. Veterans with Report of Remote Blast-Related Concussion and Current Psychopathology

    This study explored whether remote blast-related MTBI and/or current Axis I psychopathology contribute to neuropsychological outcomes among OEF/OIF veterans with varied combat histories. OEF/OIF veterans underwent structured interviews to evaluate history of blast-related MTBI and psychopathology and were assigned to MTBI (n = 18), Axis I (n = 24), Co-morbid MTBI/Axis I (n = 34), or post-deployment control (n = 28) groups. A main effect for Axis I diagnosis on overall neuropsychological performance was identified (F(3,100) = 4.81; p = .004), with large effect sizes noted for the Axis I only (d = .98) and Co-morbid MTBI/Axis I (d = .95) groups relative to the control group. The latter groups demonstrated primary limitations on measures of learning/memory and processing speed. The MTBI only group demonstrated performances that were not significantly different from the remaining three groups. These findings suggest that a remote history of blast-related MTBI does not contribute to objective cognitive impairment in the late stage of injury. Impairments, when present, are subtle and most likely attributable to PTSD and other psychological conditions. Implications for clinical neuropsychologists and future research are discussed.     Branching coral growth and visual health during bleaching and recovery on the central Great Barrier Reef

    Coral reefs are under threat from cumulative impacts such as cyclones, crown-of-thorns starfish (COTS) outbreaks and climate-driven coral bleaching events. Branching corals are more severely impacted by these events than other coral morphologies due to their sensitivity to heat stress and weaker skeletons and COTS preferred prey. The central Great Barrier Reef experienced unprecedented back-to-back bleaching events in 2016 and 2017. This study commenced in 2017 at the peak of heat stress and examined the impact of the heatwave on the survival and recovery of corals by assessing the growth, health (based on the visual health index) and physiological parameters (chlorophyll a, zooxanthellae density, lipid and protein content) of two species, Acropora millepora and Pocillopora acuta (N = 60 colonies for each species). It was conducted across a gradient of turbidity at three reefs, Pandora, Orpheus and Rib, that experienced in April 2017, degree heating weeks (DHW) of 9, 8 and 7, respectively. Orpheus experienced the worst bleaching, based on visual health score, followed by Rib and Pandora. Rib experienced the greatest mortality (78% by Nov 2017); however, this was attributed to the presence of actively feeding crown-of-thorns starfish. Growth rates of A. millepora were almost twice the rate of P. acuta. Both species showed significant seasonal variation with growth of A. millepora and P. acuta 35–40% and 23–33% significantly greater in the summer, respectively. Differences in growth rates were best explained by indicators of energy acquisition. For example, the most important predictor variable in determining higher growth rates and visual health score in A. millepora was chlorophyll a content. For P. acuta, visual health score was the best predictor variable for higher growth rates. This study highlights the important role that chlorophyll a and associated symbionts play in growth and survival in these corals during and after a heat stress event

    Core competencies for pain management: results of an interprofessional consensus summit.

    ObjectiveThe objective of this project was to develop core competencies in pain assessment and management for prelicensure health professional education. Such core pain competencies common to all prelicensure health professionals have not been previously reported.MethodsAn interprofessional executive committee led a consensus-building process to develop the core competencies. An in-depth literature review was conducted followed by engagement of an interprofessional Competency Advisory Committee to critique competencies through an iterative process. A 2-day summit was held so that consensus could be reached.ResultsThe consensus-derived competencies were categorized within four domains: multidimensional nature of pain, pain assessment and measurement, management of pain, and context of pain management. These domains address the fundamental concepts and complexity of pain; how pain is observed and assessed; collaborative approaches to treatment options; and application of competencies across the life span in the context of various settings, populations, and care team models. A set of values and guiding principles are embedded within each domain.ConclusionsThese competencies can serve as a foundation for developing, defining, and revising curricula and as a resource for the creation of learning activities across health professions designed to advance care that effectively responds to pain

    Surgical management of primary colonic lymphoma: Big data for a rare problem

    Background and ObjectivesPrimary colonic lymphoma (PCL) is rare, heterogeneous, and presents a therapeutic challenge for surgeons. Optimal treatment strategies are difficult to standardize, leading to variation in therapy. Our objective was to describe the patient characteristics, short‐term outcomes, and five‐year survival of patients undergoing nonpalliative surgery for PCL.MethodsWe performed a retrospective cohort analysis in the National Cancer Database. Included patients underwent surgery for PCL between 2004 to 2014. Patients with metastases and palliative operations were excluded. Univariate predictors of overall survival were analyzed using multivariable Cox proportional hazard analysis.ResultsWe identified 2153 patients. Median patient age was 68. Diffuse large B‐cell lymphoma accounted for 57% of tumors. 30‐ and 90‐Day mortality were high (5.6% and 11.1%, respectively). Thirty‐nine percent of patients received adjuvant chemotherapy. For patients surviving 90 days, 5‐year survival was 71.8%. Chemotherapy improved survival (surgery+chemo, 75.4% vs surgery, 68.6%; P = .01). Adjuvant chemotherapy was associated with overall survival after controlling for age, comorbidity, and lymphoma subtype (HR 1.27; 95% CI, 1.07‐1.51; P = .01).ConclusionsPatients undergoing surgery for PCL have high rates of margin positivity and high short‐term mortality. Chemotherapy improves survival, but <50% receive it.
