    Stability studies of nano-cream containing piroxicam

    The aim of this study is to study the stability of the nano-cream formula containing the newly synthesized palm oil esters when stored for reasonable storage duration. The prepared 0.5% piroxicam nano-cream formula contained phosphate buffer as external phase, palm oil esters as the oil phase and a combination of (80:20) of Tween 80 and Span 20 as emulsifier at a ratio of 37:25:38, respectively. Piroxicam is a hydro-lipophobic drug. Stability on storage is an important aspect which ensures the dosage form can exert the effects it is supposed to exert for the duration of storage. Droplets size, electrical conductivity, drug content, pH and rheological parameters are the parameters that have been assessed under different temperature to evaluate the stability of nano-cream preparation. Thus, experiments which measure the above parameters were conducted at storage temperatures of 4, 25 and 40ÎżC.The data obtained from the stability study conducted on nano-cream formula showed that this formulation was stable for the whole 3 months period of the study when stored at tested several temperatures.Keywords: Palm oil esters; Nonionic surfactant; Piroxicam; Nano-crea

    Formulation and characterization of palm oil esters based nano-cream for topical delivery of piroxicam

    Palm oil esters are high molecular weight esters oil that has been newly synthesized by University Putra Malaysia researchers. It has received a lot of attention for its pharmaceutical and chemical application. Piroxicam is a nonsteroidal anti-inflammatory drug with analgesic and antipyretic activity. It has low solubility in water as well as in oil with Log P value of 1.8. Generally, drugs with Log P value of more than 0.5 are needed to be formulated into a modified dosage form. One of these formulations is nano sized cream. Hence, the ability of formulating of these tricky drugs into dispersed system is questionable. The aim of this study is to investigate the ability of palm oil esters to be the oil phase for formulation of piroxicam into O/W nano-cream. Three points were selected from prepared pseudoternary diagram of palm oil esters and different Tween and Span mixtures. Piroxicam solubility and partition coefficient in oil and external phase was detected. Rheological properties, droplet size, structural properties and zeta potential of the dispersion system containing piroxicam were measured. O/W cream was formed with droplet size measurement by TEM of less than 100 nm. It could be concluded that palm oil esters is suitable oil for the formulation of suitable nano-cream containing piroxicam.Keywords: Palm oil esters; Piroxicam Solubility; partition coefficient; Rheology; Surface activity

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030

    The hazards of plastic waste (PW) from polymers (e.g., polyethylene terephthalate (PET), high-density polyethylene (HDPE), low-density polyethylene (LDPE), polyvinyl chloride (PVC), nylon, polystyrene (PS), etc.), the mechanism of its spread in general, and its ubiquity in our daily lives as a continuously and/or frequently expelled product are a crisis of the twenty-first century, as reported by the United Nations in 2019, especially after the outbreak of the COVID-19 pandemic. This research included the process of modifying the rheological properties of asphalt to obtain asphalt suitable for use in a high-humidity atmosphere. The Iraqi climate is characterized by heat that reaches the point of harshness in the summer and coldness that falls below zero on some winter days. From this point of view, our recent study focuses mainly on making rheological and chemical modifications to asphalt using spent polymeric materials and used lubricating oils (ULO), thus achieving two important goals, namely obtaining asphalt with rheological properties resistant to the Iraqi atmosphere as well as eliminating both solid and liquid environmental pollutants.     Investigating the Effect of Gamma and Neutron Irradiation on Portland Cement Provided with Waste Silicate Glass

    In this study, samples of commercial Portland cement mixed with 30% weight of crushed waste silicate glass were prepared in the shape of well-dried cylinders. Then, their physical and mechanical properties were investigated for two types of samples: samples without exposure and samples with exposure to gamma-ray and neutron irradiation. A notable deterioration of the physical properties of the irradiated samples relative to the non-irradiated ones was recorded. All the spectroscopic analyses were performed for the samples with exposure and without exposure to gamma-ray and neutron irradiation. The XRD emerging peaks of irradiated samples were studied to estimate the presence and stabilities of major peaks indicating the presence of the main compositions of cement with the amorphous nature of glass. FT-IR transmittance spectra were identified and the bonds were located close to those of identical glasses. Moreover, SEM images and EDX analysis were conducted on the two types of composite samples (without exposure and with exposure to gamma and neutron irradiation) to specify the change in the physical appearance and the chemical composition after irradiation. The attenuation parameters were computed theoretically with the assistance of Phy-X/PSD software to evaluate the gamma-ray and neutron shielding properties by defining the composition and the density of the samples. The irradiation was found to have a negative impact on the shielding ability of the prepared samples where there was an over-reduction in the parameters calculated with the probability that the damage may increase with longer exposure to the radiation

    In vitro and in vivo evaluation of the antiangiogenic activities of Trigonella foenum-graecum extracts

    Objective: To assess the antiangiogenic activity of fenugreek. Methods: Different fractions of fenugreek crude extracts were prepared and their antiangiogenic properties were assessed using the ex vivo rat aortic ring assay and in vivo chicken embryo chorioallantoic membrane (CAM) assay. They were investigated for their direct cytotoxic activity in the MCF7 cells using the MTT assay. Results: The ethanol extract showed 100% inhibition of blood vessel outgrowth from primary tissue explants in the rat aortic ring assay at a concentration of 100 μg/mL while the other extracts did not show significant antiangiogenic activity. The ethanol extract was therefore investigated at varying concentrations and exhibited a significant dose dependent effect. The CAM assay coincided with the results of the aortic ring assay as ethanol extract showed a significant inhibition of formation of new blood vessels. The extracts only showed anti-proliferative activity at the highest concentration of 400 μg/mL towards MCF7 breast cancer cell lines in the MTT assay. Conclusions: Findings of the both assays confirmed that the ethanol extract inhibited vascularization significantly. Further studies on the ethanol extract would be beneficial in isolating the active ingredient responsible for the inhibition
