    Ocular Brucellosis: A Brief Update

    Brucellosis is a zoonotic disease of important health significance in endemic areas such as Iran. Brucellosis can affect any organ or system in the human body. Ocular involvement is rare and can lead to poor visual prognosis if not treated early. The most common ocular manifestation of brucellosis is uveitis, although any ocular structure can be involved. The diagnosis of ocular brucellosis is based on clinical examination as well as culture and serological tests. The classical treatment regimen includes doxycycline and rifampin. Early diagnosis and prompt treatment are effective in preventing blindness. Here we breifly review the manifestations, diagnostic tools, and treatment of ocular brucellosis

    Studying the flourishing of Iranian students based on Seligman model and its comparison based on demographic variables

    Introduction: The basic objective of this paper was to study the flourishing of Iranian students based on Seligman model and its comparison based on demographic variables.  Methods: 400 subjects of all students enrolled in the second semester of academic year of 20142015 in Ferdowsi University of Mashhad were randomly classified and studied as the sample group. Flourishing questionnaire was used to test the hypotheses. The flourishing questionnaire includes four components of positive emotions, relationships, meaning and achievement. To analyze the research data, the followings were used: percentage, mean, system dynamics (SD), multivariate analysis of variance, univariate analysis of variance and post hoc least significant difference (LSD) test.   Results: Descriptive results showed that 5% of students are at wilting point; 30% have relative mental health and 65% are at flourishing state. The highest percentage of flourishing students was in the meaning component (72.5%), while the lowest percentage of them was found in the positive emotions component (59.75%). The results of MANOVA test and follow-up tests also showed that the flourishing rate of the students is not affected by father's education, mother's education, father's occupation, mother's occupation, school and residence of the students (native or non-native); however, the flourishing rate of the students in the relationships component was significantly different with regard to the gender and education of the students; This means that the female students compared to the male students, and master and PhD students compared to the undergraduate students had earned higher scores in the relationships component. Conclusion: At the end, according to the results of the present study, it is suggested to develop a unique intervention program to promote and foster flourishing of the students, while considering gender and educational differences in the relationships component.     Introduction: The purpose of this study was to clarify whether type 2 diabetic patients with coronary disease are subject to similar benefits in heart rate recovery (HRR) as non-diabetic counterpatrs after cardiac rehabilitation, assessing men and women subjects separately. Material and methods: The data used for this analysis were from an eight-week, phase-II cardiac rehabilitation including 284 patients with ischaemic heart disease who were managed at Tehran Heart Centre between July 2004 and January 2006. The heart rate parameters were compared between diabetic and non-diabetic patients before and after cardiac rehabilitation. Diabetic and non-diabetic patients had similar age and left ventricular ejection fraction. Results: Among men, the non-diabetic patients achieved a greater improvement in peak heart rate and heart rate recovery (HRR). Additionally, lower resting heart rate was found in nondiabetic men after rehabilitation. In the women &#8805; 50 years old, there was no significant difference between diabetic and non-diabetic. The non-diabetic women < 50 years old showed significantly higher peak heart rate and HRR compared with diabetic women. Conclusions: These results indicate that the benefit of cardiac rehabilitation in HRR is significantly lower in type 2 diabetic men. Improvement of HRR is not associated with diabetic status in women &#8805; 50 years old. The response to cardiac rehabilitation in women may appear to be influenced more by age at menopause rather than diabetes mellitus.Wstęp: Celem badania było wyjaśnienie czy pacjenci z cukrzycą typu 2 i chorobą wieńcową odnoszą podobne korzyści z rehabilitacji kardiologicznej dotyczące normalizacji częstości rytmu serca (HRR, heart rate recovery) jak osoby z chorobą wieńcową bez cukrzycy. Osobno oceniano mężczyzn i kobiety. Materiał i metody: Dane wykorzystane w analizie pochodziły z 8-tygodniowego II stadium rehabilitacji kardiologicznej przeprowadzonej u 284 pacjentów z chorobą niedokrwienną serca leczonych w Tehran Heart Center w okresie pomiędzy lipcem 2004 roku a styczniem 2006 roku. Porównywano parametry opisujące częstość rytmu serca u osób z cukrzycą i bez cukrzycy, przed i po rehabilitacji kardiologicznej. Pacjenci z cukrzycą i bez cukrzycy charakteryzowali się podobnym wiekiem i zbliżoną frakcją wyrzutową lewej komory. Wyniki: U mężczyzn bez cukrzycy uzyskano większą poprawę dotyczącą szczytowej częstości rytmu serca i normalizacji HRR. Dodatkowo po rehabilitacji, u mężczyzn bez cukrzycy stwierdzono mniejszą spoczynkową częstość rytmu serca. Nie zaobserwowano znamiennych różnic pomiędzy kobietami z cukrzycą i bez cukrzycy w wieku 50 lat i starszych. Kobiety bez cukrzycy poniżej 50. roku życia charakteryzowały się istotnie większą szczytową częstością rytmu serca i HRR w porównaniu z kobietami z cukrzycą. Wnioski: Uzyskane wyniki świadczą o tym, że korzyści z rehabilitacji kardiologicznej dotyczące HRR są istotnie gorsze u mężczyzn z cukrzcą typu 2. Poprawa dotycząca HRR u kobiet w wieku 50 lat i starszych nie zależała od obecność cukrzycy. Wydaje się, że u kobiet odpowiedź na rehabilitację kardiologiczną w większym stopniu zależy od wieku, w którym wystąpiła menopauza niż od obecności cukrzycy

    Comparison of Iris Fixated and Scleral Fixated intra Ocular Lens for Correction of Aphakia in Traumatic Cataract with no Proper Capsular Support

    Purpose: To compare visual outcomes and complications of iris fixated and scleral fixated intra ocular lens (IOL) implantation in patients suffering from traumatic cataract with no proper capsular support.Patients and Methods: This prospective interventional study was performed in Farabi Eye Hospital, Tehran, Iran, between May 2015 and May 2016. Twenty five patients with traumatic cataract and no proper capsular support for IOL implantation in the bag or sulcus were included and randomly underwent either iris fixated or sulcus fixated IOL implantation. Follow up visits were performed one day, one week, four weeks, three months and six months after surgery. In each visit visual acuity, intra ocular pressure, placement of IOL and anterior chamber reaction were studied. Results: Thirteen eyes of 13 patients underwent iris fixation and 12 eyes of 12 patients underwent scleral fixation of IOL. The average patient age at presentation was 29.12 ± 16.32. In the follow up visit one week after IOL fixation three patients in each group had significant anterior chamber inflammation. There was no statistically significant difference between the two groups regarding the number of patients with elevated IOP (P = 0.96), dislocated IOL (P = 0.480) and complications such as wound dehiscence and iridocorneal adhesion. Also no statistically significant difference regarding the mean BCVA three months after surgery was observed (P = 0.55).Conclusion: We did not observe any significant difference in outcome of iris and scleral fixation of IOL in traumatic eye injuries with no effective capsular support.&nbsp

    A study to measure the impact of organizational culture and organizational excellence

    Organizational culture plays an important role on increasing organization excellence and there are many evidences through different studies on this relationship. In this paper, we re-examine this relation between organizational culture and six other important factors including quality of leadership, quality of different strategies, quality of human resources, quality of participating in organizational resources, quality of organizational processes and methods of assessment. The study was held among 70 employees of one of distance learning universities located in province of Esfahan, Iran. The study uses Pearson correlation ratio as well as linear regression technique to investigate the relationships. The results confirmed that there are positive and meaningful relationship between organizational culture and quality of leadership, quality of different strategies, quality of human resources, quality of participating in organizational resources, quality of organizational processes but it does not find any meaningful relationship between organizational culture and methods of assessment
