    Small Colonic Cancer with Invasion of the Subserosal Layer. Report of a Case

    Small, flat-type advanced colonic adenocarcinomas are rare. We present a case of small colonic carcinoma invading the subserosal layer. A 61-year-old asymptomatic man was admitted for further examination of positive occult blood test. Barium enema and endoscopic examination revealed a small (10 mm in diameter) flat lesion with elevated margins and a central depression, in the transverse colon. Biopsy specimen taken from the tumor showed poorly differentiated adenocarcinoma. Based on radiologic and endoscopic evaluation, a provisional diagnosis was made of colonic tumor with invasion of the deep submucosal layer. Surgical resection of the transverse colon was performed. The tumor was macroscopically a type IIa + IIc lesion measuring 10 mm in diameter. Histological examination showed poorly differentiated adenocarcinoma infiltrating the subserosal layer. Awareness of this type of tumor should allow early diagnosis and treatment, resulting in improved prognosis

    Superficial Depressed Type (IIc) Early Cancer of the Colon : Report of Two Cases

    Two cases of superficial depressed type (IIc) early cancer of the colon are reported. Case 1 was a 65-year-old male and case 2 a 69-year old male. The lesion was located in the descending colon in both cases, and was removed by strip biopsy endoscopically in the former and surgically in the latter. The size of the lesion after resection was 6 mm in case 1 and 5 mm in case 2. Histopathologically, both cases were well differentiated adenocarcinoma without adenomatous components, and carcinoma developed de novo by submucosal (sm) invasion. As to the immunohistochemical staining of the cancer tissue by tumor associated antigen, case 1 showed a strong expression of carcinoembrionic antigen (CEA) and partial expression of sialyl Lewisx, and case 2 showed expressions of both CEA and sialyl Lewisx . The nuclear DNA content by flow cytometry was aneuploid only in case 1. Thus, although the two cases were morphologically the same IIc type cancer, the process of carcinogenesis and secondary phenomena varied

    This paper is the first report of the comparative study on subject-type and non-subject-type ski practice. The purpose of this study was to investigate the evaluation of students, who participated in a ski intensive course as a practice in the curriculum of the department of Physical education (subject-type ski practice). This evaluation was compared with that of participants in a ski intensive course as a service program of the student\u27s association (non-subject-type ski practice). The results were summarized as follows: 1. In the ski experience and the preliminary learning quantity, there was no difference in both groups before participating in each practice. 2. The improvement of the ski skill was the major motive and expectations of the participants in subject-type ski practice. On the other hand, it was the human alternating current through the sport in non-subject-type ski practice. 3. It was found in both practices that the will, which continues a ski, is proportional to the comprehensive satisfaction of participating in a practice. Some improvements were pointed out to both practices

    Cooperation for disaster prevention between local autonomous disaster prevention organizations in Tokyo and public elementary schools were clarified. Interviews were conducted with the principal of three elementary schools in three districts of Tokyo that were designated to be used as shelters, and as bases for rescue operations at a time of catastrophic calamity. Results indicated that the shelters might not be promptly established at a time of a disaster, and that stockpiles of emergency materials were insufficient. Interactions had taken place between the autonomous disaster prevention organization and all the schools, but shelter management training between the school and the local area had not been conducted. The results of this study indicated the need for cooperative events, such as emergency shelter management simulation training and for strengthening cooperation between local resident and the school

    The purpose of this study was to reveal the actual conditions and problems from activity in local autonomous disaster prevention organizations in Tokyo and Chiba prefectures. Some problems that inhibited disaster prevention were found by this research. There was under estimation for disaster prevention by the administration. There was indifference towards disaster prevention by the residents and the staff of local autonomous disaster prevention organizations. Also, there was little interaction between old residents and new residents. In addition, human relation problems existed between elderly members and young members in local autonomous disaster prevention organizations. We list up six necessary things to be done: (1) To prepare an adequate stockpile of food and tools for, rescuing. (2) To clearly designate a leader for disaster prevention. (3)To set up an organization management emergency shelter independent of the administration staff. (4) To set up an organization to promote disaster prevention for residents of apartment houses, and workers in small and medium-scale enterprises. (5) To make local autonomous disaster prevention organization centering on PTA in elementary schools. (6)To teach disaster prevention to elementary. school students and junior high school students as they will be the future leaders for disaster prevention

    Differences between disaster preventive actions and measures taken by the general population, and improvements to the disaster prevention system were compared between Tokyo; where a large earthquake is predicted, and Kobe, where the Hanshin-Awaji (Kobe) Earthquake was experienced in January 1995. A questionnaire survey was conducted with the resident of two districts in Tokyo and Kobe. Results indicated that the sample of residents in Tokyo did not understand how to prepare for disasters, with the exception of refuge management provisions, which were better than in Kobe. In addition, a tendency for not making concrete preparations was identified, despite the worry about the possibility of a large earthquake happening. Moreover, there was a tendency to value local human networks. Finally, the sample in Kobe was more optimistic than the one in Tokyo

    Nutritional effects of branched chain amino acids and lipids in surgical stress after partial hepatectomy in the rat

    The nutritional influence of branched chain amino acids (BCAA) and lipids in surgical stress was examined after 70% hepatectomy in rats. Two types of elemental diet (ED) were given through the gastrostomy tude for 7 days. Group A : ED containing 33% BCAA and 30% lipid (medium chain triglycerides 8.5g+soy bean oil 1.5g/300 kcal). Group B : ED containing 17% BCAA and 1.5% lipid (soy bean oil 0.49g/300 kcal). Up to the 7th postoperative day, the excretion of nitrogen into urine decreased in group A, but no change occurred in group B. Up to the 5th postoperative day, molar ratio of 3MH to creatinine in urine in group A decreased more rapidly than in group B. Plasma concentration of albumin in group A was higher than in group B on the 7th postoperative day. Fischer ratio was significantly higher in group A than in group B on the 7th postoperative day. Both plasma and muscle levels of Tyr, Met and Gln were higher in group B than in group A, Glu and Ala were lower in group B than in group A. Essential fatty acid deficiency occurred more severely in group B than in group A. While fatty livers were microscopically observed in some livers in group B, there were few fatty deposits in group A
