    Psychological state in cancer patients

    The psychological state and changes over time of cancer patients in the active treatment stage were classified into emotions by the speech and behavior of the patient described in the medical record article of the cancer psychological interview record, and the analysis of the “emotional state map” was attempted. In all cases, positive / negative emotions were mixed and appeared with variation, but a relatively large number of positive emotions, including , , and , were manifested, and the same was true in patients who experienced stressful treatment events. In the background, the existence of appropriate support from medical professionals and psychological characteristics peculiar to the stage of active treatment was inferred, such as the active treatment of the target patient, the hospitalization environment in which mental and physical pain promptly appealed to medical professionals, and the influence of psychological interviews. Cancer patients during active treatment perceived and expressed changes in the body and pain caused by the disease, and after responses from medical professionals and family members, they were conscious of their physical condition and emotions. It is suggested that this analysis method helps to objectively understand and explain the invisible and ever-changing psychological state of cancer patients in the active treatment stage

    Association between upper limb movements during drumming and cognition in older adults with cognitive impairment and dementia at a nursing home: a pilot study

    BackgroundDespite the association between motor dysfunction and dementia, quantitative assessment of dementia-related specific motor dysfunction in patients with severe dementia is difficult. Thus, this study aimed to develop a new method to measure upper limb motor function in people with dementia.MethodsWe examined the relationship between dementia severity and dementia-related specific motor dysfunction using the Mini-Mental State Examination (MMSE), a dementia screening test. Participants comprised 16 nursing home residents with a mean age of 86 years and MMSE score of 15.75 (range, 1–27) Points. Participants were seated in a circle and instructed to play a drum that was placed in their lap using mallets (drumsticks) in their dominant hand. Acceleration and gyroscopic sensors were attached to their wrists to collect data on arm movements while drumming. Upper limb motor characteristics were confirmed by recording acceleration and arm movement during drumming and analyzing the correlation with handgrip strength.ResultsHandgrip strength was correlated with arm elevation angle during drumming. The arm elevation angle displayed a significant regression equation with the MMSE score and showed the best regression equation along with handgrip strength (adjusted R2 = 0.6035, p = 0.0009).ConclusionWe developed a new method using drums to measure upper limb motor function in people with dementia. We also verified that the average arm elevation angle during drumming could predict cognitive dysfunction. This system may be used to monitor people with dementia in a simple and safe way

    Impact of Gestational Age at Delivery on Fetal Heart Rate Monitoring During the Second Stage of Labor

    Objective: To clarify whether gestational age at delivery affects the interpretation level of fetal heart rate (FHR) tracings in the second stage of labor in primiparous and singleton pregnancies.Methods: The database at one tertiary hospital in Japan was retrospectively reviewed for women with singleton fetuses in cephalic presentation and vaginal labor at ≥37+0 gestational weeks between June 1, 2011, and March 31, 2013. Continuous FHR tracings in the second stage of labor were subdivided into 10-minute intervals, each of which we called a window, from the beginning through delivery, and were assessed according to the five-tier classification proposed by the Japan Society of Obstetrics and Gynecology, in which level 1 is normal, level 2 is subnormal, and levels 3-5 are abnormal patterns. Subjects were divided into two groups, including early term (37 0/7 to 38 6/7 weeks) and full term (39 0/7 to 41 6/7 weeks), according to the delivery age.Results: In total, 914 parturient women were eligible for the study protocol, including 228 and 686 women in early and full term, respectively. Vacuum extraction was more often observed in full term than in early term (P=0.007). Although the maximal level, number of level-5 windows, number of level-4 windows, and number of level-3 and level-4 windows were similar between the groups, the summation of level-4 windows ≥3 was significantly higher in full term than in early term (P=0.004).Conclusion: Full term delivery has a higher risk than does early term delivery from the standpoint of FHR monitoring

    Real-time PCR for VZV keratitis

    Pupose: To determine the efficacy of real-time PCR for the diagnosis and prognosis of varicella-zoster virus (VZV) keratitis. Study design: Retrospective case series. Methods: Patients: 545 consecutive patients with keratitis were examined to quantify copy numbers of VZV DNA by real-time PCR. Association of copy numbers of DNA of VZV to clinical signs and disease course was assessed using logistic regression analysis and Cox proportional hazard model. Results: Of the 545 eyes, 38 eyes (6.9%) were diagnosed as VZV keratitis. The copy numbers of the DNA of VZV (median: 104.19 copy) was significantly associated with diagnosis of VZV keratitis with the highest odds ratio (OR) of 3390 (for median copy) compared to clinical signs. Diagnostic accuracy of the VZV DNA copy indicated good diagnostic value of area under the curve (0.92) by receiver operating characteristic analysis, and detection of unrelated VZV DNA from the cornea was very rare (0.2%). When the VZV DNA copy and clinical signs were assessed for association with the disease course after herpes zoster ophthalmicus, the disease duration was significantly prolonged in VZV keratitis cases with higher numbers of VZV DNA copies, iritis, and history of recurrences. The amount of VZV DNA led to a continuous risk to prolong disease duration until the ocular inflammation subsides (hazard ratio (HR) 0.17, 95%CI: 0.07 - 0.42, for median copies). Conclusions: Higher VZV DNA copy numbers are associated with the refractoriness of VZV keratitis, and its evaluation may be a clinically useful way to diagnose and manage VZV keratitis
