    Cancer of the Pancreas in 125 Patients

    In this report, we did a retrospective study of 125 patients with cancer of the pancreas. Seventeen (36.2%) of the 47 patients with cancer of the pancreatic head underwent resection; 11.2% died within 30 days of surgery, 33.3 survived the first year, and 13.3 survived the first three years. The longest survival was for five years. Nine (23%) of the 39 patients with cancer of the pancreatic body and tail underwent resection; 11.1% died within 30 days, 37.5% survived the first year, and none survived more than three years. The longest survival was for 35 months. Important factors affecting the prognosis were invasion by the cancer of veins or lymph vessels in the surrounding tissues, and metastasis to regional lymph nodes. Lymphatic drainage of small cancers of the pancreatic head were established early. Invasion to the retroperitoneal tissue in cancer of the pancreatic body and tail was more common than in cancer of the pancreatic head. For the patient of early cancer of the pancreas, we strongly recommend total pancreatectomy over WHIPPLE\u27s pancreatoduodenectomy, based on results of our analysis of the outcome in our patients following these two operations.     Rearrangement of the breakpoint cluster region in Philadelphia chromosome positive acute leukemia.

    The rearrangement of breakpoint cluster region (ber) was examined in leukemic cells obtained from 3 patients initially diagnosed as having Ph+ acute leukemia, 2 with acute lymphocytic leukemia (ALL) and one with acute mixed leukemia. DNA was digested with Bgl II and BamH I. The ber rearrangement was present in the case of acute mixed leukemia (Case 1), but was absent in the 2 cases of ALL (Cases 2 and 3). These results suggest that Case 1 represented a type of blast crisis of chronic myelocytic leukemia which was unusual in the sense of the occurrence of a myeloid-lymphoid conversion and lack of an apparent chronic phase. Cases 2 and 3 appeared to be de novo Ph+ ALL.</p

    Clinical Outcome of Patients with Pelvic and Retroperitoneal Bone and Soft Tissue Sarcoma : A Retrospective Multicenter Study in Japan

    This study aimed to retrospectively analyze the clinical outcomes of patients with pelvic and retroperitoneal bone and soft tissue sarcoma (BSTS). Overall, 187 patients with BSTS in the pelvis and retroperitoneal region treated at 19 specialized sarcoma centers in Japan were included. The prognostic factors related to overall survival (OS), local control (LC), and progression-free survival (PFS) were evaluated. The 3-year OS and LC rates in the 187 patients were 71.7% and 79.1%, respectively. The 3-year PFS in 166 patients without any distant metastases at the time of primary tumor diagnosis was 48.6%. Osteosarcoma showed significantly worse OS and PFS than other sarcomas of the pelvis and retroperitoneum. In the univariate analyses, larger primary tumor size, soft tissue tumor, distant metastasis at the time of primary tumor diagnosis, P2 location, chemotherapy, and osteosarcoma were poor prognostic factors correlated with OS. Larger primary tumor size, higher age, soft tissue tumor, chemotherapy, and osteosarcoma were poor prognostic factors correlated with PFS in patients without any metastasis at the initial presentation. Larger primary tumor size was the only poor prognostic factor correlation with LC. This study has clarified the epidemiology and prognosis of patients with pelvic and retroperitoneal BSTS in Japan
