    Image Correction Methods for Regions of Interest of Cirrhosis Liver Classification on CNNs

    The average error rate in liver cirrhosis classification on B-mode ultrasound images using the traditional pattern recognition approach is still too high. In order to improve the liver cirrhosis classification performance, image correction methods and a convolution neural network (CNN) approach are focused on. The impact of image correction methods on region of interest (ROI) images that are input into the CNN for the purpose of classifying liver cirrhosis based on data from B-mode ultrasound images is investigated. In this paper, image correction methods based on tone curves are developed. The experimental results show positive benefits from the image correction methods by improving the image quality of ROI images. By enhancing the image contrast of ROI images, the image quality improves and thus the generalization ability of the CNN also improves

    Repeated talaporfin sodium photodynamic therapy for esophageal cancer: safety and efficacy

    [Background] Talaporfin sodium photodynamic therapy (tPDT) is an effective salvage treatment for local failure after chemoradiotherapy for esophageal cancer. Repeated tPDT could also be indicated for local recurrence or residue after the first salvage tPDT. However, the safety and efficacy of repeated tPDT have not been elucidated. [Methods] We reviewed 52 patients with esophageal cancer who were treated with the first tPDT at Kyoto University Hospital between October 2015 and April 2020. [Results] Among 52 patients, repeated tPDT after the first tPDT was indicated for 13 patients (25%), of which six had residual tumor, four had local recurrence after complete response (CR) after the first tPDT at the primary site, and six had metachronous lesion. The total session of repeated tPDT was 25; 16 were for primary sites and nine were for metachronous sites. Among them, six patients (46.2%) achieved local (L)-CR and nine lesions (56.3%) achieved lesion L-CR. By session, 10 sessions (40%) achieved L-CR. There were no severe adverse events except for one patient; this patient showed grade 3 esophageal stenosis and perforation after the third tPDT on the same lesion that was previously treated with porfimer sodium photodynamic therapy four times. [Conclusion] Repeated tPDT could be an effective and safe treatment for local failure even after salvage tPDT for esophageal cancer

    Pyridoxal 5′-phosphate and related metabolites in hypophosphatasia: Effects of enzyme replacement therapy

    Objective To investigate the utility of serum pyridoxal 5′-phosphate (PLP), pyridoxal (PL), and 4-pyridoxic acid (PA) as a diagnostic marker of hypophosphatasia (HPP) and an indicator of the effect of, and patient compliance with, enzyme replacement therapy (ERT), we measured PLP, PL, and PA concentrations in serum samples from HPP patients with and without ERT. Methods Blood samples were collected from HPP patients and serum was frozen as soon as possible (mostly within one hour). PLP, PL, and PA concentrations were analyzed using high-performance liquid chromatography with fluorescence detection after pre-column derivatization by semicarbazide. We investigated which metabolites are associated with clinical phenotypes and how these metabolites change with ERT. Results Serum samples from 20 HPP patients were analyzed. The PLP-to-PL ratio and PLP concentration were elevated in all HPP patients. They correlated negatively with serum alkaline phosphatase (ALP) activity and showed higher values in more severe phenotypes (perinatal severe and infantile HPP) compared with other phenotypes. PL concentration was reduced only in perinatal severe HPP. ERT reduced the PLP-to-PL ratio to mildly reduced or low-normal levels and the PLP concentration was reduced to normal or mildly elevated levels. Urine phosphoethanolamine (PEA) concentration did not return to normal levels with ERT in most patients. Conclusions The serum PLP-to-PL ratio is a better indicator of the effect of ERT for HPP than serum PLP and urine PEA concentrations, and a PLP-to-PL ratio of <4.0 is a good indicator of the effect of, and patient compliance with, ERT

    Clonal evolution and clinical implications of genetic abnormalities in blastic transformation of chronic myeloid leukaemia

    Blast crisis (BC) predicts dismal outcomes in patients with chronic myeloid leukaemia (CML). Although additional genetic alterations play a central role in BC, the landscape and prognostic impact of these alterations remain elusive. Here, we comprehensively investigate genetic abnormalities in 136 BC and 148 chronic phase (CP) samples obtained from 216 CML patients using exome and targeted sequencing. One or more genetic abnormalities are found in 126 (92.6%) out of the 136 BC patients, including the RUNX1-ETS2 fusion and NBEAL2 mutations. The number of genetic alterations increase during the transition from CP to BC, which is markedly suppressed by tyrosine kinase inhibitors (TKIs). The lineage of the BC and prior use of TKIs correlate with distinct molecular profiles. Notably, genetic alterations, rather than clinical variables, contribute to a better prediction of BC prognosis. In conclusion, genetic abnormalities can help predict clinical outcomes and can guide clinical decisions in CML

    Daytime nap and nighttime breastfeeding are associated with toddlers\u27 nighttime sleep

    The purpose of the present study is to examine the association between toddlers\u27 sleep arrangements and their nighttime sleep duration and other sleep variables. For this investigation, we performed a study in which child activity and sleep levels were recorded using actigraphy. The parents of 1.5-year-old toddlers (n = 106) were asked to attach an actigraphy unit to their child’s waist with an adjustable elastic belt and complete a sleep diary for 7 consecutive days. Questionnaires were used to assess the sleep arrangements of the toddlers. There was a significant negative correlation between nap duration and nighttime sleep duration, suggesting that longer nap sleep induces shorter nighttime sleep duration. Among the sleep arrangements, such as nighttime breastfeeding or co-sleeping, only nighttime breastfeeding predicted shorter nighttime sleep duration. Our findings indicate that shorter naps induce a longer nighttime sleep in 1.5-year-old toddlers while nighttime breastfeeding decreases their nighttime sleep duration

    Sleep maturation influences cognitive development of preterm toddlers

    Our recent study on full-term toddlers demonstrated that daytime nap properties affect the distribution ratio between nap and nighttime sleep duration in total sleep time but does not affect the overall total amount of daily sleep time. However, there is still no clear scientific consensus as to whether the ratio between naps and nighttime sleep or just daily total sleep duration itself is more important for healthy child development. In the current study, to gain an answer to this question, we examined the relationship between the sleep properties and the cognitive development of toddlers born prematurely using actigraphy and the Kyoto scale of psychological development (KSPD) test. 101 premature toddlers of approximately 1.5 years of age were recruited for the study. Actigraphy units were attached to their waist with an adjustable elastic belt for 7 consecutive days and a child sleep diary was completed by their parents. In the study, we found no significant correlation between either nap or nighttime sleep duration and cognitive development of the preterm toddlers. In contrast, we found that stable daily wake time was significantly associated with better cognitive development, suggesting that sleep regulation may contribute to the brain maturation of preterm toddlers

    A number of studies have been made on the sleep characteristics of children born preterm in an attempt to develop methods to address the sleep problems commonly observed among such children. However, the reported sleep characteristics from these studies vary depending on the observation methods used, i.e., actigraphy, polysomnography and questionnaire. In the current study, to obtain reliable data on the sleep characteristics of preterm-born children, we investigated the difference in sleep properties between 97 preterm and 97 term toddlers of approximately 1.5 years of age using actigraphy. Actigraphy units were attached to the toddlers’ waists with an adjustable elastic belt for 7 consecutive days, and a child sleep diary was completed by their parents. In the study, we found that preterm toddlers had more nocturnal awakenings and more daytime activity, suggesting that preterm-born children may have a different process of sleep development in their early development

    歩行条件に応じた歩行時間および歩数の認識について : 歩行時間と歩数の予測値は自己認識指標として有用か?

    本研究の目的は、歩行時間と歩数の予測値が自己認識指標として有用であるか否かについて検討することである. 健常成人17名を対象に、20mの直線歩行路を歩行するのに要する歩行時間と歩数の予測値および実測値を計測した. なお、歩行条件は、対象者が任意に設定した「自然歩行」、「遅い歩行」、「速い歩行」の3条件とした. その結果、歩行条件に応じて歩行時間と歩数を変化させなければならないことは自己認識できたものの、歩行時間と歩数を正確に認識することは困難であることが示された. したがって、20mの直線歩行路を歩行するのに要する歩行時間と歩数の予測値を自己認識指標として用いることは、信頼性と妥当性に欠き臨床応用することが困難であると考えられた.The purpose of this study was to determine if estimated walking time and number of steps can be a useful self-reported index for clinical application to improve the safety of individuals with gait difficulty. Seventeen healthy adults were asked to estimate the time and the number of steps needed to walk in a 20m straight line in three different walking conditions, "natural walking","slow walking"and "fast walking", and the estimated values were compared with the actual time and steps taken to walk the distance. The results suggest that, even though subjects were aware that the time taken and steps needed to walk the distance needed to be changed for each condition, they had difficulty estimating the values accurately. Because of tha lack of reliability and validity, the use of estimated walking time and number of steps needed to walk in a 20m straight line as a self-reported index does not appear to be useful for clinical application
