Steroid-eluting epicardial pacing in children
Background: Permanent cardiac pacing is a known method of the treatment for children
with bradycardia. Epicardial pacing is required in small children and in children with some
congenital heart defects. Steroid-eluting leads (SEL) have been introduced to reduce implant
site fibrosis and to retain permanent low-pacing threshold values. Our aim was to evaluate the
pacing characteristic and follow-up of children with epicardial steroid-eluting pacing.
Material and methods: We implanted steroid-eluting epicardial pacing systems in 53 children
(age at implantation: 2 days - 17.5 years, mean 4.6 years), of whom 37 (70%) had
congenital heart disease and 22 of these had already had cardiac surgery. These children
formed group I. Group II was the control group and consisted of 29 children (age at implantation:
10 days - 13 years, mean 6.5 years) with non-steroid epicardial pacing systems; 15 patients
(51%) had diagnosed congenital heart disease, and 9 of these had had cardiac surgery. The
pacing threshold (PT) was obtained during implantation, before discharge, 1-3 and 6 months
following implantation and then every 6 months.
Results: In group I the mean PT during the implantation procedure was 1.6 V/0.4 ms and
decreased significantly before discharge. In group II the PT during the implantation procedure
was low and increased before discharge. In group I the PT of the atrial leads was low and
stable during the follow-up period, while the PT of the ventricular leads slowly increased and
four years following implantation was similar to that of group II.
Conclusions: In children with permanent epicardial pacing the ventricular PT was significantly
lower when steroid-eluting leads had been used then when these had not been used, but
during the follow-up period the PT slowly increased, while the pacing threshold of atrial
steroid-eluting leads remained stable
Nagłe zatrzymanie krążenia kluczem do rozpoznania rodzinnie występującego zespołu wydłużonego QT typu 1 — czy musiało tak być?
Long QT syndrome (LQTS) is one of genetic arrhythmic disorders. It is featured by prolonged repolarization period reflected by prolongation of QT interval in surface electrocardiogram (ECG). It facilitates occurrence of life-threatening polymorphic ventricular tachycardia-torsade de pointes (TdP) and ventricular fibrillation (VF). Depending on duration of ventricular arrhythmia, it may manifest as presyncope, syncope or as sudden cardiac death in apparently healthy people. The incidence of LQTS is estimated at 1 to 2000 live births. Familial occurrence of the disease is an indication for diagnosing relatives potentially at risk. In this paper we present a family with LQT1 diagnosed after cardiac arrest in 16-year-old girl. At the age of eight the girl demonstrated syncope suggestive of their vaso-vagal etiology. Registering electrocardiogram that time might have helped in diagnosing life-threatening arrhythmia. Implementing proper therapy would had prevented the sudden cardiac arrest.Zespół wydłużonego QT (Long QT Syndrome - LQTs) zaliczany jest do genetycznie uwarunkowanych arytmogennych chorób serca. Charakteryzuje się wydłużonym okresem repolaryzacji komór, którego odzwierciedleniem jest wydłużenie odstępu QT w standardowym zapisie EKG. Sprzyja to wystąpieniu groźnych dla życia komorowych zaburzeń rytmu serca z typowym wielokształtnym częstoskurczem komorowym typu torsade de pointes (TdP), czy migotaniem komór. W zależności od czasu trwania arytmii, zespół LQT może objawiać się zasłabnięciami, omdleniami, może doprowadzić do nagłej śmierci sercowej u pozornie zdrowych młodych osób. Częstość występowania LQTs ocenia się na 1 do 2000 żywo urodzonych dzieci. Rodzinne występowanie zespołu wskazuje na konieczność wykonania badań diagnostycznych u potencjalnie zagrożonych krewnych pacjenta. W niniejszej pracy przedstawiamy rodzinę, u której rozpoznano i potwierdzono genetycznie zespół wydłużonego QT typu 1 po zatrzymaniu krążenia u 16-latki. W 8. roku życia dziewczynka demonstrowała omdlenia sugerujące ich charakter wazo-wagalny. Wykonanie zapisu elektrokardiograficznego w tym okresie być może przyczyniłoby się do wcześniejszego rozpoznania groźnego dla życia zespołu. Wdrożenie leczenia prawdopodobnie pozwoliłoby uniknąć nagłego zatrzymania krążenia
Andersen-Tawil syndrome: report of 3 novel mutations and high risk of symptomatic cardiac involvement.
IntroductionAndersen-Tawil syndrome (ATS) is a potassium channelopathy affecting cardiac and skeletal muscle. Periodic paralysis is a presenting symptom in some patients, whereas, in others, symptomatic arrhythmias or prolongation of QT in echocardiographic recordings will lead to diagnosis of ATS. Striking intrafamilial variability of expression of KCNJ2 mutations and rarity of the syndrome may lead to misdiagnosis.MethodsWe report 15 patients from 8 Polish families with ATS, including 3 with novel KCNJ2 mutations.ResultsAll patients had dysmorphic features; periodic paralysis affected males more frequently than females (80% vs. 20%), and most attacks were normokalemic. Two patients (with T75M and T309I mutations) had aborted sudden cardiac death. An implantable cardioverter-defibrillator was utilized in 40% of cases.ConclusionsKCNJ2 mutations cause a variable phenotype, with dysmorphic features seen in all patients studied, a high penetrance of periodic paralysis in males and ventricular arrhythmia with a risk of sudden cardiac death
Analiza widmowa zmienności rytmu zatokowego u dzieci zdrowych
Cel pracy: Ocena wartości analizy widmowej zmienności rytmu zatokowego u dzieci zdrowych.
Materiał i metody: Badaniami objęto 175 dzieci zdrowych w wieku 4–18 lat (śr. 11 ± 4): 88 chłopców i 87 dziewczynek. W zależności od wieku wyodrębniono 4 grupy pacjentów: grupa 1 — 4–7 lat (n = 27); grupa 2 — 7–11 lat (n = 64); grupa 3 — 11–15 lat (n = 58); grupa 4
— 15–18 lat (n = 26). Z 24-godzinnego zapisu EKG ocenie poddano 5-minutowe fragmenty z okresu czuwania i snu. Analizę częstotliwościową zmienności rytmu serca przeprowadzono za pomocą algorytmu szybkiej transformacji Fouriera (FFT). Wartości mocy widma uzyskano w następujących pasmach: ULF (0–0,0033 Hz), VLF (0,0033–0,04 Hz), LF (0,04–0,15 Hz), HF (0,15–0,4 Hz). Wartości wyznaczano w ms2, jako logarytm naturalny (ln) i w jednostkach znormalizowanych (nu). Oceniano również całkowitą moc widma (TP — bez składowej ULF) oraz iloraz wartości LF/HF.
Wyniki: Nie stwierdzono znamiennych różnic między grupami wiekowymi w wartościach TP i VLF w ciągu dnia i podczas snu oraz LF i LF/HF w czasie snu. W czasie czuwania wartości LF (ms˛ i ln) były istotnie większe w grupie 4 niż w grupach 1 i 2, a LF (nu) — istotnie większe w grupach 3 i 4 w porównaniu z grupą 1. Wartości HF (nu) były znamiennie większe w grupie 1 niż w grupie 4, zarówno w czasie czuwania jak i w nocy. Iloraz LF/HF był istotnie większy w grupie 4 niż w grupach 1 i 2 (w czasie czuwania).

Risk factors for cardiac arrhythmias in children with congenital heart disease after surgical intervention in the early postoperative period
ObjectiveEarly postoperative arrhythmias are a recognized complication of pediatric cardiac surgery.MethodsDiagnosis and treatment of early postoperative arrhythmias were prospectively analyzed in 402 consecutive patients aged 1 day to 18 years (mean 29.5 months) who underwent operation between January and December 2005 at our institute. All children were admitted to the intensive care unit, and continuous electrocardiogram monitoring was performed. Risk factors, such as age, weight, Aristotle Basic Score, cardiopulmonary bypass time, aortic crossclamp time, and use of deep hypothermia and circulatory arrest, were compared. Statistical analysis using the Student t test, Mann–Whitney U test, or Fisher exact test was performed. Multivariate stepwise logistic regression was used to assess the risk factors of postoperative arrhythmias.ResultsArrhythmias occurred in 57 of 402 patients (14.2%). The most common types of arrhythmia were junctional ectopic tachycardia (21), supraventricular tachycardia (15), and arteriovenous block (6). Risk factors for arrhythmias, such as lower age (P = .0041⁎), lower body weight (P = .000001⁎), higher Aristotle Basic Score (P = .000001⁎), longer cardiopulmonary bypass time (P = .000001⁎), aortic crossclamp time (P = .000001⁎), and use of deep hypothermia and circulatory arrest (P = .0188⁎), were identified in a univariate analysis. In the multivariate stepwise logistic regression, only higher Aristotle Basic Score was statistically significant (P = .000003⁎) compared with weight (P = .62) and age (P = .40); in the cardiopulmonary bypass group, only longer aortic crossclamp time was statistically significant (P = .007⁎).ConclusionLower age, lower body weight, higher Aristotle Basic Score, longer cardiopulmonary bypass time, aortic crossclamp time, and use of deep hypothermia and circulatory arrest are the risk factors for postoperative arrhythmias. Junctional ectopic tachycardia and supraventricular tachycardia were the most common postoperative arrhythmias
