    Study of Non-Appropriate Medications Among Elderly Patients in Hospitals Affiliated with IUMS Using The STOPP Screening Tool in 2013

    Background: The use of inappropriate medications in the elderly is an important issue in the health care system. This issue can increase the side effects and the costs. This study aimed to investigate the prescribing of inappropriate medications in elderly patients. Materials & Methods: In this cross- sectional study, 400 elderly people were selected by simple random sampling at four teaching and non- teaching in hospitals affiliated with IUMS during six months of the 2013 year. The data was collected from the profiles of hospitalization of patients and check lists of STOPP criteria and was analyzed using the chi- square test, Mann- Whitney, Spearman correlation coefficients and Kolmogorov- Smirnov. Results: The mean age of the elderly was 73.4 year. The total number of prescribed drugs for studied patients was 4744 of which at least 124(31.1 Percent) inappropriate medication was prescribed for a patient. According to STOPP criteria, most inappropriate prescriptions were in the non- teaching hospitals related to drug classes Benzodiazepine (13.1 Percent) and Long-acting metabolites (6.2 Percent) and the teaching hospitals Benzodiazepine (9 Percent) and Anti-diabetic (2.7 Percent).Prescribed inappropriate drugs have had a significant relationship with the type of hospitalization section and the length of patients’ residence in the teaching hospitals while the number of inappropriate drugs prescribed was increased as the number of diseases of the elderly in non- teaching hospitals was increased significantly. Discussion: The rate of prescribing inappropriate medications among nursing was not considered in good standing. It seems that the introduction of screening tools can improve the accuracy of medication administration and physician partners and in addition to improving the quality of prescribing in the elderly it is required to design a comprehensive instruction for the country

    Introduction: New organization’s success depends on the efficient use of human resources In this study, we aimed to estimate the staffing needs in hospitals of Shiraz University of Medical Sciences (SUMS) according to model proposed by the Ministry of Health and Medical Education in 2012. Method: This is a cross-sectional definitive study conducted in three general and six specialized SUMS hospitals. The research tool used was a checklist that determines the number of nurses, paraclinic and service employees and finally the decrease and increase of human resources in the departments of the hospitals regarding Iranian Ministry of Health (MOH) issues. The data were collected and analyzed using SPSS software to determine the differences between the current situation in accordance to MOH issues. Results: Results showed that of the nine teaching hospitals of SUMs in 2012, Namazi hospital had 288 redundant staff and Khalili hospital had a shortage of manpower in 8 places . We observeda deficiency in human resources in all the studied hospitals. Also, the distribution of human resources among most of the hospital departments was not conform with MOH issues. Conclusion: Various models have been proposed for estimating human resources of hospitals. Because of better ergometer and time to estimate the correct manpower, the model introduced by the Department of Health is suitable for planning to increase the efficiency and effectiveness of the hospitals

    Human Resources Distribution Among Tehran University Of Medical Sciences Hospitals

    Background and Aim: Today, healthcare system is exposed to inappropriate human resources distribution challenges in all over the world. So far there is not an appropriate policy for human resources distribution in Iran. This deficiency may cause several problems for providing healthcare services. This research was aimed to determine the situation of human resources distribution among Tehran University Of Medical Sciences (TUMS) Hospitals. Materials and Methods: This is a cross-sectional definitive study induced in four general and special TUMS hospitals. The research tool was a checklist that determine the number of nurses, paraclinic and supportive employees and finally the decrease and increased of the human resources among the departments of the hospitals regarding Iranian Ministry of Health (MOH) issues. The data was collected and analyzed by SPSS software and determined the differences between current situation in accordance to MOH issues. Results: We observed the deficiency of human resources among all studied hospitals. Also the distribution of human resources among most of the hospitals departments was not coordinated with MOH issues. Conclusion : It seems the distribution of human resources among Iranian healthcare system is not followed by a special model. Therefore, we suggest the model of health human resources planning should be determined and related by information, providers, services, education and policy as healthcare system factors and overlapping of these factors
