    Evaluation of Max Pereira alar reconstruction technique modification in the total nasal reconstruction protocol of the Hospital of Clinics of Porto Alegre

    Introduction: Nasal reconstruction is the oldest plastic surgery technique. The nasal anatomy is complex and requires an association of techniques for the restoration of function and adequate nasal esthetics. Pereira et al. described a technique that allows total nasal reconstruction of the alar cartilage through the use of an auricular cartilage graft, with minimal deformity secondary to the donor site. The objective of the present study is to present a modification, by Collares et al., of the technique described above, which allows the reconstruction of another anatomical region of the nose without increasing morbidity, and its insertion into the total nasal reconstruction protocol of Hospital de Clínicas of Porto Alegre. Methods: A retrospective study was conducted. We evaluated technique modification in 10 patients who underwent total nasal reconstructions. Results: After examining the 10 patients who were treated with the modified total nasal reconstruction protocol at the Hospital de Clínicas of Porto Alegre, we observed an improvement in the nose shape and internal nasal valve with preservation of function, without sequelae secondary to auricular graft removal. Conclusion: In this case series, the modification of the Max Pereira technique resulted in adequate aesthetic-functional treatment when implemented in the total nasal reconstruction protocol of the Hospital de Clínicas of Porto Alegre, without increasing the morbidity in the donor area

    Expressão de CD45+ na medula óssea e sangue periférico de cães com linfoma tratados com alta dose de ciclofosfamida suportada por transplante autólogo de medula óssea

    O presente estudo teve como objetivos promover a avaliação seqüencial e temporal do quadro leucocitário de cães portadores de linfoma, em remissão, submetidos à alta dose de ciclofosfamida seguida do transplante autólogo de medula óssea, bem como quantificar células CD45+ na medula óssea e sangue periférico dos referidos cães. O transplante consistiu de colheita de MO, preparo e congelamento da bolsa que continha células medulares em suspensão, condicionamento do paciente com 500mg/m2 de ciclofosfamida, infusão das células hematopoéticas e aplicação de fator estimulador de colônia. Para avaliar a recuperação hematopoética pós-transplante, realizaram-se leucograma e análise citométrica do sangue e medula óssea, por 28 dias após o transplante. O nadir médio dos neutrófilos segmentados foi 513 células/mL, e ocorreu 3 a 7 dias pós- TMO. A duração média da neutropenia foi de 3,2 dias. As mais elevadas contagens de leucócitos, para animais do grupo tratado que receberam G-CSF, foram observadas no último dia de sua aplicação, exceto para um animal. Nenhum animal apresentou febre ou sepse após a alta dose de ciclofosfamida. Desta maneira, podese comprovar o fato de que o uso do filgrastim leva a reduções significativas na incidência, severidade e duração da neutropenia. O aumento na quantidade de células CD45+ na medula óssea de cães tratados deveu-se aos efeitos combinados do autotransplante de células hematopoéticas e à ação do G-CSF.The present study has the goal to promote sequential and timing evaluation of leucocytes of dogs bearers of lymphoma, in remission, submitted to the high dose of cyclophosphamide followed by the autologous bone marrow transplant, as well as to quantify CD45+ cells in the bone marrow and blood of the abovementioned dogs. The transplant consisted of MO collect, preparation and freezing of the bag that contained medullar cells, conditioning of the patient with 500mg/m2 of cyclophosphamide, infusion of the hematopoietics cells and application of colonystimulating factor. In order to evaluate post-transplant hematopoietic recovery, leucogram and cytometric analysis of the blood and bone marrow were carried out for 28 days after the transplantation. The medium nadir of the segmented neutrophils was 513 cells/mL, and it occurred / took place 3 to 7 days after TMO. The medium duration of the neutropenia was of 3,2 days. The highest counts of leucocytes, to animals of the treated group that received G-CSF, could be observed on the last day of its application, except for one animal. None of the animals presented either fever or sepse after the high dose of cyclophosphamide. Thus, it can be verified that the use of filgrastim leads to significant reductions on the incidence, severeness and duration of the neutropenia.     Considering the relevance of canine lymphoma in veterinary medicine and the short survival achieved with chemotherapy protocols employed, new treatment options are being studied. In the last decades, various studies have been conducted to correlate the development and progression of certain types of cancer with the overproduction of prostaglandins in tumor sites and inflammatory cells, due to increased production of COX-2, leading to cell proliferation, the suppression of apoptosis , the induction of angiogenesis and metastasis, these actions that count toward their oncogenic function. One strategy for reducing the levels of PGE2 is the use of COX-2 in anticancer treatment. In this study 20 dogs with diagnosis of multicentric lymphoma, divided in two groups were evaluated. Animals in group 1 were treated with the standard Madison-Wisconsin chemotherapy protocol, and group 2, with this protocol modified by the replacement of prednisone for a selective inhibitor of COX-2. Before the first (T1) and fifth (T2) chemo, blood samples and measurement of plasma PGE2 by ELISA were performed. We observed a higher number of female animals, Rottweiler breeds and mixed breed, middle age animals, who presented with greater frequency, multicentric lymphoma in stage IVa, B cell origin, and diffuse large cell. Leukocytosis with neutrophilia or lymphocytosis was observed for some animals at the moment of diagnosis, and after the first cycle of chemotherapy, seven cases of lymphopenia. Dogs with clinical signs of remission after the first cycle of chemotherapy show lower plasma PGE2 when compared with those without remission. The association of chemotherapy treatment with a COX-2 was able to reduce concentrations of PGE2 in 50% of patients, while only 10% of those in group 1 decreased. However, it was not possible to confirme the role of PGE2 as a marker of prognosis and survival time for dogs with lymphomaConsiderando a relevância do linfoma canino na rotina veterinária e o curto período de sobrevida alcançado com os protocolos quimioterápicos empregados, novas alternativas de tratamento estão sendo estudadas. Nas últimas décadas, diversos estudos foram realizados correlacionando o desenvolvimento e progressão de alguns tipos de neoplasias com a superprodução de prostaglandinas nas células tumorais e inflamatórias locais, em consequência à maior produção de COX-2, que leva à proliferação celular, à supressão da apoptose, à indução da angiogênese e da metástase, ações estas que contam para sua função oncogênica. Uma das estratégias para a redução das concentrações de PGE2 é a utilização de inibidores de COX-2 no tratamento antineoplásico. No presente estudo foram avaliados 20 cães com diagnóstico de linfoma multicêntrico, divididos em dois grupos. Os animais do grupo 1 foram tratados com o protocolo quimioterápico de Madson-Wisconsin convencional, e os do grupo 2, com o protocolo modificado pela substituição da prednisona por um inibidor seletivo de COX-2. Antes da primeira (T1) e da quinta (T2) sessão de quimioterapia, foram realizados hemograma e dosagem de PGE2 plasmática por ELISA. Observou-se um maior número de animais fêmeas, das raças Rottweiler e sem raça definida, de meia idade, que apresentaram com maior freqüência, o linfoma multicêntrico em estádio IVa, de origem celular B, e difuso de grandes células. No leucograma, notou-se em alguns animais, no momento do diagnóstico, a presença de leucocitose por neutrofilia ou por linfocitose, e após o primeiro ciclo de quimioterapia, sete casos de linfopenia. Os cães com remissão dos sinais clínicos após o primeiro ciclo de quimioterapia, apresentam menores concentrações de PGE2 plasmática quando comparados com aqueles sem remissão.     O linfoma é uma neoplasia de elevada incidência na população canina mundial, e que apresenta sinais clínicos diversos, dependentes da classificação anatômica e da extensão da doença. Este estudo objetivou avaliar as características clínicas e hematológicas de cães com linfoma no momento do diagnóstico, além da evolução clínica dos mesmos ao longo do tratamento com o protocolo de Madison-Wisconsin. Dos 18 cães inseridos no estudo, 50% apresentaram a forma multicêntrica da doença e 33% a forma cutânea. A manifestação clínica mais comum foi a linfadenomegalia superficial, acompanhada dos sinais sistêmicos de hiporexia, apatia e perda de peso. As principais alterações hematólogicas foram anemia normocítica normocrômica, trombocitopenia e leucocitose, associadas às síndromes paraneoplásicas. Vinte e sete por cento dos animais atingiram remissão completa da doença e apenas 22% atingiram sobrevida de um ano. Houve correlação positiva entre o valor do hematócrito e o tempo de sobrevida para os cães que morreram, demonstrando a influência da anemia no prognóstico dos animais. Sugere-se que o estágio avançado da doença no momento do diagnóstico tenha influenciado as baixas taxas de remissão e sobrevida obtidas neste estudo.Lymphoma is a neoplasm of high incidence in dogs, and has several clinical signs, depending on the tumor anatomical area and the extent of the disease. This study aimed to evaluate the clinical and hematological findings in dogs with lymphoma at diagnosis, and the clinical evolution during Madison-Wisconsin chemotherapy protocol. of the 18 dogs that underwent the study, 50% of the dogs had the multicentric form and 33% the cutaneous forms of the disease. The most common clinical sign was superficial lymphadenomegaly, combined to systemic signs of hyporexia, apathy and weight loss. The main hematological changes were anemia, thrombocytopenia and leukocytosis, associated with paraneoplastic syndromes. Complete remission was achieved in 27% of the animals and the one-year survival rate was 22%. There was a positive correlation between the hematocrit and survival time in dogs that died, which indicates the importance of anemia in the prognosis.     Autologous bone marrow transplantation in a dog with lymphoma: a clinical study Transplante autólogo de medula óssea em um cão com linfoma: ensaio clínico

    The objective of this study is to provide the first report of bone marrow transplantation (BMT) in dogs in Brazil. A Rottweiler with cutaneous lymphoma was submitted to a twelve-week Madison-Wisconsin chemotherapy protocol followed by autologous bone marrow transplantation. For this, 10mL kg-1 of bone marrow was collected simultaneously from both iliac crests and cryopreserved in a freezer at -80°C. The conditioning step was performed by administering cyclophosphamide by intravenous route at 400mg m-2. Bone marrow was reinfused after defrosting in a water bath at 37°C. Bone marrow nucleated cell counts before and after freezing, showed a small relative loss of nucleated cells (35.10 and 31.80x10³µL-1 , respectively). Cyclophosphamide induced neutropenia which was reverted by a granulocyte colony-stimulating factor (G-CSF) capable of stimulating hematopoetic reconstitution. On the day 360 after transplant the patient was found to be in complete remission. This study indicates that autologous BMT in a dog with lymphoma submitted to myelosuppressive chemotherapy was potentially safe and effective.Este estudo teve como objetivo descrever o primeiro relato de transplante de medula óssea (TMO) em cães no Brasil.     NEOTROPICAL CARNIVORES: a data set on carnivore distribution in the Neotropics

    Mammalian carnivores are considered a key group in maintaining ecological health and can indicate potential ecological integrity in landscapes where they occur. Carnivores also hold high conservation value and their habitat requirements can guide management and conservation plans. The order Carnivora has 84 species from 8 families in the Neotropical region: Canidae; Felidae; Mephitidae; Mustelidae; Otariidae; Phocidae; Procyonidae; and Ursidae. Herein, we include published and unpublished data on native terrestrial Neotropical carnivores (Canidae; Felidae; Mephitidae; Mustelidae; Procyonidae; and Ursidae). NEOTROPICAL CARNIVORES is a publicly available data set that includes 99,605 data entries from 35,511 unique georeferenced coordinates. Detection/non-detection and quantitative data were obtained from 1818 to 2018 by researchers, governmental agencies, non-governmental organizations, and private consultants. Data were collected using several methods including camera trapping, museum collections, roadkill, line transect, and opportunistic records. Literature (peer-reviewed and grey literature) from Portuguese, Spanish and English were incorporated in this compilation. Most of the data set consists of detection data entries (n = 79,343; 79.7%) but also includes non-detection data (n = 20,262; 20.3%). Of those, 43.3% also include count data (n = 43,151). The information available in NEOTROPICAL CARNIVORES will contribute to macroecological, ecological, and conservation questions in multiple spatio-temporal perspectives. As carnivores play key roles in trophic interactions, a better understanding of their distribution and habitat requirements are essential to establish conservation management plans and safeguard the future ecological health of Neotropical ecosystems. Our data paper, combined with other large-scale data sets, has great potential to clarify species distribution and related ecological processes within the Neotropics. There are no copyright restrictions and no restriction for using data from this data paper, as long as the data paper is cited as the source of the information used.
