    Case report : Jejunal diverticulosis with chronic interstitial and mesenteric adhesions, chronic mesenteric volvulus, and decompensated small-bowel obstruction

    Publisher Copyright: © 2024 The AuthorsIntroduction: Jejunal diverticulosis has not gained significant attention because of its rarity and typically asymptomatic course as well as the relative diagnostic inaccessibility of the jejunum. This report presents a rare case of jejunal diverticulosis complicated with chronic interstitial and mesenteric adhesions, chronic mesenteric volvulus, and decompensated small-bowel obstruction. Presentation of case: An 84-year-old man was admitted to the emergency room with a 24-h history of acute colicky abdominal pain. He denied other signs or symptoms. The preoperative diagnosis based on physical and radiologic evaluations was challenging, and only diagnostic laparoscopy revealed extensive small-bowel diverticulosis. Midline laparotomy was performed as definitive surgery, revealing diverticulosis in the proximal 2-m section of the jejunum, starting approximately 20 cm from Treitz's ligament; the affected section was resected. The postoperative recovery was excellent. The histopathologic report confirmed substantial jejunal diverticulosis with chronic fibrosis, adhesions, and strictures. Discussion: Histopathologic evaluation is necessary because tumors can be misdiagnosed as diverticula. This case report should serve as a reminder for surgeons to be cognizant of the signs of uncommon conditions, such as jejunal diverticulosis. Conclusion: Albeit rare, jejunal diverticulosis should be considered in the differential diagnosis of acute abdomen.Peer reviewe

    What Does the Uniting for Peace Resolution Mean for the Role of the UN Security Council?

    The Uniting for Peace resolution together with the UN Charter prescribes a certain role for the General Assembly with regard to international peace and security. Larry Johnson addresses that role, but he does not consider a second question: how does the Uniting for Peace resolution affect the UN Security Council? The normative role of the Council is influenced not only by the Charter, but also by general international law. In this comment, I explore the normative role of the Council in fulfilling the Charter’s purpose to maintain international peace and security. I argue that the text of the Charter and the prior practice of both the Assembly and the Council help to determine the proper division of these organs’ respective tasks within the Charter system. I conclude that the Council alone exercises the constant control needed to enforce measures of collective security effectively, and that the Assembly is limited to recommending the consequences for states when threats or breaches of the peace occur.</jats:p

    The history and development of St. Anthony\u27s College and San Vicente Institute: two schools managed by the Franciscan Apostolic Sisters (FAS)

    This study traces the historical background of St. Anthony\u27s College (SAC) and San Vicente Institute (SVI), two schools managed by the Franciscan Apostolic Sisters (FAS). The researcher employed the historical and descriptive survey types of research. The principal instrument used was a questionnaire supplemented by the interview, observation and documentary analysis. Sources of data were the responses of Father Gerard Filippetto, O.F.M. and Father Felicisimo Herrera, founders of the two schools, responses of past and present administrators, graduates and selected people in the community to the questionnaire given to them. Data was treated statistically using percentages, simple frequency count and rank order. From the findings of the study the following conclusions were drawn: 1) St. Anthony\u27s College and San Vicente Institute were established by the two priests mainly for the welfare of the people in terms of strengthening the Catholic faith in their respective communities and in providing Catholic education to the people at the least expense. 2) SAC and SVI have undertaken long, hard and gradual process of development. 3) The strengths and weaknesses of SAC and SVI are different from each other. SAC has its strength in her sufficient facilities, equipment, textbooks and library materials while SVI has its strength in the administrative staff. SAC is weak in her poor faculty line-up while SVI has inadequate facilities, equipment and library holdings and also in having a poor faculty line-up. 4) A great majority of the graduates of SAC and SVI contributed in one way or another, to the religious educational, economic, social and cultural development of the communities of Sta. Ana and Solana, Cagayan. 5) It appeared that SAC, a congregational school has an advantage over SVI which is a diocesan school. 6) The Franciscan Apostolic Sisters have contributed in their own little way to the growth and development of SAC and SVI

    Study of Patients with First unprovoked Seizure in the Eastern Part of Nepal

    Background: About 5-10% of the population get at least one seizure in their lifetime. Treatment is started in patients with first unprovoked seizure if the risk of seizure recurrence is predicted to be high. If patients with first seizure are not treated 40-50% of patients develop recurrence within 2 years of the initial seizure. Starting treatment may cause reduction in the risk of recurrence by almost one half. The aim of the study was to identify the factors causing recurrence in patients with first unprovoked seizure.&#x0D; Materials and Methods: It is a prospective cross-sectional study conducted at Nobel Medical College from March 2015 to March 2019. Patients who presented to Nobel Medical College with first unprovoked seizure were enrolled in the study with follow up during the hospital visit.&#x0D; Results: Eighty six patients participated in our study. Recurrence of seizure occurred in 21(24.4%) patients within the study period of 4 years. Abnormal Electroencephalography was significantly associated with recurrence of seizure in patients with first seizure. (P value&lt;0.001) Neuro imaging abnormality was also associated with increased risk of seizure recurrence (Pvalue&lt;0.001) .Starting an antiepileptic after first seizure reduced the risk of further seizure.&#x0D; Conclusion: Recurrence of seizure was observed in almost a quarter of patients within the study period of four years. Recurrence risk was higher in those patients with abnormal Electro encephalography and in those patients with identified cause than those patients whose seizure was assumed to be idiopathic.</jats:p

    The Effects of Nicotine Pouches and E-Cigarettes on Oral Microbes: A Pilot Study

    It remains uncertain whether nicotine pouches and electronic cigarettes alter the oral environment and result in a high presence of periodontopathogenic bacteria in saliva, compared to that among cigarette users or non-tobacco users. In this study, saliva samples were collected from respondents using nicotine pouches, electronic cigarettes, and conventional cigarettes, alongside a control group of non-tobacco users. Polymerase chain reaction was used to identify clinical isolates of the following periodontal bacteria: Porphyromonas gingivalis, Tannerella forsythia, Prevotella intermedia, Fusobacterium nucleatum, Fusobacterium periodonticum, Porphyromonas endodontalis, and Rothia mucilaginosa. The presence of some periodontal pathogens was detected in the saliva samples from users of nicotine pouches, electronic cigarettes, and conventional cigarettes but not in samples taken from the control group. Therefore, the initial results of this pilot study suggest that the presence of periodontopathogenic bacteria in the saliva of nicotine pouch and electronic cigarette users could alter the oral microbiome, leading to periodontal diseases.
