Conservative surgical treatment in cervical dysplastic lesions associated with cystorectocele

    Get PDF
The treatment of cervical intraepithelial neoplasia aims at removing the squamocolumnar junction area, including abnormal tissues, up to the healthy tissue. Old postpartum perineal tears associated with cystorectocele, hypertrophic cervical elongation, and first and second degree uterine prolapse are pelvic static disorders. Particular aspects of pelvic-genital static disorder are related to the vulnerability of the pelvic-perineal floor during birth. On the occasion of birth, especially when there are different forms of dystocia during labor, the degradation of soft pelvic, genital, and vaginal-perineal tissues can occur. The objective of this study was to re-evaluate the conservative treatment of cervical dysplasia, depending on the degree of cervical lesions, the surface extension, the age of the patients, and the pathology associated with cervical lesions – colpocele, cystorectocele, and urinary incontinence in the old postpartum perineal tears. In order to solve the three types of concomitant lesions, we used the Manchester operation: the anterior colporrhaphy with the recalibration of the urethra and the suspension of the cystocele, the minimal colpectomy, the lesional cervical amputation with the anterior fixation of the parameters, and the posterior colpoperineorrhaphy with high myorrhaphy of the levator ani muscles. The decision on the management of cervical dysplasia has taken into account the degree of cervical lesions, the extension on the surface, the patients’ age and the pregnancy planning, and the pathology associated with cervical lesions. The Manchester operation is a conservative surgical procedure, effective in women under 45 years old, multiparous, with present genital activity, with dysplastic cervical lesions and cystorectocele. It also solves cystorectocele, cervical dysplasia, hypertrophic cervical elongation, and first and second degree uterine prolapse

Unsupervised Adversarial Depth Estimation using Cycled Generative Networks

While recent deep monocular depth estimation approaches based on supervised regression have achieved remarkable performance, costly ground truth annotations are required during training. To cope with this issue, in this paper we present a novel unsupervised deep learning approach for predicting depth maps and show that the depth estimation task can be effectively tackled within an adversarial learning framework. Specifically, we propose a deep generative network that learns to predict the correspondence field i.e. the disparity map between two image views in a calibrated stereo camera setting. The proposed architecture consists of two generative sub-networks jointly trained with adversarial learning for reconstructing the disparity map and organized in a cycle such as to provide mutual constraints and supervision to each other. Extensive experiments on the publicly available datasets KITTI and Cityscapes demonstrate the effectiveness of the proposed model and competitive results with state of the art methods. The code and trained model are available on https://github.com/andrea-pilzer/unsup-stereo-depthGAN.Comment: To appear in 3DV 2018. Code is available on GitHu

INTERNAL CONTROL MECHANISMS IN THE CONTEXT OF THE CHALENGE FINANCIAL CRISIS

Prevention and earlier detection of fraudulent financial reporting must start with the entity that prepares financial reports. Thus the first focus of the Sarbannes-Oxley Actâ€(tm)s recommendations is the public company. These recommendations, taken together, will improve a company's overall financial reporting process and increase the likelihood of preventing fraudulent financial reporting and detecting it earlier when it occurs. For some companies, implementing these recommendations will require little or even no change from current practices; for other companies, it will mean adding or improving a recommended practice. Whether it means adding or improving a practice, the benefits justify the costs. The Sarbanes-Oxley Act is a direct response to the recent scandals in the US corporate world. Governance, compliance, risks and internal controls are mounting concerns for almost all organizations. As the numbers of rules, regulations and contractual obligations steadily rise, management is growing more and more concerned about their exposure on day-to-day operational decisions!fraud, internal control, COSO, risks based audit

Connections between Some Concepts of Polynomial Trichotomy for Noninvertible Evolution Operators in Banach Spaces

    Get PDF
The present paper treats three concepts of nonuniform polynomial trichotomies for noninvertible evolution operators acting on Banach spaces. The connections between these concepts are established through numerous examples and counterexamples for systems defined on the Banach space of square-summable sequences

Technical Aspects Regarding the Preservation of Dry Onions in Different Storage Conditions

    Get PDF
Research refers to the ability to maintain the quality of dry onions in different conditions of temperature, the three varieties used in experimentation (De Buzau, Daytona and Countach) being stored after proper preparation at ambient temperature (+20…+22°C), refrigerated (+10…+12°C) and cold conditions (+3…+5°C). Storage life, the level of weight (mass) and decay losses and evolution of some chemical components determined from the 9 variants led to the conclusion that the best results were obtained by De Buzau variety for storage under ambient conditions and Daytona variety for storage under refrigerated and cold conditions. Moreover large differences between varieties and their behavior depending on storage conditions require choosing resistant cultivars and optimum storage temperatures according to destination and period of marketing or consumption

Mandatory multidisciplinary approach for the evaluation of the lymph node status in rectal cancer

    Get PDF
Colorectal cancer is the third most frequently reported malignancy and also the third leading cancer-related cause of death worldwide. Lymph node evaluation, both preoperatively and postoperatively, represents an important aspect of the diagnosis and therapeutic strategy in colorectal cancer, such that an accurate preoperative staging is required for a correct therapeutic strategy. Treatment of rectal cancer with positive lymph nodes, a very important predictive prognostic parameter, is currently based on neoadjuvant chemoradiotherapy followed by total/ surgical mesorectal excision and adjuvant regimen. Preoperative evaluation of the lymph node status in rectal cancer is based on endoscopic ultrasound and magnetic resonance imaging, but their accuracy, specificity, and sensitivity still require improvement. Postoperative evaluation also presents points of debate, especially related to the role of sentinel lymph node mapping and their final implication, represented by detection of micrometastases and isolated tumor cells. The pathologic interpretation of tumor deposits represents other points in discussion. From a surgical perspective, extended lateral lymph node dissection vs. abstinence and (neo)adjuvant therapeutic approach represent another unresolved issue. This review presents the major controversies existing today in the treatment and pathologic interpretation of the lymph nodes in rectal cancer, the role/ indication and value of the lateral pelvic lymph node dissection, and the postoperative interpretation of the value of the micrometastatic disease and tumor deposits

Corruption and Organised Crime Signalling Indicators for Foreign Investors as Applied to Eastern European Countries

    Get PDF
The former communist countries in Central and Eastern Europe and, implicitly, their economies, went through specific and sometimes asymmetric developments since the fall of the “iron curtain” at the end of the 1980s. During those years, they faced many challenges to create a secure and predictable economic environment for individuals and businesses, local or foreign. We propose a framework that considers legal, social, and economic indicators which can be used by foreign investors to make wise and efficient business decisions. Our main purpose is to expand the traditional economic analysis framework and enrich it with new institutional and societal indicators from the social, quality of life and legal layers of research. This new landscape could offer relevant information on vulnerabilities that could favour organized crime and corruption in some Central and Eastern European countries, a factor that foreign investors may need to address before deciding to invest in the region. Keywords: foreign direct investments, organised crime, corruption, economic vulnerabilities, GD
