    L'explication du diabète dans une séance d'éducation thérapeutique : une activité discursive contribuant au développement professionnel des soignants

    Drawing on sociocultural psychology, the clinic of activity and discourse analysis, this study analysed a session of health education that brought together a nurse, Inès, and an adolescent living with diabetes. It aimed, first, at investigating the mechanisms of shared understanding and reciprocity involved in explanations, second, at bringing about professional development. It analysed an explanation sequence and in a second step used a method that confronted the nurse to her activity (method of simple and cross self-confrontation). Results showed that in the session of health education, medical discourse tended to dominate, and created a strong asymmetry between the participants, thus raising doubts about the patient’s comprehension. Whereas in the simple self-confrontation Inès’ comments focused on the patient’s comprehension and emotional states, in the cross self-confrontation, her colleague’s comments led her to question her own emotional states and interpretations. The method of self-confrontation seems then adequate to capture the psychological mechanisms underlying the explanations provided by practitioners and to foster a form of professional development

    A systematic review of glomerular hyperfiltration assessment and definition in the medical literature.

    BACKGROUND AND OBJECTIVES: Evaluation of glomerular hyperfiltration (GH) is difficult; the variable reported definitions impede comparisons between studies. A clear and universal definition of GH would help in comparing results of trials aimed at reducing GH. This study assessed how GH is measured and defined in the literature. DESIGN, SETTING, PARTICIPANTS, &amp; MEASUREMENTS: Three databases (Embase, MEDLINE, CINAHL) were systematically searched using the terms "hyperfiltration" or "glomerular hyperfiltration". All studies reporting a GH threshold or studying the effect of a high GFR in a continuous manner against another outcome of interest were included. RESULTS: The literature search was performed from November 2012 to February 2013 and updated in August 2014. From 2013 retrieved studies, 405 studies were included. Threshold use to define GH was reported in 55.6% of studies. Of these, 88.4% used a single threshold and 11.6% used numerous thresholds adapted to participant sex or age. In 29.8% of the studies, the choice of a GH threshold was not based on a control group or literature references. After 2004, the use of GH threshold use increased (P&lt;0.001), but the use of a control group to precisely define that GH threshold decreased significantly (P&lt;0.001); the threshold did not differ among pediatric, adult, or mixed-age studies. The GH threshold ranged from 90.7 to 175 ml/min per 1.73 m(2) (median, 135 ml/min per 1.73 m(2)). CONCLUSION: Thirty percent of studies did not justify the choice of threshold values. The decrease of GFR in the elderly was rarely considered in defining GH. From a methodologic point of view, an age- and sex-matched control group should be used to define a GH threshold

    Physical fitness and sport activity of children and adolescents: methodological aspects of a regional survey

    Measurement of physical fitness and physical activity in children and adolescents raise a lot of methodological issues, explaining the scarcity of surveys in European countries and in Switzerland. This article exposes the design and the methods used in a survey on physical fitness, physical activity and health conducted in a region of Switzerland, and discuss the choice of the instruments and the quality control procedure selected to measure physical activity and physical fitness. The survey was conducted in a sample of 3540 students 9-19 years-old and included a battery of physical fitness tests, anthropometrics measurements and a self-report questionnaire on physical activity, sports activity and life styles. An ancillary study in a sub sample assessed daily physical activity with a pedometer, dietary intake with a 3-day dietary record, serum lipids and nutritional status. Some results are displayed as example. Quality control techniques are exposed and the choice of the instrument to assess physical fitness, physical activity, sports, and dietary intake are discussed. Local reference tables are now available for fitness tests and the practicability of fitness testing has been demonstrated in physical education. The research process has induced the sensitisation of schools toward health promotion through physical activity
