Right heart failure: toward a common language
Abstract In this guideline, the International Right Heart Foundation Working Group moves a step forward to develop a common language to describe the development and defects that exemplify the common syndrome of right heart failure. We first propose fundamental definitions of the distinctive components of the right heart circulation and provide consensus on a universal definition of right heart failure. These definitions will form the foundation for describing a uniform nomenclature for right heart circulatory failure with a view to foster collaborative research initiatives and conjoint education in an effort to provide insight into mechanisms of disease unique to the right heart
The role of emergency department HIV care in resource-poor settings: lessons learned in western Kenya
The human immunodeficiency virus (HIV) pandemic in sub-Saharan Africa and other high prevalence regions continues to overwhelm health care systems. While there has been a global response to improve the delivery of antiretroviral therapy in these high prevalence regions, there are few models that have developed an adequate plan to deal with HIV specifically in resource-poor emergency department settings. In this manuscript, we report on the experience scaling up HIV care at one emergency department in a large referral hospital located in western Kenya. Specifically, we describe how rapid bedside HIV testing helps to narrow the differential diagnosis of disease processes in acute care patients and how HIV screening of patients discharged from the emergency department can detect HIV-infected individuals

Free Energy of an Inhomogeneous Superconductor: a Wave Function Approach
A new method for calculating the free energy of an inhomogeneous
superconductor is presented. This method is based on the quasiclassical limit
(or Andreev approximation) of the Bogoliubov-de Gennes (or wave function)
formulation of the theory of weakly coupled superconductors. The method is
applicable to any pure bulk superconductor described by a pair potential with
arbitrary spatial dependence, in the presence of supercurrents and external
magnetic field. We find that both the local density of states and the free
energy density of an inhomogeneous superconductor can be expressed in terms of
the diagonal resolvent of the corresponding Andreev Hamiltonian, resolvent
which obeys the so-called Gelfand-Dikii equation. Also, the connection between
the well known Eilenberger equation for the quasiclassical Green's function and
the less known Gelfand-Dikii equation for the diagonal resolvent of the Andreev
Hamiltonian is established. These results are used to construct a general
algorithm for calculating the (gauge invariant) gradient expansion of the free
energy density of an inhomogeneous superconductor at arbitrary temperatures.Comment: REVTeX, 28 page
miR-515-5p controls cancer cell migration through MARK4 regulation
Here, we show that miR-515-5p inhibits cancer cell migration and metastasis. RNA-seq analyses of both oestrogen receptor receptor-positive and receptor-negative breast cancer cells overexpressing miR-515-5p reveal down-regulation of NRAS, FZD4, CDC42BPA, PIK3C2B and MARK4 mRNAs. We demonstrate that miR-515-5p inhibits MARK4 directly 3' UTR interaction and that MARK4 knock-down mimics the effect of miR-515-5p on breast and lung cancer cell migration. MARK4 overexpression rescues the inhibitory effects of miR-515-5p, suggesting miR-515-5p mediates this process through MARK4 down-regulation. Furthermore, miR-515-5p expression is reduced in metastases compared to primary tumours derived from both in vivo xenografts and samples from patients with breast cancer. Conversely, miR-515-5p overexpression prevents tumour cell dissemination in a mouse metastatic model. Moreover, high miR-515-5p and low MARK4 expression correlate with increased breast and lung cancer patients' survival, respectively. Taken together, these data demonstrate the importance of miR-515-5p/MARK4 regulation in cell migration and metastasis across two common cancers
