    At the end of 2021, there were 836,300 Somali refugees and asylum seekers worldwide; in Minnesota alone, there are more than 70,000 Somali refugees, which is the largest concentration in the United States (Culture Care Connection, 2022; The UN Refugee Agency, n.d.). As with many refugee populations, Somali refugees experience many healthcare inequities including limited understanding of the resettled nation\u27s healthcare system, poor functional health literacy, provider and/or interpreter gender preferences, discrimination, and limited cultural competence of providers (Eklöf et al., 2019; Fang et al., 2015; Gele et al., 2016; Houston et al., 2021; Njenga, 2022; Wångdahl et al., 2018). This negatively affects Somali refugees during healthcare encounters and has detrimental impacts on their health and well-being during and following discharge from a healthcare setting

A critically appraised topic (CAT) is an overview of scientific literature about an intervention or practical issue and how it relates to practice (Barends et al., 2017). The use of the Ecological Human Performance (EHP) model is to guide this CAT research with a theoretical basis. EHP was chosen for its emphasis on the person, environment, and tasks. The person is described in EHP as including their experiences, sensorimotor, cognition, and psychosocial abilities (Dunn et al., 1994).     Rituximab therapy for refractory interstitial lung disease related to antisynthetase syndrome

    SummaryObjectiveTo report our experience using rituximab as therapy for refractory antisynthetase syndrome (ASS)-associated interstitial lung disease.MethodsWe retrospectively evaluated the medical records of 7 ASS patients with refractory interstitial lung disease, which had previously failed to respond to prednisone and/or other cytotoxic drugs. All 7 patients received rituximab therapy, i.e.: 1 g at days 0 and 14 and at 6-month follow-up. Data on pulmonary symptoms, pulmonary function tests and high resolution computed tomography (HRCT) scan of the lungs were collected: 1) before rituximab initiation; and 2) at 6-month and one-year follow-up after the first infusion of rituximab.ResultsAt one-year follow-up, ASS patients had resolution (n = 2) or improvement of pulmonary clinical manifestations. Patients also exhibited significant improvement of interstitial lung disease parameters: 1) on pulmonary function tests: FVC (p = 0.03) and DLCO (p = 2 × 10−5); 2) and HRCT-scan of the lungs. Due to clinical resolution/improvement of interstitial lung disease, the median daily dose of oral prednisone could be reduced in these 7 ASS patients at one-year follow-up, compared with baseline (20 mg/day vs. 9 mg/day; p = 0.015).ConclusionOur findings suggest that rituximab may be a helpful therapy for refractory interstitial lung disease in patients with ASS

    Neuron-Derived Semaphorin 3A Is an Early Inducer of Vascular Permeability in Diabetic Retinopathy via Neuropilin-1

    SummaryThe deterioration of the inner blood-retinal barrier and consequent macular edema is a cardinal manifestation of diabetic retinopathy (DR) and the clinical feature most closely associated with loss of sight. We provide evidence from both human and animal studies for the critical role of the classical neuronal guidance cue, semaphorin 3A, in instigating pathological vascular permeability in diabetic retinas via its cognate receptor neuropilin-1. We reveal that semaphorin 3A is induced in early hyperglycemic phases of diabetes within the neuronal retina and precipitates initial breakdown of endothelial barrier function. We demonstrate, by a series of orthogonal approaches, that neutralization of semaphorin 3A efficiently prevents diabetes-induced retinal vascular leakage in a stage of the disease when vascular endothelial growth factor neutralization is inefficient. These observations were corroborated in TgCre-Esr1/Nrp1flox/flox conditional knockout mice. Our findings identify a therapeutic target for macular edema and provide further evidence for neurovascular crosstalk in the pathogenesis of DR

    The combined immunodetection of AP-2α and YY1 transcription factors is associated with ERBB2 gene overexpression in primary breast tumors

    INTRODUCTION: Overexpression of the ERBB2 oncogene is observed in about 20% of human breast tumors and is the consequence of increased transcription rates frequently associated with gene amplification. Several studies have shown a link between activator protein 2 (AP-2) transcription factors and ERBB2 gene expression in breast cancer cell lines. Moreover, the Yin Yang 1 (YY1) transcription factor has been shown to stimulate AP-2 transcriptional activity on the ERBB2 promoter in vitro. In this report, we examined the relationships between ERBB2, AP-2alpha, and YY1 both in breast cancer tissue specimens and in a mammary cancer cell line. METHODS: ERBB2, AP-2alpha, and YY1 protein levels were analyzed by immunohistochemistry in a panel of 55 primary breast tumors. ERBB2 gene amplification status was determined by fluorescent in situ hybridization. Correlations were evaluated by a chi2 test at a p value of less than 0.05. The functional role of AP-2alpha and YY1 on ERBB2 gene expression was analyzed by small interfering RNA (siRNA) transfection in the BT-474 mammary cancer cell line followed by real-time reverse transcription-polymerase chain reaction and Western blotting. RESULTS: We observed a statistically significant correlation between ERBB2 and AP-2alpha levels in the tumors (p < 0.01). Moreover, associations were found between ERBB2 protein level and the combined high expression of AP-2alpha and YY1 (p < 0.02) as well as between the expression of AP-2alpha and YY1 (p < 0.001). Furthermore, the levels of both AP-2alpha and YY1 proteins were inversely correlated to ERBB2 gene amplification status in the tumors (p < 0.01). Transfection of siRNAs targeting AP-2alpha and AP-2gamma mRNAs in the BT-474 breast cancer cell line repressed the expression of the endogenous ERBB2 gene at both the mRNA and protein levels. Moreover, the additional transfection of an siRNA directed against the YY1 transcript further reduced the ERBB2 protein level, suggesting that AP-2 and YY1 transcription factors cooperate to stimulate the transcription of the ERBB2 gene. CONCLUSION: This study highlights the role of both AP-2alpha and YY1 transcription factors in ERBB2 oncogene overexpression in breast tumors. Our results also suggest that high ERBB2 expression may result either from gene amplification or from increased transcription factor levels

    Analysis of 339 pregnancies in 181 women with 13 different forms of inherited thrombocytopenia

    65Pregnancy in women with inherited thrombocytopenias is a major matter of concern as both the mothers and the newborns are potentially at risk of bleeding. However, medical management of this condition cannot be based on evidence because of the lack of consistent information in the literature. To advance knowledge on this matter, we performed a multicentric, retrospective study evaluating 339 pregnancies in 181 women with 13 different forms of inherited thrombocytopenia. Neither the degree of thrombocytopenia nor the severity of bleeding tendency worsened during pregnancy and the course of pregnancy did not differ from that of healthy subjects in terms of miscarriages, fetal bleeding and pre-term births. The degree of thrombocytopenia in the babies was similar to that in the mother. Only 7 of 156 affected newborns had delivery-related bleeding, but 2 of them died of cerebral hemorrhage. The frequency of delivery-related maternal bleeding ranged from 6.8% to 14.2% depending on the definition of abnormal blood loss, suggesting that the risk of abnormal blood loss was increased with respect to the general population. However, no mother died or had to undergo hysterectomy to arrest bleeding. The search for parameters predicting delivery-related bleeding in the mother suggested that hemorrhages requiring blood transfusion were more frequent in women with history of severe bleedings before pregnancy and with platelet count at delivery below 50 × 10(9)/L.openopenPatrizia Noris; Nicole Schlegel; Catherine Klersy; Paula G.     Oral Probiotic Control Skin Inflammation by Acting on Both Effector and Regulatory T Cells

    Probiotics are believed to alleviate allergic and inflammatory skin disorders, but their impact on pathogenic effector T cells remains poorly documented. Here we show that oral treatment with the probiotic bacteria L. casei (DN-114 001) alone alleviates antigen-specific skin inflammation mediated by either protein-specific CD4+ T cells or hapten-specific CD8+ T cells. In the model of CD8+ T cell-mediated skin inflammation, which reproduces allergic contact dermatitis in human, inhibition of skin inflammation by L. casei is not due to impaired priming of hapten-specific IFNγ-producing cytolytic CD8+ effector T cells. Alternatively, L. casei treatment reduces the recruitment of CD8+ effector T cells into the skin during the elicitation (i.e. symptomatic) phase of CHS. Inhibition of skin inflammation by L. casei requires MHC class II-restricted CD4+ T cells but not CD1d-restricted NK-T cells. L casei treatment enhanced the frequency of FoxP3+ Treg in the skin and increased the production of IL-10 by CD4+CD25+ regulatory T cells in skin draining lymph nodes of hapten-sensitized mice. These data demonstrate that orally administered L. casei (DN-114 001) efficiently alleviate T cell-mediated skin inflammation without causing immune suppression, via mechanisms that include control of CD8+ effector T cells and involve regulatory CD4+ T cells. L. casei (DN-114 001) may thus represent a probiotic of potential interest for immunomodulation of T cell-mediated allergic skin diseases in human

    The diet composition of two troops of Macaca sylvanus has been studied in two different habitats of the Djurdjura National Park (Algeria). 1. - In the evergreen Cedar-Green Oak forest (Tigounatine Range), quantitative observations were carried out during 16 months (April 1983 to March 1985). 2. - On the rocky ridges of the Icetcifène Range, without any arborescent vegetation, only qualitative observations were made during 1 2 months (April 1983 to August 1984). On the Tigounatine Range, the troop spent 93 % of its time feeding on plant foods, from which 75 % were leaves and seeds. Great seasonal variations were observerd : Barbary Macaques ate mainly grass and/or Cedar leaves in spring and winter. They turned to a granivorous diet in summer and autumn, consumming mainly acorns and Asphodelus microcarpus seeds. They can become insectivorous in spring, when caterpillars are swarming. These Barbary macaques spent 75 % of their feeding time on the ground. Throughout the year, young animais (0,5-3 years) and subadults (3-4 years) spent more time feeding in trees than adults (> 4 years), but the kind of food items eaten remained similar. Adults ate preferentialy fallen acorns and caterpillars creeping on the ground, whereas young macaques gathered both acorns and caterpillars in trees. One hundred thirty six food categories were identified in the Barbary Macaques diet : However, only Quercus ilex, Cedrus atlantica, Asphodelus microcarpus, nine grass species and caterpillars contributed to 62 % of the diet. Furthermore, only one or three specie s (out of the 12 to 52 consumed each month) contributed to 43 to 85 % of the diet. Fifty nine percent of the food items originated from the grass layer, and 34 % from the arboreal layer. On the Icetcifène range, the troop's diet was mainly made of plant material.
