    The value of preoperative diagnostic tests in acute appendicitis, retrospective analysis of 196 patients

    <p>Abstract</p> <p>Background</p> <p>In this study our aim was to evaluate the diagnostic value of preoperative laboratory and radiological studies for appendicitis.</p> <p>Methods</p> <p>The clinical data of 196 patients who have undergone conventional appendectomy between March 2007 and April 2008 were collected retrospectively. Patients were examined for age, sex, white blood cell count, ultrasonography results, histopathological diagnosis and hospital stay.</p> <p>Results</p> <p>Negative appendectomy rate was 17.3% (27% for female, 11.5% for male). White blood cell counts were found to be high in 83% for acute appendicitis group and %61 for negative appendectomy group. There were 66 (34%) patients who had negative USG findings for acute appendicitis. Of these patients, histopathological examination revealed acute appendicitis in 46 patients whereas 20 patients had normal appendix. Hospital stays were 2.79 +/- 1.9 and 2.66 +/- 1.7 days for negative and positive appendicectomies respectively.</p> <p>Conclusions</p> <p>Besides the improvement of diagnostic tests for acute appendicitis, we could not sufficiently reduce the negative appendectomy rate.</p

    Inguinal hernia repair is commonly performed operation in surgical practice. Thirty-five years old female patient was operated with the diagnosis of inguinal hernia. The inguinal defect was misdiagnosed as femoral vein aneurysm in surgical exploration. The postoperative diagnostic imaging revealed that the inguinal defect had been confused as femoral vein aneurysm due to intra-abdominal hemorrhogical fluid after an ovarian cyst rupture

    Prolonged venous bleeding due to traditional treatment with leech bite: a case report

    <p>Abstract</p> <p>Introduction</p> <p>The medicinal leech, <it>Hirudo medicinalis</it>, has been used in the treatment of many diseases for thousands of years. In Turkey, it is used most commonly in the management of venous diseases of lower extremities.</p> <p>Case presentation</p> <p>A 25-year-old Turkish woman presented to our emergency room with bleeding from her left leg. She had been treated for varicose veins in her lower extremities with leeches about 24 hours before admission to the emergency room. The bleeding was controlled by applying pressure with sterile gauze upon the wound, and she was discharged. She returned after four hours having started bleeding again. Hemostasis was achieved by vein ligation under local anesthesia.</p> <p>Conclusions</p> <p>Leech bite should be evaluated as a special injury. Prolonged bleeding can be seen after leech bites. In such cases, hemostasis either with local pressure or ligation of the bleeding vessel is mandatory.</p

    Strangulated inguinal hernia accompanied by paratesticular leiomyosarcoma

    Sarcomas that arise from the spermatic cord constitute 2.1% of soft tissue sarcomas and are observed at a rate of 1% to 2% in the genitourinary system. A 74-year-old patient presented at the emergency department with complaints of groin pain and swelling persisting for 3 days. On physical examination, a firm mass that was approximately 5x5 cm in size and could not be reduced was observed at the right inguinal area as well as a right inguinal hernia. Computed tomography revealed a large hernia sac in the right inguinal area and a mass that was 77x55 mm in size within the hernial sac. A radical orchiectomy and hernia repair were performed. A diagnosis of leiomyosarcoma was made based on the pathological evaluation of the mass. At the 7-month postoperative follow-up, no local relapse or distant metastasis was found. To conclude, although paratesticular leiomyosarcoma is rare, it should be kept in mind as one of the possible diagnoses for older patients presenting with an inguinal mass

    Trocar-Site Hernia After Diagnostic Laparoscopy

    Trocar-site hernias occur due to various reasons and may cause life-threatening complications. Early diagnosis and treatment reduces morbidity and mortality rates. In this paper, we present a 43-year-old patient who presented to our emergency department with the complaints of nausea, vomiting, distention and inability to defecate occurred after a diagnostic laparoscopy for an abdominal stab wound. A computed tomography revealed a trocar-site hernia involving a small bowel segment. Trocar site was evaluated under general anesthesia and, after reduction of the bowel, the defect was closed primarily. On the first postoperative day, he was discharged uneventfully. Immediate mechanical bowel obstruction after laparoscopic surgery should be kept in mind to deal with these complications
