Interventional Radiology Readiness Assessment Tool for Global Health
The Interventional Radiology Readiness Assessment Tool for Global Health is a new tool to methodically evaluate the environment of a medical institution for interventional radiology services given the existing infrastructure. Global health provides an exciting opportunity for interventional radiology to impact health outcomes in developing countries. A systematic and thoughtful approach to integrating interventional radiology services in the health care institutions of resource poor countries is needed in order to maximize global health efforts and outcomes.

Partial Truths: Adults Choose to Mention Agents and Patients in Proportion to Informativity, Even If It Doesnât Fully Disambiguate the Message
How do we decide what to say to ensure our meanings will be understood? The Rational Speech Act model (RSA; Frank & Goodman, 2012) asserts that speakers plan what to say by comparing the informativity of words in a particular context. We present the first example of an RSA model of sentence-level (who-did-what-to-whom) meanings. In these contexts, the set of possible messages must be abstracted from entities in common ground (people and objects) to possible events (Jane eats the apple, Marco peels the banana), with each word contributing unique semantic content. How do speakers accomplish the transformation from context to compositional, informative messages? In a communication game, participants described transitive events (e.g., Jane pets the dog), with only two words, in contexts where two words either were or were not enough to uniquely identify an event. Adults chose utterances matching the predictions of the RSA even when there was no possible fully âsuccessfulâ utterance. Thus we show that adultsâ communicative behavior can be described by a model that accommodates informativity in context, beyond the set of possible entities in common ground. This study provides the first evidence that adultsâ language production is affected, at the level of argument structure, by the graded informativity of possible utterances in context, and suggests that full-blown natural speech may result from speakers who model and adapt to the listenerâs needs

Linking Language and Events: Spatiotemporal Cues Drive Childrenâs Expectations About the Meanings of Novel Transitive Verbs
How do children map linguistic representations onto the conceptual structures that they encode? In the present studies, we provided 3-4 year old children with minimal-pair scene contrasts in order to determine the effect of particular event properties on novel verb learning. Specifically, we tested whether spatiotemporal cues to causation also inform childrenâs interpretation of transitive verbs either with or without the causal/inchoative alternation (She broke the lamp/the lamp broke). In Experiment 1, we examined spatiotemporal continuity. Children saw scenes with puppets that approached a toy in a distinctive manner, and toys that lit up or played a sound. In the causal events, the puppet contacted the object, and activation was immediate. In the noncausal events, the puppet stopped short before reaching the object, and the effect occurred after a short pause (apparently spontaneously). Children expected novel verbs used in the inchoative transitive/intransitive alternation to refer to spatiotemporally intact causal interactions rather than to 'gap' control scenes. In Experiment 2, we manipulated the temporal order of sub-events, holding spatial relationships constant, and provided evidence for only one verb frame (either transitive or intransitive). Children mapped transitive verbs to scenes where the agent's action closely preceded the activation of the toy over scenes in which the timing of the two events was switched, but did not do so when they heard an intransitive construction. These studies reveal that childrenâs expectations about transitive verbs are at least partly driven by their nonlinguistic understanding of causal events: children expect transitive syntax to refer to scenes where the agent's action is a plausible cause of the outcome. Pediatric Emergency Medicine Physiciansâ Use of Pointâofâcare Ultrasound and Barriers to Implementation: A Regional Pilot Study
ObjectivesPointâofâcare ultrasound (POCUS) has been identified as a critical skill for pediatric emergency medicine (PEM) physicians. The purpose of this study was to profile the current status of PEM POCUS in pediatric emergency departments (EDs).MethodsAn electronic survey was distributed to PEM fellows and attending physicians at four major pediatric academic health centers. The 24âitem questionnaire covered professional demographics, POCUS experience and proficiency, and barriers to the use of POCUS in pediatric EDs. We used descriptive and inferential statistics to profile respondentâs PEM POCUS experience and proficiency and Rasch analysis to evaluate barriers to implementation.ResultsOur return rate was 92.8% (128/138). Respondents were attending physicians (68%) and fellows (28%). Most completed pediatric residencies prior to PEM fellowship (83.6%). Almost all had some form of ultrasound education (113/128, 88.3%). Approximately half (46.9%) completed a formal ultrasound curriculum. More than half (53.2%) said their ultrasound education was pediatricâspecific. Most participants (67%) rated their POCUS proficiency low (Levels 1â2), while rating proficiency in other professional competencies (procedures 52%, emergency stabilization 70%) high (Levels 4â5). There were statistically significant differences in POCUS proficiency between those with formal versus informal ultrasound education (p < 0.001) and those from pediatric versus emergency medicine residencies (p < 0.05). Participants identified both personal barriers discomfort with POCUS skills (76.7%), insufficient educational time to learn POCUS (65%), and negative impact of POCUS on efficiency (58.5%)âand institutional barriers to the use of ultrasoundâconsultants will not use ultrasound findings from the ED (60%); insufficient mentoring (64.7%), and POCUS not being a departmental priority (57%).ConclusionsWhile POCUS utilization continues to grow in PEM, significant barriers to full implementation still persist. Interplay between Single Resistance-Associated Mutations in the HIV-1 Protease and Viral Infectivity, Protease Activity, and Inhibitor Sensitivity
ABSTRACT Resistance-associated mutations in the HIV-1 protease modify viral fitness through changes in the catalytic activity and altered binding affinity for substrates and inhibitors. In this report, we examine the effects of 31 mutations at 26 amino acid positions in protease to determine their impact on infectivity and protease inhibitor sensitivity. We found that primary resistance mutations individually decrease fitness and generally increase sensitivity to protease inhibitors, indicating that reduced virion-associated protease activity reduces virion infectivity and the reduced level of per virion protease activity is then more easily titrated by a protease inhibitor. Conversely, mutations at more variable positions (compensatory mutations) confer low-level decreases in sensitivity to all protease inhibitors with little effect on infectivity. We found significant differences in the observed effect on infectivity with a pseudotype virus assay that requires the protease to cleave the cytoplasmic tail of the amphotropic murine leukemia virus (MuLV) Env protein. Additionally, we were able to mimic the fitness loss associated with resistance mutations by directly reducing the level of virion-associated protease activity. Virions containing 50% of a D25A mutant protease were 3- to 5-fold more sensitive to protease inhibitors. This level of reduction in protease activity also resulted in a 2-fold increase in sensitivity to nonnucleoside inhibitors of reverse transcriptase and a similar increase in sensitivity to zidovudine (AZT), indicating a pleiotropic effect associated with reduced protease activity. These results highlight the interplay between enzyme activity, viral fitness, and inhibitor mechanism and sensitivity in the closed system of the viral replication complex
