    Awareness and perceptions of electroconvulsive therapy among psychiatric patients: a cross-sectional survey from teaching hospitals in Karachi, Pakistan

    <p>Abstract</p> <p>Background</p> <p>Electroconvulsive therapy (ECT) is shown to be effective in many psychiatric illnesses, but its distorted projection by the Pakistani media and its unregulated use by many physicians across the country have adversely affected its acceptability. Given this situation we aimed to assess the awareness and perceptions regarding ECT as a treatment modality among the psychiatric patients.</p> <p>Methods</p> <p>This was a questionnaire based cross-sectional study carried out at 2 tertiary care hospitals in Karachi, Pakistan.</p> <p>Results</p> <p>We interviewed 190 patients of which 140 were aware of ECT. The study showed that the level of education had a significant impact on the awareness of ECT (p = 0.009). The most common source of awareness was electronic and print media (38%), followed by relatives (24%) and doctors (23%). Physical injuries (42%) and neurological (12%) and cognitive disturbances (11%) were the commonly feared side effects. The most popular belief about ECT was that it was a treatment of last resort (56%). Thirty-nine percent thought that ECT could lead to severe mental and physical illness and 37% considered it inhumane. Patients' willingness to receive ECT was dependant on whether or not they were convinced of its safety (p = 0.001) and efficacy (p = 0.0001).</p> <p>Conclusion</p> <p>We identified a serious lack of dissemination of information regarding ECT by the psychiatrists and the mental health care providers. This may be the result of an inadequate postgraduate training in Pakistan or just a lack of concern about the mentally ill patients. The media seemed to be the major source of information for our patients. We also saw the prevalence of a variety of myths regarding ECT in our society, which we feel may be responsible for the patients' adverse attitudes. Given the widespread applicability of ECT there is a dire need to dispel these misconceptions and improve its acceptability.</p

    Sidelobe Reduction in Cognitive Radio Systems Using Hybrid Technique

    Orthogonal frequency division multiplexing (OFDM) is one of the best candidates for dynamic spectrum access due to its flexibility of spectrum shaping. However, the high sidelobes of the OFDM signal that result in high out-of-band radiation, introduce significant interference to the users operating in its vicinity. This problem becomes more critical in cognitive radio (CR) system that enables the secondary users (SUs) users to access the spectrum holes not used by the primary users (PUs) at that time. In this paper, we present a generalized OFDM framework that has a capability of describing any sidelobe suppression techniques, despite of whether one or a number of techniques are used. Based on that framework, we propose cancellation carrier (CC) technique in conjunction with the generalized sidelobe canceller (GSC) to reduce the out-of-band radiation in the region where the licensed users are operating. Simulation results show that the proposed technique can reduce the out-of-band radiation better when compared with the existing techniques found in the literature

    Neonatal haemolytic disease with co-existing Anti-D and Anti-C antibodies: an unusual experience

    Neonatal haemolytic disease in the new-born remains of prime importance for paediatricians due to high perinatal morbidity and mortality rates. The Rh antigen family comprises several different antigens, out of which, D antigen incompatibility is well known for causing severe haemolytic disease in the foetus. Although the current literature shows anomalous cases where coexisting non-D-Rh and D-Rh antigens are the causative agents, there is very little information regarding post-natal outcomes in neonates bearing two different incompatibilities simultaneously. Herein, we discuss an unusual case of anti-D as well as anti-C antibodies (non-D-Rh) in a male neonate born to a Rh-negative mother, who developed jaundice and haemolysis in post-natal life. The neonate underwent exchange transfusion and photo therapy due to raised serum bilirubin levels, supplemented with repeated blood transfusions, intravenous immunoglobulin therapy, and immunosuppressive therapy. --Continu

    Application of Machine Learning Algorithms for Sustainable Business Management Based on Macro-Economic Data: Supervised Learning Techniques Approach

    Macroeconomic indicators are the key to success in the development of any country and are very much important for the overall economy of any country in the world. In the past, researchers used the traditional methods of regression for estimating macroeconomic variables. However, the advent of efficient machine learning (ML) methods has led to the improvement of intelligent mechanisms for solving time series forecasting problems of various economies around the globe. This study focuses on forecasting the data of the inflation rate and the exchange rate of Pakistan from January 1989 to December 2020. In this study, we used different ML algorithms like k-nearest neighbor (KNN), polynomial regression, artificial neural networks (ANNs), and support vector machine (SVM). The data set was split into two sets: the training set consisted of data from January 1989 to December 2018 for the training of machine algorithms, and the remaining data from January 2019 to December 2020 were used as a test set for ML testing. To find the accuracy of the algorithms used in the study, we used root mean square error (RMSE) and mean absolute error (MAE). The experimental results showed that ANNs archives the least RMSE and MAE compared to all the other algorithms used in the study. While using the ML method for analyzing and forecasting inflation rates based on error prediction, the test set showed that the polynomial regression (degree 1) and ANN methods outperformed SVM and KNN. However, on the other hand, forecasting the exchange rate, SVM RBF outperformed KNN, polynomial regression, and ANNs
