    Managements of sleep bruxism in adult : A systematic review

    This systematic review aimed to update the management of sleep bruxism (SB) in adults, as diagnosed using polysomnography (PSG) and/or electromyography (EMG). Management methods covered were oral appliance therapy (OAT) with stabilization splints, cognitive-behavioral therapy (CBT), biofeedback therapy (BFT), and pharmacological therapy. A comprehensive search was conducted on MEDLINE, Cochrane Library, and Web of Science up to October 1st, 2021. Reference list searches and hand searches were also performed by an external organization. Two reviewers for each therapy independently performed article selection, data extraction, and risk of bias assessment. The reviewers resolved any disagreements concerning the assortment of the articles by discussion. Finally, 11, 3, 14, and 22 articles were selected for each therapy. The results suggested that OAT tended to reduce the number of SB events, although there was no significant difference compared to other types of splints, that the potential benefits of CBT were not well supported, and that BFT, rabeprazole, clonazepam, clonidine, and botulinum toxin type A injection showed significant reductions in specific SB parameters, although several side effects were reported. It can be concluded that more methodologically rigorous randomized large-sample long-term follow-up clinical trials are needed to clarify the efficacy and safety of management for SB

    Radiological evaluation of joint space width in medial knee osteoarthritis

    Background. Although joint space width on weight-bearing radiographs of the knee is critical for early diagnosis and grading knee osteoarthritis, the optimal method with which to accurately measure this value remains controversial. The purpose of this study was to investigate and quantify the effects of the radiographic technique on joint space width in medial knee osteoarthritis.Materials and Methods. We compared maximum plateau gaps and minimum joint space widths on bilateral weight-bearing plain radiographs acquired using three different methods in 31 patients with medial knee osteoarthritis (56 knee joints): standing with the knee extended (standard imaging); SynaFlexer method; and Rosenberg method. Measured values were compared statistically, with values of P < 0.05 considered significant.Results. Maximum plateau gap in the medial compartment was significantly lower with the SynaFlexer method (3.2 ± 1.5 mm) and Rosenberg method (2.2 ± 1.2 mm) than with standard imaging (4.7 ± 2.2 mm; P < 0.05 each). Minimum width of the medial joint space was also significantly lower with the SynaFlexer method (3.1 ± 1.4 mm) and Rosenberg method (2.3 ± 1.4 mm) than with standard imaging (4.1 ± 1.4 mm; P < 0.05 each).Conclusion. The Rosenberg method appears beneficial for diagnosing early knee osteoarthritis, while the SynaFlexer method seems more appropriate for assessing disease severity or progression in patients with painful intermediate to severe knee osteoarthritis

    Metastases of soft tissue sarcoma to the liver: A Historical Cohort Study from a Hospital-based Cancer Registry

    Background: Hepatic metastasis of soft tissue sarcoma is rare compared to lung metastasis, and the literature is scarce. We examined the risk of hepatic metastasis according to the site of occurrence and histological type. Methods: From a Hospital-based Cancer Registry, 658 patients registered between 2007 and 2017 with soft tissue sarcomas were evaluated. The exclusion criteria were gastrointestinal stromal tumors, tumors of unknown origin, and follow-up periods of less than 1 month. SPSS 25 was used for statistical analysis. Results: The risk of hepatic metastasis was significantly higher in the retroperitoneum (HR, 5.981; 95% CI, 2.793-12.808) and leiomyosarcoma (HR, 4.303; 95% CI, 1.782-10.390). Multivariate analysis showed that the risk of hepatic metastasis as first distant metastasis was high in leiomyosarcoma (HR, 4.546; 95% CI, 2.275-9.086) and retroperitoneal onset (HR, 4.588; 95% CI, 2.280-9.231). The 2-year survival rate after hepatic metastasis was 21.7%. Conclusions: The onset of hepatic metastasis indicates a poor prognosis. However, hepatic metastasis from retroperitoneal sarcoma and leiomyosarcoma may be the first distant metastasis in some cases. For retroperitoneal sarcoma and leiomyosarcoma, additional screening for hepatic metastasis such as contrast CT should be considered during staging and follow-up after treatment.ArticleCancer medicine 17(17) : 6159-6165(2020)journal articl

    Introduction: Upshaw-Schulman syndrome (USS) is a congenital form of thrombotic thrombocytopenic purpura (TTP) associated with loss-of-function mutations in the ADAMTS13 gene, possibly leading to aberrant complement activation and vascular injury. However, USS is extremely rare, and there have been no systematic studies correlating histopathological severity with local ADAMTS13 expression and complement activation. Materials and methods: Here, we compared histopathological features, ADAMTS13 immunoreactivity, and immunoreactivity of complement proteins C4d and C5b-9 among renal biopsy tissues from five USS cases, ten acquired TTP cases, and eleven controls. Results: Pathological analysis revealed chronic glomerular sclerotic changes in the majority of USS cases (4 of 5), with minor glomerular pathology in the remaining case. In two of these four severe cases, more than half of the glomerular segmental sclerosis area was localized in the perihilar region. The average number of ADAMTS13-positive cells per glomerulus was significantly lower in USS cases than controls (p < 0.05). Conversely, C4d staining was significantly more prevalent in the glomerular capillary walls of USS cases than controls (p < 0.05), while C5b-9 staining did not differ significantly among groups. Conclusions: These findings suggest that the severity of glomerular injury in USS is associated with deficient ADAMTS13 expression and local complement activation, particularly in vascular regions with higher endothelial shear stress.     Sonodynamic Therapy With Anticancer Micelles and High-Intensity Focused Ultrasound in Treatment of Canine Cancer

    Sonodynamic therapy (SDT) is a minimally invasive anticancer therapy involving a chemical sonosensitizer and high-intensity focused ultrasound (HIFU). SDT enables the reduction of drug dose and HIFU irradiation power compared to those of conventional monotherapies. In our previous study, mouse models of colon and pancreatic cancer were used to confirm the effectiveness of SDT vs. drug-only or HIFU-only therapy. To validate its usefulness, we performed a clinical trial of SDT using an anticancer micelle (NC-6300) and our HIFU system in four pet dogs with spontaneous tumors, including chondrosarcoma, osteosarcoma, hepatocellular cancer, and prostate cancer. The fact that no adverse events were observed, suggests the usefulness of SDT

    A Study of Clinical Features and the Worsening Factors Among Patients with Behget\u27s Disease in Japan

    A cross-sectional survey of Beliefs disease in Japan was held to investigate the correlation of clinical-features and HLA-B51, pathergy test and the worsening factors among patients. The questionnaires were sent to 200 hospitals selected geographically at random in Japan. The diagnosis was made with Japanese Criteria for the classification and patients who did not satisfy the criteria were excluded from the study. The differences in data were analyzed by Pearson\u27s corrected chi-square test, and multiple logistic regression analyses with SPSS. A total of 180 patients (80 men and 100 women) were used as subjects. The rate of positive HLA-B51 was 57.4% (66.7% in men, 44.8% in women). HLA-B51 positivity was significantly higher in patients with eye lesions than without, and significantly lower in patients with gastrointestinal lesions than without. The rate of positive pathergy test was 53.0 % (67.3 % in men, 42.4 % in women). The pathergy test positivity was significantly higher in male patients, patients with skin lesions, and with central nerve system symptoms. The number of patients with worsening factors affecting the symptoms, such as seasonal influences, infectious states, psychological fatigue and physical fatigue was 88, with significantly more among patients with skin lesions and those with arthritis, and less among those with eye lesions. As the results of multiple regression analyses show, HLA-B51 negativity was most associated with patients with gastrointestinal lesions and the positivity was most associated with male patients. The pathergy test positivity was most associated with male patients. It was suggested that the rates of positive HLA - B51 and pathergy test were significantly higher among male patients. As the worsening factors might be concerned with the etiology of Behcet\u27s disease, this study only provided some suggestions for further examination

In particular, studies of gene expression dynamics and regulation landscape in COVID-19 infected individuals are limited. Here, we report on a thorough analysis of whole blood RNA-seq data from 465 genotyped samples from the Japan COVID-19 Task Force, including 359 severe and 106 non-severe COVID-19 cases. We discover 1169 putative causal expression quantitative trait loci (eQTLs) including 34 possible colocalizations with biobank fine-mapping results of hematopoietic traits in a Japanese population, 1549 putative causal splice QTLs (sQTLs; e.g. two independent sQTLs at TOR1AIP1), as well as biologically interpretable trans-eQTL examples (e.g., REST and STING1), all fine-mapped at single variant resolution. We perform differential gene expression analysis to elucidate 198 genes with increased expression in severe COVID-19 cases and enriched for innate immune-related functions. Finally, we evaluate the limited but non-zero effect of COVID-19 phenotype on eQTL discovery, and highlight the presence of COVID-19 severity-interaction eQTLs (ieQTLs; e.g., CLEC4C and MYBL2). Our study provides a comprehensive catalog of whole blood regulatory variants in Japanese, as well as a reference for transcriptional landscapes in response to COVID-19 infection
