    Prolonged Effects of Acute Stress on Decision-Making under Risk: A Human Psychophysiological Study

    This study investigates the prolonged effects of physiological responses induced by acute stress on risk-taking in decision-making. Participants were divided into a Stress group (N = 14) and a Control group (N = 12). The Trier Social Stress Test was administered as an acute stressor, and reading was administered as a control task; thereafter, participants performed a decision-making task in which they needed to choose a sure option or a gamble option in Gain and Loss frame trials 2 hours after (non-) exposure to the stressor. Increased cortisol, adrenaline, heart rate (HR), and subjective stress levels validated acute stress manipulation. Stressed participants made fewer risky choices only in the Gain domain, whereas no effect of stress was shown in the Loss domain. Deceleration of HR reflecting attention was greater for Gains compared with Losses only in the Stress group. Risk avoidance was determined by increased levels of cortisol caused by acute stress. These results suggest that processes regarding glucocorticoid might be involved in the prolonged effects of acute stress on the evaluation of risks and the monitoring of outcomes in decision-making

    Asymmetry of tensile vs. compressive elasticity and permeability contributes to the regulation of exchanges in collagen gels

    The Starling principle describes exchanges in tissues based on the balance of hydrostatic and osmotic flows. This balance neglects the coupling between mechanics and hydrodynamics, a questionable assumption in strained elastic tissues due to intravascular pressure. Here, we measure the elasticity and permeability of collagen gels under tensile and compressive stress via the comparison of the temporal evolution of pressure in an air cavity sealed at the outlet of a collagen slab with an analytical kinetic model. We observe a drop in the permeability and enhanced strain-stiffening of native collagen gels under compression, both effects being essentially lost after chemical cross-linking. Further, we prove that this asymmetric response accounts for the accumulation of compressive stress upon sinusoidal fluid injection, which modulates the material's permeability. Our results thus show that the properties of collagen gels regulate molecular exchanges and could help understand drug transport in tissues

    MALIGNANT ONCOCYTOMA OF THE PARANASAL SINUS

    A case of malignant oncocytoma developing in the paranasal sinus of a 37-year-old Japanese man with a habit of heavy smoking is described. The primary tumor was found in the right maxillary sinus with local invasion to the nose and right ethmoidal sinus ; it was composed of nests of large oncocytic cells, with finely granular and eosinophilic cytoplasm, nuclear pleomorphism and prominent nucleoli, surrounded by proliferation of spindle-shaped cells in some areas. Electron microscopically, the cytoplasm of oncocytic tumor cells was characterized by abundant mitochondria, and immunohistochemical investigation revealed positive binding for antibodies to S-100 protein and α₁-antitrypsin, but a negative reaction for both cytokeratin and vimentin. At autopsy, metastatic nodules of tumor in the lungs, liver and pancreas independently exhibited both oncocytic cell- and sarcomatous cell compartments. This phenomenon is rare, and to the authors' knowledge, this would be histologically an ususual report of malignant oncocytoma with sarcomatous metastatic nodules arising from the paranasal sinus to be described in the literature

    A Retrospective Analysis of Transfusion Management for Obstetric Hemorrhage in a Japanese Obstetric Center

    Background. Since cryoprecipitate, fibrinogen concentrate, or recombinant activated factor VII is not approved by public medical insurance in Japan, we retrospectively assessed blood product usage in patients with obstetric hemorrhage at our tertiary obstetric center. Material and Methods. 220 patients with obstetric hemorrhagic disorders who underwent blood product transfusion in our institution during a 5-year period were reviewed for the types and volumes of blood products transfused. Results. There was a significant positive correlation (P< 0.001) between the volume of RCC (red blood cell concentrate) transfused and that of FFP (fresh frozen plasma), irrespective of underlying obstetric disorders. The median of FFP to RCC ratio in each patient was 1.3–1.4, when 6 or more units of RCC were transfused. Conclusions. In transfusion for massive obstetric hemorrhage in terms of appropriate supplementation of coagulation factors, the transfusion of RCC : FFP = 1 : 1.3–1.4 may be desirable

    High Current-Induced Electron Redistribution in a CVD-Grown Graphene Wide Constriction

    Investigating the charge transport behavior in one-dimensional quantum confined system such as the localized states and interference effects due to the nanoscale grain boundaries and merged domains in wide chemical vapor deposition graphene constriction is highly desirable since it would help to realize industrial graphene-based electronic device applications. Our data suggests a crossover from interference coherent transport to carriers flushing into grain boundaries and merged domains when increasing the current. Moreover, many-body fermionic carriers with disordered system in our case can be statistically described by mean-field Gross-Pitaevskii equation via a single wave function by means of the quantum hydrodynamic approximation. The novel numerical simulation method supports the experimental results and suggests that the extreme high barrier potential regions on graphene from the grain boundaries and merged domains can be strongly affected by additional hot charges. Such interesting results could pave the way for quantum transport device by supplying additional hot current to flood into the grain boundaries and merged domains in one-dimensional quantum confined CVD graphene, a great advantage for developing graphene-based coherent electronic devices

    Platelet-to-Lymphocyte Ratio Multiplied by the Cytokeratin-19 Fragment Level as a Predictor of Pathological Response to Neoadjuvant Chemotherapy in Esophageal Squamous Cell Carcinoma

    [Background] The standard treatment for resectable advanced esophageal squamous cell carcinoma in Japan is surgery followed by neoadjuvant chemotherapy, and it is important to predict the effect of neoadjuvant chemotherapy before treatment. Therefore, this study aims to extract conventional blood examination data, such as tumor markers and/or inflammatory/nutritional index levels, that can predict the pathological response of patients with esophageal squamous cell carcinoma to neoadjuvant chemotherapy. [Methods] We retrospectively analyzed the medical records of 66 patients with thoracic esophageal squamous cell carcinoma who received neoadjuvant chemotherapy, followed by curative esophagectomy at Tottori University Hospital between June 2009 and December 2019. [Results] We demonstrated that the product of the platelet-to-lymphocyte ratio (PLR) multiplied by the cytokeratin-19 fragment (CYFRA) level, which was termed “PLR-CYFRA,” is the most accurate indicator that predicts the pathological response to neoadjuvant chemotherapy, with the highest area under the curve [0.795 (95% confidence interval: 0.665–0.925), P < 0.001] in receiver operating characteristic analyses. Therefore, we divided patients into the PLR-CYFRALow (< 237.6, n = 21) and PLR-CYFRAHigh (≥ 237.6, n = 45) groups and found that the percentage of PLR-CYFRALow was significantly higher in patients with a better pathological response (P < 0.001). Furthermore, patients with good pathological response had significantly better prognoses in terms of disease-specific survival (P = 0.014), recurrence-free survival (P = 0.014), and overall survival (P = 0.032). In the multivariate analysis, PLR-CYFRA was an independent predictor of the pathological response of patients with esophageal squamous cell carcinoma to neoadjuvant chemotherapy (P = 0.002). [Conclusion] Pretreatment PLR-CYFRA might be a useful and simple tool that predicts the pathological effect of neoadjuvant chemotherapy in esophageal squamous cell carcinoma
