Postoperative Komplikationen nach Stomarückverlagerungsoperation – Ergebnisse nach prophylaktischer Verstärkung der Bauchwand mit GORE® BIO-A® - Netz in offener Sublay - Technik und primärem Wundverschluss

EINLEITUNG: In der Klinik für Chirurgie des Klinikum Mittelbaden Baden-Baden Balg (Prof. Dr. Dieter Berger) wurden in dem Zeitraum 15.11.2010 bis 11.03.2015 an 127 Patienten eine Stomarückverlagerungs-Operation durchgeführt. Alle 127 Patienten wurden mit einem resorbierbaren synthetischen Netz (GORE® BIO-A®- Netz) in sublay-Technik im Rahmen des Bauchdecken-Inzisionsverschlusses versorgt. In allen Fällen wurde ein primärer Wundverschluss vorgenommen. METHODIK: Alle 127 operierten Patienten wurden in einer prospektiven Studie erfasst. In der Nachuntersuchung (Follow-up Minimum 2 Monate) wurden 104 Patienten klinisch und durch bildgebende Verfahren kontrolliert. Lost to follow-up Patienten n=23 ERGEBNISSE: Insgesamt zeigte sich eine Hernienrate von 7,7 % (n=8) sowie relevante Wundinfektionen von 32,7% (n=42). Diese unterteilen sich in oberflächliche und tiefe Wundinfekte. Den Status der Wundheilung haben wir bei allen 127 Patienten erfasst. SCHLUSSFOLGERUNG: • Die prophylaktische Anwendung des resorbierbaren synthetischen Netzes GORE®BIO-A® führt in offener sublay-Technik bei Stoma-RV zu einer deutlichen Minderung der Hernienrate bei 7,7% . • Die Rate an Wundheilungsstörungen ist mit 32,7% nach wie vor hoch, allerdings erfolgte nur bei 6,7% aller nachuntersuchten Patienten der Netzausbau. • In dieser Studie gibt es keinen Zusammenhang zwischen Wundheilungsstörung und Hernieninzidien
