    Still Unequal at Birth - Birth Weight, Socioeconomic Status and Outcomes at Age 9

    Birth weight is an important aspect of public health which has been linked to increased risk of infant death, increased cost of care, and a range of later life outcomes. Using data from a new Irish cohort study, I document the relationship between birth weight and socioeconomic status. A strong asso- ciation with maternal education does not appear to be due to the timing of birth or complications during pregnancy, even controlling for a wide range of background characteristics. However, results do suggest intergenerational persistence in the transmission of poor early life conditions. A compar- ison with the UK Millennium Cohort Study reveals similar social gradients in both countries. Birth weight predicts a number of outcomes at age 9, including test scores, hospital stays and health. An advantage of the data is that I am able to control for a number of typically unmeasured variables. I determine whether parental investments as measured by the quality of interaction with the child, parenting style, or school quality mediate the association between birth weight and later indicators. For test scores, there is evidence of non-linearity. Boys are more adversely affected than girls, and I find that the effects of low birth weight (Early Life Conditions, Birth Weight, Health Inequalities, Test Scores

    Vaccination and All Cause Child Mortality 1985-2011: Global Evidence from the Demographic and Health Surveys

    Based on models with calibrated parameters for infection, case fatality rates, and vaccine efficacy, basic childhood vaccinations have been estimated to be highly cost effective. We estimated the association of vaccination with mortality directly from survey data. Using 149 cross-sectional Demographic and Health Surveys, we determined the relationship between vaccination coverage and the probability of dying between birth and 5 years of age at the survey cluster level. Our data included approximately 1 million children in 68,490 clusters from 62 countries. We considered the childhood measles, bacillus Calmette-Guérin, diphtheria-pertussis-tetanus, polio, and maternal tetanus vaccinations. Using modified Poisson regression to estimate the relative risk of child mortality in each cluster, we also adjusted for selection bias that resulted from the vaccination status of dead children not being reported. Childhood vaccination, and in particular measles and tetanus vaccination, is associated with substantial reductions in childhood mortality. We estimated that children in clusters with complete vaccination coverage have a relative risk of mortality that is 0.73 (95% confidence interval: 0.68, 0.77) times that of children in a cluster with no vaccinations. Although widely used, basic vaccines still have coverage rates well below 100% in many countries, and our results emphasize the effectiveness of increasing coverage rates in order to reduce child mortality

    Pharmacologic Considerations in Oncology Critical Care

    Critical care in the oncology population consists of diverse levels of diseases, syndromes, and emergencies that are not observed in typical medically-ill patients and, with it, comes even more specialized treatment strategies. Therefore, the uncommon or less well-understood pharmacologic considerations in this population must be discussed to better assist any clinician at the bedside. This chapter outlines some of the situations commonly encountered in this setting such as the challenge of treating and preventing infectious diseases when the patient lacks the ability to mount appropriate immune responses to conventional therapy, the paradigm of treating thromboembolism in the group of patients who are at highest risk for both bleeding and clotting and treatment of acute and long-term consequences of cancer or chemotherapy requiring escalation of care to the intensive care unit (ICU)

    Surface Gravities for 228 M, L, and T Dwarfs in the NIRSPEC Brown Dwarf Spectroscopic Survey

    We combine 131 new medium-resolution (R~2000) J-band spectra of M, L, and T dwarfs from the Keck NIRSPEC Brown Dwarf Spectroscopic Survey (BDSS) with 97 previously published BDSS spectra to study surface-gravity-sensitive indices for 228 low-mass stars and brown dwarfs spanning spectral types M5-T9. Specifically, we use an established set of spectral indices to determine surface gravity classifications for all M6-L7 objects in our sample by measuring equivalent widths (EW) of the K I lines at 1.1692, 1.1778, 1.2529 um, and the 1.2 um FeHJ absorption index. Our results are consistent with previous surface gravity measurements, showing a distinct double peak - at ~L5 and T5 - in K I EW as a function of spectral type. We analyze K I EWs of 73 objects of known ages and find a linear trend between log(Age) and EW. From this relationship, we assign age ranges to the very low gravity, intermediate gravity, and field gravity designations for spectral types M6-L0. Interestingly, the ages probed by these designations remain broad, change with spectral type, and depend on the gravity sensitive index used.     National South African HIV prevalence estimates robust despite substantial test non-participation

    Background. South African (SA) national HIV seroprevalence estimates are of crucial policy relevance in the country, and for the worldwide HIV response. However, the most recent nationally representative HIV test survey in 2012 had 22% test non-participation, leaving the potential for substantial bias in current seroprevalence estimates, even after controlling for selection on observed factors. Objective. To re-estimate national HIV prevalence in SA, controlling for bias due to selection on both observed and unobserved factors in the 2012 SA National HIV Prevalence, Incidence and Behaviour Survey. Methods. We jointly estimated regression models for consent to test and HIV status in a Heckman-type bivariate probit framework. As selection variable, we used assigned interviewer identity, a variable known to predict consent but highly unlikely to be associated with interviewees’ HIV status. From these models, we estimated the HIV status of interviewed participants who did not test. Results. Of 26 710 interviewed participants who were invited to test for HIV, 21.3% of females and 24.3% of males declined. Interviewer identity was strongly correlated with consent to test for HIV; declining a test was weakly associated with HIV serostatus. Our HIV prevalence estimates were not significantly different from those using standard methods to control for bias due to selection on observed factors: 15.1% (95% confidence interval (CI) 12.1 - 18.6) v. 14.5% (95% CI 12.8 - 16.3) for 15 - 49-year-old males; 23.3% (95% CI 21.7 - 25.8) v. 23.2% (95% CI 21.3 - 25.1) for 15 - 49-year-old females. Conclusion. The most recent SA HIV prevalence estimates are robust under the strongest available test for selection bias due to missing data. Our findings support the reliability of inferences drawn from such data

    Discovery of a Very Young Field L Dwarf, 2MASS J01415823-4633574

    While following up L dwarf candidates selected photometrically from the Two Micron All Sky Survey, we uncovered an unusual object designated 2MASS J01415823-4633574. Its optical spectrum exhibits very strong bands of vanadium oxide but abnormally weak absorptions by titanium oxide, potassium, and sodium. Morphologically such spectroscopic characteristics fall intermediate between old, field early-L dwarfs (log(g)~5) and very late M giants (log(g)~0), leading us to favor low gravity as the explanation for the unique spectral signatures of this L dwarf. Such a low gravity can be explained only if this L dwarf is much lower in mass than a typical old field L dwarf of similar temperature and is still contracting to its final radius. These conditions imply a very young age. Further evidence of youth is found in the near-infrared spectrum, including a triangular-shaped H-band continuum reminiscent of young brown dwarf candidates discovered in the Orion Nebula Cluster. Using the above information along with comparisons to brown dwarf atmospheric and interior models, our current best estimate is that this L dwarf has an age of 1-50 Myr and a mass of 6-25 M_Jupiter. The location of 2MASS 0141-4633 on the sky coupled with a distance estimate of ~35 pc and the above age estimate suggests that this object may be a brown dwarf member of either the 30-Myr-old Tucana/Horologium Association or the ~12-Myr-old beta Pic Moving Group.Comment: Accepted for publication in the 10 March 2006 issue (volume 639) of the Astrophysical Journa

    The NIRSPEC Brown Dwarf Spectroscopic Survey. I. Low-Resolution Near-Infrared Spectra

    We present the first results of a near-infrared (0.96-2.31 micron) spectroscopic survey of M, L, and T dwarfs obtained with NIRSPEC on the Keck II telescope. Our new survey has a resolving power of R = 2000 and is comprised of two major data sets: 53 J-band (1.14-1.36 micron) spectra covering all spectral types from M6 to T8 with at least two members in each spectral subclass (wherever possible), and 25 flux-calibrated spectra from 1.14 to 2.31 microns for most spectral classes between M6 and T8. Sixteen of these 25 objects have additional spectral coverage from 0.96-1.14 microns to provide overlap with optical spectra. Spectral flux ratio indexes for prominent molecular bands are derived and equivalent widths (EWs) for several atomic lines are measured. We find that a combination of four H2O and two CH4 band strengths can be used for spectral classification. Weak (EW~1-2 angstrom) atomic lines of Al I and Ca I disappear at the boundary between M and L types.Comment: 60 pages, 25 figures. To appear in the Astrophysical Journal Vol 596.
