    Relationship between magnetic resonance imaging findings and histological grade in spinal peripheral nerve sheath tumors in dogs

    BackgroundPeripheral nerve sheath tumors (PNSTs) are a group of neoplasms originating from Schwann cells or pluripotent cell of the neural crest. Therapeutic options and prognosis are influenced by their degree of malignancy and location.Hypothesis/ObjectivesIdentify magnetic resonance imaging (MRI) features predictive of PNST histologic grade.AnimalsForty‐four dogs with histopathological diagnosis of spinal PNSTs and previous MRI investigation.MethodsA multicenter retrospective study including cases with (a) histopathologic diagnosis of PNST and (b) MRI studies available for review. Histologic slides were reviewed and graded by a board‐certified pathologist according to a modified French system (FNCLCC) for grading soft tissue sarcomas. The MRI studies were reviewed by 2 board‐certified radiologists blinded to the grade of the tumor and the final decision on the imaging characteristics was reached by consensus. Relationships between tumor grade and histological and MRI findings were assessed using statistical analysis.ResultsForty‐four cases met inclusion criteria; 16 patients were PNSTs Grade 1 (low‐grade), 19 were PNSTs Grade 2 (medium‐grade), and 9 were PNSTs Grade 3 (high‐grade). Large volume (P = .03) and severe peripheral contrast enhancement (P = .04) were significantly associated with high tumor grade. Degree of muscle atrophy, heterogeneous signal and tumor growth into the vertebral canal were not associated with grade.Conclusions and Clinical ImportanceGrade of malignancy was difficult to identify based on diagnostic imaging alone. However, some MRI features were predictive of high‐grade PNSTs including tumor size and peripheral contrast enhancement

    Clinical Features, Cardiovascular Risk Profile, and Therapeutic Trajectories of Patients with Type 2 Diabetes Candidate for Oral Semaglutide Therapy in the Italian Specialist Care

    Introduction: This study aimed to address therapeutic inertia in the management of type 2 diabetes (T2D) by investigating the potential of early treatment with oral semaglutide. Methods: A cross-sectional survey was conducted between October 2021 and April 2022 among specialists treating individuals with T2D. A scientific committee designed a data collection form covering demographics, cardiovascular risk, glucose control metrics, ongoing therapies, and physician judgments on treatment appropriateness. Participants completed anonymous patient questionnaires reflecting routine clinical encounters. The preferred therapeutic regimen for each patient was also identified. Results: The analysis was conducted on 4449 patients initiating oral semaglutide. The population had a relatively short disease duration (42%  60% of patients, and more often than sitagliptin or empagliflozin. Conclusion: The study supports the potential of early implementation of oral semaglutide as a strategy to overcome therapeutic inertia and enhance T2D management

    Stereotyping Roma people in Italy: IRT models for ambivalent prejudice measurement

    \u2018Gypsy\u2019 is a name that conjures up a wide variety of images in peoples\u2019 minds. Romanticised for their freedom or reviled for their antisocial behaviour, Roma people have in turns been the subject of both exotic myth and virulent prejudice. Roma are depicted as romantic or criminal outsiders: anything from thieves to talented and artistic people. Exploiting the potentiality of Item Response Theory models, this study aims to assess the level of consolidation of positive and negative stereotypes on Roma people in Italy. In addition, we investigate how socio-demographic covariates affect the degree of acceptance of a clich\ue9d depiction of Roma. Results suggest that images and representations of the \u2018Gypsies\u2019, which confirm a distinction between nature and culture, persist in our culture. On the one hand, Roma are perceived as \u2018free\u2019 from societal constrains (nature); on the other hand, they are portrayed as criminals and untruthful, relational qualities that arise in a societal organised condition (culture). This distinction continues to be misused to delegitimise minority groups, especially Roma

    The Influence of Temperature on the Larval Development of Aelurostrongylus abstrusus in the Land Snail Cornu aspersum

    The metastrongyloid Aelurostrongylus abstrusus has an indirect lifecycle involving gastropod intermediate hosts. The widespread snail Cornu aspersum is an efficient intermediate host of A. abstrusus. As the temperature may influence the developmental rate of metastrongyloids from first (L1) to the third infective larval stage (L3) inside molluscs, this study evaluated the effect of two controlled temperatures on the development of A. abstrusus in C. aspersum. Overall, 300 snails were infected with 500 L1 of A. abstrusus and kept at ∼25 °C. Fifteen days post infection (D15), the overall developmental rate to L3 (0.8%) was assessed in a subset of 20 snails. The remaining gastropods were divided in 2 groups, i.e., 180 still kept at ∼25 °C (G1) and 100 hibernated at ∼4 °C (G2). On D30, the larval development was evaluated in 20 snails from each group, while another batch of 80 snails was selected random from G1 and hibernated at ∼4 °C (G3). The larval developmental rate was determined digesting 20 snails from each of the three groups on D45, D60, and D75. The higher mean developmental rate was registered in G1 (3.8%) compared to G2 (1.9%) and G3 (2.3%), indicating that the development to L3 of A. abstrusus in C. aspersum is positively influenced by the increase of temperature

    Ultrasound-Guided Procedures in Rheumatology Daily Practice: Feasibility, Accuracy, and Safety Issues

    BACKGROUND/OBJECTIVE:The aims of this study were to describe in detail the ultrasound (US)-guided procedures in our daily rheumatology practice, to evaluate the feasibility and accuracy of US-guided procedures, and to test the efficacy of our disinfection protocol in preventing infectious complications. METHODS: This was a cross-sectional and monocentric study. Information regarding patients' age, sex, body mass index, rheumatic disease, US pathological findings, aspirated and/or injected anatomical site, US equipment (ie, probe type and frequency), and needle type was consecutively collected for each US-guided procedure in a third-level rheumatology center. RESULTS: A total of 643 US-guided procedures were performed, with a mean of 5.2 procedures per working-day. In 94.2% of the patients, only one procedure was carried out, whereas in 5.8%, more than one. The mean time was 7 ± 2.5 minutes. Ultrasound-guided procedures were highly accurate (accuracy rate higher than 95%) and safe (adverse events were reported in 0.8%). Our disinfection protocol was effective in preventing infectious complications. Probes with frequency values between 8 to 13 MHz and 20-, 21-, and 22-gauge needles were the most frequently used at shoulder, knee, wrist, elbow, and ankle level. High-frequency linear probes (ranging between 18 and 22 MHz) and 23- and 25-gauge needles were used for injecting small joints of the hands and feet. Convex low-frequency probe (2-7 MHz) and 18- and 20-gauge needles were the most used for performing hip joint aspirations and/or injections. CONCLUSIONS: This study reports useful information for setting up a service providing US-guided procedures in rheumatology and supports the feasibility, accuracy, and safety of US-guided procedures

    Multi-Systemic Alterations by Chronic Exposure to a Low Dose of Bisphenol A in Drinking Water: Effects on Inflammation and NAD+-Dependent Deacetylase Sirtuin1 in Lactating and Weaned Rats

    Bisphenol A (BPA) is largely used as a monomer in some types of plastics. It accumulates in tissues and fluids and is able to bypass the placental barrier, affecting various organs and systems. Due to huge developmental processes, children, foetuses, and neonates could be more sensitive to BPA-induced toxicity. To investigate the multi-systemic effects of chronic exposure to a low BPA dose (100 μg/L), pregnant Wistar rats were exposed to BPA in drinking water during gestation and lactation. At weaning, newborn rats received the same treatments as dams until sex maturation. Free and conjugated BPA levels were measured in plasma and adipose tissue; the size of cerebral ventricles was analysed in the brain; morpho-functional and molecular analyses were carried out in the liver with a focus on the expression of inflammatory cytokines and Sirtuin 1 (Sirt1). Higher BPA levels were found in plasma and adipose tissue from BPA treated pups (17 PND) but not in weaned animals. Lateral cerebral ventricles were significantly enlarged in lactating and weaned BPA-exposed animals. In addition, apart from microvesicular steatosis, liver morphology did not exhibit any statistically significant difference for morphological signs of inflammation, hypertrophy, or macrovesicular steatosis, but the expression of inflammatory cytokines, Sirt1, its natural antisense long non-coding RNA (Sirt1-AS LncRNA) and histone deacetylase 1 (Hdac1) were affected in exposed animals. In conclusion, chronic exposure to a low BPA dose could increase the risk for disease in adult life as a consequence of higher BPA circulating levels and accumulation in adipose tissue during the neonatal period
