    Pain perception and stabilometric parameters in people with chronic low back pain after a pilates exercise program: A randomized controlled trial

    Various exercise interventions, such as Pilates exercises and traditional physical therapy methods, are employed to decrease low back pain (LBP). Nonspecific low back pain (NSLBP) is distinct from LBP, however, as the distribution of pain is restricted to the region between the costal margin and the inferior gluteal. The aim of our randomized controlled trial was to evaluate the effects of a program of Pilates exercises on pain perception and stabilometric parameters in patients with NSLBP.Thirty-eight participants were randomly allocated, using a 1:1 scheme, to either the experimental group (EG) or control group (CG). The EG completed a 14-week program of Pilates exercises, performed thrice per week under the supervision of an exercise specialist, while the CG was managed with a social program only. Measures of posturography and Oswestry Disability Index (ODI) for pain perception were obtained at baseline (T0) and after the 14 weeks of intervention (T)1.Posturography measures improved for patients in the EG, with both eyes open and eyes closed (P\u200a<\u200a0.05). There were no statistical differences in posturography in the CG. ODI decreased significantly in both groups over the 14 weeks of the study protocol: EG, T0, 13.7\u200a\ub1\u200a5.0 compared with T1, 6.5\u200a\ub1\u200a4.0 (P\u200a<\u200a0.001); and CG, T0, 10.7\u200a\ub1\u200a7.8 compared with T1, 8.4\u200a\ub1\u200a7.8 (P\u200a<\u200a0.01). A greater extent of reduction in pain was achieved in the EG.The Pilates exercise program yielded improvements in pain and posturography outcomes. Our study also confirms the applicability of posturography in evaluating postural instability in patients with NSLBP. Due to our relatively small study group, future studies would be necessary to confirm our findings

    Factors influencing the development of otitis media among Sicilian children affected by upper respiratory tract infections

    Introduction: Upper respiratory tract infection is a nonspecific term used to describe an acute infection involving the nose, paranasal sinuses, pharynx and larynx. Upper respiratory tract infections in children are often associated with Eustachian tube dysfunction and complicated by otitis media, an inflammatory process within the middle ear. Environmental, epidemiologic and familial risk factors for otitis media (such as sex, socioeconomic and educational factors, smoke exposure, allergy or duration of breastfeeding) have been previously reported, but actually no data about their diffusion among Sicilian children with upper respiratory tract infections are available. Objective: To investigate the main risk factors for otitis media and their prevalence in Sicilian children with and without upper respiratory tract infections. Methods: A case-control study of 204 children with upper respiratory tract infections who developed otitis media during a 3 weeks monitoring period and 204 age and sex-matched healthy controls. Seventeen epidemiologically relevant features were inventoried by means of standardized questionnaires and skin tests were performed. Univariate analysis and multivariate logistic regression analysis were used to examine the association between risk factors and occurrence of otitis media. Results: Otitis media resulted strongly associated to large families, low parental educational attainment, schooling within the third years of life (p &lt;. 0.05); children were more susceptible to develop otitis media in the presence of asthma, cough, laryngopharyngeal reflux disease, snoring and apnea (p &lt;. 0.05). Allergy and urban localization increased the risk of otitis media in children exposed to smoke respectively of 166% and 277% (p &lt;. 0.05); the joint effect of asthma and presence of pets in allergic population increased the risk of recurrence of 11%, while allergy, cough and runny nose together increased this risk of 74%. Conclusions: Upper respiratory tract infections and otitis media are common childhood diseases strongly associated with low parental educational attainment (p = 0.0001), exposure to smoke (p = 0.0001), indoor exposure to mold (p = 0.0001), laryngopharyngeal reflux disease (p = 0.0002) and the lack of breast-feeding (p = 0.0014); an increased risk of otitis media recurrences was observed in the presence of allergy, persistent cough and runny nose (p = 0.0001). The modification of the identified risk factors for otitis media should be recommended to realize a correct primary care intervention

    Effects of pilates exercise programs in people with chronic low back pain: a systematic review

    The Pilates method has recently become a fast-growing popular way of exercise recommended for healthy individuals and those engaged in rehabilitation. Several published studies have examined the effects of Pilates method in people with chronic low back pain (LBP).The objective of this study is to describe and provide an extensive overview of the scientific literature comparing the effectiveness of the Pilates method on pain and disability in patients with chronic nonspecific LBP. The study is based on the data from the following sources: MEDLINE-NLM, MEDLINE-EBSCO, Scopus Elsevier, Cochrane, DOAJ, SciELO, and PLOSONE.Original articles and systematic reviews of adults with chronic nonspecific LBP that evaluated pain and/or disability were included in this study; studies in which the primary treatment was based on Pilates method exercises compared with no treatment, minimal intervention, other types of intervention, or other types of exercises.The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) were adopted. The literature search included 7 electronic databases and the reference list of relevant systematic reviews and original articles to July 2014. Two independent investigators conducted the literature search and performed the synthesis as follows: Study Design; Sample (n); Disability measure; Intervention; and Main results.The searches identified a total of 128 articles. From these, 29 were considered eligible and were included in the analysis. The items were stratified as follows: Pilates method versus other kind of exercises (n = 6 trials) and Pilates method versus no treatment group or minimal intervention for short-term pain (n = 9 trials); the therapeutic effect of the Pilates method in randomized cohorts (n = 5); and analysis of reviews (n = 9).We found that there is a dearth of studies that clearly demonstrates the efficacy of a specific Pilates exercise program over another in the treatment of chronic pain. However, the consensus in the field suggests that Pilates method is more effective than minimal physical exercise intervention in reducing pain. These conclusions need to be supported by other proper investigations

    Influence of occlusal vertical dimension on cervical spine mobility in sports subjects

    Introduction: In the scientific literature, a number of studies have reported conflicting results regarding the effects of occlusal vertical dimension (OVD) on sports-related skills. The purpose of this study was to increase OVD in sports subjects so as to specifically investigate the influence on cervical spine mobility. In particular, we measured cervical range of motion (ROM) before and after increasing OVD in individuals, either with or without malocclusion, analyzing both sports and sedentary subjects. Materials and methods: Participants were divided into two groups: a sports group (SG) and a control group (CG), each including 18 subjects. The SG was composed of sports subjects (age: 20.11±3.45 yrs; BMI: 25.39±2.32 kg/m2), whereas the CG consisted of age-matched sedentary subjects (age: 25.78±2.26 yrs; BMI: 24.88±2.87 kg/m2). Cervical range of motion (ROM) was evaluated, by way of an accelerometer (Moover®, Sensor Medica®), before (pre-test) and after (post-test) increasing OVD. Results: The main finding of this study was that sports subjects showed no significant difference, compared to control subjects, in cervical ROM in response to increased OVD. Moreover, we found that sports and sedentary subjects alike showed no significant change in cervical spine mobility as a result of increased OVD, regardless of whether they were affected by malocclusion (class II) or represented subjects with normocclusion (class I). Conclusion: In accord with several studies reported in the literature, the findings of our study indicate that occlusal splints failed to significantly improve the physical-performance endpoint measured, i.e. cervical ROM, in sports subjects as compared to sedentary individuals. Due to the paucity of studies, characterized by conflicting results, there is as yet no compelling scientific evidence as to whether OVD positively impacts sports performance or not. Accordingly, we suggest that further scientific investigation, regarding the relationship between sports performance and OVD, be conducted in the field of sport and exercise sciences

    Mediterranean diet and dietary protein supplementation as possible predicting variables of weight management: An update of the protein project

    Introduction: The Mediterranean diet is known to support the prevention of several chronic diseases and excessive weight gains. However, relevant investigations were performed in a clinical setting and healthy and physical active individuals were not considered. The aim of this study was therefore to understand the effects of protein supplements intake on body mass index (BMI) in healthy active individuals following the Mediterranean diet. Materials and methods: A face-to-face questionnaire was administered to 667 subjects, 627 living in a Mediterranean area (MD) and 40 in a non-Mediterranean area (NMD); questionnaire enquired dietary behaviors, including a comparison between dietary patterns and protein consumption between these two populations and within each population. Results: Dietary patterns significantly varied between the MD and NMD populations (p < 0.001), although, BMI was not significantly different. The major significant differences were found between the BMI of protein supplement users and non-protein supplement users within and between the two populations (p < 0.01). No differences were found between the BMI of the protein supplement users of MD and NMD, while moderate differences were displayed between the non-supplement users of the two populations (p < 0.05). Conclusions: The Mediterranean diet does not appear to affect BMI. However, BMI significantly differed between protein supplement users and non-protein supplements users indicating a direct influence of such products on body weight and consequently BMI

    Comparison of Postural Features and Muscle Strength between Children with Idiopathic Short Stature and Healthy Peers in Relation to Physical Exercise

    Previous research has reported that children with idiopathic short stature (ISS) showed functional and cognitive impairments. The purpose of this study was to compare muscle strength and body posture between children with ISS treated with growth hormone (GH) and healthy peers (healthy children, HC), and to analyze whether these parameters were affected by physical exercise. Eighteen children for the ISS group (mean age: 10.96 ± 1.68 years) and 26 children for the HC group (mean age: 10.19 ± 1.06 years) were recruited for the study. All participants performed the following assessments: handgrip and Sargent test for the muscle strength evaluation; baropodometric and stabilometric test for the posturographic measures. Data were analyzed with analysis of covariance (ANCOVA) using height and weight as covariate. Groups were then stratified into active and inactive and independent t-tests were used to determine differences between variables. Significance level was set to p &lt; 0.05. Our results showed a significantly lower performance for both hands (p &lt; 0.01) and a greater difference of plantar loading distribution between feet in the ISS compared to the HC groups (p &lt; 0.01). In relation to physical exercise, the HC active group showed the highest handgrip strength values for both hands among the analyzed groups and, moreover, handgrip strength of both ISS active and inactive groups was significantly lower than corresponding CH peers for both hands. Although ISS and HC inactive groups reported an unequal plantar loading distribution between feet (p &lt; 0.05), this asymmetry was not present in both ISS and HC active groups. We assume that GH therapy integrated with physical exercise in young patients with ISS could be suggested to increase muscle strength and body posture improving their quality of life

Case presentation: The authors report the 5th case of EMP of the tonsil in the literature occurred in a 57-year-old Caucasian male. Conclusions: Through a review of the relevant literature, we consider adjuvant radiotherapy not necessary for EMP of tonsil because of an adequate resection achieved by surgery

    The relation between moderate hearing loss with balance and postural control

    Background. Balance is a complex process that involves multiple sensory integrations. The auditory, visual and vestibular systems are the main contributors. Hearing loss or hearing impairment may induce inappropriate postural strategies that could affect the spine and consequently balance. The aim of this study was to understand if hearing loss could influence balance and posture Methods. 13 patients (61±13 year; 161.8±11.0 cm; 70.5±15.9 Kg) with moderate hearing loss (Right ear -60±21 dB; Left ear -61±24 dB) underwent: 1) an audiometric examination 2) a postural examination (with open and closed eyes) through a stabilometric platform, and 3) a sternocleidomastoid EMG examination. Results. No differences were found between right and left hemibody between the audiometric, posturographic, and the EMG amplitude. EMG parameters have shown no association with hearing loss, for both right or left head rotation. Multiple regression analysis has shown a negative regression coefficient (R2 -0.69) between hearing loss and the posturographic parameters. Conclusions. Hearing loss is associated to increased posturographic measures (CoP, ellipse X and Y deviations) underlining a reduced postural control in people with hearing impairment. No association has been found between neck activation and hearing loss
