    SPINE20 A global advocacy group promoting evidence-based spine care of value

    PURPOSE: The Global Burden of Diseases (GBD) Studies have estimated that low back pain is one of the costliest ailments worldwide. Subsequent to GBD publications, leadership of the four largest global spine societies agreed to form SPINE20. This article introduces the concept of SPINE20, the recommendations, and the future of this global advocacy group linked to G20 annual summits. METHODS: The founders of SPINE20 advocacy group coordinated with G20 Saudi Arabia to conduct the SPINE20 summit in 2020. The summit was intended to promote evidence-based recommendations to use the most reliable information from high-level research. Eight areas of importance to mitigate spine disorders were identified through a voting process of the participating societies. Twelve recommendations were discussed and vetted. RESULTS: The areas of immediate concern were "Aging spine," "Future of spine care," "Spinal cord injuries," "Children and adolescent spine," "Spine-related disability," "Spine Educational Standards," "Patient safety," and "Burden on economy." Twelve recommendations were created and endorsed by 31/33 spine societies and 2 journals globally during a vetted process through the SPINE20.org website and during the virtual inaugural meeting November 10-11, 2020 held from the G20 platform. CONCLUSIONS: This is the first time that international spine societies have joined to support actions to mitigate the burden of spine disorders across the globe. SPINE20 seeks to change awareness and treatment of spine pain by supporting local projects that implement value-based practices with healthcare policies that are culturally sensitive based on scientific evidence

    Lo scopo di questo lavoro è capire come funzionasse la mediazione linguistica tra popoli di continenti diversi e lontani. L’elaborato finale si sviluppa in quattro macrocapitoli. Nel primo parlerò della comunicazione e della mediazione in senso generale e cercherò di fornirne una panoramica a tinte storiche e sociali. Nel secondo accennerò alla figura del mediatore culturale dal punto di vista storico e la analizzerò in un contesto preciso: la colonizzazione delle Americhe da parte degli europei, nei secoli XVI e XVII. Il terzo macrocapitolo introduce due personaggi chiave (Gerónimo de Aguilar e Gonzalo Guerrero) che legano la mediazione di epoca coloniale alla figura de La Malinche e pertanto questo funge da ponte tra il secondo ed il quarto. Nell'ultimo macrocapitolo proverò a raccontare la storia de La Malinche, l'interprete personale del conquistador spagnolo Hernán Cortés, durante la colonizzazione della penisola dello Yucatán, in Messico. Le conclusioni alle quali sono giunto coincidono con le risposte concrete al mio quesito su come funzionasse la mediazione linguistica nel mondo antico. In particolare ho individuato alcune vie principali attraverso le quali è nata la figura del mediatore-interprete. Nello specifico, queste vie sono: l'intreccio amoroso tra conquistatori e donne indigene; lo studio delle lingue native da parte degli ecclesiasti; il rapimento di giovani indigeni o lo scambio di giovani tra europei e nativi così che apprendessero la lingua degli altri e viceversa; lo sfruttamento di schiavi o servi che avevano appreso la lingua tramite la cosiddetta “immersione”; l'inserimento in tribù indigene di giovani avventurieri europei i quali imparavano la lingua per immersione; il caso

    Objectives In the context of pediatric sports injuries, the epiphyseal and apophyseal knee fractures represent rather peculiar lesions. The most frequently involved anatomical area is the knee. The peculiar function of the physis and the need to preserve their integrity, makes choosing what treatment methods to employ very important. Objective of this study is to assess the kind and the effectiveness of the most suitable treatment in the apophyseal and epiphyseal knee lesions occurring in the adolescents Materials and Methods From 2006 to 2011, were treated 41 patients (34 M-7 F) between the ages of 10 and 15, with a diagnosis of traumatic knee injury caused by sports activities. Traumatic physeal fractures of the distal femur, the proximal tibia and its anterior tuberosity and the avulsion of the intercondylar eminence were the lesions that occurred most frequently. The treatment belonged to the type of lesion: closed reduction or percutaneous fixation with K-wires/ screws and a femoral-podalic plaster cast, ORIF with K-wires/screws, arthroscopic reduction and internal fixation using absorbable screws. All the patients were given the POSNA questionnaire at the end of the follow up. Results The follow up was on average 5 years (4-10 years). We considered as excellent the results obtained in 26 patients, as fair in 12 patients, in 1 case the result obtained was considered as poor. 2 cases were lost during follow up. The average POSNA score at the end of the follow-up was 98.51. Any early complications recorded were the following: in 1 case infection of the K-wires 32 days after pinning and a reported compression of the popliteal neurovascular bundle, due to a displaced tibial physeal fracture. Discussion Since sport during childhood and adolescence is now practiced more and more frequently, also at a competitive level, these kinds of fractures in children between the ages of 10 and 15 have been occurring more often, especially in male patients. Sports traumatology of the knee in this age group is characterized by a typology of injuries that are very particular. The knowledge of the anatomy and physiology of children, with an appropriate diagnostic assessment, is essential to identify the most appropriate treatment options for each specific injury. As the nucleus of proximal tibial growth plate progressively closes from posterior to medial side, in patients between 11 and 13 years of age with an apophyseal displacement of the tibial tuberosity, you should always perform a CT exam, to exclude an intra-articular physeal fracture

    High Prevalence of Scoliosis in a Large Cohort of Patients with Prader-Willi Syndrome

    The characteristics of scoliosis were investigated in a large cohort of children and adults with Prader–Willi syndrome (PWS), analysing the role of age, gender, puberty, body mass index (BMI), genotype and growth hormone therapy (GHT) on its onset and severity. A retrospective cross-sectional study was performed in 180 patients with genetically confirmed PWS (96 females), aged 17.6 ± 12 years. Eighty-five subjects (47%) were obese. One hundred and fifty subjects (83.3%) were on GHT, while 30 patients had never been treated. Overall, 150 subjects (83.3%) were affected by scoliosis, 80.2% of children and adolescents and 87.8% of adults. A mild degree of scoliosis was observed in 58 patients (38.7%), moderate in 43 (28.7%) and severe in 49 (32.6%). Median age at diagnosis of scoliosis was 6.3 years, while the severe forms were diagnosed earlier (median age: 3.8 years). The cumulative probability at 5 years of age was equal to 0.403 and almost doubled at 15 years. No significant associations were found between scoliosis and genotype, gender, pubertal stage, GHT and BMI. A corset was prescribed to 75 subjects (50%) at a median age of 7.5 years, while 26 subjects (17.3%) underwent surgery at a median age of 13.1 years. Our data indicate that scoliosis is one of the major concerns for PWS patients that increases with age, and therefore suggest the need for regular systematic monitoring of spinal deformity from paediatric age
