    The Role of Oxytocin in the Stress and Anxiety Response.

    Centrally released oxytocin (OT) is believed to attenuate stress-induced activation of the hypothalamic pituitary adrenal (HPA) axis as well as being anxiolytic. Therefore, it is expected that OT deficient (OT-/-) mice that do not synthesize or release OT centrally or peripherally will display enhanced HPA axis activation, as well as increased anxiety-related behavior compared to wildtype (OT+/+) mice. To test this hypothesis, OT-/- mice were exposed to shaker stress, (psychogenic stressor), cholecystokinin- (CCK) administration (physical stressor), or the elevated plus maze (EPM), a behavioral test of anxiety. Female OT-/- mice released more corticosterone than OT+/+ mice in response to shaker stress. Shaker stress exposure activated Fos in OT neurons of the paraventricular nucleus of the hypothalamus (PVN) of male and female OT+/+ mice and corticotropin-releasing hormone (CRH) within the PVN of male and female mice of both genotypes. In addition, shaker stress exposure revealed that Fos expression in the medial nucleus of the amygdala (MeA) was lower in female OT-/- than OT+/+ mice. Genotypic differences in corticosterone release and Fos activation of the MeA in response to shaker stress exposure were not observed in male mice. Furthermore, similar genotypic (and/or sex) differences were not revealed in response to CCK-administration. OT is also anxiolytic in female mice. Female OT-/- mice tested in the EPM displayed increased anxiety-related behavior compared to OT+/+ mice. In response to EPM exposure Fos expression in the MeA was greater in female OT-/- mice than OT+/+ mice. Surprisingly, male OT-/- mice tested in the EPM displayed decreased anxiety-related behavior compared to OT+/+ mice, but did not display genotypic differences in the Fos expression within the MeA. The results of this thesis suggest that OT is anxiolytic and attenuates HPA activation in female, but not male mice. Furthermore, it appears that OT plays a modulatory role in the processing of psychogenic stressors, but may not be involved in the processing of physical or systemic stressors. More specifically, it is possible that OT plays a role in behavioral and physiological responses that depend upon neuronal processing within the MeA

    Pro-Ana Web-Log Uses and Gratifications towards Understanding the Pro-Anorexia Paradox

    Eating disorders have the highest mortality rate of all mental illnesses. Currently, web-logs are hosts to thousands of pro-ana (short for pro-anorexia) blogring participants, joining together with a common claim that eating disorders are lifestyles not illnesses. This study explored participation motives among pro-ana blogring participants, relationships to individual psycho-social factors, and to unique web-log features. Results from the cross-sectional online survey suggest three main participation motives among blogring participants (community, obtaining social/ emotional support, and ego-protection). Results also indicate individual factors such as; social support satisfaction, drive-for-thinness, and BMI were related to blogger motives in a various diverse ways. Analyses of the findings from the present study reflect the self-disclosive nature of the blogging process. The results have several implications for eating disorder, computer-mediated communication, internet social support, and blog/blogring research

    25-hydroxyvitamin D levels and bone histomorphometry in hemodialysis renal osteodystrophy.BackgroundThe importance of 25-hydroxyvitamin D (25-OHD) serum levels in hemodialysis chronic renal failure has not been so far histologically evaluated. Information still lacking relate to the effect of 25-OHD deficiency on serum parathyroid hormone (PTH) levels and on bone and its relationship with calcitriol levels.MethodsThis retrospective study has been performed on a cohort of 104 patients on hemodialysis from more than 12 months, subjected to transiliac bone biopsy for histologic, histomorphometric, and histodynamic evaluation. The patients, 61 males and 43 females, mean age 52.9 ± 11.7 years, hemodialysis length 97.4 ± 61.4 months, were treated with standard hemodialysis and did not receive any vitamin D supplementation. Treatment with calcitriol was not underway at the time of the biopsy. Transiliac bone biopsies were performed after double tetracycline labels. In addition, serum intact PTH (iPTH), alkaline phosphatase, and 25-OHD were measured. Calcitriol serum levels was also measured in a subset of patients (N = 53). The patients were divided according to serum 25-OHD levels in three groups: (1) 0 to 15 (15 patients), (2) 15 to 30 (38 patients), and (3) >30 ng/mL (51 patients).ResultsThere was no significant difference in average age, hemodialysis age, serum PTH [490 ± 494, 670 ± 627, and 489 ± 436 pg/mL, respectively (mean ± SD)], alkaline phosphatase, and calcitriol between the three groups. The parameters double-labeled surface, trabecular mineralizing surface, and bone formation rate were significantly lower in group 1 than in the other groups (P < 0.03, < 0.03, and < 0.02, respectively). Osteoblast surface and adjusted apposition rate were borderline significantly lower in group 1 (P < 0.06 and < 0.10). There was no statistical difference in the biochemical and bone parameters between groups 2 and 3. A positive significant correlation was found between several bone static and dynamic parameters and 25-OHD levels in the range 0 to 30 ng/mL, showing a vitamin D dependence of bone turnover at these serum levels. However, actual evidence of an effect on bone of 25-OHD deficiency was found at serum levels below 20 ng/mL. With increasing 25-OHD levels beyond 40 ng/mL, a downslope of parameters of bone turnover was also observed.ConclusionSince PTH serum levels are equally elevated in low and high 25-OHD patients, while calcitriol levels are constantly low, an effect of 25-OHD deficiency (group 1) on bone, consisting of a mineralization and bone formation defect, can be hypothesized. The effect of vitamin D deficiency or bone turnover is found below 20 ng/mL. The optimal level of 25-OHD appears to be in the order of 20 to 40 ng/mL. Levels of the D metabolite higher than 40 ng/mL are accompanied by a reduction of bone turnover

    Epidemiología y manejo de las fracturas de fémur por arma de fuego. Nuestra experiencia

    Introducción: Cada vez con más frecuencia, la población civil sufre lesiones por proyectil de arma de fuego. El 57% de los pacientes presenta compromiso óseo y la fractura de fémur es la más común. La elevada incidencia y la ausencia de un protocolo estandarizado para su tratamiento motivaron este estudio. Materiales y Métodos: Estudio retrospectivo, descriptivo. Entre 2019 y 2021, se incluyeron pacientes con fracturas de fémur causadas por arma de fuego. Se analizaron las siguientes variables: región anatómica involucrada, clasificación, tratamiento y complicaciones. Resultados: La muestra incluyó a 35 pacientes, 25 (71,43%) con fracturas completas y 10 (28,57%), con fracturas incompletas. Según la localización, el fémur distal fue la zona más afectada (48,57%). Veintiséis pacientes fueron tratados mediante reducción y osteosíntesis y 9, de forma incruenta. Conclusiones: Recurrimos a una clasificación sencilla que divide a las fracturas en completas o incompletas. Todas las fracturas completas se consideraron inestables independientemente de su localización; y las incompletas, estables, salvo las del tercio proximal, donde es conveniente realizar una fijación profiláctica. Las fracturas diafisarias incompletas pueden tratarse de forma incruenta y todas las fracturas completas se trataron con reducción y osteosíntesis. El clavo endomedular es el método de elección para las fracturas en las zonas I y II. En la zona III, se requiere un análisis individualizado para cada patrón. Creemos que el manejo inicial y la correcta selección del implante según la zona afectada son factores determinantes para lograr resultados satisfactorios

    Screening for neurocysticercosis in internationally adopted children: yield, cost and performance of serological tests, Italy, 2001 to 2016

    IntroductionNeurocysticercosis (NCC) is one of the leading causes of epilepsy worldwide. The majority of cases in Europe are diagnosed in immigrants. Currently in Italy, routine serological screening for cysticercosis is recommended for internationally adopted children (IAC) coming from endemic countries. Methods: We retrospectively analyse the results of the serological screening for cysticercosis in IAC 16 years old or younger, attending two Italian third level paediatric clinics in 2001-16. Results: Of 2,973 children included in the study, 2,437 (82.0%) were screened by enzyme-linked immune electro transfer blot (EITB), 1,534 (51.6%) by ELISA, and 998 (33.6%) by both tests. The seroprevalence of cysticercosis ranged between 1.7% and 8.9% according to EITB and ELISA, respectively. Overall, 13 children were diagnosed with NCC accounting for a NCC frequency of 0.4% (95% confidence interval (CI): 0.2-0.6%). Among the 168 seropositive children, only seven (4.2%) were diagnosed with NCC. Of these children, three were asymptomatic and four presented epilepsy. Among seronegative children (n\u2009=\u20092,805), seven presented with neurological symptoms that lead to the diagnosis of NCC in six cases. The sensitivity, specificity, positive and negative predictive value for the diagnosis of NCC were 54.5%, 98.6%, 14.6%, 99.8% for EITB and 22.2%, 91.1%, 1.4%, 99.5% for ELISA. The yield of the screening programme was 437 NCC cases per 100,000. The number needed to screen to detect one NCC case was 228. The cost per NCC case detected was EUR 10,372. Conclusion: On the base of our findings we suggest the ongoing serological screening for cysticercosis to be discontinued, at least in Italy, until further evidence in support will be available

    No confirmed cases of Taenia solium taeniasis in a group of recently arrived Sub-Saharan migrants to Italy

    One-hundred and sixty-four migrants from Sub-Saharan Africa to Italy were screened with the Taenia solium specific enzyme-linked immunosorbent assay coproantigen (ELISA CoAg) and four (2.4%) were recorded as positive, but with optical density values near to the cut-off. No ELISA CoAg positive samples were confirmed by parasitological methods. Low positivity could be attributed to false positive result or cross-reaction with other Taenia species. Further studies are needed to assess the role of migration on sporadic autochthonous transmission of T. solium taeniasis/cysticercosis in Europe
