    Ablation of Post Transplant Atrial Flutter and Pseudo-fibrillation Using Magnetic Navigation via a Superior Approach.

    Ablation of cavotricuspid ishtmus flutter and atrial tachycardia in a complex substrate has never been reported using remote navigation via superior approach. Venous access was obtained via right internal jugular for ablation and left subclavian for duodecapolar catheter placement into the coronary sinus. In a posttransplant patient presenting with both regular and irregular tachycardia, both cavotricuspid isthmus flutter in the donor and atrial tachycardia in the recipient was mapped using a two catheter approach. Successful ablation of typical atrial flutter and anastomotic block was achieved. This is the first report of successful ablation of cavotricuspid isthmus flutter and posttransplant atrial tachycardia using magnetic navigation via superior approach. Using only two catheters, this approach is logical and feasible in complex substrates with interrupted inferior venous access

    Influence of PVD AlCrN Coating and Surface Treatments on Characteristics and Performance of Cutting Tools during Machining of Incoloy 825

    With the increasing levels technology, advanced engineering materials have been developed regularly to fulfill the dedicated application effectively. These are categorized under difficult-to-cut materials owing to their peculiar properties. The machining of these advanced superalloys demands the cutting tools with superiority in various physical, mechanical properties and functional attributes. Although coated tools are there to serve this purpose, still there is significant scope to augment the properties and performance of coated tools in environment friendly dry machining. One of the possible ways to enhance the properties of coated tools is by surface treatment. The current work employed microblasting prior to and after the coating deposition owing to it’s promising results as a surface treatment. AlCrN coating is deposited on cemented carbide tool insert by physical vapor deposition (PVD) with prior as well as post treatment to recommend an optimal treatment technique. Critical examination of physical and mechanical characteristics such as surface morphology, microstructure, chemical composition, crystallite size, lattice strain, surface roughness and residual stress has been made to reveal the influence of micro-blasting prior and after the coating deposition on PVD coated tools. Evaluated the machining performance of coated tools with surface treatment during dry turning of Incoloy 825 in terms of cutting force, machined surface roughness and tool wear. Uncoated tool considered as reference to compare the machining performance. The results of the work witnessed the superior performance of AlCrN coated tool with pre-treatment only because its stronger adhesion and higher wear resistance in dry turning

    Catheter ablation of accessory pathways near the coronary sinus: value of defining coronary arterial anatomy.

    BackgroundAccessory pathways can lie near or within the coronary sinus (CS). Radiofrequency catheter ablation of accessory pathways is a well-established treatment option, but this procedure can cause damage to adjacent coronary arteries.ObjectiveThe purpose of this study was to evaluate the anatomic relationship between the coronary arteries and the CS.MethodsRetrospective data of patients who underwent catheter ablation of supraventricular tachycardia between June 2011 and August 2013 was reviewed. In addition, detailed analysis of coronary computed tomographic angiography (CTA) data from 50 patients was performed.ResultsBetween June 2011 and August 2013, 427 patients underwent catheter ablation of supraventricular tachycardia, of whom 105 (age 28 ± 17 years, 60% male) had accessory pathway-mediated tachycardia. Of these, 23 patients had accessory pathways near the CS, and 60% (N = 14) underwent concurrent coronary angiography. In 4 patients, the posterolateral (inferolateral) branch (PLA) of the right coronary artery was in close proximity to the CS, and 2 patients (18%) had stenosis of the PLA at the site of ablation. On CTA at their closest proximity, the PLA was 1.9 ± 1.3 mm and the left circumflex artery (LCx) was 2.0 ± 0.8 mm from the body of the CS, in right and left coronary artery-dominant patients, respectively. CS ostium and PLA were 3.6 ± 1.9 mm apart. In left-dominant patients, LCx and CS ostium were 3.8 ± 1.2 mm apart.ConclusionThe PLA and LCx are in close proximity to the anteroinferior aspect of the CS ostium and proximal CS. The relationship of the CS and coronary arteries should be evaluated before ablation at these sites

    Quantitative Analysis of Localized Sources Identified by Focal Impulse and Rotor Modulation Mapping in Atrial Fibrillation

    BackgroundNew approaches to ablation of atrial fibrillation (AF) include focal impulse and rotor modulation (FIRM) mapping, and initial results reported with this technique have been favorable. We sought to independently evaluate the approach by analyzing quantitative characteristics of atrial electrograms used to identify rotors and describe acute procedural outcomes of FIRM-guided ablation.Methods and resultsAll FIRM-guided ablation procedures (n=24; 50% paroxysmal) at University of California, Los Angeles Medical Center were included for analysis. During AF, unipolar atrial electrograms collected from a 64-pole basket catheter were used to construct phase maps and identify putative AF sources. These sites were targeted for ablation, in conjunction with pulmonary vein isolation in most patients (n=19; 79%). All patients had rotors identified (mean, 2.3±0.9 per patient; 72% in left atrium). Prespecified acute procedural end point was achieved in 12 of 24 (50%) patients: AF termination (n=1), organization (n=3), or >10% slowing of AF cycle length (n=8). Basket electrodes were within 1 cm of 54% of left atrial surface area, and a mean of 31 electrodes per patient showed interpretable atrial electrograms. Offline analysis revealed no differences between rotor and distant sites in dominant frequency or Shannon entropy. Electroanatomic mapping showed no rotational activation at FIRM-identified rotor sites in 23 of 24 patients (96%).ConclusionsFIRM-identified rotor sites did not exhibit quantitative atrial electrogram characteristics expected from rotors and did not differ quantitatively from surrounding tissue. Catheter ablation at these sites, in conjunction with pulmonary vein isolation, resulted in AF termination or organization in a minority of patients (4/24; 17%). Further validation of this approach is necessary
