    Comparison of Two Lipid Emulsions on the Incidence of Parenteral Nutrition Associated Cholestasis in Neonates

    OBJECTIVE: Lipid emulsion contributes to parenteral nutrition associated cholestasis (PNAC). For decades, soybean oil-based intravenous lipid emulsion (SO-ILE) was the predominant product. Recently, a multicomponent lipid emulsion containing soybean oil, medium-chain triglycerides, olive oil and fish oil (SMOF-ILE) has been used off-label in neonatal care. This study evaluates the incidence of PNAC in neonates who received SMOF-ILE or SO-ILE. METHODS: This was a retrospective review of neonates who received SMOF-ILE or SO-ILE for at least 14 days. Patients receiving SMOF-ILE were matched based on gestational age (GA) and birth weight to a historical cohort receiving SO-ILE. The primary outcomes were the incidences of PNAC among all patients and patients without intestinal failure. The secondary outcomes were clinical outcomes and incidence of PNAC stratified by GA. Clinical outcomes included liver function tests, growth parameters, and development of retinopathy of prematurity and intraventricular hemorrhage. RESULTS: Forty-three neonates who received SMOF-ILE were matched to 43 neonates who received SOILE. There were no significant differences in baseline characteristics. The incidence of PNAC in the total population was 12% in the SMOF-ILE cohort and 23% in the SO-ILE cohort (p = 0.26). The lipid dosage of SMOF-ILE was significantly higher at time of peak direct serum bilirubin concentration compared with SO-ILE cohort (p = 0.05). Clinically significant differences were noted in laboratory endpoints in several subgroups. CONCLUSIONS: There was no significant difference in the incidence of PNAC among neonates in a SMOFILE cohort compared with a historical SO-ILE cohort

    Oil bearing seasonal crops in India:energy and phytoremediation potential

    Purpose: A variety of biomass plantations are being raised for energy production. This case study is on energy production potential of seasonal oil bearing crops in India. These crops have the advantage of producing oil (liquid fuel) as well as biomass as agro residue (solid fuel). The purpose of the study is to estimate total energy yields of oil bearing crops and compare with other types of energy plantations. Also oil bearing crops bioaccumulate metals and thus phytoremediate soil. This provides scope for waste water irrigation. Design/methodology/approach: Relevant published papers on energy production by raising oil bearing crops have been analyzed. The effect of waste water irrigation and agronomic practices on increasing productivity is given special attention. Findings: It is shown that the seasonal oil bearing crops such as castor have a high potential to generate energy and this is comparable to energy produced by many perennial grasses. The energy yields of castor under irrigated condition was 196×103 MJ/ha and this is comparable to the reed canary grass which yields 195×103 MJ/ha. Some of the oil bearing crops are also super accumulators of certain toxic metals. Research limitations/implications: In this study, only all the accessible papers on the topic could be analyzed. Practical implications: This case study indicates that raising oil bearing crops such as castor using waste water has many advantages which include high energy yields, utilization of waste water for productive purpose and phytoremediation of soil. Originality/value: The comparison made between various types of energy crops for their energy generation is an original contribution. Findings of economic and environmental benefits by waste water irrigation are also of value

    Randomized Controlled Trial of Enteral Vitamin D Supplementation (ViDES) in Infants \u3c28 Weeks Gestational Age or \u3c1000 G Birth Weight: Study Protocol

    BACKGROUND: Vitamin D is necessary to develop healthy lungs and other organs early in life. Most infants born before 28 weeks\u27 gestation have low vitamin D levels at birth and a limited intake during the first month. Enteral vitamin D supplementation is inexpensive and widely used. The appropriate supplementation regimen for extremely preterm infants is controversial, and the effect of different regimens on their blood levels and outcomes is unclear. METHODS: Randomized, blinded comparative effectiveness trial to compare two vitamin D supplementation regimens for inborn infants(400 IU/day with established feedings) or increased supplementation (800 IU/day with any feedings) during the first 28 days after birth. We hypothesize that the higher and early vitamin D dose (800 IU/day with early feeding) compared to placebo plus routine dose (400 IU/day with established feeding) will substantially increase total 25-hydroxyvitamin D3 levels measured as state-of-art at 1 month, reduce respiratory support at 36 weeks\u27 postmenstrual age (on an ordinal scale predictive of later adverse outcomes), and improve or at least not worsen other important secondary outcomes. The infants in the study will follow up at 22-26 months\u27 corrected age (~2 years) with blinded certified examiners to evaluate neurodevelopmental outcomes. The sample size of a minimum of 180 infants provides \u3e90% power to detect a \u3e95% posterior probability of a 33% increase in serum 25-hydroxy vitamin D3 and \u3e80% power to detect a \u3e80% posterior probability of a relative risk decrease of 20% of reducing respiratory support by intention-to-treat Bayesian analyses using a neutral prior probability. DISCUSSION: Our study will help clarify the uncertain relationship of vitamin D supplementation and its associated serum metabolites to clinical outcomes of extremely preterm infants. Confirmation of our hypotheses would prompt reconsideration of the supplementation regimens used in extremely preterm infants and justify a large multicenter study to verify the generalizability of the results

    Comparative evaluation of real-time PCR and conventional RT-PCR during two year surveillance for Influenza and RSV among children with acute respiratory infections in Kolkata reveals distinct seasonality of infection

    Acute respiratory tract infections (ARTI) are one of the most common cause of morbidity and mortality in young children all over the world. Influenza and Respiratory Syncytial viruses (RSV) are the predominant etiology during seasonal epidemics and thus rapid and sensitive molecular tests for screening & timely identification of epidemics are required. In this study we report comparison of real time PCR (Q-PCR) with conventional RT-PCR for parallel identification of Influenza A or B (Inf-A or -B) and RSV. A total of 1091 respiratory samples were examined from children with suspected ARTI during January 2007- December 2008. Of these 1091 samples, 275 (25.21%) were positive for either Influenza or RSV by Q-PCR compared to 262 (24%) positives by RT-PCR. Overall Inf-A, -B and RSV were detected in a total of 121 (11.075%), 59 (5.38%) and 95 (8.68%) samples respectively. In spite of overlapping clinical symptoms, RSV and Influenza showed distinct seasonal peaks. Inf-A positively and RSV, negatively correlated with rainfall and temperature. No distinct seasonality was observed in Inf-B infections. This is the first report of a systemic surveillance of respiratory viruses with seasonal correlation and prevalence rate from Eastern India. The two year comparative analysis also confirmed feasibility of using Q-PCR in developing countries, which will not only improve scope for prevention of epidemics but also provide crucial epidemiological data from the tropical regions

    Analysis of crystallization phenomenon in Indian honey using molecular dynamics simulations and artificial neural network

    Molecular dynamics simulation was performed on sugar profile and moisture content-based mixture systems of six Indian honey samples. Comparative studies were performed to understand the interactive effects of fructose, glucose, sucrose, maltose and water on crystallization. All simulations led to formation of stable crystal but with different interaction energies. Post-simulation analysis showed that Fructose:Glucose of 1.18 formed the most stable crystal with highest van der Waals and electrostatic interactions. The stability of crystal was further validated with least gyration radius (209±1.81 nm2), accessible surface area (4.09±0.04 nm) and root mean square displacement (3.51±0.00261nm). Results indicated that not only Fructose:Glucose ratio but also sucrose, maltose and water had a significant effect on the overall crystallization process. The simulation data was used to train the artificial neural network which predicted the stability of honey crystallization depending on Fructose:Glucose and Glucose:Water ratios

    Induced Systemic Resistance and Evaluation of Bio-control Agents for Management of Pigeonpea Wilt Caused by Fusarium udum

    The performance of the four fungal and two bacterial bioagents were evaluated for the bicontrol efficacy and ability to induce systemic resistance against Fusarium udum causing wilt of pigeonpea which is posing a serious threat to pigeonpea growing regions of India. Among the six isolates, maximum mycelial inhibition was noticed in Trichoderma harzianum (Th- R) as compared to other biocontrol agents. Among contact fungicides, maximum inhibition (> 75 %) of mycelium was recorded in Mancozeb and capton at 0.20 and 0.3 % concentrations. More than 90% inhibition was recorded among the systemic fungicides at all the all the concentrations except thiophanate methyl which recorded 53.67 % inhibition at 0.05 % concentration. Among different treatment combinations of biocontrol agents, the highest vigour index was recorded in P. fluorescense (RP- 46) + P. putida (RP- 56) treated seeds in both the cultivars (Moderately resistant and susceptible). The level of expression of defense related enzymes (PO, PPO & PAL) was more in moderately resistant cultivar(BSMR- 736) rather than susceptible one(ICP- 2376). In glass house experiment seeds treated with P. flourescens (RP- 46) + P. putida (RP-56) recorded least wilt incidence as compared to other treatments. In both Kharif seasons of 2013/14 and 2014/15 recorded significantly lowest wilt incidence and highest yield in soil drenching with 0.2 % Carbendazim fungicide. Among the biocontrol agents, seed treatment @4 g / kg seeds + soil application of PGPR (P. flourescens & P. putida) consortium @ 25kg/ ha in FYM @ 50 kg/ ha, recorded least wilt incidence and highest yield

    Genetic enhancement of Trichoderma asperellum biocontrol potentials and carbendazim tolerance for chickpea dry root rot disease management

    Advances in biocontrol potentials and fungicide resistance are highly desirable for Trichoderma. Thus, it is profitable to use mutagenic agents to develop superior strains with enhanced biocontrol properties and fungicide tolerance in Trichoderma. This study investigates the N-methyl-n-nitro-N-nitrosoguanidine (NTG) (100 mg/L) induced mutants of Trichoderma asperellum. Six NTG (3 each from 1st & 2nd round) induced mutants were developed and evaluated their biocontrol activities and carbendazim tolerance. Among the mutant N2-3, N2-1, N1 and N2-2 gave the best antagonistic and volatile metabolite activities on inhibition of chickpea F. oxysporum f. sp. ciceri, B. cinerea and R. bataticola mycelium under in vitro condition. Mutant N2-2 (5626.40 μg/ml) showed the highest EC50 value against carbendazim followed by N2-3 (206.36 μg/ml) and N2-1 (16.41 μg/ml); and succeeded to sporulate even at 2000 μg/ml of carbendazim. The biocontrol activity of N2-2 and N2 with half-dose of carbendazim was evaluated on chickpea dry root rot under controlled environment. Disease reduction and progress of the dry root rot was extremely low in T7 (N2-2 + with half-dose of carbendazim) treatment. Further, carbendazim resistant mutants demonstrated mutation in tub2 gene of β-tubulin family which was suggested through the 37 and 183 residue changes in the superimposed protein structures encoded by tub2 gene in N2 and N2-2 with WT respectively. This study conclusively implies that the enhanced carbendazim tolerance in N2-2 mutant did not affect the mycoparasitism and plant growth activity of Trichoderma. These mutants were as good as the wild-type with respect to all inherent attributes
