    Application of Remote Patient Monitoring Technology in Medical Rehabilitation Programs

    AIM. To develop an original information technology for remote monitoring and to conduct a scientific justification for its use for monitoring and evaluating the motor regimen and the main physiological parameters of patients in a rehabilitation center. MATERIAL AND METHODS. The original architecture of the software and hardware complex was used (certificate of state registration of the computer program No. 2022611766 of February 1, 2022), allowing to the attending physician’s PC and wearable devices (smart watches) of the patient with the function of the 24-hour registration of motor activity and heart rate indicators of users. The study involved 21 patients, who underwent medical rehabilitation courses in a round-the-clock hospital. 11 patients underwent cardiorehabilitation programs for the diagnosis of chronic coronary heart disease, myocardial infarction in the past (I25.2 according to ICD-10); 10 patients due to a condition after COVID-19 (U09.9 according to ICD-10). RESULTS AND DISCUSSION. When comparing the motor characteristics of the 2 study groups, it was found that the average number of steps per day was 9004.6 in the group of patients with a diagnosis of I25.2, which equals the distance of 6.6±2.63 km, and 10072.4 steps per day and 7.45±3.12 km in the group with a diagnosis of U09.9, respectively (p<0.001). In both groups, during the course of medical rehabilitation, as a rule, 3 peaks of an increase in motor load were observed – on days 2-3, 5-6 and 8. The period of physical activity in group I25.2 averaged 5:45±0:33 hours, and in group U09.9 – 5:30±0:23 hours. The total density of motor load in both groups corresponded to ≈ 35-36%. However, the average time of motor load spent on active physical rehabilitation procedures in the group with a diagnosis of I25.2 was 1:45±0:17 hours, and in the group U09.9 only 1:05±0:14 (p<0.01). In this connection, the motor density of the motor load of rehabilitation measures in the groups corresponded to 26.6% and 19.8%. CONCLUSION. The software package for remote monitoring of the motor regime and the main physiological parameters of patients has demonstrated the possibility of its use in medical practice, namely for patients at the 2nd stage of medical rehabilitation in a round-the-clock hospital and can be recommended for inclusion in programs of spa treatment

    The sequence for the development of the multiple case methodology (training technology) for accreditation of health care professionals has been presented. The multidimensionality of cases allows to cover all functions of professional standards. To improve the technique reliability, the multiple-choice tests have been offered. The requirement of local independence of tasks has been implemented. The results of approbation of cases with participation of 114 graduates (6th year) of 5 medical universities have been analysed. The interpretation of the analysis results and their use for assessing the professional readiness of health care professionals have been offered

    Adamantane-Monoterpenoid Conjugates Linked via Heterocyclic Linkers Enhance the Cytotoxic Effect of Topotecan

    Inhibiting tyrosyl-DNA phosphodiesterase 1 (TDP1) is a promising strategy for increasing the effectiveness of existing antitumor therapy since it can remove the DNA lesions caused by anticancer drugs, which form covalent complexes with topoisomerase 1 (TOP1). Here, new adamantane&ndash;monoterpene conjugates with a 1,2,4-triazole or 1,3,4-thiadiazole linker core were synthesized, where (+)-and (&minus;)-campholenic and (+)-camphor derivatives were used as monoterpene fragments. The campholenic derivatives 14a&ndash;14b and 15a&ndash;b showed activity against TDP1 at a low micromolar range with IC50 ~5&ndash;6 &mu;M, whereas camphor-containing compounds 16 and 17 were ineffective. Surprisingly, all the compounds synthesized demonstrated a clear synergy with topotecan, a TOP1 poison, regardless of their ability to inhibit TDP1. These findings imply that different pathways of enhancing topotecan toxicity other than the inhibition of TDP1 can be realized

    Risk of hematological malignancies among Chernobyl liquidators

    International audienceA case-control study of hematological malignancies was conducted among Chernobyl liquidators (accident recovery workers) from Belarus, Russia and Baltic countries to assess the effect of low- to medium-dose protracted radiation exposures on the relative risk of these diseases. The study was nested within cohorts of liquidators who had worked around the Chernobyl plant in 1986-1987. A total of 117 cases [69 leukemia, 34 non-Hodgkin lymphoma (NHL) and 14 other malignancies of lymphoid and hematopoietic tissue] and 481 matched controls were included in the study. Individual dose to the bone marrow and uncertainties were estimated for each subject. The main analyses were restricted to 70 cases (40 leukemia, 20 NHL and 10 other) and their 287 matched controls with reliable information on work in the Chernobyl area. Most subjects received very low doses (median 13 mGy). For all diagnoses combined, a significantly elevated OR was seen at doses of 200 mGy and above. The excess relative risk (ERR) per 100 mGy was 0.60 [90% confidence interval (CI) _0.02, 2.35]. The corresponding estimate for leukemia excluding chronic lymphoid leukemia (CLL) was 0.50 (90% CI _0.38, 5.7). It is slightly higher than but statistically compatible with those estimated from A-bomb survivors and recent low-doserate studies. Although sensitivity analyses showed generally similar results, we cannot rule out the possibility that biases and uncertainties could have led to over- or underestimation of the risk in this study

    New Hybrid Compounds Combining Fragments of Usnic Acid and Thioether Are Inhibitors of Human Enzymes TDP1, TDP2 and PARP1

    Tyrosyl-DNA phosphodiesterase 1 (TDP1) catalyzes the cleavage of the phosphodiester bond between the tyrosine residue of topoisomerase 1 (TOP1) and the 3′ phosphate of DNA in the single-strand break generated by TOP1. TDP1 promotes the cleavage of the stable DNA–TOP1 complexes with the TOP1 inhibitor topotecan, which is a clinically used anticancer drug. This article reports the synthesis and study of usnic acid thioether and sulfoxide derivatives that efficiently suppress TDP1 activity, with IC50 values in the 1.4–25.2 μM range. The structure of the heterocyclic substituent introduced into the dibenzofuran core affects the TDP1 inhibitory efficiency of the compounds. A five-membered heterocyclic fragment was shown to be most pharmacophoric among the others. Sulfoxide derivatives were less cytotoxic than their thioester analogs. We observed an uncompetitive type of inhibition for the four most effective inhibitors of TDP1. The anticancer effect of TOP1 inhibitors can be enhanced by the simultaneous inhibition of PARP1, TDP1, and TDP2. Some of the compounds inhibited not only TDP1 but also TDP2 and/or PARP1, but at significantly higher concentration ranges than TDP1.
