Reduced healthcare utilisation following successful HCV treatment in HIV co-infected patients with mild liver disease
New direct-acting antivirals (DAA) for hepatitis C virus (HCV) infection have achieved high cure rates in many patient groups previously considered difficult-to-treat, including those HIV/HCV co-infected. The high price of these medications is likely to limit access to treatment, at least in the short term. Early treatment priority is likely to be given to those with advanced disease, but a more detailed understanding of the potential benefits in treating those with mild disease is needed. We hypothesized that successful HCV treatment within a co-infected population with mild liver disease would lead to a reduction in the use and costs of healthcare services in the 5 years following treatment completion. We performed a retrospective cohort study of HIV/HCV-co-infected patients without evidence of fibrosis/cirrhosis who received a course of HCV therapy between 2004 and 2013. Detailed analysis of healthcare utilization up to 5 years following treatment for each patient using clinical and electronic records was used to estimate healthcare costs. Sixty-three patients were investigated, of whom 48 of 63 (76.2%) achieved sustained virological response 12 weeks following completion of therapy (SVR12). Individuals achieving SVR12 incurred lower health utilization costs (£5000 per-patient) compared to (£10 775 per-patient) non-SVR patients in the 5 years after treatment. Healthcare utilization rates and costs in the immediate 5 years following treatment were significantly higher in co-infected patients with mild disease that failed to achieve SVR12. These data suggest additional value to achieving cure beyond the prevention of complications of disease
Symmetry breaking in crossed magnetic and electric fields
We present the first observations of cylindrical symmetry breaking in highly
excited diamagnetic hydrogen with a small crossed electric field, and we give a
semiclassical interpretation of this effect. As the small perpendicular
electric field is added, the recurrence strengths of closed orbits decrease
smoothly to a minimum, and revive again. This phenomenon, caused by
interference among the electron waves that return to the nucleus, can be

Diffusion Monte Carlo calculations for the ground states of atoms and ions in neutron star magnetic fields
The diffusion quantum Monte Carlo method is extended to solve the old
theoretical physics problem of many-electron atoms and ions in intense magnetic
fields. The feature of our approach is the use of adiabatic approximation wave
functions augmented by a Jastrow factor as guiding functions to initialize the
quantum Monte Carlo prodecure. We calcula te the ground state energies of atoms
and ions with nuclear charges from Z= 2, 3, 4, ..., 26 for magnetic field

Classical, semiclassical, and quantum investigations of the 4-sphere scattering system
A genuinely three-dimensional system, viz. the hyperbolic 4-sphere scattering
system, is investigated with classical, semiclassical, and quantum mechanical
methods at various center-to-center separations of the spheres. The efficiency
and scaling properties of the computations are discussed by comparisons to the
two-dimensional 3-disk system. While in systems with few degrees of freedom
modern quantum calculations are, in general, numerically more efficient than
semiclassical methods, this situation can be reversed with increasing dimension
of the problem. For the 4-sphere system with large separations between the
spheres, we demonstrate the superiority of semiclassical versus quantum
calculations, i.e., semiclassical resonances can easily be obtained even in
energy regions which are unattainable with the currently available quantum
techniques. The 4-sphere system with touching spheres is a challenging problem
for both quantum and semiclassical techniques. Here, semiclassical resonances
are obtained via harmonic inversion of a cross-correlated periodic orbit

Setting up tunneling conditions by means of Bohmian mechanics
Usually tunneling is established after imposing some matching conditions on
the (time-independent) wave function and its first derivative at the boundaries
of a barrier. Here an alternative scheme is proposed to determine tunneling and
estimate transmission probabilities in time-dependent problems, which takes
advantage of the trajectory picture provided by Bohmian mechanics. From this
theory a general functional expression for the transmission probability in
terms of the system initial state can be reached. This expression is used here
to analyze tunneling properties and estimate transmissions in the case of
initial Gaussian wave packets colliding with ramp-like barriers.Comment: 18 pages, 4 figure
Feasibility, safety and acceptability of select outcome measures in a physiotherapy study protocol for boys with haemophilia.
BACKGROUND: There is a lack of functional performance measures for children and young people with haemophilia (CYPwH) with associated control data from typically developing boys (TDB). The literature advocates development of a core set of outcome measures for different chronic conditions. As medical treatment improves, CYPwH are experiencing better outcomes; therefore, more challenging measures are required to monitor physical performance. Such testing is not performed routinely, due to practical and safety concerns. AIM: Evaluate the feasibility, safety and acceptability of select outcome measures as part of a study protocol testing CYPwH; including myometry, 10 metre incremental shuttle walk test (10-m ISWT), iSTEP (an incremental step test, with data from TDB), and 1 week of accelerometry-wear at home. METHODS: Sixty-six boys aged 6-15 years with mild, moderate or severe haemophilia A or B (including inhibitors) attending routine clinics at Great Ormond Street Hospital were approached to participate. Descriptive statistics and content analysis were used to assess outcomes of feasibility, safety and acceptability, which included recruitment/retention rates, protocol completion within routine appointment timeframes, performance testing without serious adverse events/reactions (SAE/SARs), and acceptability to CYPwH of high-level performance measures. RESULTS: Outcomes were met: 43 boys completed testing at clinic review (Jan-Nov 2018) within a 10-month timeframe, retention was 95% at completion of protocol and no SAE/SARs were reported throughout testing. CONCLUSION: Feasibility, safety and acceptability of the study protocol have been established in this population. Both high-level performance tests, iSTEP and 10-m ISWT, were an acceptable addition to boys' routine clinic appointments and could be safe, acceptable choices of outcome measure as part of a core set of tests for CYPwH. Further investigation of the psychometric properties for the iSTEP is now justified, in order for it to be used as a standardised, validated, reliable outcome measure in clinical or research settings. Improvement in IAPT outcomes over time: are they driven by changes in clinical practice?
Treatment outcomes across Improving Access to Psychological Therapies (IAPT) services in England have improved year-on-year, with the national average proportion of patients in recovery at the end of treatment now exceeding the 50% target. This is despite the number of referrals and numbers of treated patients also increasing year-on-year, suggesting that services have evolved local practices and treatment delivery to meet needs whilst improving performance. This study explores whether there have been changes in clinical practice with regard to: (1) the number of sessions and length of treatments; (2) the number of cancellations and non-attendance; and (3) the recording of problem descriptor information, and the association with treatment outcomes in IAPT. Routinely collected data from seven IAPT services involved in the North and Central East London (NCEL) IAPT Service Improvement and Research Network (SIRN) were brought together to form a dataset of nearly 88,000 patients who completed a course of IAPT treatment. Results showed that there was a slight increase in the average number of sessions, and decreases in the length of time in treatment, as well as decreases in both the number of non-attended appointments and the use of inappropriate problem descriptors. These findings highlight a number of areas where potentially small changes to clinical practice may have had positive effects on patient outcomes. Real-world effectiveness of airway clearance techniques in children with cystic fibrosis

Access to the data, data dictionary and informed consent forms through the DRE is available with permission from the corresponding author.Background Cystic fibrosis (CF) is commonly characterised by thick respiratory mucus. From diagnosis, people with CF are prescribed daily physiotherapy, including airway clearance techniques (ACTs). ACTs consume a large proportion of treatment time, yet the efficacy and effectiveness of ACTs are poorly understood. This study aimed to evaluate associations between the quality and quantity of ACTs and lung function in children and young people with CF. Methods Project Fizzyo, a longitudinal observational cohort study in the UK, used remote monitoring with electronic pressure sensors attached to four different commercial ACT devices to record real-time, breath-by-breath pressure data during usual ACTs undertaken at home over 16 months in 145 children. ACTs were categorised either as conformant or not with current ACT recommendations based on breath pressure and length measurements, or as missed treatments if not recorded. Daily, weekly and monthly associations between ACT category and lung function were investigated using linear mixed effects regression models adjusting for clinical confounders. Results After exclusions, 45 224 ACT treatments (135 individuals) and 21 069 days without treatments (141 individuals) were analysed. The mean±SD age of participants was 10.2±2.9 years. Conformant ACTs (21%) had significantly higher forced expiratory volume in 1 s (FEV1) (mean effect size 0.23 (95% CI 0.19–0.27) FEV1 % pred per treatment) than non-conformant (79%) or missed treatments. There was no benefit from non-conformant or missed treatments and no significant difference in FEV1 between them (mean effect size 0.02 (95% CI −0.01–0.05) FEV1 % pred per treatment). Conclusions ACTs are beneficial when performed as recommended, but most people use techniques that do not improve lung function. Work is needed to monitor and improve ACT quality and to increase the proportion of people doing effective airway clearance at home.UK Research and Innovation

Power Laws, Precursors and Predictability During Failure
We investigate the dynamics of a modified Burridge-Knopoff model by
introducing a dissipative term to mimic the bursts of acoustic emission (AE)
from rock samples. The model explains many features of the statistics of AE
signals observed in experiments such as the crossover in the exponent value
from relatively small amplitude AE signals to larger regime, and their
dependence on the pulling speed. Significantly, we find that the cumulative
energy dissipated identified with acoustic emission can be used to predict a
major slip event. We also find a data collapse of the acoustic activity for
several major slip events describable by a universal stretched exponential with
