    A Proposed Approach to Mechatronics Design and Implementation Education-Oriented Methodology

    Mechatronics engineer is expected to design engineering systems with synergy and integration toward constrains like higher performance, speed, precision, efficiency, lower costs and functionality. The key element in success of a mechatronics engineering education-program, and correspondingly, Mechatronics engineering graduates, is directly related to a well-structured mechatronic system design course and the applied structural design methodology. Guidelines for structural design methodology and tools for the development process of mechatronic products, that can be applied in educational process is highly required. This paper proposes mechatronics systems design education-oriented methodology, which aims to integrate multidisciplinary knowledge, in various stages through the design process and development of mechatronics product. The proposed mechatronics design methodology is described, discussed and applied with the help of example student final year graduation project; design and implementation of mechatronics mobile robotic guidance system in the from of smart wheelchair- Mechatronics Motawif, to help and support people with disabilities and special needs to perform specific predetermined tasks, particularly, performing Al Omrah and motion around holy Kaba, Makka.     Usefulness of the Global Echo-Doppler Score (GEDS) in selection of patients with mitral stenosis for percutaneous balloon mitral valvuloplasty

    Background: We aimed to create a novel modified score by combining anatomic and hemodynamic Doppler-echocardiographic measures for selection of suitable patients with mitral stenosis for percutaneous balloon mitral valvuloplasty (PBMV) and its impact in prediction of outcome. Methods: 262 consecutive patients candidate for PBMV were enrolled. Wilkins score and a global score based on anatomical parameters (Wilkins score, posterior to anterior mitral leaflet ratio [PMVL/AMVL ratio]; left atrial diameter [LAD]) and hemodynamic parameters (mitral regurgitation [MR]; atrioventricular compliance [CN]; systolic pulmonary artery pressure [SPAP]) were assessed. Patients were classified into two groups according to their outcomes. Results: Global Echo-Doppler Score (GEDS) for patients with favorable vs. those with unfavorable outcomes was (5.0 ± 0.9 vs. 8.9 ± 1.3; p < 0.001). Sensitivity, specificity, and accuracy of a GEDS ≥ 7 for prediction of cardiac events were 97.5%, 88%, and 97.5%, respectively. The area under the receiver operating characteristic curve was 0.95 (p < 0.001). The correlation coefficient was 0.852 (p < 0.0001) for GEDS 0.531 (p < 0.002), for Wilkins score 0.315 (p < 0.02), for PMVL/AMVL 0.460 (p < 0.01), for LAD; MR: Pre-PBMV (r = 0.348, p < 0.03); CN [mL/mm Hg] (r = 0.579, p < 0.01) and SPAP [mm Hg] (r = 0.499, p < 0.01). In the regression analysis, GEDS, Wilkins score, and LAD were entered into the model. The regression coefficient (r = 0.695) of GEDS was much higher than those of the other 2 factors. Conclusions: GEDS is an independent predictor of PBMV success and clinical outcome and may be formulated in a scoring system that would help to identify the proper timing and best candidates for PBMV

    SPECTROPHOTOMETRIC DETERMINATION OF OXYBUTININE HYDROCHLORIDE BY ION-PAIR EXTRACTIONIN IN PHARMACEUTICAL PREPARATIONS

    Objective: Simple, sensitive, precise, reproducible and validated visible spectrophotometric methods have been developed for the determination of an antimuscarinic drug, namely oxybutynin hydrochloride (OXB) in pure form and in pharmaceutical preparations.Methods: Two spectrophotometric methods are based on the formation of yellow colored ion-pair complexes between the studied drug, and two sulphonphthalein acid dyes, bromocresol purple (BCP) and bromophenol blue (BPB) with absorption maxima at 410 and 416 nm, respectively.Results: The stoichiometric ratio of the formed ion-pair complexes was found to be 1:1 (drug: reagent) for both methods as deduced by Job's method of continuous variation. Several parameters such as pH, buffer type, and reagent volume, sequence of addition and effect of extracting solvent were optimized to achieve high sensitivity, stability, low blank reading and reproducible results. Under the optimum reaction conditions, linear relationships with good correlation coefficients (0.9996-0.9999) were found between the absorbance's and the concentrations of oxybutynin over the concentration ranges of 1.0–8.0 μg ml-1 and 1.0–12 μg ml-1 with LOD of 0.21 and 0.19 μg ml-1, using BCP and BPB methods, respectively. Various analytical parameters have been evaluated and the results have been validated by statistical data.Conclusion: The proposed methods were validated in accordance with ICH guidelines and successfully applied to the analysis of pharmaceutical formulation. Statistical comparison of the results obtained by applying the proposed methods with those of the reference method revealed good agreement and proved that there was no significant difference in the accuracy and precision between the results.Â

    Investigation of pulse dispersion in a carrier-based UWB system with LO leakage cancellation

    Local oscillator (LO) leakage in a carrier-based ultrawideband (UWB) system is a major design concern. In many cases, mixer LO-RF isolation is not sufficient and the LO leakage is well above the useful UWB signal. However, this leakage can be substantially reduced by using a notch filter located before the UWB transmitting antenna as long as it will not lead to unacceptable signal distortion. Therefore, various filter parameters, such as the filter order and 3 dB rejection bandwidth, have been studied to see their effects on providing sufficient band rejection level to reduce the unwanted LO leakage while minimizing the transmitted pulse dispersion. Time domain simulations and measurements have been utilized to evaluate the pulse dispersion using both the relative signal's first pulse amplitude and the pulse time delay spread. © 2009 Wiley Periodicals, Inc.     The association between right ventricular function and exercise capacity for hypertensive patients

    Background: Transthoracic echocardiography (TTE) would be used to evaluate right ventricular (RV) function in patients with arterial hypertension, and the link between RV dysfunction and exercise capacity was studied in the study population.Objective: To establish a correlation with exercise capability in the study group by using transthoracic echocardiography (TTE) to evaluate RV function in patients with arterial hypertension.Patients and Methods: Patients with hypertension were surveyed in a cross-sectional research; those known and treated; whether controlled or not and patients who were recently discovered to be hypertensive. Ninety patients were included; gender and age were taken into account. A thorough examination was performed on each patient (TTE) as well as six minute walking distance (6MWD). For all we had a written informed consent prior to enrollment. Results: Patients with untreated and uncontrolled hypertension showed significantly reduced right ventricular systolic and diastolic functions compared to those with well-controlled hypertension. Patients with untreated or uncontrolled hypertension had significant (P <0.001) deterioration on S', wave and diastolic dysfunction parameters (E/A, DTt, E/e t and IVRT). Untreated and uncontrolled hypertension patients showed a significant decrease in 6MWD compared to the well-controlled individuals (P <0.001). Conclusion: Untreated or ineffectively treated hypertension individuals had considerably reduced right ventricular function and exercise capacity, according to our findings.     Current Status and Utilization of Oral and Dental Health Services in Gezira State (2005-2009)

     Objective:  This study aimed to assess the status of the present dental and oral health services   in the Gezira state.  Methods: The study areas comprise of three clusters: urban, suburban and rural. Five study sites in the urban area, four sites in suburban area around Wad Medani and 12 sites in the rural area were chosen from the six localities of Gezira State . The households study sample was as follows: 100 in the urban area, 108 in the rural area and 52 in the suburban area. The study tools were direct pre-coded questionnaires, household survey and observation checklist. The procedure involved household interview to evaluate dental  status of dental health professionals, equipment and materials in order to assess dental health services. Descriptive statistics was used to analyze and present the data. The SPSS Statistical Programme using Microsoft Windows was used. Results: This result revealed absolute lack of dental services in the rural and relative lack of services in suburban areas. The present unit is old and most of them were incomplete and not functioning. This was the reason that some of rural and suburban populations seek oral health services in Wad Medani health institutions. Most of the patients irrespective of their residence preferred treatment at private dental clinics because they expect better care due to good facilities Recommendation: Establishing new dental units with good facilities distributed in different localities and rehabilitation of dental units provided to Elhasaheisa and Elmanagil dental departments with equipments and dental specialties to become specialized dental hospitals to cover the population needs and demand dental services for the surrounding areas

    Diagnostic and Therapeutic Advances in Blastic Plasmacytoid Dendritic Cell Neoplasm: A Focus on Hematopoietic Cell Transplantation

    AbstractBlastic plasmacytoid dendritic cell neoplasm (BPDCN) is an exceedingly rare disorder categorized under acute myeloid leukemia by the World Health Organization. Phenotypically, malignant cells coexpress CD4+ and CD56+ without coexpressing common lymphoid or myeloid lineage markers. BPDCN frequently expresses CD123, TCL1, BDCA-2, and CD2AP. Restriction of CD2AP expression to plasmacytoid dendritic cells makes it a useful tool to help confirm diagnosis. Clonal complex chromosome aberrations are described in two-thirds of cases. Eighty percent of BPDCN cases present with nonspecific dermatological manifestations, prompting inclusion in the differential diagnosis of atypical skin rashes refractory to standard treatment. Prognosis is poor, with a median survival of less than 18 months. No prospective randomized data exist to define the most optimal frontline chemotherapy. Current practice considers acute myeloid leukemia-like or acute lymphoblastic leukemia–like regimens acceptable for induction treatment. Unfortunately, responses are short-lived, with second remissions difficult to achieve, underscoring the need to consider hematopoietic cell transplantation early in the disease course. Allografting, especially if offered in first remission, can result in long-term remissions. Preclinical data suggest a potential role for immunomodulatory agents in BPCDN. However, further research efforts are needed to better understand BPDCN biology and to establish evidence-based treatment algorithms that might ultimately improve overall prognosis of this disease

    SIMULTANEOUS EXTRACTION AND RAPID HPLC BASED QUANTIFICATION OF CROCIN AND SAFRANAL IN SAFFRON (CROCUS SATIVUS L.)

    Objective: Saffron (Crocus sativus L.) is one of the most valuable crops with high medicinal values used in many diseases. The aim of the study was to establish the reliability and rapid HPLC method for analysis of crocin and safranal concentration present in stigmata of saffron in the market as well as field grown samples.Methods: Field grown and market stigmata of saffron were used for extraction of crocin and safranal. The linear dynamic ranges were established after validating the robustness of critical method parameters. The Agilent1260-Infinity Quaternary LC system was used for the preparation of calibration standards and quantification of crocin and safranal in C. sativus stigmata.Results: A good linearity was achieved in the range of 10-30 µg for each compound with the determination coefficient (R2). The calibration curves revealed linear regression (r. 0.997) for this rapid HPLC method and limit of quantifications (LOQs) were achieved in the range of 3.4 µg/ml for crocin and 10.2 µg/ml for safranal. The limit of detection (LODs) for all standards was ≤4.2 µg/ml. The range of crocin content (10.43-16.32 mg/g) and safranal (5.19-5.21 mg/g) was estimated in saffron samples.Conclusion: This method may serve the purpose of accurate quantification of crocin and safranal present in the stigmata of the plant in a quick time period.Keywords: Saffron, Crocin, Safranal, Extraction, Quantification, HPL

    Seroprevalence of hepatitis B and C virus infections among health students and health care workers in the Najran region, southwestern Saudi Arabia: The need for national guidelines for health students

    BACKGROUND: The objectives of the study were to study the seroprevalence of hepatitis B virus (HBV) and hepatitis C virus (HCV) infections among health college students (HS) and health care workers (HCWs) in the Najran Region of south-western Saudi Arabia and to study the students’ knowledge of occupational exposure to blood-borne viral infections. METHODS: A cross-sectional study of a representative sample of 300 HS and 300 HCWs was conducted. RESULTS: An overall seroprevalence of HBV of 1.7% and 8.7% was found among HS and HCWs, respectively. Two-thirds of HS (66.7%, 200) and 23.3% (70) of HCWs lack anti-HBs and are susceptible to HBV infection. An overall seroprevalence of HCV of 0% and 0.3% was found among the HS and HCWs, respectively. The present study indicates poor knowledge among HS and moderate knowledge among HCWs regarding occupationally transmitted blood-borne diseases, safe injection practices, and standard precautions to prevent occupationally transmitted blood-borne infections. CONCLUSION: It is mandatory to develop a structured program to raise awareness among HS, and current health colleges’ curricula should be upgraded to address these issues early. The HS should be considered new recruits to health services in terms of their initial screening for blood-borne infections and vaccination against HBV. The development of a novel continuing medical education and pre-employment awareness program for HCWs is recommended to address the following: blood-borne diseases transmitted occupationally, standard precautions to prevent occupationally transmitted blood borne infections, and safe injection practices
