Celluloid Savior: Jesus in the Movies
Films about Jesus attempt to interpret the gospel in light of both cultural issues and the concerns of individual filmmakers. Yet both 1) dominant assumptions about how to interpret Jesus and 2) the difficulty of presenting the narrative structures and worldview inherent in ancient texts within the forms of modern narrative cinema make this a difficult task. Smartphone-based pathogen diagnosis in urinary sepsis patients.
BackgroundThere is an urgent need for rapid, sensitive, and affordable diagnostics for microbial infections at the point-of-care. Although a number of innovative systems have been reported that transform mobile phones into potential diagnostic tools, the translational challenge to clinical diagnostics remains a significant hurdle to overcome.MethodsA smartphone-based real-time loop-mediated isothermal amplification (smaRT-LAMP) system was developed for pathogen ID in urinary sepsis patients. The free, custom-built mobile phone app allows the phone to serve as a stand-alone device for quantitative diagnostics, allowing the determination of genome copy-number of bacterial pathogens in real time.FindingsA head-to-head comparative bacterial analysis of urine from sepsis patients revealed that the performance of smaRT-LAMP matched that of clinical diagnostics at the admitting hospital in a fraction of the time (~1 h vs. 18-28 h). Among patients with bacteremic complications of their urinary sepsis, pathogen ID from the urine matched that from the blood - potentially allowing pathogen diagnosis shortly after hospital admission. Additionally, smaRT-LAMP did not exhibit false positives in sepsis patients with clinically negative urine cultures.InterpretationThe smaRT-LAMP system is effective against diverse Gram-negative and -positive pathogens and biological specimens, costs less than $100 US to fabricate (in addition to the smartphone), and is configurable for the simultaneous detection of multiple pathogens. SmaRT-LAMP thus offers the potential to deliver rapid diagnosis and treatment of urinary tract infections and urinary sepsis with a simple test that can be performed at low cost at the point-of-care. Gaining the PROMIS perspective from children with nephrotic syndrome: a Midwest pediatric nephrology consortium study
Background and objectives
Nephrotic syndrome (NS) represents a common disease in pediatric nephrology typified by a relapsing and remitting course and characterized by the presence of edema that can significantly affect the health-related quality of life in children and adolescents. The PROMIS pediatric measures were constructed to be publically available, efficient, precise, and valid across a variety of diseases to assess patient reports of symptoms and quality of life. This study was designed to evaluate the ability of children and adolescents with NS to complete the PROMIS assessment via computer and to initiate validity assessments of the short forms and full item banks in pediatric NS. Successful measurement of patient reported outcomes will contribute to our understanding of the impact of NS on children and adolescents.
Design
This cross-sectional study included 151 children and adolescents 8-17 years old with NS from 16 participating institutions in North America. The children completed the PROMIS pediatric depression, anxiety, social-peer relationships, pain interference, fatigue, mobility and upper extremity functioning measures using a web-based interface. Responses were compared between patients experiencing active NS (n = 53) defined by the presence of edema and patients with inactive NS (n = 96) defined by the absence of edema.
Results
All 151 children and adolescents were successfully able to complete the PROMIS assessment via computer. As hypothesized, the children and adolescents with active NS were significantly different on 4 self-reported measures (anxiety, pain interference, fatigue, and mobility). Depression, peer relationships, and upper extremity functioning were not different between children with active vs. inactive NS. Multivariate analysis showed that the PROMIS instruments remained sensitive to NS disease activity after adjusting for demographic characteristics.
Conclusions
Children and adolescents with NS were able to successfully complete the PROMIS instrument using a web-based interface. The computer based pediatric PROMIS measurement effectively discriminated between children and adolescents with active and inactive NS. The domain scores found in this study are consistent with previous reports investigating the health-related quality of life in children and adolescents with NS. This study establishes known-group validity and feasibility for PROMIS pediatric measures in children and adolescents with NS
A Theory for the High-T_c Cuprates: Anomalous Normal-State and Spectroscopic Properties, Phase Diagram, and Pairing
A theory of highly correlated layered superconducting materials isapplied for
the cuprates. Differently from an independent-electron approximation, their
low-energy excitations are approached in terms of auxiliary particles
representing combinations of atomic-like electron configurations, where the
introduction of a Lagrange Bose field enables treating them as bosons or
fermions. The energy spectrum of this field accounts for the tendency of
hole-doped cuprates to form stripe-like inhomogeneities. Consequently, it
induces a different analytical behavior for auxiliary particles corresponding
to "antinodal" and "nodal" electrons, enabling the existence of different
pairing temperatures at T^* and T_c. This theory correctly describes the
observed phase diagram of the cuprates, including the non-Fermi-liquid to FL
crossover in the normal state, the existence of Fermi arcs below T^* and of a
"marginal-FL" critical behavior above it. The qualitative anomalous behavior of
numerous physical quantities is accounted for, including kink- and
waterfall-like spectral features, the drop in the scattering rates below T^*
and more radically below T_c, and an effective increase in the density of
carriers with T and \omega, reflected in transport, optical and other
properties. Also is explained the correspondence between T_c, the
resonance-mode energy, and the "nodal gap".Comment: 28 pages, 7 figure
