Depression - cause or result of obesity?
Otyłość stanowi ważny problem zdrowia publicznego zarówno
z powodu stałego wzrostu jej występowania, jak i kosztów leczenia
jej powikłań. W ostatnich latach coraz większą uwagę zwraca się
na psychologiczne skutki otyłości, takie jak obniżona samoocena,
brak akceptacji własnego wyglądu i izolacja społeczna. Z jednej
strony te psychologiczne problemy mogą być przyczyną zwiększonego
poziomu stresu, obniżenia nastroju i wystąpienia depresji.
Z drugiej strony przewlekły stres, obniżenie nastroju i występowanie
depresji oraz związane z nimi zmiany zachowań żywieniowych,
takie jak pocieszanie się jedzeniem i kompulsywne jedzenie, mogą
być przyczyną rozwoju otyłości. Należy podkreślić, że niezależnie
od tego, która z tych chorób jest przyczyną, a która skutkiem, ich
współwystępowanie może prowadzić do wzrostu śmiertelności,
ponieważ zarówno otyłość, jak i depresja stanowią niezależne czynniki
ryzyka chorób układu krążenia. Prezentowana praca stanowi
przegląd danych dotyczących związków między otyłością a depresją.Obesity is a major problem of public health, due to its increasing
prevalence as well as costs of treatment of its complications. Recently,
there has been increasing attention on the psychological effects of obesity, such as a decreased self-assessment, lack of
acceptance of own appearance and social isolation. From one side
these psychological problems may be a cause for increased level
of stress, decrease of the mood and development of depression.
On the other hand, chronic stress, decrease of mood, depression
and changes of nutritional behavior such as food console and compulsive
eating may be a cause for development of obesity. It should
be emphasized that regardless of which of these diseases is
a cause or which is a result, their simultaneous development may
lead to increased of mortality, because both obesity and depression
are independent risk factors for coronary heart disease.
The aim of present study is to review the current literature on connection
between obesity and depression
The role of a very low calorie diet (VLCD) in the treatment of obesity
The basis for the treatment of obesity is a negative energy balance
that is achieved by reduced energy intake and increased physical activity. Among the methods used in the dietary treatment of obesity
are very low calorie diets (VLCD) supplying 200–800 kcal a day.
VLCD in the form of sachets of powder to prepare meals have
been developed for the treatment of hospitalized patients. These
are high-protein and low-carbohydrate as well as very low-fat diets
enriched with vitamins, minerals and essential fatty acids. Caloric
content and composition of the VLCD cause a development of
ketosis state, which suppresses the feeling of hunger.
The aim of this paper is to present a role of VLCD in the treatment
of obesity, including contraindications for VLCD and use of VLCD
products as a component of low energy diet (reduction of daily
energy consumption by about 600 calories compared to the estimated
energy expenditure of a patient).Podstawą leczenia otyłości jest uzyskanie ujemnego bilansu energetycznego,
co jest możliwe dzięki wprowadzeniu diety o obniżonej energetyczności
w stosunku do zapotrzebowania pacjenta i zwiększeniu
aktywności fizycznej. Jedną z metod wykorzystywanych w dietetycznym
leczeniu otyłości jest stosowanie diet bardzo niskoenergetycznych
(VLCD) dostarczających 200–800 kcal na dobę. Takie diety, w postaci
saszetek z proszkiem do przygotowania posiłków, opracowano na
potrzeby leczenia pacjentów hospitalizowanych. Są to diety wysokobiałkowe
oraz bardzo niskowęglowodanowe i bardzo niskotłuszczowe,
wzbogacone w witaminy i mikroelementy oraz niezbędne kwasy
tłuszczowe. Kaloryczność i skład diety powodują wytworzenie się
w organizmie stanu ketozy, która hamuje odczuwanie głodu.
Celem niniejszego artykułu jest przedstawienie roli VLCD w leczeniu
otyłości z uwzględnieniem przeciwwskazań oraz zastosowań produktów
VLCD jako elementów diety niskokalorycznej (ograniczenie
spożycia o ok. 600 kcal dziennie w stosunku do oszacowanego
wydatku energetycznego pacjenta)
The effect of weight loss on serum concentrations of nitric oxide, TNF-alfa and soluble TNF-alfa receptors
Wstęp: Celem prezentowanej pracy była ocena wpływu
obniżenia masy ciała na stężenie w surowicy tlenku azotu
(NO, nitric oxide) i czynnika martwicy nowotworów α (TNF-α,
tumor necrosis factor α) oraz zbadanie czy istnieje związek
między aktywnością układu TNF α stężeniem w surowicy
NO po zmniejszeniu masy ciała.
Materiał i metody: Badaną grupę stanowiły 43 otyłe kobiety
(wiek 41,8 ± 11,9 lat, masa ciała 95,2 ± 15,0 kg, wskaźnik
masy ciała [BMI, body mass index] 36,5 ± 4,6 kg/m2). Badane
poddano 3-miesięcznej kompleksowej grupowej kuracji
odchudzającej. Zalecono im dietę 1000-1200 kcal i regularną
aktywność fizyczną. Nie stosowano leczenia farmakologicznego.
Przed kuracją i po kuracji oznaczono stężenie w surowicy metabolitów
tlenku azotu, TNF-α i jego rozpuszczalnych receptorów
(sTNFR1, sTNFR2) za pomocą metody Enzyme-Linked ImmunoSorbent Assay (ELISA), insuliny przy użyciu
metody radioimmunologicznej (RIA, Radioimmunoassay),
a stężenie glukozy, cholesterolu, cholesterolu frakcji HDL
i triglicerydów - metodą enzymatyczną. Skład ciała oceniono
za pomocą metody bioimpedancji przy użyciu aparatu
Bodystat.
Wyniki: Średni ubytek masy ciała wynosił 8,3 ± 4,3 kg. Po
obniżeniu masy ciała stężenie w surowicy TNF-α obniżyło
się istotnie (p < 0,000), a obu receptorów sTNFR1 i sTNFR2
istotnie się podwyższyło (p < 0,000). Nie obserwowano natomiast zmian stężenia NO po zmniejszeniu masy ciała.
Przeprowadzono analizę regresji wieloczynnikowej ze
zmiennymi zależnymi, takimi jak ΔTNF-α, ΔsTNFR1,
ΔsTNFR2 i ΔNO. Zaobserwowano istotne korelacje między
ΔNO a wyjściowym stężeniem w surowicy TNF-α, sTNFR1
i sTNFR2.
Wnioski: Po obniżeniu masy ciała zaobserwowano zmniejszenie
stężenia w surowicy TNF-α i wzrost stężenia obu receptorów
TNF, nie stwierdziliśmy, natomiast zmian stężenia
w surowicy NO. Wydaje się, że zmiany aktywności układu
TNF mogą być mechanizmem kontrregulacyjnym, który
hamuje dalszą redukcję masy ciała. Związku między zmianami
aktywności układu TNF α stężeniem w surowicy NO
po redukcji masy ciała nie wykazano.Introduction: The aims of the present study were to evaluate
the effect of weight-loss treatment on serum concentrations
of NO and TNF-α and to examine whether there is an
association between TNF-system activity and serum concentrations
of NO after weight loss.
Material and methods: The study group involved 43 obese
women (aged 41.8 ± 11.9 years, weight 95.2 ± 15.0 kg, BMI
36.5 ± 4.6 kg/m2). The women were subjected to threemonth
complex weight-loss treatment. Patients were advised
to keep to a 1000-1200 kcal diet and to exercise regularly.
Pharmacological treatment was not administered. Serum
concentrations of nitric oxide metabolites, TNF-α and its
soluble receptors (sTNFR1, sTNFR2) were measured by
ELISA kits; insulin was measured by RIA and glucose, cholesterol,
HDL cholesterol and triglicerydes by an enzymatic
procedure before and after weight loss. Body composition
was determined by impedance analysis using Bodystat.
Results: The mean weight loss during treatment was 8.3 ± 4.3 kg. The serum concentrations of TNF-α decreased significantly
(p < 0.000) and both receptors sTNFR1 and
sTNFR2 increased significantly (p < 0.000) after weight loss.
No significant changes in serum concentrations of NO were
observed after weight loss. A multiple regression analysis
was performed using ΔTNF-α, ΔsTNFR1, ΔTNFR2 and ΔNO as dependent variables. A significant correlation was
observed between DNO and initial plasma concentrations
of TNF-α, sTNFR1 and sTNFR2.
Conclusions: This study demonstrates a decrease in serum
TNF-α concentration as well as an increase in plasma concentration
of both TNF receptors but does not show any
change in serum concentrations of NO after weight-loss treatment
in obese women. It seems that changes in TNF-system
activity may be a counter-regulating mechanism, which
inhibits further body mass loss. We did not observe any association
between changes in TNF-system activity and serum
concentrations of NO after weight loss
The role of glucagon in the pathogenesis of type 2 diabetes
Cukrzyca typu 2 jest chorobą metaboliczną spowodowaną opornością
tkanek obwodowych na działanie insuliny. Obecnie znana
jest rola aktywnych metabolicznie substancji wydzielanych przez
tkankę tłuszczową (adipokin) w rozwoju insulinooporności tkanki
tłuszczowej, mięśniowej i hepatocytów. Początkowo hiperinsulinemia
kompensuje insulinooporność, ale stopniowe zmniejszanie
się liczby komórek β trzustki z powodu zmian inwolucyjnych,
w których również uczestniczą adipokiny, powoduje wystąpienie
klinicznie jawnej cukrzycy typu 2. Upośledzenie poposiłkowego
wydzielania insuliny jest efektem dysfunkcji układu inkretynowego
(zmniejszone poposiłkowe wydzielanie glukagonopodobnego
peptydu-1). Najmniej poznane ogniwo patogenezy tej choroby stanowi
glukagon, główny antagonista insuliny. Celem niniejszego
opracowania było podsumowanie dotychczasowej wiedzy dotyczącej
roli glukagonu w rozwoju nietolerancji węglowodanów
i cukrzycy typu 2.

of peripheral tissues to insulin action. Currently of the role of metabolically
active agents secreted by adipose tissue (adipokines) in
the development of insulin resistance of adipose tissue, myocytes
and hepatocytes is growing is known initially in the course of type
2 diabetes the insulin resistance is compensated by hyperinsulinaemia,
however a gradual involution of beta-cells in the pancreas
with some contribution of adipokines, followed by the decrease of
their number is leading to the appearance of type 2 diabetes. Impaired
postprandial secretion of insulin is related to the abnormal
function of incretin system (decreased secretion of glucagon like
peptide-1). Although the participation of glucagon a main insulin
antagonist is hardly comprehensible. The aim of this review is to
summarize the current knowledge concerning the role of glucagon
in the development of impaired carbohydrate tolerance and
type 2 diabetes.

Factors affecting the choice of drospirenone as the component of combined contraceptive pill in daily clinical practice: the results of nation-wide survey
Objectives: The aim of the multicenter, open-label, post-marketing, observational survey was to assess doctors’ preferences in choosing the progestogen component of the combined contraceptive pill (CCP) and factors affecting this choice in daily clinical practice as well as non-contraceptive reasons use of CCP containing drospirenone (CCPD) and patients’ tolerance and satisfaction with the treatment. Material and methods: This multicenter, open-label, post-marketing, survey was performed nation-wide with the participation of 222 doctors involving and 10,345 patients treated with CCPD. The study questionnaire included questions concerning factors affecting the choice of drospirenone as a component of CCP and assessing prescription pattern of the drug as well as tolerance and satisfaction with the use of CCPD. Results: The doctors frequently declared their choice of drospirenone as the progestogen component of CCP. The most important factors affecting the choice of drospirenone, declared by doctors, were tolerance level, consistent regulation of menstrual cycle and not causing spotting. CCPD was prescribed to patients with irregular menstrual cycles (62.7%) and painful menstruation (46.8%). During follow-up, significantly increased percentage of patients assessed the tolerance of treatment with CCPD as very good (52.5% vs 68.0%; p < 0.01) and very satisfied with its use (61.9% vs 77.8%, p < 0.01). Conclusions: 1) Drospirenone is frequently chosen progestogen component in CCP by Polish gynecologists due to its good tolerance, consistent regulation of the menstrual cycle and no spotting in patients opinion. 2) CCPD was most frequently used in patients with irregular menstrual cycles and painful menstruation. 3) The patients were satisfied with the use CCPD and treatment was well tolerated
Quality of life in arterial hypertension
Choroba przewlekła, jaką jest nadciśnienie tętnicze, stanowi
przyczynę obniżenia jakości życia (QoL), wywierając wpływ nie
tylko na jej wymiar subiektywny, związany z pogorszeniem samopoczucia,
ale również na wymiar obiektywny, uniemożliwiając
pełnienie dotychczasowych funkcji społecznych i pogarszając
status ekonomiczny, z powodu konieczności rezygnacji
z pracy zawodowej i izolacji społecznej. Zdrowie to przede wszystkim
pełnia dobrostanu fizycznego, psychicznego i społecznego,
dlatego miarą sukcesu terapeutycznego nie mogą być tylko klasyczne,
ilościowe wskaźniki, takie jak właściwa kontrola ciśnienia
tętniczego i, w perspektywie, wydłużenie przeżycia. Trzeba również
uwzględnić czynniki jakościowe, do których należą: zmniejszenie
nasilenia objawów choroby, poprawa sprawności psychofizycznej,
poprawa nastroju, dobre samopoczucie, utrzymanie aktywności
zawodowej i seksualnej. Monitorowanie jakości życia
u chorych z nadciśnieniem tętniczym, u których powrót do zdrowia
może być jedynie częściowy, a leczenie trwa najczęściej do końca
życia, może być przydatne również w aspekcie modyfikacji sposobu
leczenia oraz stratyfikacji ryzyka zgonu czy kolejnych hospitalizacji.
W niniejszej pracy przedstawiono przegląd piśmiennictwa
obejmujący wzajemne powiązania między nadciśnieniem tętniczym,
farmakologicznymi i niefarmakologicznymi metodami leczenia
oraz jakością życia i efektywnością leczenia tej choroby.

179–185Any chronic disease, including arterial hypertension, is the cause
of lower quality of life (QoL) exerting detrimental impact on subjective
aspects, related to the impairment of wellbeing as well as objective
measures: restriction of the previous social functions, worsening
of economic status related to the necessity of withdrawal
from working life, social isolation. As health is not merely the absence
of disease but a state of complete physical, mental, and
social well-being, the measure of therapeutic efficacy should in
addition to classical quantitative measures as appropriate control
of blood pressure and expected elongation of life, include qualitative
measures, as alleviation of sign and symptoms of the disease,
improvement of psycho-physical efficiency, of mood, and well-being,
subsistence of labour and sexual activity. Quality of life monitoring
in patients with arterial hypertension, a disease with only
partially recovery, and the necessity of lifetime lasting therapy, can
also be useful for modification of therapy, and stratification for risk
of death and hospitalization. This current review summarizes data
concerning the interrelation between arterial hypertension, its pharmacotherapy
and non-pharmacological methods and quality of life
and efficacy of therapy of the disease.

The effect of weight loss on serum concentrations of FAS and tumour necrosis factor alpha in obese women
Wstęp: Proces apoptozy może wpływać zarówno na masę, jak i na rozmieszczenie tkanki tłuszczowej. Nadrodzina receptorów dla czynnika
martwicy nowotworów (TNF, tumour necrosis factor) stymuluje apoptozę. Celem pracy była ocena stężeń TNF-α, rozpuszczalnych
form receptorów dla TNF oraz FAS w surowicy otyłych kobiet oraz ich zmiany po zmniejszeniu masy ciała.
Materiał i metody: Grupę 23 otyłych kobiet bez chorób towarzyszących (średni wiek 36,6 ± 10,9 lat) poddano 3-miesięcznej kuracji
odchudzającej składającej się z diety 1000 kcal oraz aktywności fizycznej. Grupę kontrolną stanowiło 17 zdrowych kobiet z prawidłową
masą ciała (średnia wieku 40,3 ± 5,5 roku). Próbkę krwi pobierano na czczo w godzinach porannych. Stężenia TNF-α, receptorów dla TNF
oraz FAS oznaczono metodą ELISA. Stężenie insuliny oznaczono przy użyciu metody RIA. Stężenia glukozy, cholesterolu całkowitego,
cholesterolu frakcji HDL oraz triglicerydów oznaczono metodą enzymatyczną.
Wyniki: Średnie zmniejszenie masy ciała w trakcie 3-miesięcznej kuracji odchudzającej wyniosło 11,4 ± 3,1 kg. Stężenie TNF-α uległo
obniżeniu (7,3 ± 3,0 vs. 5,4 ± 1,6 pg/ml; p < 0,005), stężenie sTNFRs uległo podwyższeniu (odpowiednio: 1222,6 ± 211,8 vs. 1325,6 ± 261,6
pg/ml; p < 0,05 oraz 1881,5 ± 337,2 vs. 2057,4 ± 358,7 pg/ml; p < 0,05). Nie zaobserwowano zmian w stężeniu FAS po zmniejszeniu masy
ciała.
Wnioski: U otyłych kobiet zaobserwowano wyższe stężenie TNF-α w osoczu w porównaniu z osobami z prawidłową masą ciała, nie
zaobserwowano różnic w stężeniu FAS. Stężenie TNF uległo obniżeniu, a stężenie sTNFRs wzrosło po zmniejszeniu masy ciała. Redukcja
masy ciała nie spowodowała zmian w stężeniu FAS. Endokrynol Pol 2008; 59 (1): 18-22Introduction: Apoptosis can influence both adipose tissue mass and its distribution. The suprafamily of tumour necrosis factor (TNF)
receptors stimulate apoptosis. The aim of the study was to assess serum concentrations of tumour necrosis factor alpha (TNF-α), TNF
soluble receptors (sTNFRs) and FAS in obese subjects and to examine the changes in these parameters after weight loss.
Material and methods: The study group consisted of 23 obese women without additional disease aged 36.6 ± 10.9 years. These were
examined before and after three-month weight reduction treatment consisting of a diet of 1000 kcal/day and physical exercise. The control
group comprised 17 lean healthy women aged 40.3 ± 5.5 years. Blood samples were taken in the morning after an overnight fast. Serum
concentrations of TNF-α, sTNFRs and FAS were measured by enzyme linked immunosorbent assay (ELISA). Serum concentrations of
insulin were measured by RIA. Serum concentrations of glucose, total cholesterol, HDL cholesterol and triglycerides were measured by an
enzymatic procedure.
Results: The mean weight loss over the three-month treatment was 11.4 ± 3.1 kg. Following weight loss, serum TNF-α concentrations
decreased significantly (7.3 ± 3.0 vs. 5.4 ± 1.6 pg/ml; p < 0.005) and concentrations of sTNFRs increased significantly (1222.6 ± 211.8 vs.
1325.6 ± 261.6 pg/ml; p < 0.05 and 1881.5 ± 337.2 vs. 2057.4 ± 358.7 pg/ml; p < 0.05 respectively). However, no changes in serum concentrations
of FAS were observed after weight loss.
Conclusion: We observed increased serum concentrations of TNF-α but not of FAS in obese women. The concentrations of TNF decreased

Postpartum hypothalamic dysfunction - a case report
Zaburzenia czynności podwzgórza stanowią rzadko rozpoznawaną jednostkę kliniczną, stanowiącą konsekwencję różnych procesów
patologicznych toczących się w obrębie tej struktury mózgu. Charakteryzuje ją złożony obraz kliniczny, na który składają się bóle głowy,
nieprawidłowa regulacja łaknienia i pragnienia, zaburzenia snu i termoregulacji oraz nieprawidłowe wydzielanie hormonów. W niniejszym
artykule opisano przypadek 29-letniej kobiety, u której doszło do rozwoju zaburzeń czynności podwzgórza w następstwie powikłań
okołoporodowych. Postawienie prawidłowego rozpoznania wymagało wykonania wielu oznaczeń laboratoryjnych i przeprowadzenia
badań obrazowych. Otyłość, która wystąpiła u pacjentki, wykazywała oporność na klasyczne leczenie (dietetyczne i farmakologiczne)
i dopiero leczenie bariatryczne spowodowało redukcję masy ciała. Opisywana pacjentka jest pierwszą chorą z poporodową dysfunkcją
podwzgórza, u której wykonano oznaczenia stężeń w osoczu adipokin, neuropeptydów i hormonów przewodu pokarmowego uczestniczących
w fizjologicznej regulacji poboru pokarmu. (Endokrynol Pol 2010; 61 (4): 396-399)Hypothalamic dysfunction is a rarely diagnosed endocrine disorder resulting from various pathological processes affecting this brain
region. It is characterized by a complex clinical manifestation, including headaches, abnormal regulation of various behaviours, abnormalities
in sleeping and thermoregulation, and inappropriate secretion of many hormones. In our paper, we report the case of a 29-year-old
female in whom hypothalamic dysfunction was induced by delivery complications. Accurate diagnosis of this syndrome required complex
laboratory and imaging tests. The disease-related obesity was resistant to conventional treatment (diet and pharmacotherapy) and
only bariatric surgery caused a reduction of body mass. The described patient is the first with postpartum hypothalamic dysfunction in
whom plasma levels of adipokines, neuropeptides, and alimentary tract hormones contributing to physiological regulation of food intake
