    Efficacy of autologous bone marrow buffy coat grafting combined with core decompression in patients with avascular necrosis of femoral head: a prospective, double-blinded, randomized, controlled study

    Introduction Avascular necrosis of femoral head (ANFH) is a progressive disease that often leads to hip joint dysfunction and even disability in young patients. Although the standard treatment, which is core decompression, has the advantage of minimal invasion, the efficacy is variable. Recent studies have shown that implantation of bone marrow containing osteogenic precursors into necrotic lesion of ANFH may be promising for the treatment of ANFH. Methods A prospective, double-blinded, randomized controlled trial was conducted to examine the effect of bone-marrow buffy coat (BBC) grafting combined with core decompression for the treatment of ANFH. Forty-five patients (53 hips) with Ficat stage I to III ANFH were recruited. The hips were allocated to the control group (core decompression + autologous bone graft) or treatment group (core decompression + autologous bone graft with BBC). Both patients and assessors were blinded to the treatment options. The clinical symptoms and disease progression were assessed as the primary and secondary outcomes. Results At the final follow-up (24 months), there was a significant relief in pain (P \u3c0.05) and clinical joint symptoms as measured by the Lequesne index (P \u3c0.05) and Western Ontario and McMaster Universities Arthritis Index (P \u3c0.05) in the treatment group. In addition, 33.3% of the hips in the control group have deteriorated to the next stage after 24 months post-procedure, whereas only 8% in the treatment group had further deterioration (P \u3c0.05). More importantly, the non-progression rates for stage I/II hips were 100% in the treatment group and 66.7% in the control group. Conclusion Implantation of the autologous BBC grafting combined with core decompression is effective to prevent further progression for the early stages of ANFH.     Burden of psoriasis in young adults worldwide from the global burden of disease study 2019

    BackgroundTo determine the global burden of psoriasis in young adults, i.e., those aged 15–49, from 1990 to 2019 and predict trends in this burden for 2020 to 2030MethodsAge-standardized disease burden indicators and their estimated annual percentage changes were assessed and used to compare the estimated burden between regions. In addition, generalized additive models were used to predict the burden in this population from 2020 to 2030.ResultsFrom 1990 to 2019, the overall burden of psoriasis in young adults worldwide trended downward, as the age-standardized incidence rate and the age-standardized disability-adjusted life year rate decreased. From 1990 to 2019, there were gender differences in the burden of psoriasis between regions with different Socio-demographic index. Specifically, there was a smaller increase in the burden in young men than in young women in middle- and low–middle-Socio-demographic index areas. In 2019, Western Europe, Australasia, and Southern Latin America had the highest age-standardized incidence rate of psoriasis in young adults, whereas age-standardized disability-adjusted life year rates of psoriasis in young adults were highest in high-income North America. In 2019, the psoriasis burden in young adults was the highest in high-Socio-demographic index areas and the lowest in low-Socio-demographic index regions. We predict that from 2020 to 2030, the incidence rate and disability-adjusted life year rate of psoriasis in all age groups of young adults will continue to decline, but the burden in those aged 30–39 will increase.ConclusionFrom 1990 to 2019, the overall burden of psoriasis in each age group trended downward in this period.     The Immune Cell Landscape in Different Anatomical Structures of Knee in Osteoarthritis: A Gene Expression-Based Study

    Background. Immunological mechanisms play a vital role in the pathogenesis of knee osteoarthritis (KOA). Moreover, the immune phenotype is a relevant prognostic factor in various immune-related diseases. In this study, we used CIBERSORT for deconvolution of global gene expression data to define the immune cell landscape of different structures of knee in osteoarthritis. Methods and Findings. By applying CIBERSORT, we assessed the relative proportions of immune cells in 76 samples of knee cartilage, 146 samples of knee synovial tissue, 40 samples of meniscus, and 50 samples of knee subchondral bone. Enumeration and activation status of 22 immune cell subtypes were provided by the obtained immune cell profiles. In synovial tissues, the differences in proportions of plasma cells, M1 macrophages, M2 macrophages, activated dendritic cells, resting mast cells, and eosinophils between normal tissues and osteoarthritic tissues were statistically significant (P<0.05). The area under the curve was relatively large in resting mast cells, dendritic cells, and M2 macrophages in receiver operating characteristic analyses. In subchondral bones, the differences in proportions of resting master cells and neutrophils between normal tissues and osteoarthritic tissues were statistically significant (P<0.05). In subchondral bones, the proportions of immune cells, from the principle component analyses, displayed distinct group-bias clustering. Resting mast cells and T cell CD8 were the major component of first component. Moreover, we revealed the potential interaction between immune cells. There was almost no infiltration of immune cells in the meniscus and cartilage of the knee joint. Conclusions. The immune cell composition in KOA differed substantially from that of healthy joint tissue, while it also differed in different anatomical structures of the knee. Meanwhile, activated mast cells were mainly associated with high immune cell infiltration in OA. Furthermore, we speculate M2 macrophages in synovium and mast cells in subchondral bone may play an important role in the pathogenesis of OA

    Realization of a Z classified chiral-symmetric higher-order topological insulator in a coupling-inversion acoustic crystal

    Higher-order topological band theory has transformed the landscape of topological phases in quantum and classical systems. Here, we experimentally demonstrate a two-dimensional (2D) higher-order topological phase (HOTP), referred to as the multiple chiral topological phase (MCTP), which is protected by a multipole chiral number (MCN). Our realization differs from previous HOTPs in that it possesses a larger-than-unity MCN, which arises when the nearest-neighbor couplings (NNCs) are weaker than long-range couplings (LRCs). Our phase has an MCN of 4, protecting the existence of 4 mid-gap topological corner modes (TCMs) at each corner. The multiple TCMs demonstrated here could lead to enhanced quantum-inspired devices for sensing and computing. Our study also highlights the rich and untapped potential of LRC manipulation for future research in topological phases

    High temperature stability of H-diamond high frequency MOSFET with 300°C grown Al2O3 dielectric

    The high frequency H-diamond metal-oxide-semiconductor field effect transistors (MOSFETs) were fabricated on single diamond substrate using 300°C ALD grown Al2O3 as gate dielectric and passivation layer. The devices gate length, gate/drain spacing and dielectric thickness are 100 nm, 2 μm, and 10 nm, respectively. The direct-current and frequency characteristics were investigated. The device shows a maximum saturation drain current of −492.6 mA/mm and gm of 135.2 mS/mm. The device shows good high temperature working performance, and the maximum saturation drain current only has a little decreasing of 7.6%. at 200°C. In addition, the device exhibits a maximum cut-off frequency of 36.2 GHz and maximum oscillation frequency of 70.5 GHz. The transient drain current response measurement indicates that the drain current can follow the changing of gate voltage at the frequency of 1 MHz. These results indicate that the Al2O3 dielectric is suitable for using in high frequency or the high-speed switching devices

    High frequency single crystalline diamond MOSFET with high temperature (300 °C) ALD grown Al2O3 dielectric

    In this paper, hydrogen terminated metal oxide semiconductor field effect transistors (MOSFETs) having a sub-micron gate length were fabricated on a single crystalline diamond sample using the 300 °C ALD grown Al2O3 as gate dielectric and passivation layer. The device shows a maximum output current density of 942 mA/mm and a lowest on-resistance of 6.2 Ω·mm at a VGS of −3 V. A maximum transconductance of 284 mS/mm was achieved. In addition, a high cut-off frequency of 41.3 GHz and a maximum oscillation frequency of 80.6 GHz were achieved. These are the highest values among the reported H-diamond MOSFETs with high temperature (>200 °C) grown dielectrics. The hole mobility is estimated to be 138 cm2/Vs at VGS = -3 V. The high carrier mobility contributes to the achievement of the high output current density. These results demonstrate that the high temperature grown Al2O3 dielectric has also a great potential to be used in high frequency H-diamond MOSFETs
