Pharmacological cardioversion for atrial fibrillation and flutter*
Atrial fibrillation is the commonest cardiac dysrhythmia. It is associated with significant morbidity and mortality. There are two approaches to the management of atrial fibrillation: controlling the ventricular rate or converting to sinus rhythm in the expectation that this would abolish its adverse effects.
The objective of this review was to assess the effects of pharmacological cardioversion of atrial fibrillation in adults on the annual risk of stroke, peripheral embolism, and mortality.
We made a thorough search for existing evidence in the following databases: the Cochrane Controlled Trials Register (Issue 3, 2002), MEDLINE (2000 to 2002), EMBASE (1998 to 2002), CINAHL (1982 to 2002), Web of Science (1981 to 2002). We also handsearched the following journals: Circulation (1997 to 2002), Heart (1997 to 2002), European Heart Journal (1997-2002), Journal of the American College of Cardiology (1997-2002) and selected abstracts published on the web site of the North American Society of Pacing and Electrophysiology (2001, 2002). We selected trials based on the following criteria: randomised controlled trials or controlled clinical trials of pharmacological cardioversion versus rate control in adults (>18 years) with acute, paroxysmal or sustained atrial fibrillation or atrial flutter, of any duration and of any aetiology.
We identified two completed studies AFFIRM (n=4060) and PIAF (n=252). We found no difference in mortality between rhythm control and rate control - relative risk 1.14 (95% confidence interval 1.00 to 1.31). Both studies show significantly higher rates of hospitalisation and adverse events in the rhythm control group and no difference in quality of life between the two treatment groups. In AFFIRM there was a similar incidence of ischaemic stroke, bleeding and systemic embolism in the two groups. Certain malignant dysrhythmias were significantly more likely to occur in the rhythm control group. There were similar scores of cognitive assessment in both groups. In PIAF, cardioverted patients enjoyed an improved exercise tolerance but there was no overall benefit in terms of symptom control or quality of life.
There is no evidence that pharmacological cardioversion of atrial fibrillation to sinus rhythm is superior to rate control. Rhythm control is associated with more adverse effects and increased hospitalisation. It does not reduce the risk of stroke. These conclusions cannot be generalised to all people with atrial fibrillation as most of the patients included in these studies were relatively older (>60 years) with significant cardiovascular risk factors
Improving CMB non-Gaussianity estimators using tracers of local structure
Local non-Gaussianity causes correlations between large scale perturbation
modes and the small scale power. The large-scale CMB signal has contributions
from the integrated Sachs Wolfe (ISW) effect, which does not correlate with the
small scale power. If this ISW contribution can be removed, the sensitivity to
local non-Gaussianity is improved. Gravitational lensing and galaxy counts can
be used to trace the ISW contribution; in particular we show that the CMB
lensing potential is highly correlated with the ISW signal. We construct a
nearly-optimal estimator for the local non-Gaussianity parameter \fnl and
investigate to what extent we can use this to decrease the variance on
{\fnl}. We show that the variance can be decreased by up to at Planck
sensitivity using galaxy counts. CMB lensing is a good bias-independent ISW
tracer for future more sensitive observations, though the fractional decrease

Nationwide forestry applications program. Analysis of forest classification accuracy
The development of LANDSAT classification accuracy assessment techniques, and of a computerized system for assessing wildlife habitat from land cover maps are considered. A literature review on accuracy assessment techniques and an explanation for the techniques development under both projects are included along with listings of the computer programs. The presentations and discussions at the National Working Conference on LANDSAT Classification Accuracy are summarized. Jupiter's radiation belts: Can Pioneer 10 survive?
Model calculations of Jupiter's electron and proton radiation belts indicate that the Galilean satellites can reduce particle fluxes in certain regions of the inner magnetosphere by as much as six orders of magnitude. Average fluxes should be reduced by a factor of 100 or more along the Pioneer 10 trajectory through the heart of Jupiter's radiation belts in early December. This may be enough to prevent serious radiation damage to the spacecraft
Absorption of trapped particles by Jupiter's moons
Absorption effects of the four innermost moons in the radial transport equations for electrons and protons in Jupiter's magnetosphere are presented. The phase space density n at 2 R sub J for electrons with equatorial pitch angles less than 69 deg is reduced by a factor of 4.2 x 1000 when lunar absorption is included in the calculation. For protons with equatorial pitch angles less than 69 deg, the corresponding reduction factor is 3.2 x 100000. The effect of the satellites becomes progressively weaker for both electrons and protons as equatorial pitch angles of pi/2 are approached, because the likelihood of impacting a satellite becomes progressively smaller. The large density decreases which we find at the orbits of Io, Europa, and Ganymede result in corresponding particle flux decreases that should be observed by spacecraft making particle measurements in Jupiter's magnetosphere. The characteristic signature of satellite absorption should be a downward pointing cusp in the flux versus radius curve at the L-value corresponding to each satellite

Investigating the effects of inter-annual weather variation (1968- 2016) on the functional response of cereal grain yield to applied nitrogen, using data from the Rothamsted Long-Term experiments
The effect of weather on inter-annual variation in the crop yield response to nitrogen (N) fertilizer for winter wheat (Triticum aestivvum L.) and spring barley (Hordeum vulgare L.) was investigated using yield data from the Broadbalk Wheat and Hoosfield Spring Barley long-term experiments at Rothamsted Research. Grain yields of crops from 1968 to 2016 were modelled as a function of N rates using a linear-plus-exponential (LEXP) function. The extent to which inter-annual variation in the parameters of these responses was explained by variations in weather (monthly summarized temperatures and rainfall), and by changes in the cultivar grown, was assessed. The inter-annual variability in rainfall and underlying temperature influenced the crop N response and hence grain yields in both crops. Asymptotic yields in wheat were particularly sensitive to mean temperature in November, April and May, and to total rainfall in October, February and June. In spring barley asymptotic yields were sensitive to mean temperature in February and June, and to total rainfall in April to July inclusive and September.
The method presented here explores the separation of agronomic and environmental (weather) influences on crop yield over time. Fitting N response curves across multiple treatments can support an informative analysis of the influence of weather variation on the yield variability. Whilst there are issues of the confounding and collinearity of explanatory variables within such models, and that other factors also influence yields over time, our study confirms the considerable impact of weather variables at certain times of the year. This emphasizes the importance of including weather temporal variation when evaluating the impacts of climate change on crops
Barrier Energies in MIM Structures from Photoresponse: Effect of Scattering in the Insulating Film
Scattering of electrons photoexcited into the insulator conduction band prevents photoresponse from following the Fowler relation in MIM structures. However, barrier energies
can be obtained without specific knowledge of the scattering process either by measuring
the threshold for photo response directly, or by applying sufficiently large voltages across
the insulator
Optical Polarization of 52 Radio-Loud QSOs and BL Lac Objects
Polarization measurements are presented for 52 radio-loud QSOs and BL Lac
objects. For 9 highly polarized (p >3%) AGN, these are the first published
polarization measurements. Of these 9, 7 are highly-polarized QSOs (HPQs), one
is a BL Lac object and another is a likely BL Lac object. Polarization
variability is confirmed for some of these new and previously known
highly-polarized AGN. While 6 of the HPQs have flat radio spectra are almost
certainly blazars, PKS 1452-217 is probably a new member of the rare class of
radio-loud QSOs that show high polarization by scattering, and is therefore
important for testing orientation Unified Schemes. In competition for the
highest redshift HPQ are the well-observed QSO PKS 0438-43 at z = 2.85, with
maximum p = 4.7%, and PKS 0046-315 at z = 2.72, for which we find p = 13%.Comment: 6 pages. Astronomical Journal, in pres
The Presence of Deep Levels in Ion Implanted Junctions
It has been found that ion implantation doping results in the generation and diffusion of defect species, forming deep trapping levels. The effect of these levels on the electrical characteristics of zinc‐implanted GaAs diodes has been observed for the case of 70‐kV implantation at 400°C into substrates with n‐type concentrations ranging from 1 × 10^16 to 1.8 × 10^18 atoms/cm^3. Capacitance‐voltage measurements have indicated the presence of a semi‐insulating layer in the diodes, varying in thickness from 0.18 μ for the most heavily doped substrate to 2.7 μ for the lightest. Frequency dependence of the junction capacitance and power law variation of forward current vs voltage have also been observed and are attributed to deep levels
