Evolved stars in the Local Group galaxies. I. AGB evolution and dust production in IC 1613
We used models of thermally-pulsing asymptotic giant branch (AGB) stars, that
also describe the dust-formation process in the wind, to interpret the
combination of near- and mid-infrared photometric data of the dwarf galaxy IC
1613. This is the first time that this approach is extended to an environment
different from the Milky Way and the Magellanic Clouds (MCs). Our analysis,
based on synthetic population techniques, shows a nice agreement between the
observations and the expected distribution of stars in the colour-magnitude
diagrams obtained with JHK and Spitzer bands. This allows a characterization of
the individual stars in the AGB sample in terms of mass, chemical composition,
and formation epoch of the progenitors. We identify the stars exhibiting the
largest degree of obscuration as carbon stars evolving through the final AGB
phases, descending from 1-1.25Msun objects of metallicity Z=0.001 and from
1.5-2.5Msun stars with Z=0.002. Oxygen-rich stars constitute the majority of
the sample (65%), mainly low mass stars (<2Msun) that produce a negligible
amount of dust (<10^{-7}Msun/yr). We predict the overall dust-production rate
from IC 1613, mostly determined by carbon stars, to be 6x10^{-7}Msun/yr with an
uncertainty of 30%. The capability of the current generation of models to
interpret the AGB population in an environment different from the MCs opens the

The Hot R Coronae Borealis Star DY Centauri is a Binary
The remarkable hot R Coronae Borealis (RCB) star DY Cen is revealed to be the first and only binary system to be found among the RCB stars and their likely relatives, including the extreme helium stars and the hydrogen-deficient carbon stars. Radial velocity determinations from 1982 to 2010 have shown that DY Cen is a single-lined spectroscopic binary in an eccentric orbit with a period of 39.67 days. It is also one of the hottest and most H-rich member of the class of RCB stars. Hot bottom burning and s-process nucleosynthesis in massive AGB stars at the beginning of the thermally-pulsing phase
We report the first spectroscopic identification of massive Galactic
asymptotic giant branch (AGB) stars at the beginning of the thermal pulse (TP)
phase. These stars are the most Li-rich massive AGBs found to date, super
Li-rich AGBs with logE(Li)~3-4. The high Li overabundances are accompanied by
weak or no s-process element (i.e. Rb and Zr) enhancements. A comparison of our
observations with the most recent hot bottom burning (HBB) and s-process
nucleosynthesis models confirms that HBB is strongly activated during the first
TPs but the 22Ne neutron source needs many more TP and third dredge-up episodes
to produce enough Rb at the stellar surface. We also show that the short-lived
element Tc, usually used as an indicator of AGB genuineness, is not detected in
massive AGBs which is in agreement with the theoretical predictions when the

Active galactic nuclei synapses: X-ray versus optical classifications using artificial neural networks
(Abridged) Many classes of active galactic nuclei (AGN) have been defined
entirely throughout optical wavelengths while the X-ray spectra have been very
useful to investigate their inner regions. However, optical and X-ray results
show many discrepancies that have not been fully understood yet. The aim of
this paper is to study the "synapses" between the X-ray and optical
classifications.
For the first time, the new EFLUXER task allowed us to analyse broad band
X-ray spectra of emission line nuclei (ELN) without any prior spectral fitting
using artificial neural networks (ANNs). Our sample comprises 162 XMM-Newton/pn
spectra of 90 local ELN in the Palomar sample. It includes starbursts (SB),
transition objects (T2), LINERs (L1.8 and L2), and Seyferts (S1, S1.8, and S2).
The ANNs are 90% efficient at classifying the trained classes S1, S1.8, and
SB. The S1 and S1.8 classes show a wide range of S1- and S1.8-like components.
We suggest that this is related to a large degree of obscuration at X-rays. The
S1, S1.8, S2, L1.8, L2/T2/SB-AGN (SB with indications of AGN), and SB classes
have similar average X-ray spectra within each class, but these average spectra
can be distinguished from class to class. The S2 (L1.8) class is linked to the
S1.8 (S1) class with larger SB-like component than the S1.8 (S1) class. The L2,
T2, and SB-AGN classes conform a class in the X-rays similar to the S2 class
albeit with larger fractions of SB-like component. This SB-like component is
the contribution of the star-formation in the host galaxy, which is large when
the AGN is weak. An AGN-like component seems to be present in the vast majority
of the ELN, attending to the non-negligible fraction of S1-like or S1.8-like
component. This trained ANN could be used to infer optical properties from

Living with chronic migraine: a qualitative study on female patients' perspectives from a specialised headache clinic in Spain
OBJECTIVES: The aim of this study was to explore the views and experiences of a group of Spanish women suffering from chronic migraine (CM).
SETTING: Headache clinic at a university hospital in Madrid (Spain).
PARTICIPANTS: Purposeful sampling of patients that attended a specialised headache clinic for the first time between June 2016 and February 2017 was performed. The patients included were females aged 18–65 and with positive diagnoses of CM according to the International Classification of Headache disorders (third edition, beta version), with or without medication overuse. Accordingly, 20 patients participated in the study with a mean age of 38.65 years (SD 13.85).
DESIGN: Qualitative phenomenological study.
METHODS: Data were collected through in-depth interviews, researchers’ field notes and patients’ drawings. A thematic analysis was performed following appropriate guidelines for qualitative research.
RESULTS: Five main themes describing the significance of suffering emerged: (a) the shame of suffering from an invisible condition; (b) treatment: between need, scepticism and fear; (c) looking for physicians’ support and sincerity and fighting misconceptions; (d) limiting the impact on daily life through self-control; and (e) family and work: between understanding and disbelief. The disease is experienced as an invisible process, and the journey to diagnosis can be a long and tortuous one. Drug prescription by the physician is greeted with distrust and scepticism. Patients expect sincerity, support and the involvement of their doctors in relation to their disease. Pain becomes the main focus of the patient’s life, and it requires considerable self-control. The disease has a strong impact in the work and family environment, where the patient may feel misunderstood.
CONCLUSIONS: Qualitative research offers insight into the way patients with CM experience their disease and it may be helpful in establishing a more fruitful relationship with these patients
Probing restarting activity in hard X-ray selected giant radio galaxies
With their sizes larger than 0.7 Mpc, Giant Radio Galaxies (GRGs) are the
largest individual objects in the Universe. To date, the reason why they reach
such enormous extensions is still unclear. One of the proposed scenarios
suggests that they are the result of multiple episodes of jet activity.
Cross-correlating the INTEGRAL+Swift AGN population with radio catalogues
(NVSS, FIRST, SUMSS), we found that 22% of the sources are GRG (a factor four
higher than those selected from radio catalogues). Remarkably, 80% of the
sample shows signs of restarting radio activity. The X-ray properties are
consistent with this scenario, the sources being in a high-accretion,
high-luminosity state with respect to the previous activity responsible for the

Cost-Utility Analysis of a Medication Review with Follow-Up Service for Older Adults with Polypharmacy in Community Pharmacies in Spain: The conSIGUE Program

Background: The concept of pharmaceutical care is operationalized through pharmaceutical professional services, which are patient-oriented to optimize their pharmacotherapy and to improve clinical outcomes. Objective: The objective of this study was to estimate the incremental cost-effectiveness ratio (ICER) of a medication review with follow-up (MRF) service for older adults with polypharmacy in Spanish community pharmacies against the alternative of having their medication dispensed normally. Methods: The study was designed as a cluster randomized controlled trial, and was carried out over a time horizon of 6 months. The target population was older adults with polypharmacy, defined as individuals taking five or more medicines per day. The study was conducted in 178 community pharmacies in Spain. Cost-utility analysis adopted a health service perspective. Costs were in euros at 2014 prices and the effectiveness of the intervention was estimated as quality-adjusted life-years (QALYs). In order to analyze the uncertainty of ICER results, we performed a non-parametric bootstrapping with 5000 replications. Results: A total of 1403 older adults, aged between 65 and 94 years, were enrolled in the study: 688 in the intervention group (IG) and 715 in the control group (CG). By the end of the follow-up, both groups had reduced the mean number of prescribed medications they took, although this reduction was greater in the IG (0.28 ± 1.25 drugs; p < 0.001) than in the CG (0.07 ± 0.95 drugs; p = 0.063). Older adults in the IG saw their quality of life improved by 0.0528 ± 0.20 (p < 0.001). In contrast, the CG experienced a slight reduction in their quality of life: 0.0022 ± 0.24 (p = 0.815). The mean total cost was €977.57 ± 1455.88 for the IG and €1173.44 ± 3671.65 for the CG. In order to estimate the ICER, we used the costs adjusted for baseline medications and QALYs adjusted for baseline utility score, resulting in a mean incremental total cost of −€250.51 ± 148.61 (95 % CI −541.79 to 40.76) and a mean incremental QALY of 0.0156 ± 0.004 (95 % CI 0.008–0.023). Regarding the results from the cost-utility analysis, the MRF service emerged as the dominant strategy. Conclusion: The MRF service is an effective intervention for optimizing prescribed medication and improving quality of life in older adults with polypharmacy in community pharmacies. Mapping The Interstellar Medium With Near-Infrared Diffuse Interstellar Bands
We map the distribution and properties of the Milky Way's interstellar medium as traced by diffuse interstellar bands (DIBs) detected in near-infrared stellar spectra from the SDSS-III/APOGEE survey. Focusing exclusively on the strongest DIB in the H band, at lambda similar to 1.527 mu m, we present a projected map of the DIB absorption field in the Galactic plane, using a set of about 60,000 sightlines that reach up to 15 kpc from the Sun and probe up to 30 mag of visual extinction. The strength of this DIB is linearly correlated with dust reddening over three orders of magnitude in both DIB equivalent width (Wpm) and extinction, with a power law index of 1.01 +/- 0.01, a mean relationship of W-DIB/A(v) = 0.1 angstrom mag(-1) and a dispersion of similar to 0.05 angstrom mag(-1) at extinctions characteristic of the Galactic midplane. These properties establish this DIB as a powerful, independent probe of dust extinction over a wide range of Av values. The subset of about 14,000 robustly detected DIB features have a W-DIB distribution that follows an exponential trend. We empirically determine the intrinsic rest wavelength of this transition to be lambda(0) = 15 272.42 angstrom and use it to calculate absolute radial velocities of the carrier, which display the kinematical signature of the rotating Galactic disk. We probe the DIB carrier distribution in three dimensions and show that it can be characterized by an exponential disk model with a scale height of about 100 pc and a scale length of about 5 kpc.
