    Bridging the gap: Increasing awareness of sex differences in opioid addiction using social media platforms

    HONORABLE MENTION Background: Current efforts to combat the opioid epidemic in the United States do not take into account the sex differences in opioid response, prescription, use, and addiction. There is a need for medical professionals and the general public to understand these differences to properly prevent, address, and treat opioid addiction. Healthcare professionals and patients often use the Internet and social networking websites to access and share medical information. These platforms provide an innovative way to educate these groups on various topics in medicine. The American Medical Women’s Association (AMWA) created the Opioid Addiction in Women Task Force to positively impact care for women with opioid addiction. Objectives: Increase health literacy surrounding sex differences in opioid addiction in the general public and healthcare providers via social media platforms Methods/Research: The AMWA Opioid Task Force consists of physicians and medical students across the country who meet virtually each month. With guidance from supervising physicians, educational infographics, pamphlets, and videos are created to facilitate the dissemination of complex scientiffc data in a simple and engaging manner. Information is subsequently shared via social media platforms, such as Instagram, Twitter and YouTube, where they can be easily accessed by healthcare providers, medical students, and the public. Conclusions/Impact: This is one of the few organizations with national exposure that highlights sex and gender differences in the opioid crisis and aims to increase health literacy for both the general public and health care providers. We have successfully distributed information regarding sex differences in opioid addiction during Women’s Health Week 2020 on the national AMWA website, Instagram, and Twitter. Our future plans include producing and sharing videos on YouTube about opioids, addiction, and pain management, especially in women.     The clinical features of the piriformis syndrome: a systematic review

    Piriformis syndrome, sciatica caused by compression of the sciatic nerve by the piriformis muscle, has been described for over 70 years; yet, it remains controversial. The literature consists mainly of case series and narrative reviews. The objectives of the study were: first, to make the best use of existing evidence to estimate the frequencies of clinical features in patients reported to have PS; second, to identify future research questions. A systematic review was conducted of any study type that reported extractable data relevant to diagnosis. The search included all studies up to 1 March 2008 in four databases: AMED, CINAHL, Embase and Medline. Screening, data extraction and analysis were all performed independently by two reviewers. A total of 55 studies were included: 51 individual and 3 aggregated data studies, and 1 combined study. The most common features found were: buttock pain, external tenderness over the greater sciatic notch, aggravation of the pain through sitting and augmentation of the pain with manoeuvres that increase piriformis muscle tension. Future research could start with comparing the frequencies of these features in sciatica patients with and without disc herniation or spinal stenosis

    Small RNA interference-mediated gene silencing of heparanase abolishes the invasion, metastasis and angiogenesis of gastric cancer cells

    <p>Abstract</p> <p>Background</p> <p>Heparanase facilitates the invasion and metastasis of cancer cells, and is over-expressed in many kinds of malignancies. Our studies indicated that heparanase was frequently expressed in advanced gastric cancers. The aim of this study is to determine whether silencing of heparanase expression can abolish the malignant characteristics of gastric cancer cells.</p> <p>Methods</p> <p>Three heparanase-specific small interfering RNA (siRNAs) were designed, synthesized, and transfected into cultured gastric cancer cell line SGC-7901. Heparanase expression was measured by RT-PCR, real-time quantitative PCR and Western blot. Cell proliferation was detected by MTT colorimetry and colony formation assay. The <it>in vitro </it>invasion and metastasis of cancer cells were measured by cell adhesion assay, scratch assay and matrigel invasion assay. The angiogenesis capabilities of cancer cells were measured by tube formation of endothelial cells.</p> <p>Results</p> <p>Transfection of siRNA against 1496-1514 bp of encoding regions resulted in reduced expression of heparanase, which started at 24 hrs and lasted for 120 hrs post-transfection. The siRNA-mediated silencing of heparanase suppressed the cellular proliferation of SGC-7901 cells. In addition, the <it>in vitro </it>invasion and metastasis of cancer cells were attenuated after knock-down of heparanase. Moreover, transfection of heparanase-specific siRNA attenuated the <it>in vitro </it>angiogenesis of cancer cells in a dose-dependent manner.</p> <p>Conclusions</p> <p>These results demonstrated that gene silencing of heparanase can efficiently abolish the proliferation, invasion, metastasis and angiogenesis of human gastric cancer cells <it>in vitro</it>, suggesting that heparanase-specific siRNA is of potential values as a novel therapeutic agent for human gastric cancer.</p

    Biomechanical comparison of anterior cervical plating and combined anterior/lateral mass plating.

    BACKGROUND CONTEXT: Previous studies showed anterior plates of older design to be inadequate for stabilizing the cervical spine in all loading directions. No studies have investigated enhancement in stability obtained by combining anterior and posterior plates. PURPOSE: To determine which modes of loading are stabilized by anterior plating after a cervical burst fracture and to determine whether adding posterior plating further significantly stabilizes the construct. STUDY DESIGN/SETTING: A repeated-measures in vitro biomechanical flexibility experiment was performed to investigate how surgical destabilization and subsequent addition of hardware components alter spinal stability. PATIENT SAMPLE: Six human cadaveric specimens were studied. OUTCOME MEASURES: Angular range of motion (ROM) and neutral zone (NZ) were quantified during flexion, extension, lateral bending, and axial rotation. METHODS: Nonconstraining, nondestructive torques were applied while recording three-dimensional motion optoelectronically. Specimens were tested intact, destabilized by simulated burst fracture with posterior distraction, plated anteriorly with a unicortical locking system, and plated with a combined anterior/posterior construct. RESULTS: The anterior plate significantly (p\u3c.05) reduced the ROM relative to normal in all modes of loading and significantly reduced the NZ in flexion and extension. Addition of the posterior plates further significantly reduced the ROM in all modes of loading and reduced the NZ in lateral bending. CONCLUSIONS: Anterior plating systems are capable of substantially stabilizing the cervical spine in all modes of loading after a burst fracture. The combined approach adds significant stability over anterior plating alone in treating this injury but may be unnecessary clinically. Further study is needed to assess the added clinical benefits of the combined approach and associated risks
