    Heart To Heart: Educating Allied Healthcare Students and Professionals About Heart Failure

    Heart failure (HF) is a health problem which providing necessary education to healthcare professionals who serve individuals with HF allows for continuity of care and the promotion of disease management for this population. The intent of this research was to determine if providing an educational in-service to allied healthcare students and practitioners increased their knowledge of OT’s role with HF clients and about HF itself as a condition interfering with occupation. The design of this study involved collecting pre/post survey data to evaluate the knowledge of the participants about the HF population and gathering participant satisfaction data regarding the in-service. A three-hour in-service was developed covering areas of best practice management for clients with HF, OT’s role is working with clients on disease self-management of HF, and the nonmedical needs of clients with HF. A total of 22 healthcare practitioners and 32 allied healthcare students participated in the study. Data was collected and analyzed based on participants’ scores from the 20 question surveys and were compared as a group. Participants pre-survey mean score was 55% compared to post-survey mean score of 77%. Paired t-tests of the data determined that knowledge gained by students and practitioners from participating in the educational in-service was statistically significant (p\u3c0.05). Participants demonstrated increased knowledge about the importance of OT’s role in disease self-management with HF patients and best practice management of HF. The results of the study support providing structured inter/intraprofessional education for increasing understanding of OT’s role with and providing integrated care for complex clients

    Informed choice? How the United Kingdom's Key Information Set fails to represent pedagogy to potential students

    This paper explores the ways in which information about course pedagogy has been represented to potential students through national descriptors and specifications such as the United Kingdom’s Key Information Set. It examines the extent to which such descriptors provide helpful information about pedagogy, for example innovative uses of technology. The paper starts by exploring the wider context within which these descriptors have been developed, including a comparison of similar descriptions internationally. This is followed by a comparative analysis, in which two courses (one single honours undergraduate degree, one Massive Open Online Course) are classified and compared. This serves to illustrate the blind spots in classifications such as the Key Information Set. The paper concludes by arguing that further work is needed to develop classification schemes that both address explicitly the interests of potential students and are able to represent the pedagogic decisions that differentiate teaching in contemporary higher education

    On the Computational Complexity of Vertex Integrity and Component Order Connectivity

    The Weighted Vertex Integrity (wVI) problem takes as input an nn-vertex graph GG, a weight function w:V(G)Nw:V(G)\to\mathbb{N}, and an integer pp. The task is to decide if there exists a set XV(G)X\subseteq V(G) such that the weight of XX plus the weight of a heaviest component of GXG-X is at most pp. Among other results, we prove that: (1) wVI is NP-complete on co-comparability graphs, even if each vertex has weight 11; (2) wVI can be solved in O(pp+1n)O(p^{p+1}n) time; (3) wVI admits a kernel with at most p3p^3 vertices. Result (1) refutes a conjecture by Ray and Deogun and answers an open question by Ray et al. It also complements a result by Kratsch et al., stating that the unweighted version of the problem can be solved in polynomial time on co-comparability graphs of bounded dimension, provided that an intersection model of the input graph is given as part of the input. An instance of the Weighted Component Order Connectivity (wCOC) problem consists of an nn-vertex graph GG, a weight function w:V(G)Nw:V(G)\to \mathbb{N}, and two integers kk and ll, and the task is to decide if there exists a set XV(G)X\subseteq V(G) such that the weight of XX is at most kk and the weight of a heaviest component of GXG-X is at most ll. In some sense, the wCOC problem can be seen as a refined version of the wVI problem. We prove, among other results, that: (4) wCOC can be solved in O(min{k,l}n3)O(\min\{k,l\}\cdot n^3) time on interval graphs, while the unweighted version can be solved in O(n2)O(n^2) time on this graph class; (5) wCOC is W[1]-hard on split graphs when parameterized by kk or by ll; (6) wCOC can be solved in 2O(klogl)n2^{O(k\log l)} n time; (7) wCOC admits a kernel with at most kl(k+l)+kkl(k+l)+k vertices. We also show that result (6) is essentially tight by proving that wCOC cannot be solved in 2o(klogl)nO(1)2^{o(k \log l)}n^{O(1)} time, unless the ETH fails.Comment: A preliminary version of this paper already appeared in the conference proceedings of ISAAC 201

    Our aim, in editing the ‘London Issue’ of this journal, is to contribute to a conversation between scholars of British cinema and television, London historians and scholars of the cinematic city. In 2007, introducing the themed issue on ‘Space and Place in British Cinema and Television’, Steve Chibnall and Julian Petley observed that it would have been possible to fill the whole journal with essays about the representation of London. This issue does just that, responding to the increased interest in cinematic and, to a lesser extent, televisual, Londons, while also demonstrating the continuing fertility of the paradigms of ‘space and place’ for scholars of the moving image1. It includes a wide range of approaches to the topic of London on screen, with varying attention to British institutions of the moving image – such as Channel Four or the British Board of Film Classification – as well as to concepts such as genre, narration and memory. As a whole, the issue, through its juxtapositions of method and approach, shows something of the complexity of encounters between the terms ‘London’, ‘cinema’ and ‘television’ within British film and television studies

    Impaired CXCL12 signaling contributes to resistance of pancreatic cancer subpopulations to T cell-mediated cytotoxicity

    Pancreatic cancer remains largely unresponsive to immune modulatory therapy attributable in part to an immunosuppressive, desmoplastic tumor microenvironment. Here, we analyze mechanisms of cancer cell-autonomous resistance to T cells. We used a 3D co-culture model of cancer cell spheroids from the KPC (LSL-Kras(G12D/+)/LSL-Trp53(R172H/+)/p48-Cre) pancreatic ductal adenocarcinoma (PDAC) model, to examine interactions with tumor-educated T cells isolated from draining lymph nodes of PDAC-bearing mice. Subpopulations of cancer cells resistant to these tumor-educated T cells were isolated from the in vitro co-culture and their properties compared with sensitive cancer cells. In co-culture with resistant cancer cell subpopulations, tumor-educated T cells showed reduced effector T cell functionality, reduced infiltration into tumor cell spheroids and decreased induction of apoptosis. A combination of comparative transcriptomic analyses, cytometric and immunohistochemistry techniques allowed us to dissect the role of differential gene expression and signaling pathways between sensitive and resistant cells. A decreased expression of the chemokine CXCL12 (SDF-1) was revealed as a common feature in the resistant cell subpopulations. Adding back CXCL12 reversed the resistant phenotype and was inhibited by the CXCR4 inhibitor AMD3100 (plerixafor). We conclude that reduced CXCL12 signaling contributes to PDAC subpopulation resistance to T cell-mediated attack

    Hostility, Race, and Glucose Metabolism in Nondiabetic Individuals

    OBJECTIVE— The present study was designed to determine whether hostility is differentially related to measures of glucose metabolism in African-Americans and Caucasians. RESEARCH DESIGN AND METHODS— The relationship of hostility, as measured by a subset of the Cook-Medley hostility scale (CMHOST) inventory items, to various parameters of glucose metabolism were examined in a young, healthy sample of male and female African-American and Caucasian volunteers. Fasting blood samples were collected during an inpatient admission, at which time the CMHOST was also administered. RESULTS— In the entire sample, the CMHOST was found to be significantly correlated with fasting glucose and insulin sensitivity, as measured by the homeostatic model assessment (HOMA). However, the relationship of hostility to these parameters of glucose metabolism was different in African-American and Caucasian subjects. Hostility was significantly related to fasting glucose in African-Americans and to insulin sensitivity and fasting insulin in Caucasian subjects. The relationship of hostility to insulin sensitivity and fasting insulin was partially dependent on BMI in Caucasians, but the relationship of hostility to fasting glucose was unrelated to BMI in African-Americans. CONCLUSIONS— Our data suggest that the relationship of hostility to measures of glucose metabolism is mediated differently in these two ethnic groups. Therefore, hostility seems to be part of a constellation of risk-related behaviors related to BMI in Caucasians but independently related to fasting glucose in African-Americans

    Identity and belonging in social learning groups : the importance of distinguishing social, operational and knowledge-related identity congruence

    Collaborative learning has much to offer but not all learners participate fully and peer groups can be exclusive. The paper examines how belonging or 'congruence' in learning groups is related to identities of gender, age, ethnicity and socio-economic status. A study of student experiences of collaborative learning on three different blended learning courses illustrated how learners negotiate identity congruence with peer groups to belong and engage. An analytical framework that distinguishes social, operational and knowledge-related identity congruence has emerged. Contrary to received wisdom, the social aspect appears least important for learner engagement while knowledge-related identity congruence is fundamental. Some of the consequences of identity incongruence, particularly concerning gender and maturity, are discussed and the paper points towards the pedagogies which might enable identities of group members to shift so that collaborative learning can flourish

    Modulation of emotional appraisal by false physiological feedback during fMRI

    BACKGROUND James and Lange proposed that emotions are the perception of physiological reactions. Two-level theories of emotion extend this model to suggest that cognitive interpretations of physiological changes shape self-reported emotions. Correspondingly false physiological feedback of evoked or tonic bodily responses can alter emotional attributions. Moreover, anxiety states are proposed to arise from detection of mismatch between actual and anticipated states of physiological arousal. However, the neural underpinnings of these phenomena previously have not been examined. METHODOLOGY/PRINCIPAL FINDINGS We undertook a functional brain imaging (fMRI) experiment to investigate how both primary and second-order levels of physiological (viscerosensory) representation impact on the processing of external emotional cues. 12 participants were scanned while judging face stimuli during both exercise and non-exercise conditions in the context of true and false auditory feedback of tonic heart rate. We observed that the perceived emotional intensity/salience of neutral faces was enhanced by false feedback of increased heart rate. Regional changes in neural activity corresponding to this behavioural interaction were observed within included right anterior insula, bilateral mid insula, and amygdala. In addition, right anterior insula activity was enhanced during by asynchronous relative to synchronous cardiac feedback even with no change in perceived or actual heart rate suggesting this region serves as a comparator to detect physiological mismatches. Finally, BOLD activity within right anterior insula and amygdala predicted the corresponding changes in perceived intensity ratings at both a group and an individual level. CONCLUSIONS/SIGNIFICANCE Our findings identify the neural substrates supporting behavioural effects of false physiological feedback, and highlight mechanisms that underlie subjective anxiety states, including the importance of the right anterior insula in guiding second-order "cognitive" representations of bodily arousal state
