    Exogenous application of platelet-leukocyte gel during open subacromial decompression contributes to improved patient outcome

    Background: Platelet-leukocyte gel (PLG) is being used during various surgical procedures in an attempt to enhance the healing process. We studied the effects of PLG on postoperative recovery of patients undergoing open subacromial decompression (OSD). Methods: PLG was produced from platelet-leukocyte-rich plasma (P-LRP), prepared from a unit of whole blood. Forty patients were included in the study. Self-assessed evaluations, using the American Shoulder and Elbow Surgeons scoring system of activities of daily living (ADL), joint instability, pain levels, pain medications, and clinical evaluations for range of motion were conducted. Results: Platelet and leukocyte counts were significantly increased in the P-LRP compared to baseline counts. Treated patients demonstrated decreased visual analog scales for pain and used significantly less pain medication, had an improved range of motion during passive forward elevation, external rotation, external rotation with arm at 90 degrees abduction, internal rotation, and cross body adduction compared to control patients (p < 0.001). No differences in the instability score were observed between the groups. Furthermore, treated patients performed more ADL (p < 0.05). Conclusion: In the PLG-treated group, recovery was faster and patients returned earlier to daily activities and also took less pain medication than control subjects

    Cognitive improvement in patients with carotid stenosis is independent of treatment type

    Treatment of carotid artery stenosis decreases the long-term risk of stroke and may enhance cerebral blood flow. It is therefore expected to have the potential to prevent cognitive decline or even improve cognition over the long-term. However, intervention itself can cause peri-interventional cerebral infarcts, possibly resulting in a decline of cognitive performance, at least for a short time. We investigated the long-term effects of three treatment methods on cognition and the emotional state one year after intervention. In this prospective observational cohort study, 58 patients with extracranial carotid artery stenosis (≥ 70%) underwent magnetic resonance imaging and assessment of cognition, mood and motor speed before carotid endarterectomy (n = 20), carotid stenting (n = 10) or best medical treatment (n = 28) (i.e., time-point 1 [TP1]), and at one-year follow-up (TP2). Gain scores, reflecting cognitive change after treatment, were built according to performance as (TP2 -TP1)/TP1. Independent of the treatment type, significant improvement in frontal lobe functions, visual memory and motor speed was found. Performance level, motor speed and mood at TP1 were negatively correlated with gain scores, with greater improvement in patients with low performance before treatment. Active therapy, whether conservative or interventional, produces significant improvement of frontal lobe functions and memory in patients with carotid artery disease, independent of treatment type. This effect was particularly pronounced in patients with low cognitive performance prior to treatment

    Background and purpose Activated platelets release a cocktail of growth factors, some of which are thought to stimulate repair. We investigated whether the use of autologous platelet gel (PG) in total knee arthroplasty (TKA) would improve wound healing and knee function, and reduce blood loss and the use of analgesics. Patients and methods 102 patients undergoing TKA were randomly assigned to a PG group (n 50) or to a control (C) group (n 52). The primary analysis was based on 73 participants (PG: 32; C: 41) with comparison of postoperative wound scores, VAS, WOMAC, knee function, use of analgesics, and the pre- and postoperative hemoglobin values after a follow-up of 3 months. 29 participants were excluded due to insufficient data.     Quantum gases in trimerized kagom\'e lattices

    We study low temperature properties of atomic gases in trimerized optical kagom\'{e} lattices. The laser arrangements that can be used to create these lattices are briefly described. We also present explicit results for the coupling constants of the generalized Hubbard models that can be realized in such lattices. In the case of a single component Bose gas the existence of a Mott insulator phase with fractional numbers of particles per trimer is verified in a mean field approach. The main emphasis of the paper is on an atomic spinless interacting Fermi gas in the trimerized kagom\'{e} lattice with two fermions per site. This system is shown to be described by a quantum spin 1/2 model on the triangular lattice with couplings that depend on the bond directions. We investigate this model by means of exact diagonalization. Our key finding is that the system exhibits non-standard properties of a quantum spin-liquid crystal: it combines planar antiferromagnetic order in the ground state with an exceptionally large number of low energy excitations.     Size Dependence In The Disordered Kondo Problem

    We study here the role randomly-placed non-magnetic scatterers play on the Kondo effect. We show that spin relaxation effects (with time τso\tau_s^o)in the vertex corrections to the Kondo self-energy lead to an exact cancellation of the singular temperature dependence arising from the diffusion poles. For a thin film of thickness LL and a mean-free path \ell, disorder provides a correction to the Kondo resistivity of the form τso/(kFL2)lnT\tau_s^o/(k_FL\ell^2)\ln T that explains both the disorder and sample-size depression of the Kondo effect observed by Blachly and Giordano (PRB {\bf 51}, 12537 (1995)).Comment: 11 pages, LaTeX, 2 Postscript figure

    Correlations in the Ising antiferromagnet on the anisotropic kagome lattice

    We study the correlation function of middle spins, i. e. of spins on intermediate sites between two adjacent parallel lattice axes, of the spatially anisotropic Ising antiferromagnet on the kagome lattice. It is given rigorously by a Toeplitz determinant. The large-distance behaviour of this correlation function is obtained by analytic methods. For shorter distances we evaluate the Toeplitz determinant numerically. The correlation function is found to vanish exactly on a line J_d(T) in the T-J (temperature vs. coupling constant) phase diagram. This disorder line divides the phase diagram into two regions. For J less than J_d(T) the correlations display the features of an unfrustrated two-dimensional Ising magnet, whereas for J greater than J_d(T) the correlations between the middle spins are seen to be strongly influenced by the short-range antiferromagnetic order that prevails among the spins of the adjacent lattice axes. While for J less than J_d(T) there is a region with ferrimagnetic long-range order, the model remains disordered for J greater than J_d(T) down to T=0.Comment: 26 pages, 9 figures, published versio

    Multi-particle structure in the Z_n-chiral Potts models

    We calculate the lowest translationally invariant levels of the Z_3- and Z_4-symmetrical chiral Potts quantum chains, using numerical diagonalization of the hamiltonian for N <= 12 and N <= 10 sites, respectively, and extrapolating N to infinity. In the high-temperature massive phase we find that the pattern of the low-lying zero momentum levels can be explained assuming the existence of n-1 particles carrying Z_n-charges Q = 1, ... , n-1 (mass m_Q), and their scattering states. In the superintegrable case the masses of the n-1 particles become proportional to their respective charges: m_Q = Q m_1. Exponential convergence in N is observed for the single particle gaps, while power convergence is seen for the scattering levels. We also verify that qualitatively the same pattern appears for the self-dual and integrable cases. For general Z_n we show that the energy-momentum relations of the particles show a parity non-conservation asymmetry which for very high temperatures is exclusive due to the presence of a macroscopic momentum P_m=(1-2Q/n)/\phi, where \phi is the chiral angle and Q is the Z_n-charge of the respective particle.Comment: 22 pages (LaTeX) plus 5 figures (included as PostScript), BONN-HE-92-3

    Skin and liver diseases induced in flounder (Platichthys flesus) after long-term exposure to contaminated sediments in large-scale mesocosms.

    Disease development in flounder (Platichthys flesus) was studied over a period of 3 years in three large mesocosms (40 m x 40 m x 3 m). Two of the mesocosms contained clean sand and the third, sharing a common water circulation with one of the clean-sand mesocosms, was stocked with contaminated dredged spoil. In this way, one of the clean-sand mesocosms was indirectly polluted via the water phase, and analysis of contaminant concentrations in sediments and flounder tissues showed that it had a status intermediate between the other two. Random samples of the flounder populations from the indirectly polluted and reference mesocosms were examined every 2 months for epidermal diseases (lymphocystis, skin ulcers, fin rot) and then released. In addition, every 6 months, random samples of fish from all three mesocosms were sacrificed for histological and chemical investigation. With regard to the development of epidermal disease, the results showed little difference between the reference mesocosm and the indirectly polluted mesocosm, with the exception that lymphocystis was significantly elevated in the indirectly polluted mesocosm. Although pollution may be a risk factor in the etiology of this disease, such a relationship would probably be obscured under field conditions due to variation arising from other factors. Histopathological analysis of the livers revealed in total four cases of hepatocellular adenoma (1.5% of sampled population) in fish from the polluted mesocosms, the first occurring after 2.5 years of exposure in fish from the indirectly polluted mesocosm. Furthermore, several other liver lesions, including foci of cellular alteration and hydropic vacuolated lesions, developed during the course of the experiment before tumor formation was apparent. Prevalences of these conditions were very much lower in the reference mesocosm than in the two polluted mesocosms. Densities of melanomacrophage centers in the liver showed a similar trend. The findings clearly indicate that long-term exposure to chemically contaminated dredged spoil can induce liver neoplasia and other liver lesions in flounder at contaminant levels comparable to those found in the natural environment

    Overexpression of Cyclin E1 or Cdc25A leads to replication stress, mitotic aberrancies, and increased sensitivity to replication checkpoint inhibitors

    Oncogene-induced replication stress, for instance as a result of Cyclin E1 overexpression, causes genomic instability and has been linked to tumorigenesis. To survive high levels of replication stress, tumors depend on pathways to deal with these DNA lesions, which represent a therapeutically actionable vulnerability. We aimed to uncover the consequences of Cyclin E1 or Cdc25A overexpression on replication kinetics, mitotic progression, and the sensitivity to inhibitors of the WEE1 and ATR replication checkpoint kinases. We modeled oncogene-induced replication stress using inducible expression of Cyclin E1 or Cdc25A in non-transformed RPE-1 cells, either in a TP53 wild-type or TP53-mutant background. DNA fiber analysis showed Cyclin E1 or Cdc25A overexpression to slow replication speed. The resulting replication-derived DNA lesions were transmitted into mitosis causing chromosome segregation defects. Single cell sequencing revealed that replication stress and mitotic defects upon Cyclin E1 or Cdc25A overexpression resulted in genomic instability. ATR or WEE1 inhibition exacerbated the mitotic aberrancies induced by Cyclin E1 or Cdc25A overexpression, and caused cytotoxicity. Both these phenotypes were exacerbated upon p53 inactivation. Conversely, downregulation of Cyclin E1 rescued both replication kinetics, as well as sensitivity to ATR and WEE1 inhibitors. Taken together, Cyclin E1 or Cdc25A-induced replication stress leads to mitotic segregation defects and genomic instability. These mitotic defects are exacerbated by inhibition of ATR or WEE1 and therefore point to mitotic catastrophe as an underlying mechanism. Importantly, our data suggest that Cyclin E1 overexpression can be used to select patients for treatment with replication checkpoint inhibitors
