    The impact of climate change on household food security in the Bongo District of the Upper East Region of Ghana

    The study determined the impact of climate change on household food security, investigated awareness level and identified coping strategies used to mitigate negative impact of climate change in the Bongo District with emphasis on women. It was a cross-sectional survey conducted in four purposively selected farming communities in the District in the Upper East Region of Ghana. A sample size of 246 women participated in the study. Using a structured questionnaire, data were collected on socio-demographic characteristics of the participants, awareness of climate change, food consumption patterns, coping strategies and household food security. Data on rainfall and temperature for the past 30 years period in the District were obtained from the Ghana Meteorological Service. Crop yields data over a period of 21 years in the District was also obtained from the Ministry of Food and Agriculture (MOFA). The results revealed that rainfall in the Bongo District has been decreasing at - 0.3 mm per annum and maximum temperature has been increasing at 0.005 °C. More than half (62.6%) of the participants were aware of climate change in their communities. Majority (89.4%) of the participants reduced the quality and quantity of diets as coping strategy method during food shortage periods and almost all (97.2%) of the households were food insecure. Participants who reported to have observed decrease in rainfall were more likely to be food insecure (OR = 3.96; CI = 0.56 – 27.81). Participants were aware of climate change and employed reduction in the quality and quantity of diet as coping mechanism. Almost all households were food insecure. There is the need to intensify climate resistant agriculture technology such as irrigation methods to offset the negative impact of climate change on food security in the District.

    Food and nutrition security situation in Ghana: Nutrition implications for national development

    Achieving food and nutrition security is fundamental to the Sustainable Development Goals (SDGs) due to its strong interdependence with economic growth and development. Eradicating hunger and all forms of malnutrition (SDG 2) remain high on the global agenda and is at the forefront of high panel deliberations because poor dietary intake has long term negative consequences on individuals and economic advancement. The goal to end hunger and malnutrition can be achieved when food and nutrition insecurity is properly tackled through investment in agriculture parallel to economic and social protection programmes. In Ghana, about 1.5 million of the population are estimated to be food insecure while undernutrition, overnutrition and micronutrient deficiencies persist across the life stages. Challenges to food and nutrition security in Ghana have been identified as: poverty, climate change, rapid urbanization and population growth, gender inequalities and poor infrastructure. Poor economic growth, health, education, hygiene and environmental  exploitation are implications of these challenges. Addressing these implications sets long-term foundation for the development of the nation by prioritising policies that are nutrition sensitive which directly address the complexity of malnutrition. The level of food and nutrition insecurity in Ghana can be reduced through a national commitment towards addressing the four pillars of food security coupled with programmes that bring about resilience through sustainable systems. To this end, intervention programmes have been introduced by government to reduce the poverty burden on households. These programmes are in the form of social interventions, governmental flagship projects, and research-driven agricultural interventions to improve yield that withstand the effect of climate change. This review is aimed to present the food and nutrition security situation in Ghana and emphasise the challenges that exacerbate the problem while bringing to light the nutritional implications to national development. It is hoped that the recommendations from this review will help the government in achieving food and nutrition security in Ghana

    Hospital based maternity care in Ghana - findings of a confidential enquiry into maternal deaths

    Background: In Ghana, a universal free delivery policy was implemented to improve access to delivery care in health facilities, thereby improving access to skilled attendance and reducing maternalmortality. Objective: A confidential enquiry was conducted to ascertain if changes had occurred in the care provided by reviewing the care given to a sample of maternal deaths before and after introduction of the policy. Method: Twenty women who died as a result of pregnancy-related complications (maternal deaths) in selected hospitals in two regions were assessed by a clinical panel, guided by a maternal deathassessment form. Unlike the traditional confidential enquiry process, both adverse and favourable factors were identified. Findings: Clinical care provided before and after the introduction of the fee exemption policy did not change, though women with complications were arriving in hospital earlier after the introduction of the policy. On admission, however, they received very poor care and this, the clinical paneldeduced could have resulted in many avoidable deaths; as was the case before the implementation of the policy. Consumables, basic equipment and midwifery staff for providing comprehensive emergency obstetric care were however found to be usually available. Conclusion: Our findings suggest that the already poor delivery care services women received remained unchanged after introduction of the policy

    Introducing the Disease Outbreak Resilience Index (DORI) Using the Demographic and Health Surveys Data from sub-Saharan Africa

    Although most studies on disease emergencies underscore the need for household readiness for shocks associated with disease outbreaks, no study to date has provided a holistic measure for profiling households based on their readiness toward disease outbreaks. This paper introduces a novel Disease Outbreak Resilience Index (DORI) using a multidimensional approach that draws on the Alkire-Foster methodology. DORI measures disease outbreak resilience in four dimensions: (a) water and hygiene, (b) physical distancing, (c) energy and communication, and (d) economic security and resilience. The paper details the development of DORI and its use by presenting findings from ten countries in sub-Saharan Africa using data from the Demographic and Health Surveys (DHS) program. In addition to serving as a resilience index, we illustrate how DORI can be used to produce a disease outbreak vulnerability index (DOVI). As a versatile index, the indicators under each dimension can be tailored to meet country- and region-specific contexts based on indicators appropriate to each context

    Junior high school students’ use of their afterschool hours in Ghana: The role of household assets

    Studies have examined a broad range of factors for how students use their time, but few attempts have been made to explore the nuanced link between different types of asset ownership and students' use of study time, particularly in resource-limited countries. This study uses data from junior high school students in Ghana to examine how students spend their time after school hours, the predictive influence of different types of household assets, and the extent to which these trends and relationships vary by gender. Polynomial quantile regression models were fitted across three quantiles (24th, 53rd, and 76th percentiles) to align with one hour, one and half hours, and two hours of study time. Results show that the average student spends well above the recommended 90 minutes on their schoolwork during afterschool hours, regardless of gender. Multivariate results indicate that owning limited assets tends to have a negative relationship with use of study time, but higher levels tend to be positively related to use of study time. Also, the predictive influence of asset ownership varies by asset type, and higher levels of asset ownership favor girls more than boys. Given this study's realtively small sample size, caution must be exercised in generalizing the study findings to the general population of junior high school students in Ghana. In light of the study's limitations, the finding of varying asset effect may have practical implications for asset development programs designed to enhance the well-being of low-income families

    The importance of self-efficacy and educational aspirations for academic achievement in resource-limited countries: Evidence from Ghana

    Introduction: Research on the influence of academic self-efficacy and educational aspirations on academic performance is underdeveloped in resource-limited countries. This study replicates and expands on earlier research that investigated a complex network of relationships between academic self-efficacy, educational aspirations, and academic performance. Methods: Data from 4282 adolescents in Ghana and path analysis were used to test the causal pathways, and path invariance analysis was used to assess the moderation role of gender. Instrumental variable techniques were used to validate the path models. Results: Increase in academic self-efficacy indirectly accounts for improvement in academic performance through the mediational role of educational aspirations. The effects of self-efficacy on educational aspirations, and educational aspirations were stronger for boys than for girls. Conclusions: These findings suggest that in resource-limited countries where the financial burden of schooling tends to be a demotivating factor, interventions that target adolescents' academic self-efficacy may be an effective means to boost educational aspirations and academic performance. Interventions should be tailored to meet the needs of all students so that all children can think of school as an important part of their lives and aspire to achieve, now and in the future

    Psychosocial Syndemics and Sexual Risk Practices Among U.S. Adolescents: Findings from the 2017 U.S. Youth Behavioral Survey

    Background: The present study aims to (1) identify classes of psychosocial syndemics among adolescents in the U.S. based on psychological factors, such as depression and suicidal ideation, and social factors, such as binge drinking, alcohol use, and drug use; (2) identify correlates of psychosocial syndemics; and (3) examine the independent associations between psychosocial syndemic factors and sexual risk practices. Method: We used latent class analysis and a sample of 14,762 U.S. high school students who participated in the 2017 Youth Risk Behavior Surveillance System to examine youth population profiles based on shared characteristics of syndemics. Adjusting for sociodemographic factors, we conducted logistic regression to explore the connections between psychosocial syndemic factors and three sexual risk practices, namely, early initiation of sexual intercourse, condom use, and sex with multiple partners. Results: The study results indicate that three classes of risk exist among the sample: substance misuse (class 1, n = 3872, 26.2%), normative (class 2, n = 8791, 59.6%), and mental health problems (class 3, n = 2099, 14.2%). Class membership of psychosocial syndemics was significantly different by gender, age group, and race. The odds of initiating sexual intercourse before age 13 were positively associated with participants belonging in the substance misuse class and the mental health problem class. The odds of using condoms during the last sexual intercourse for currently sexually active adolescents were lower for participants classified in the substance misuse class. The likelihood of reporting having sex with four or more partners in a lifetime was higher among participants belonging to the substance misuse class. Conclusion: The study advances our understanding of the heterogeneity of class membership associated with psychosocial syndemic risk factors among adolescents and extends our understanding of syndemics in the area of adolescent health. Thus, practitioners and policymakers can design multicomponent and multilevel school-based HIV/STI prevention programs that meet the needs of adolescents

    Addressing the Burden of Education Financing in Low and Lower-Middle-Income Countries: The Role of Savings Accounts, Cash Transfers, and Other Income Sources

    Cost-sharing between governments and families remains a strategic part of many governments’ post-secondary education funding policies in low-income countries. This shift to more cost-sharing raises questions about how households meet their contributions to post-secondary schooling costs. This study uses data from the World Bank’s Global Financial Inclusion survey and World Development Indicators to examine how savings account ownership, cash transfers, and other forms of income shape families’ decisions about education financing in 59 low and lower-middle-income countries. Results from generalized hierarchical linear modeling and logistic regression models show that individuals with savings accounts are more likely to dedicate resources to educational purposes than those without accounts. Other forms of income (cash transfers excepted) also predict an individual’s likelihood of earmarking savings for education to a lesser degree. Our findings offer compelling evidence that greater access to formal savings services may provide a viable long-term strategy to help families prepare financially for their children’s future education. These findings may inform future programs that promote financial inclusion and expand access to formal savings services to help individuals and families save for their children’s education

    Perceived family economic hardship and student engagement among junior high schoolers in Ghana

    Research has shown that several factors influence student engagement, but little is known about the predictive role of family economic hardships on student engagement, particularly in the sub-Saharan African context. This study used data from junior high school students in Ghana to examine the association between perceived family economic hardship and students' classroom engagement, and the intervening role of future intentions. The structural equation modeling results indicate that perceived economic hardship is associated with behavioral engagement in school in two ways. The first is the direct positive link from perceived economic hardship to engagement (i.e., the motivational pathway). The second is a more nuanced channel whereby perceived economic difficulties are negatively associated with students' participation in academic work, but only through the inconsistent mediating (suppression) role of students' future intentions (i.e., the demoralizing pathway). The nuanced psychological and behavioral outcomes suggest the need for programs that cultivate educational resilience among young people
