The use of endoscopy and biopsy for the diagnosis of kidney disease in free- living birds of prey and owls

wildlebende Greifvögel und Eulen klinisch untersucht, hämatologische und
klinisch-chemische Blutwerte erhoben sowie Röntgenaufnahmen angefertigt. Nach
gegebenenfalls notwendiger Stabilisierung wurden die Tiere endoskopiert und
die linke Niere bioptiert. Ziele der Arbeit waren: 1\. die Endoskopie und
endoskopische Bioptatentnahme zur Diagnose von Nierenerkrankungen bei
Greifvögeln und Eulen zu evaluieren sowie die Aussagefähigkeit der Bioptate zu
ermitteln, 2\. die allgemeine Verträglichkeit des Verfahrens und den Einfluss
der Bioptatentnahme auf ausgewählte Blutparameter zu überprüfen, 3\. die
Ergebnisse der klinischen Untersuchung, der Blutanalyse und der
röntgenologischen Untersuchung mit den Befunden der Endoskopie und
endoskopischen Biopsie hinsichtlich Nierenaffektionen zu vergleichen. zu 1.
Nierenbioptate stellen eine sinnvolle Ergänzung zur Diagnostik von
Nierenerkrankungen dar. Der laterale Zugang durch den kaudalen thorakalen und
abdominalen Luftsack lässt die Beurteilung und Bioptatentnahme einer Niere zu.
Aufgrund der anatomischen Aufbaus ist die Ansicht der zweiten Niere durch
diesen Zugang nicht möglich. Durch die endoskopische Untersuchung wurden bei
2,2% (2/89) der Vögel hochgradige, bei 13,5% (12/89) mittelgradige, bei 52,8%
(47/89) geringgradige Nierenbefunde festgestellt. 31,5% (28/89) der Tiere
wiesen keine makroskopischen Veränderungen auf. Es wurden nur lokale und keine
generalisierten Abweichungen an den Nieren festgestellt. Bioptate mit
pathologischen Befunden (n = 66) wiesen eine (53%), zwei (37,9%), drei (7,6%)
und fünf (1,5%) verschiedene pathologische Veränderungen gleichzeitig auf. Bei
0,8% (1/126) konnten hochgradige, bei 10,3% (13/126) mittelgradige, bei 43,7%
(55/126) geringgradige und bei 45,2% (57/126) keine histologischen
Veränderungen in den Bioptaten festgestellt werden. Zu den in den
Schnittpräparaten der Bioptate diagnostizierten Befunden gehörten unter
anderem subkapsuläre Blutungen (19/126), kleinherdige entzündliche
Infiltrationen (16/126), Zellzylinder (12/126), PAS-positives Material (8/126)
und Proteinzylinder in den Tubuli (6/126). Bei 76,1% der Proben konnten
histologisch und endoskopisch übereinstimmende Befunde nachgewiesen werden.
Die Diagnosen von Biopsie und histologischer Organuntersuchung waren in 30,8%
(12/39) der Fälle übereinstimmend. 20,5% (8/39) der histopathologisch
untersuchten Tiere wiesen Befunde auf, die endoskopisch nicht nachweisbar
waren. zu 2. Die Bioptatentnahme mittels Biopsiezange erfolgte ohne
Komplikationen. Die nach der Probennahme einsetzende Blutung dauerte
durchschnittlich 67 Sekunden. Am Tag nach der Biopsie kam es zu einem
signifikanten Anstieg der Harnstoff- und Phosphorwerte. Einzelne Vögel zeigten
einen stärkeren Abfall des Hämatokrits bis maximal 10%. Die
Verlaufsuntersuchungen über bis zu fünf Tage ließen bei Hämatokrit, Phosphor,
Harnsäure und Harnstoff kein einheitliches Verhalten erkennen. Die 126
Bioptate der 89 Vögel wiesen im Durchschnitt eine Länge von 2,2 mm, eine
Breite von 1,3 mm und eine Höhe von 1 mm auf. In allen 126 Schnittpräparaten
der Bioptate befanden sich proximale und distale Tubuli sowie 1 - 89
Glomerula. Durchschnittlich waren pro Präparat 25 - 29 Glomerula vorhanden.
Ein bis vier intralobuläre Venen wiesen 38,9% (49/126) der Schnittpräparate
auf. Mit dem Nierenbioptat wurden durchschnittlich 10,7% Luftsackgewebe
entnommen. Eine sehr gute bis gute Beurteilung der Bioptate war bei 89,7%
(113/126) Proben möglich. Keine entnahmebedingten Schäden wiesen 59,5%
(75/126) der Proben auf. zu 3. Bei der allgemeinen klinischen Untersuchung vor
der Endoskopie und Biopsie wies kein Tier Hinweise auf eine Nierenveränderung
auf. Bei 28,3% (15/53) der Mäusebussarde lagen die Harnsäurewerte vor der
Endoskopie über dem Referenzwert von 8,5 mg/dl und bei 5,7% (3/53) die
Harnstoffwerte über dem entsprechenden Referenzwert von 36 mg/dl. Ein Habicht
(1/13; 7,7%) wies einen Harnsäurewert von mehr als 14,3 mg/dl auf. Zwei
Sperber lagen über dem Referenzwert von 9,1 mg/dl. Es konnte kein Zusammenhang
zwischen den Röntgenbefunden und der Bioptathistologie festgestellt werden.
Zum Teil wiesen endoskopisch sowie histologisch unveränderte Nieren
röntgenologische Befunde auf. Die mikrobiologische Untersuchung der Nieren
sezierter Vögel ergab bei 69,1% (38/55) kein Keimwachstum. In 21,8% der Proben
wurden E. coli nachgewiesen. 10,9% wiesen andere Keime auf, wie Streptokokken,
Proteus, Mikrococcaceae, coryneforme Bakterien sowie nicht näher spezifizierte
Oxidase positive und gram positive Keime auf. Bei 18,2% der pathologisch
untersuchten Vögel (n = 49) wurden Mikroorganismen in den Nieren und
histologische Befunde im Bioptat festgestellt.In the present study 89 free-living birds of prey and owls were clinically
examined. Blood analysis and radiographs were also performed. After
stabilization the birds underwent endoscopy and kidney biopsy. The aims of the

kidney biopsies for the diagnosis of renal disease in birds of prey and owls,

examination, blood tests and radiology for kidney diseases. topic one: Kidney
biopsies are a useful tool for the diagnosis of kidney diseases. The lateral
approach through the caudal thoracic and abdominal air sacs allows the
evaluation of one kidney and a specific biopsy. Due to the anatomic
arrangement only one kidney is completely visible using this approach. During
endoscopy 2.2% (2/89) of the kidneys had severe changes, 13.5% (12/89)
moderate changes and 52.8% (47/89) slight changes. 31.5% (28/89) of the birds
had no macroscopically visible abnormalities. The visible changes were only
local abnormalities. Biopsies with pathological findings (n = 66) showed one
(53%), two (37.9%), three (7.6%) and five (1.5%) concurrent pathological
conditions. In 0.8% (1/126) of the biopsies pathological alterations were
found to be severe, in 10.3% (13/126) moderate and in 43.7% to a slight
degree. 45.2% (57/126) of the biopsies showed no histological abnormalities.
Subcapsular bleeding (19/126), local inflamation (16/126), cell cylinder
(12/126), PAS-positive material (8/126) and protein cylinders (6/126) were the
most important histological findings. 76.1% of the specimens showed an
conformity between endoscopy and biopsy results. The histopathology of the
biopsy material had similar results in 30.8% (12/39) of cases when compared to
post mortal examination. No endoscopically detectable changes had 20.5% (8/39)
of the histopathologic examined animals. topic two: Taking of biopsies with
specific forceps was possible without complications. Post biopsy haemorrhage
averaged 67 seconds. The influence of the biopsy on blood values was small.
One day after the biopsy urea and phosphorus rose significantly. Some birds
showed a decrease of hematocrit up to 10%. The analysis of hematocrit,
phosphorus, uric acid and urea showed no correlation over five days of
monitoring. The 126 biopsies had in average a length of 2.2 mm, a width of 1.3
mm and a height of 1.0 mm. All samples contained proximal and distal tubuli
and 1 - 89 glomeruli. The number of the globuli was estimated and the most
biopsie samples had 25 - 29 glomeruli per histological slice. 38.9% (49/126)
of them contained one to for intralobular veins. Biopsy material consisted of
10.7% of air sac tissue. A good estimation of the samples was possible in
89.7% (113/126) cases. 59.5% (75/126) of the specimens were not damaged. topic
three: No bird showed any indication of a kidney disease based on clinical
examination. Uric acid levels above the reference value of 8.5 mg/dl had 28,3%
(15/53) of the Eurasian buzzards (Buteo buteo) and 5.7% (3/53) had urea levels
above 36 mg/dl on the day of endoscopy. One goshawk (Accipiter gentilis)
showed uric acid levels above 14.3 mg/dl. Two sparrowhawks had uric acid
levels above the reference value of 9.1 mg/dl. There was no correlation
between radiological results and the histological biopsy results. Some kidneys
without endoscopical and histological findings revealed radiological
alterations. 69.1% (38/55) of the kidneys pathological examined birds showed
no bacterial growth on microbiologic examination. E. coli was detected in
21.8% of this kidneys. 10.9% of the samples revealed Streptokokkus, Proteus,
Micrococcus, coryneform bacteria, oxidase positive and gram positive bacteria.

The bureaucratisation of Islam in Southeast Asia: transdisciplinary perspectives
Special Issue on this topic. In their specific local contexts, state- and non-state projects of bureaucratising Islam are driven by very different socio-political motivations and conditioned by equally different (albeit often interconnected and overlapping) historical trajectories and their discursive substrates. In this special issue, we examine this transnationally and transregionally observable phenomenon in the context of Southeast Asia, where the quest for "order" -no matter how messy or even entirely failed in its outcomes- is particularly strong. In this regional setting, the institutional trajectories of the "nation-state-ization" of Islam date back to colonial times, which have continued to cast a long shadow that informs the particular manifestations of bureaucratised Islam in each country
Large language model-based information extraction from free-text radiology reports: a scoping review protocol
Introduction Radiological imaging is one of the most frequently performed diagnostic tests worldwide. The free-text contained in radiology reports is currently only rarely used for secondary use purposes, including research and predictive analysis. However, this data might be made available by means of information extraction (IE), based on natural language processing (NLP). Recently, a new approach to NLP, large language models (LLMs), has gained momentum and continues to improve performance of IE-related tasks. The objective of this scoping review is to show the state of research regarding IE from free-text radiology reports based on LLMs, to investigate applied methods and to guide future research by showing open challenges and limitations of current approaches. To our knowledge, no systematic or scoping review of IE from radiology reports based on LLMs has been published. Existing publications are outdated and do not comprise LLM-based methods.
Methods and analysis This protocol is designed based on the JBI Manual for Evidence Synthesis, chapter 11.2: ‘Development of a scoping review protocol’. Inclusion criteria and a search strategy comprising four databases (PubMed, IEEE Xplore, Web of Science Core Collection and ACM Digital Library) are defined. Furthermore, we describe the screening process, data charting, analysis and presentation of extracted data.
Ethics and dissemination This protocol describes the methodology of a scoping literature review and does not comprise research on or with humans, animals or their data. Therefore, no ethical approval is required. After the publication of this protocol and the conduct of the review, its results are going to be published in an open access journal dedicated to biomedical informatics/digital health

Randomized clinical trial evaluating the effect of bandaging on the healing of sole ulcers in dairy cattle
This randomized clinical trial investigated the effect of bandaging on 56 uncomplicated sole ulcers (SU) among a sample of 52 dairy cows. Following therapeutic hoof trimming and povidone-iodine treatment, all cows received a wooden block on the unaffected claw and were randomly assigned into either a bandaged or non-bandaged group. The bandaging process was standardized and applied by the same veterinarian. Wound size and locomotion were measured weekly. Overall, 19 of 32 SU (59.4%) in the non-bandaged group were healed at week 4 compared to 7 of 24 ulcers (29.2%) in the bandaged group. Healing was significantly higher for non-bandaged than bandaged SU (P = 0.024). Bandaging had no effect on locomotion (P = 0.9). Cows with a lower locomotion (Sprecher 1 + 2) had significantly smaller lesion sizes (median lesion size, 0.5 cm², IQR = 0.21 to 0.92 cm²) than animals with locomotion score 3–5 (median lesion size, 0.9 cm², IQR = 0.42 to 1.81 cm²; P < 0.001). A multivariate cox proportional hazard regression indicated that bandaging and parity had a significant effect on healing. Specifically, bandaged SU had a significantly lower hazard ratio (HR) to cure compared to non-bandaged SU (HR = 0.32; P = 0.01; 95% CI = 0.13 to 0.78). Furthermore, cows with parity ≥3 had a significantly higher HR to cure compared to cows in their first lactation (HR = 2.73; P = 0.025; 95% CI = 1.14 to 6.56)
