    Dental extraction following zoledronate, induces osteonecrosis in rat´s jaw

    Bisphosphonate-Related Osteonecrosis of the Jaw (BRONJ) is clinically characterized by the presence of exposed bone in the oral cavity that persists for more than eight weeks. Previous attempts to establish an animal model have not sufficiently considered disease features. Our aim was to establish an inexpensive and replicable animal model that develops BRONJ in a short time. Thirty-two male Wistar rats were randomly divided into two groups: control and experimental. In the experimental group, we administered 0.06mg/kg intraperitoneal dose of zoledronic acid (ZA) 7 and 14 days prior to maxillary second molar extraction. At two, four and six weeks after tooth extraction, the animals were euthanized, and we dissected the maxilla following histological procedures. We stained serial slides with hematoxylin and eosin and Masson?s trichrome. The samples were harvested for macroscopic, radiologic and histological evaluation of bone changes. At two weeks postextraction, we observed exposed necrotic bone in dental socket areas in experimental groups. Radiological analysis revealed osteolytic lesions accompanied by extensive destruction and sequestrum formation in the same group. Histological examination confirmed the absence of necrotic bone in control groups in contrast with the experimental groups. The percentage of empty lacunae and the number of osteoclasts and the necrotic bone area were significantly increased (p<0.05) in the experimental groups. The animal model using ZA administration to prior dental extraction successfully mimicked human BRONJ lesions. Also, the model was easily replicated, inexpensive and showed different features than other previous BRONJ models

    Trends in frequency and prevalence of oral cancer and oral squamous cell carcinoma in mexicans. A 20 years retrospective study

    Objective. To establish the time trends of the frequency and prevalence of oral cavity cancer in regard to age and gender in a 20-years (time period 1989 ? 2008) cohort of Mexicans. Design and Setting. 13,235 head and neck biopsies from the archive of the Oral Pathology Laboratory, Dental School, National Autonomous University of Mexico were revised. The cases with diagnoses of oral cancer were selected. Gender and age at diagnosis was obtained from medical records. The frequency and prevalence of oral cavity cancer and oral squamous cell carcinoma were assessed biannually in regard to the total number of population served by the oral pathology laboratory. The statistical significance of trends was established using the linear logistic regression (curve estimation) test (s 0.05). Results. 298 cases (138 males; 160 females) of oral cancer were included; 167 (92 females; 75 males; female:male ratio: 1.1:1) corresponded to oral squamous cell carcinoma. From 1989 to 2008 the prevalence of oral cancer and oral squamous cell carcinoma increased 200% (s 0.05) and 100% (s 0.000) respectively. The increase of frequency and prevalence was observed in both genders however only in females was significant (s 0.000). We do not identify changes in the age at diagnosis. Conclusions. Oral cancer, specifically oral squamous cell carcinoma, has increase in Mexicans females in the last 20 years

    Active Giant Peripheral Osteoma of the mandible: presentation of one case with follow-up to 6 years

    Osteoma, a benign neoplasia of bone tissue, is considered a quiescent neoplasm. An active osteoma is an uncom- mon lesion. We presented a case of a mandibular osteoma with an unusual active behavior. In January of 2001 was admitted in the Oral Medicine Service a 11-years old female because an asymptomatic growth in the left side of their face with two years of evolution. Radiographical examination revealed a well circumscribed spherical radio- paque lesion of approximately 2 x 2 cm in diameter on lateral side of the left angle of the mandible. Microscopically the surgical sample coming from incisional biopsy showed normal bone tissue. A diagnosis of osteoid osteoma was emitted. Surgical treatment was indicated and refused by the parents of the patient. In August 2007 the same was readmitted because an increase in their facial asymmetry. Radiographical examination revealed an increase in the size of the radiopaque area to 3x3 cm. A 99m Tc MDP bone scintigraphy was indicated and performed. Because the scintigraphy showed bone activity the surgical elimination of the whole lesion was indicated and done. Mi- croscopically the lesion was composed by vital mature lamellar and trabecular bone. A diagnosis of giant active osteoma of the mandible was emitted. The maxillofacial surgeon should awake by the possibility of an osteoma shows neoplastic activity

    Prevalence and distribution of odontogenic cysts in a Mexican sample. A 753 cases study

    Odontogenic cysts (OC) are the most frequent lesions of the jaws and their constant epidemiological update is necessary and indispensable. Therefore the principal objective of this report was To determine prevalence and clinical-demographical characteristics of OC in a Mexican sample. 753 cases of OC coming from the archive of a head and neck histopathological teaching service, from January 2000 to December 2013, were included. OC cases were re-assessed according 2005 WHO classification. Chi square test was used to establish possible associations (p<0.05IC95%). From 753 OC, 369 were female and 384 male; 52.9% of them were in their 2nd- 4th decade of life. The most common location (41%) was the mandibular posterior area. Radicular cysts were more frequent in maxillary anterior zone of females (p 0.0002) at their fourth decade of life. Dentigerous cysts were more frequent in the mandibular posterior zone of males (p 0.0000) in their second decade of life. Six cases of periodontal lateral cyst; 4 cases of paradental cysts; 4 eruption cysts and 4 cases of adult gingival cyst, as well were identified. Radicular cyst and dentigerous cyst are the most prevalent odontogenic cyst in this Mexican sample. Due to their etiology, dental pulpar necrosis and impacted teeth, radicular cyst and dentigerous cyst could be prevenible. Therefore, it is necessary to establish preventive strategies to diminish dental decay and programs of prophylactic extractions of impacted teeth, to in consequence decrease the prevalence of odontogenic cysts

    Plasmacytoid myoepithelioma of the palate : report of one case and review of the literature

    Introduction: Myoepithelioma is a benign neoplasm of salivary glands, represents 1.5 % of all salivary glands neoplasm. The plasmacytoid myoepithelioma from palate salivary glands is considered as a rare entity, at date it has been reported 14 cases. It is present one case of plasmacytoid myoepithelioma of palate. Case report: A Hispanic female of 28 years old presented a not-ulcerate, painless ovoid swelling at left side of hard palate with a one year and a half of evolution. An excisional biopsy was done. The sample was fixed at 10% buffer formalin, embedded in paraffin, cuts at 5 µ and stained with H-E. Microscopically, the lesion was composed by myoepithelial neoplastic cells characterized by a round ovoid silhouette, an eccentric nuclei of dense chromatin and eosinophilic cytoplasm. In some myoepithelial neoplastic cells were identifies intranuclear cytoplasmatic inclusions. The lesion was analysed with immunohistochemical technique using the follow antibodies: vimentin, citokeratin AE1/AE3, S100 protein and actin muscle specific. We observe positive immunoreactivity against vimentin, citokeratin, S100 protein and actin muscle specific. A diagnosis of plasmacytoid myoephitelioma of palate salivary glands was done. Our findings supports the suggestion about plasmacytoid myoepithelioma is an independent entity. The histological diagnostic parameters of plasmacytoid myoepithelioma versus pleomorphic adenoma are discussed

    Impact of oral mucosa lesions on the quality of life related to oral health. An etiopathogenic study

    Background: To assess the impact of oral mucosa lesions on quality of life related to oral health (QLROH) and additionally to establish whether the etiopathogenicy of oral lesion is associated to the degree of QLROH impact. Material and Methods: In this cross-sectional study performed on a non-probability sample of 247 consecutively patients attending the oral medicine and pathology clinic the Spanish version of Oral Health Impact Profile-49 questionnaire (OHIP-49-mx) was applied. Responses were recorded on Likert-type scale whose values ranged from 0 (never) to 4 (always). Values greater than the 50 percentile (median) were considered as indicative of poor quality of life. All patients were orally examined and diagnosed. In accordance to their etiopathogenicy 6 study groups were formed: 4 corresponded to MIND classification for diseases (Metabolic, Inflammatory, Neoplastic, and Development groups), with ≥2 diseases and no-lesion group. To identify possible differences of OHIP-49 values between study groups an ANOVA (one factor) parametric and a chi square tests were performed (SPSS®20.0). Results: The OHIP-49-mx values were higher than the 50 percentile (established at 39) in metabolic, inflammatory, development, and ≥2 diseases groups, suggesting that this type of oral lesions negatively impact the quality of life. ≥2 diseasesgroup followed by metabolic and inflammatory diseases group (p 0.001) depicted worst quality of life. Functional limitation (p 0.003), pain, physical inability (p 0.001) and psychological disabilities dimensions exhibited greater values in all groups. Conclusions: Injured oral mucosa negatively impacts quality of life, specifically functional limitation, physical inability and psychological disabilities could lead to social isolation.To our knowledge, this is the first time that an association between QLROH and the etiopathogenicy of oral mucosal diseases is established

    Oral lesions and immune reconstitution syndrome in HIV+/AIDS patients receiving highly active antiretroviral therapy. : Epidemiological evidence

    Objective: To determine whether opportunistic oral infections associated to HIV infection (OOI-HIV) are found in HIV+/AIDS patients with immune reconstitution related to highly active antiretroviral therapy (HAART). Methods. From among 1100 HIV+/AIDS patients (Service of Internal Medicine, Carlos Haya Hospital, Malaga, Spain) subjected to review of the oral cavity between January 1996 and May 2007, we identified those examined in 1996 and which were again examined between 1997 and 2007, and were moreover receiving HAART. The following data were collected: age, gender, form of contagion, antiretroviral therapy at the time of review, number of CD4+ lymphocytes/ml, and viral load (from 1997 onwards). We identified those subjects with an increase in CD4+ lymphocytes/ml associated to HAART, and classified them as subjects with quantitative evidence of immune reconstitution (QEIR). Among these individuals with QEIR we moreover identified those with undetectable viral loads (QEIR+VL), and differentiated those patients with an increase in CD4+ lymphocytes >500/ml (QEIRm+VL). In each group we determined the prevalence of OOI-HIV, following the diagnostic recommendations of the EC-Clearinghouse (CDCAtlanta, USA - WHO). In addition, we analyzed the prevalence of OOI-HIV in the different groups in relation to the duration of HAART. Results. A total of 86 subjects were included (44 females and 42 males; 19 heterosexuals, 34 male homosexuals, and 33 intravenous drug abusers). Forty-two patients showed QEIR: 21 belonged to the QEIR+VL group, and 17 conformed the QEIRm+VL group. The prevalence of OOI-HIV per group was as follows: QEIR = 54.8%; QEIR+VL = 33%; QEIRm+VL = 35%. The most prevalent lesion in all groups was erythematous candidiasis. OOI-HIV increased with the duration of HAART (p = 0.008), and were seen to be dependent upon late appearance of the mycotic lesions (after 24 months under HAART). Conclusions: It is suggested that opportunistic oral infections associated to HIV infection form part of the clinical picture of immune reconstitution inflammatory syndrome, though such infections are of late onset

    Tumors of the salivary gland in Mexicans. A retrospective study of 360 cases

    Objective: To establish distribution frequency and demographic characteristics of salivary gland tumours (SGT) in order to identify possible risk profiles. Design of study: The present report constitutes an eight year retrospective study (January 2000-August 2007). The archives of the Clinical and Experimental Pathology Laboratory (Graduate and Research Division, Dental School, National Autonomous University of Mexico) as well as archives of the Surgical Pathology Service (General Hospital, Mexico City) were subject to revision in order to select all cases where SGT tumour diagnoses were emitted. Age and gender of patients as well as SGT topography were obtained from medical records. Selected cases were classified according to location of the lesion, histological lineage and biological behaviour. Results: 360 cases of SGT were included, 227 (67%) cases were benign tumours, while 83 cases (23%) were malignant tumours. SGT were most frequent in women with ages ranging from their 3 rd to 5 th decades of life. 275 tumours were located in major salivary glands, 78.9% of them were identified in the parotid gland. The most frequent location of tumours arising from minor salivary glands (33 cases, 38%) was found in the palatine glands. Tumours of epithelial lineage were the predominant histological type. The most frequent benign tumours were pleomorphic adenomas (86.1%) and papillary cystadenoma lymphomatosum (7.3%). The most frequent malignant tumours were adenoid cystic carcinomas (25%) and mucoepidermoid carcinomas (23.6%) Conclusions: Salivary gland tumours in Mexican population appear principally in major salivary glands of women in their 3 rd to 5 th decade of life. © Medicina Oral S.
