Tratamentul ortodontic în două etape în anomaliile clasa ii angle
Background. Angle class II malocclusions have a frequency influenced by many factors. Recognized
as a difficult anomaly to treat, class II Angle malocclusions require early diagnosis and treatment, the
latter often being staged. Objective of the study. Evaluation of two-stage treatment of patients with
class II angle abnormalities. Material and Methods. The study is based on data collected from a group
of 50 patients with a diagnosis of malocclusion class II / 1 Angle and class II / 2 Angle, aged 7-18 years.
They underwent clinical and paraclinical examinations pretreatment and after each stage, the changes
and the effectiveness of the applied treatment were evaluated. The applied methods were mobile /
mobilizable devices and fixed technique. Results. Following the analysis of the preliminary and final
results of the treatment in two stages from the group of 50 patients, the change of the dento-facial
parameters was observed. In the case of patients with maxillary compression, the use of mobilizable
devices had an efficiency of about 30%, and fixed expansion devices an efficiency of 80%. In patients
with medium and advanced severity overbite, the use of mobile devices showed an efficiency of 40%
of the desired result. The second stage of treatment resulted in achieving the maximum results against
the background of those obtained in the first stage. Conclusion. Evaluation of two-stage treatment of
patients with Angle class II abnormalities demonstrated efficacy at each stage. 2. The application of
early orthodontic treatment of mobile or mobilizable appliances reflects an increased success rate and a
minor risk of recurrence.
Introducere. Malocluziile clasa II Angle au o frecvență influențată de numeroși factori. Recunoscută
ca fiind o anomalie dificil de tratat, malocluziile de clasa II Angle impun un diagnostic și tratament
precoce, cel din urmă fiind adesea etapizat. Scopul lucrării. Evaluarea tratamentului în două etape a
pacienților cu anomalii clasa II Angle. Material și Metode. Studiul realizat are la bază datele culese de
la un lot de 50 de pacienți, cu diagnosticul de malocluzie clasa II/1 Angle și clasa II/2 Angle, cu vârsta
cuprinsă între 7-18 ani. Ei au fost supuși examinărilor clinice și paraclinice pretratament și după fiecare
etapă au fost evaluate modificările și eficiența tratamentului aplicat. Metodele aplicate au fost aparate
mobile/mobilizabile și tehnica fixă. Rezultate. În urma analizei rezultatelor preliminare și finale a
tratamentului în două etape din lotul de 50 de pacienți s-a observat modificarea parametrilor dentofaciali. În cazul pacienților ce prezentau compresiune maxilară, utilizarea aparatelor mobilizabile a avut
o eficiență de aproximativ 30%, iar aparatele fixe de expansiune o eficiență de 80%. În cazul pacienților
cu overbite gravitate medie și avansată, utilizarea aparatelor mobile au prezentat o eficiență de 40% din
rezultatul dorit. Etapa a doua de tratament s-a soldat prin atingerea rezultatelor maxime pe fundalul celor
obținute în prima etapă. Concluzii. 1. Evaluarea tratamentului în două etape a pacienților cu anomalii
clasa II Angle a demonstrat eficacitatea la fiecare fază. 2. Aplicarea tratamentului ortodontic anticipat
de aparate mobile sau mobilizabile oglindește o rată mărită de succes și risc minor de recidivă

În ultimul timp se detectă, tot mai frecvent, caria dentară în dinţii permanenţi în etapa prefuncţională a erupţiei lor. Acest argument ne impune
către o tactică anumită de tratament, etapizată, cît mai raţională, ţinîndu-se

Peculiarities in the orthodontic treatment of dental crowding associated with arch compression
Introducere. Micșorarea diametrelor transversale
și sagitale ale arcadelor dentare sunt niște semne caracteristice pentru pacienţii cu înghesuire dentară. De
aici și reiese că una din sarcinile tratamentului acestei
patologii este expansiunea arcadelor dentare. Scopul lucrării. Sporirea eficacității diagnosticului
și simplificarea metodelor de tratament al disarmoniilor dento-alveolare cu înghesuire prin utilizarea expansiunii arcadelor dentare.
Obiective:
1. Selectarea metodelor utile și eficiente în diagnosticul DDA cu înghesuire.
2. Studiul eficacității utilizării arcurilor rigide în
tratamentul compresiei arcadelor dentare asociate cu
înghesuire.
3. Asigurarea stabilității rezultatului tratamentului
acestei patologii prin aplicarea unor metode de contenție cu eficacitate sporită.
Material și metode. Lucrarea s-a realizat în baza
analizei unui lot de 22 de pacienți cu înghesuire dentară,
la care s-a efectuat extinderea dimensiunilor transversale și sagitale a arcadelor. 6(27,3%) erau de sex masculin,
16(72,7%) - feminin.Vârsta medie a bolnavilor a alcătuit 15,86±1,25 ani. Stabilirea diagnosticului s-a efectuat
în baza: examenului clinic, radiologic (teleradiografie
și ortopantomografie, tomografie compiuterizată), studiului biometric pe model (metodele Pont, Korkhaus,
Bolton). În tratament, la toți pacienții s-au utilizat aparate fixe de tip MBT cu slotul de .022″ în asociere cu un
dispozitiv de expansiune a arcadei dentare.
Rezultate. După tipul facial pacienţii au fost repartizaţi în: 13(68,42%) cazuri – tip euriprosop,
3(15,79%) cazuri – tip mezoprosop, 3(15,79%) cazuri
– tip facial leptoroprosop. Indicele SI mediu a fost de
31,8±0,375 mm. Pacienţi cu normodenţie au fost 18
(72,0%), cu macrodenţie - 7 (28,0%). Indicile Bolton
frontal a avut valoarea de 77,88%, cu 0,68% mai mare
decât norma. Diametrul intermolar al arcadei dentare
superioare pretratament era micșorat cu 4,77 mm, celei inferioare cu 3,51mm. La analiza spaţiului total în
arcada inferioară s-a depistat un deficit total de 8,04
mm. Durata tratamentului prin expansiunea arcadelor dentare a alcătuit 16,82 luni ± 0,78, fiind veridic
mai scurtă decât în cazurile cu extracții (p<0,05). Concluzii. În diagnosticul disarmoniilor dento-alveolare cu înghesuire e importată utilizarea analizei
spațiului total în asociere cu analiza cefalometrică.
Pentru simplificarea tratamentului e necesar de utilizat
la mai mulți pacienți posibilitatea expansiunii arcadelor prin activarea arcurilor rigide din oțel inoxidabil.
Pentru asigurarea stabilității rezultatului tratamentului și păstrarea diametrelor transversale create pot fi
recomandate folosirea capelor de contenție confecționate din plăci rigide cu grosimea de 1,5 mm.Introduction. Decreased transverse and sagital
length of dental arches are characteristic signs of patients with dental crowding. This is to conclude that
one of the orthodontic treatment aims is to expand
dental arches. Objective of the study. Increase the diagnosis efficiency and simplify the treatment methods of DDA
with crowding by using dental arches expansion.
Study goals.
1.Selection of appropriate and efficient methods in
dental crowding diagnosis.
2. Study the efficiency of rigid arches usage in the
treatment of dental arch compression associated with
crowding.
3. Ensure the treatment result stability of this pathology by applying more effective retention methods.
Methods and Materials. This study is based on
the evaluation of 22 patients with dental crowding,
in which the expansion of the transverse and sagittal
lengh of dental arches was performed. 6(27.3%) were
male, 16(72.7%) - female. The average age of the patients was 15.86±1.25 years. Diagnosis was established
based on clinical examination, X-ray examination (lateral cephalometrics and panoramic X-ray, cone beam
computed tomography), biometric study of the casts
(Pont, Korkhaus, Bolton analysis). In all patients,during the orthodontic treatment, were used fixed MBT
appliances with a .022″ slot in combination with a dental arch expansion device.
Results. According to the facial type, the patients
were divided into:13(68.42%) cases – euriprosop type,
3(15.79%) cases – mesoprosop type, 3(15.79%) cases
– leptoroprosop facial type. The SI index was 31.85
mm±0.37. There were 18(72.0%) patients with normodentia, with macrodentia - 7(28.0%). The anterior
Bolton indices had a value of 77.88%, 0.68% higher
than the norm. The intermolar width of the upper
dental arch, before treatment, was reduced with 4.77
mm, the lower one with 3.51 mm. When analyzing
the total space in the lower arch, a total deficit of 8.04
mm was detected. The duration of the treatment by expansion of the dental arches was 16.82 months ± 0.78,
being truthfully shorter than in cases with extractions
(p<0.05). Conclusions. In the diagnosis of dento-alveolar
disharmonies with crowding is important the use of
total space analysis in association with cephalometric
analysis. To simplify the treatment, it is necessary to
use in several patients the possibility of expanding the
arches by activating the rigid stainless steel wires. To
ensure the stability of the treatment result and to preserve the created transverse width, the use of retention
appliances made of rigid plates with a thickness of 1.5
mm can be recommended
DENTO-MAXILLARY ANOMALIES ASSOCIATED WITH SUPERNUMERARY TEETH. DIAGNOSIS AND TREATMENT ASPECTS

Dinți supranumerari (pleiodonție) sunt tulburări în perioada de formare, diviziune a lamei dentare și evoluție a mugurilor, însoțită de majorarea numărului de dinți. Are o frecvență de 2-4% (Brabant) din numărul total de populație, mai des se depistează în sectorul frontal al arcadei superioare. Scopul lucrării. Analiza particularităților clinice, aspectelor de diagnostic și tratament al anomaliilor dento-maxilare asociate cu prezența dinților numerari. Material și metode. Lucrarea este un studiu descriptiv realizat în baza analizei unui lot de 12 pacienți cu vârste de 6-18 ani. Criteriul de bază în selectare a fost prezenta unui sau a mai mulți dinți supranumerari în asociere cu o anomalie dento-maxilară. Stabilirea diagnosticului s-a efectuat în baza: examenului clinic, radiologic (teleradiografie și ortopantomografie, tomografie computerizată), studiului biometric de model (metodele Pont, Korkhaus, Bolton). Rezultate. Din numărul de 12 pacienți: 9 aveau anomalii de clasa I Angle, 2-clasa II, 1-clasa III. In 8 (67%) cazuri s-a depistat prezența dinților supranumerari în sectorul frontal al arcadelor dentare. În 4 cazuri dinții supranumerari erau erupți, fiind identificați la examenul clinic, la 8 pacienți au fost depistați în urma unui examen radiologic. Urmările prezenței oligodonției au fost: diastema superioară–3 cazuri, incluzie dentară–4 cazuri, înghesuire dentară la 3 pacienți, în 2 cazuri au lipsit probleme legate de prezența dintelui supranumerar. Concluzii. În diagnosticul oligodențiilor e importată utilizarea tomografiei computerizate pentru identificarea poziției dinților supranumerari și selectarea corectă a planului de tratament. În 10 cazuri acești dinți au fost supuși extracției, urmată de aplicarea unui aparat dentar pentru tratamentul anomaliei asociate.Background. Main criterion in the selection was the presence of one or more supernumerary teeth in association with a dento-maxillary anomaly. The establishment of the diagnosis was carried out according to: clinical, radiological examination (teleradiography and orthopantomography, computed tomography), biometric study on the model (Pont, Korkhaus, Bolton methods). Results. From the total number of 12 patients: 9 had Angle class I anomalies, 2-class II, 1-class III. In 8 (67%) cases, the presence of supernumerary teeth was detected in the frontal region of the dental arches. In 4 cases the supernumerary teeth were erupted and were identified during the clinical examination, in other 8 cases there were detected supernumerary teeth following a radiological examination. The consequences of the presence of oligodontia in the study group were: upper diastema–3 cases, dental inclusion–4 cases, dental crowding in 3 patients, in 2 cases there were no problems related to the presence of the supernumerary tooth. Conclusions. In the diagnosis of oligodentures, the use of computerized tomography is important for identifying the position of supernumerary teeth and the correct selection of the treatment plan. In 10 cases these teeth were subjected to extraction, followed by the application of a dental appliance for treating the associated anomaly

Analiza și sistematizarea factorilor etiologici, stabilirea metodelor de diagnostic necesare și aprecierea eficienței tratamentului funcțional al malocluziei de clasa II/2 Angle în dentiția mixtă. Material și metode. Lucrarea a fost efectuată în baza analizei unui lot de 10 pacienți cu vârste de 7-13 ani. Criteriul de bază în selectare a fost prezenta rapoartelor distalizate la nivel molar și canin. Stabilirea diagnosticului s-a efectuat în baza: examenului clinic, radiologic (teleradiografie și ortopantomografie), studiului biometric pe model (metodele Pont, Korkhaus, Bolton). Rezultate. Pacienții au prezentat următoarele acuze: poziție incorectă a dinților frontali, dereglare a esteticului facial. Clinic s-a depistat: profil facial convex, etaj inferior micșorat, arcade dentare în formă de trapeză, ocluzie adâncă acoperită. Radiologic:<ANB majorat cu 3-6⁰, <FMA micșorat cu 5-10⁰. Biometric: compresie a arcadelor cu 3-8mm.În tratament s-au utilizat Twin-blocurile Clark. După 6 luni de tratament deficitul de spațiu s-a înlăturat, s-a îmbunătățit raportul dinților laterali, au dispărut malpozițiile dentare, s-a micșorat acoperirea incisivală până la 1-2mm. Concluzii. Tratamentul funcțional al malocluziei clasa II/2 Angle în perioada dentiției mixte a avut un succes bun, a simplificat semnificativ tratamentul tardiv și a asigurat la minim riscul de recidivă, urmărind un rezultat estetic și funcțional stabil.Introduction. Analysis and systematization of etiological factors, establishing the necessary diagnostic methods and assessing the effectiveness of functional treatment of Angle Class II / 2 malocclusion in mixed dentition. Material and methods. The work was performed based on the analysis of a group of 10 patients aged 7-13 years. After 6 months of treatment, the space deficit was removed, the ratio of the lateral teeth was improved, the dental malpositions disappeared, and the incisor coverage was reduced to 1-2 mm. Conclusions. Functional treatment of malocclusion class II / 2 Angle during mixed dentition was a good success, significantly simplified late treatment and ensured the risk of recurrence to a minimum, aiming at a stable aesthetic and functional result

The role of the inflammatory process in orthodontic tooth movement
Introducere: Recunoscut este faptul că deplasarea
dentară ortodontică conduce la apariția zonelor de
presiune și de tensiune în periodonțiu. Zona de presiune se caracterizează prin colaps vascular, cu scăderea
parțială și/sau completă a fluxului sangvin local, fapt
care conduce inevitabil la ischemie și moartea unor
celule. Reacția fiziologică tipică a organismului vital
constă în declanșarea unui proces inflamator. Deși
practicienii sunt conștienți de morfofiziologia țesuturilor parodontale în deplasarea ortodontică a dinților,
încă nu sunt întotdeauna clare limitele și mecanismul
inflamației provocate, beneficiile și/sau riscurile posibile în tratamentul ortodontic.Introduction: It is now well-known that an orthodontic tooth movement will always create areas
of pressure and tension within periodontium. The
pressure area is characterized by partial and complete
interruption of the local blood flow which will inevitably result in death of some cells. As a typical physiological reaction of any vital organism an inflammation process soon emerges. Although practitioners are
now aware of inflammation during orthodontic tooth
movement (OTM) it is still not always clear what is
its purpose and whether it’s beneficial or harmful for
orthodontic treatment

Recognized as a difficult anomaly to treat, class II Angle malocclusions
require early diagnosis and treatment, the latter often being staged. A goal
of “early” orthodontic treatment is to correct existing or developing skeletal,
dentoalveolar, and muscular imbalances to improve the orofacial
environment before the permanent teeth eruption is complete.
Early intervention may reduce the overall need for complex orthodontic
treatment like permanent tooth extraction or orthognathic surgery. Purpose: Evaluation of two-stage treatment of patients with
Cass II Angle abnormalities. Material and methods: The study is based on data collected from a group

Parodontal diseases in dento-maxillary anomalies. Assessment and aspects of the complex orthodontic treatment
Introduction: The emergence of the inflammation and parodontal disease is due to the activity of two
complexes: the primary causal complex which includes the dental plaque and the secondary causal complex with its local factors and actions upon the primary causal complex. The dental plaque is the main cause
of occurrence of the inflammatory parodontal disease. Local factors with negative influence on parodontal
disease are dento-maxillary anomalies, occlusal trauma, form and integration of the labial frenulum, etc.
Abnormalities in tooth size, position and shape can cause disorders in gingival architecture, which lead to
the development of problem areas with difficulties in self-cleaning and as a result, the progressive accumulation of dental deposits. Also, the dento-maxillary anomalies create overload conditions for the periodontal support, with development of the occlusal trauma and later with an eventual gingival recession.
The purpose of the work was to assess the parodontal status in the case of dento-maxillary anomalies
and the clinical and paraclinical supervision within a complex orthodontic treatment.
Materials and methods: The research involved the examination and treatment of 98 patients with
different dento-maxillary anomalies, with an average age of 13,77 ± 0,36 in a range of 8-24 years old. The
algorithm for the investigation of patients included exo- and endooral examination, fotostatic test, model
biometric study; X-ray examination before and after orthodontic treatment, assessment of the parodontal disease through radiological and endooral clinical examination, by determining the gingival index of
Parma, the papillary hemorrhage index of Miihlemann; and assessing of the oral hygiene index (OHI-S).
There were diagnosed parodontal diseases in 77 patients (78, 6%) were diagnosed with dento-maxillary anomalies and 21 patients (21, 4%) were considered parodontally healthy.
The orthodontic treatment was realized using fixed appliances with .022 slot brackets, Straight Wire
Technique, Roth prescription, with individual elements for 41 patients (41, 8%) and with removable and
functional appliances for 57 patients (58, 2%). The parodontal therapy included topical administration
of 0,1% gel BioR, in the first group (of the research), and in the second group (of reference) - parodontal
therapy with routine preparations.
Results: After a course of active orthodontic treatment the results of the complete healing were different -76,3% for the first group and 66,9% for the patients of the second group. The results varied in
dependence of the used orthodontic appliances, 83,1% for the patients with fixed appliances and 62,7%
for the patients treated with removable appliances.
Conclusion: The complex orthodontic treatment of dento-maxillary anomalies truthfully leads to
parodontal healing granting the fixed appliances treatment
