    Developing recovery oriented services and co-production in mental healthcare: Building- up on existing promising organisational practices

    Recovery, as a patient-centred emergent transformative concept in mental healthcare, requires a change in the culture and practice of organisations at different levels. This paper investigates the potential of nurturing existing recovery oriented initiatives as promising practices for the re-orientation of mental healthcare provision. In the field of social innovation, promising practices are intended as very context-linked sustainable practices which open up possibilities of societal radical transformation based on people’s real needs and existing assets. Similarly, in mental healthcare services, the authors argue that emergent promising recovery oriented and co-produced practices can favour the shift from a traditional top-down culture to a more collaborative one. This paper is based on an experimental action-research project, Recovery CO–LAB, developed in collaboration with the Mental Health Department of Spedali Civili di Brescia, aiming to explore how service design could help the organisation to increase its orientation toward recovery

    The European grape berry moth, Eupoecilia ambiguella (Hübner) (Lepidoptera: Tortricidae), since its first identification in 1796, was defined as a key pest for European viticulture despite its polyphagy. Although between the late 1800s and early 1900s its presence and spread in Europe was of concern, to date its populations are low and limited to cooler and wetter areas, leading to a decline in its importance. In the present work, we reviewed its global distribution as well as its morphology, biology, and ecology. Considering the monitoring and management of this pest, the present review summarised insecticidal, agronomic, and cultural control strategies. Moreover, given the need to reduce the impact of agriculture on the environment, humans, and non-target species, we focused on control strategies relying on pheromones and biological control agents (e.g. predators and parasitoids) involved in conservation biological control

    Investigation on the microbiological hazards in an artisanal soft cheese produced in northern Italy and its production environment in different seasonal periods

    The present study aimed at assessing the occurrence of microbiological hazards (Listeria monocytogenes, Staphylococcus aureus, Salmonella spp. and Escherichia coli O157) in an artisanal soft cheese produced in northern Italy. In the same product total bacterial count, lactic acid bacteria and Enterobacteriaceae were enumerated, and pH and water activity measured in two batches sampled in summer and winter. Samples of raw materials, environmental swabs from the production processes and cheese during 15 days of storage at 2 and 8°C as well as dynamic temperature of 2°C for 5 days and 8°C for 10 days were collected and tested. The load of total bacterial count was significantly higher in the winter batch in comparison to the summer one, with a significant increase at the end of the storage period also noticed for lactic acid bacteria. Statistical higher values of pH were registered in raw materials and end of storage in winter batch. S. aureus was con-firmed only in the winter batch within samples (n=4) of stored cheese. On plates used for E. coli O157 detection, colonies of Klebsiella pneumoniae and Klebsiella oxy-toca were isolated. The results suggest that the highest bacterial population in the winter batch was associated to a higher pH in stored cheese and a higher number of bio-logical hazards identified. Their isolation started in the maturation room suggesting this step as relevant for possible cheese contamination

    Erupted cumulate fragments in rhyolites from Lipari (Aeolian Islands)

    Over the last similar to 267 ky, the island of Lipari has erupted magmas ranging in compositions from basaltic andesites to rhyolites, with a notable compositional gap in the dacite field. Bulk geochemical and isotopic compositions of the volcanic succession, in conjunction with major and trace elemental compositions of minerals, indicate that the rhyolites were dominantly generated via crystal fractionation processes, with subordinate assimilation. Radiogenic (Sr, Nd, and Pb) and stable (O) isotopes independently suggest <= 30 % of crustal contamination with the majority of it occurring in mafic compositions, likely relatively deep in the system. Within the rhyolites, crystal-rich, K2O-rich enclaves are common. In contrast to previous interpretations, we suggest that these enclaves represent partial melting, remobilization and eruption of cumulate fragments left-over from rhyolite melt extraction. Cumulate melting and remobilization is supported by the presence of (1) resorbed, low-temperature minerals (biotite and sanidine), providing the potassic signature to these clasts, (2) reacted Fo-rich olivine, marking the presence of mafic recharge, (3) An(38-21) plagioclase, filling the gap in feldspar composition between the andesites and the rhyolites and (4) strong enrichment in Sr and Ba in plagioclase and sanidine, suggesting crystallization from a locally enriched melt. Based on Sr-melt partitioning, the high-Sr plagioclase would require similar to 2300 ppm Sr in the melt, a value far in excess of Sr contents in Lipari and Vulcano magmas (50-1532 ppm) but consistent with melting of a feldspar-rich cumulate. Due to the presence of similar crystal-rich enclaves within the rhyolites from Vulcano, we propose that the eruption of remobilized cumulates associated with high-SiO2 rhyolites may be a common process at the Aeolian volcanoes, as already attested for a variety of volcanic systems around the world

    Investigation on the microbiological hazards in an artisanal salami produced in Northern Italy and its production environment in different seasonal periods

    In the present study, the occurrence of Listeria monocytogenes, Staphylococcus aureus, Salmonella spp. and Escherichia coli VTEC was investigated in two batches of artisanal Italian salami tested in winter and summer. Moreover, enumerations of total bacterial count, lactic acid bacteria and Enterobacteriaceae were performed as well as monitoring of water activity and pH. Samples were taken from raw materials, production process environment, semi-finished product and finished products. The results revealed an overall increase of total bacterial count and lactic acid bacteria during the ripening period, along with a decrease of Enterobacteriaceae, pH and water activity. No significant difference was observed between the two batches. The enterobacterial load appeared to decrease during the maturation period mainly due to a decrease in pH and water activity below the limits that allow the growth of these bacteria. E. coli VTEC, Salmonella spp. or L. monocytogenes were not detected in both winter and summer batches. However, Klebsiella pneumoniae was detected in both summer and winter products. Except for one isolate, no biological hazards were detected in the finished salami, proving the efficacy of the ripening period in controlling the occurrence of microbiological hazard in ripened salami. Further studies are required to assess the virulence potential of the Klebsiella pneumoniae isolates

    Frequent activity on Vulcano (Italy) spanning the last 80 ky: New insights from the chemo-stratigraphy of the Brown Tuffs

    The Brown Tuffs (BT) are widespread reddish-brown to grey, ash-rich pyroclastic deposits recognized in the stratigraphic sequences of the Aeolian Islands and Capo Millazzo peninsula (Sicily) that span the last 80 ky. They have very homogeneous lithological, textural and sedimentological features which make it difficult to reliably correlate units on the islands to proximal units in the source areas. Here we carefully re-interpret the stratigraphic profiles of the BT on Vulcano and Lipari where the deposits are thickest and present the most complete succession. The investigation is based on a large dataset of major and minor element geochemistry of juvenile glass components for the majority of the recognized BT depositional units, whilst also providing new radiocarbon ages. The distinctive chemical groupings observed within the glass analyses, both temporally and spatially, allow us to fingerprint the three main stratigraphically defined macro-units in which the BT succession can be sub-divided using prominent tephra marker beds, the Ischia Tephra (Monte Epomeo Green Tuff; 56 ky) and Monte Guardia pyroclastics from Lipari (herein radiocarbon dated to 27\u201326 ky). The Lower (80\u201356 ky; LBT), Intermediate (56\u201327 ky; IBT) and Upper BT (here dated at 24\u20136 ky; UBT) macro-units display K-series volcanic glasses ranging from basaltic trachy-andesites, through trachy-andesites, to more evolved trachytes, all consistent with an origin on Vulcano. The UBT are clearly distinguished from the lower macro units by their higher-SiO2 trachy-andesite to trachytic glasses, which extend to noticeably lower TiO2, CaO and MgO contents. These features make it possible to re-define the geochemical-evolutionary boundary between IBT and UBT as corresponding to the 24 ky Spiaggia Lunga scoria bed on Vulcano, which is stratigraphically higher (and younger) than the previous boundary marker (Monte Guardia). The glass compositions of the LBT, IBT and UBT are used to: (1) assess links to known proximal eruption units outcropping on Vulcano; (2) validate medial-distal BT occurrences across the Aeolian archipelago (Salina, Filicudi and Panarea) and on Capo Millazzo; (3) confirm that the BT are responsible for distal volcanic ash layers preserved in Central Mediterranean marine sedimentary archives. Interestingly, the glass compositions of the UBT are very similar to those of the Punte Nere unit, the earliest pyroclastic products erupted from the currently active La Fossa cone on Vulcano, indicating the corresponding magmatic system has likely erupted similar melts and products over the last 24 ky and thus extending its life cycle. Such information is crucial for evaluating the long-term eruption scenarios underpinning hazard assessment of the La Fossa caldera magmatic system

    P2X7 mRNA expression in non-small cell lung cancer: MicroRNA regulation and prognostic value

    The human P2X7 receptor is significant and exhibits several functions in neoplasia. At present, little is known with regard to its regulation. P2X7 expression may be regulated post-transcriptionally and putative microRNA (miRNA) binding sites are considered to be involved. The aim of this study was to determine whether miRNAs (miR-21, let-7 g and miR-205) regulate P2X7 mRNA stability. In addition, the impact of P2X7 expression in patients with non-small cell lung cancer (NSCLC) was investigated. P2X7 mRNA and mature Let-7 g, miR-21, and miR-205 expression levels were quantified in 96 NSCLC cases using quantitative reverse transcription polymerase chain reaction. In all samples, epidermal growth factor receptor and K-Ras mutational analysis was also performed. Samples with low P2X7 expression were found to exhibit a higher fold change in miR-21 expression when compared with samples exhibiting high P2X7 expression. Significantly higher miR-21 expression was observed in the tumors of NSCLC patients with a K-Ras mutation when compared with patients who had K-Ras wild-type tumors (P=0.003). Additionally, to evaluate the association between P2X7 expression and prognosis in NSCLC patients, survival analysis was performed using the Kaplan-Meier method. A significant difference in the progression-free survival and overall survival in the NSCLC patients with high P2X7 expression was identified, when compared with that of patients with low expression (P=0.03 and P=0.02, respetively). Therefore, we hypothesized that high levels of miR-21 expression in NSCLC patients with K-Ras mutations may be regulated by a complex circuit, including P2X7 downregulation and together these processes may promote tumor progression

    The resistome of commensal Escherichia coli isolated from broiler carcasses “produced without the use of antibiotics” -

    Several strategies have been in place in food animal production to reduce the unnecessary use of antimicrobial agents. Beyond the monitoring of their use, the evaluation of the effect of these strategies on the occurrence and types of antimicrobial resistance (AMR) associated genes is crucial to untangle the potential emergence and spread of AMR to humans through the food chain. In the present study, the occurrence of these genes was evaluated in commensal Escherichia coli isolated from broiler carcasses “produced without the use of antibiotics” in three antibiotic-free (AB-free) farms in Italy in 2019. Sequenced data were analyzed along with publicly available genomes of E. coli collected in Italy from the broiler food chain from previous years (2017 to 2018). The genetic relationships among all 93 genomes were assessed on de novo assemblies by in silico MLST and SNP calling. Moreover, the resistomes of all genomes were investigated. According to SNP calling, genomes were gathered in three clades. Clade A encompassed, among others, ST117, ST8070 and ST1011 genomes. ST10 belonged to clade B, whereas Clade C included ST58, ST297, ST1101 and ST23 among others. Regarding the occurrence of AMR genes, a statistically significant lower occurrence of these genes in the genomes of this study in comparison to the public genomes was observed considering the whole group of genes as well as genes specifically conferring resistance to aminoglycosides, ÎČ-lactams, phenicols, trimethoprim and lincosamides. Moreover, significant reductions were observed by comparing the whole group of AMR associated mutations, as well as those specifically for fluoroquinolones and fosfomycin resistance. Although the identification of 3° generation cephalosporin resistance associated genes in AB-free E. coli is a concern, this study provides a first indication of the impact of a more prudent use of antimicrobial agents on the occurrence of AMR genes in Italian broiler production chain. More studies are needed in next years on a higher number of genomes to confirm this preliminary observation

    Tumour necrosis factor- α and transforming growth factor- ÎČ are significantly associated with better prognosis in non-small cell lung carcinoma: putative relation with BCL -2-mediated neovascularization

    Recent in vivo and in vitro studies have demonstrated a wide spectrum of biologic activities of cytokines in the pathogenesis and progression of malignancy. Tumour necrosis factor alpha (TNF-α) and transforming growth factor beta (TGF-ÎČ) have emerged as two of the many host-derived mediators that seem to interfere with both antiproliferative and tumorigenic effects in malignant tumours including lung cancer. However, their association with tumour prognosis or prognostic factors has not yet been completely clarified. In this study, we assessed TNF-α and TGF-ÎČ mRNA expression by RT-PCR technique in 61 NSCLC samples, demonstrating the presence of TNF-α and TGF-ÎČ mRNA in 55.74% and 45.9% of cases, respectively. We also evaluated the expression of the two distinct transmembrane TNF receptors. TNFR-I and TNFR-II, with a PCR-positive signal in 70.49% and 65.57% of cases, respectively. In 49 of the 61 cases, we evaluated the prognostic impact of the two growth-inhibiting factors using the Kaplan–Meier analysis. In the univariate analysis patients without nodal metastatic involvement (P = 0.02), less advanced tumour stage (P = 0.02) or TNF-α and TGF-ÎČ positive cancers (P = 0.01 and P = 0.03) showed a favourable prognosis in terms of overall survival. Since our previous studies demonstrated a significant association between NSCLC behaviour, neoangiogenesis and bcl -2 expression, we investigated the putative relation between TNF-α and TGF-ÎČ on the one hand, and vascular count (as a measure of tumour angiogenesis) and bcl -2 protein expression, on the other hand. Our results showed a significant direct association between TNF-α and bcl -2 (P = 0.05) and an inverse association between TNF-α and microvessel count (P = 0.03). Moreover, as previously demonstrated, we observed a significant inverse correlation between bcl -2 protein expression and vascular count (P = 0.05), suggesting that the favourable effect of TNF-α on clinical outcome may be related to a bcl -2-mediated low neovascular development. © 2000 Cancer Research Campaig

    Genomic features of Klebsiella isolates from artisanal ready-to-eat food production facilities

    Increasing reports on K. pneumoniae strains with antimicrobial resistance and virulence traits from food and farm animals are raising concerns about the potential role of Klebsiella spp. as a foodborne pathogen. This study aimed to report and characterize Klebsiella spp. isolates from two artisanal ready-to-eat food (soft cheese and salami) producing facilities, and to track similar genotypes in different ecological niches. Over 1170 samples were collected during the whole production chain of different food batches. The overall Klebsiella prevalence was 6%. Strains were classified into the three Klebsiella species complexes: K. pneumoniae (KpSC, n = 17), K. oxytoca (KoSC, n = 38) and K. planticola (KplaSC, n = 18). Despite high genetic diversity we found in terms of known and new sequence types (STs), core genome phylogeny revealed clonal strains persisting in the same processing setting for over 14 months, isolated from the environment, raw materials and end-products. Strains showed a natural antimicrobial resistance phenotype-genotype. K. pneumoniae strains showed the highest virulence potential, with sequence types ST4242 and ST107 strains carrying yersiniabactin ybt16 and aerobactin iuc3. The latter was detected in all K. pneumoniae from salami and was located on a large conjugative plasmid highly similar (97% identity) to iuc3+ plasmids from human and pig strains circulating in nearby regions of Italy. While identical genotypes may persist along the whole food production process, different genotypes from distinct sources in the same facility shared an iuc3-plasmid. Surveillance in the food chain will be crucial to obtain a more comprehensive picture of the circulation of Klebsiella strains with pathogenic potential
