    Relevance of progesterone receptor immunohistochemical staining to Oncotype DX recurrence score

    AbstractObjective/BackgroundProgesterone-receptor negativity (PR−) is predictive of adverse outcomes in estrogen receptor-positive (ER+) breast cancer. The Oncotype DX assay provides risk stratification for hormone receptor-positive (HR+) invasive breast cancer; however, the association of PR status and Oncotype DX recurrence scores (RSs) is less clear.MethodsWe designed an analysis to determine whether a significant difference exists in the RS for ER+/PR− tumors when compared with ER+/PR+ breast cancer. Three hundred and fifty patients with HR+ invasive breast cancer who underwent Oncotype DX testing at our institution from December 2006 to October 2013 were included. We also examined the concordance in the HR status reported by immunohistochemical (IHC) and reverse transcriptase-polymerase chain reaction (RT-PCR) analyses. The data were analyzed by analysis of variance, F test, t test, and chi-square tests. Multivariate linear regression was used to determine significant predictors of Oncotype DX RS.ResultsA total of 301 patients had ER+/PR+ tumors and 47 patients had ER+/PR− tumors by IHC. PR− tumors had a significantly higher RS than PR+ tumors (24.7±8.53 vs. 17.3±7.38; p<.001), predicting a greater 10-year risk of distant recurrence. Multivariate linear regression showed PR status and tumor grade to be significant predictors of Oncotype DX RS (p<.0001). A total of 284 patients had HR status reported by Oncotype DX assay. Concordance between IHC and RT-PCR was 99.3% for ER and 88.7% for PR.ConclusionOur study shows that ER+/PR− breast cancer tumors are associated with a significantly higher Oncotype DX scores; this interprets into a higher risk of recurrence. Our data also show that the concordance between IHC and RT-PCR was 99.3% for ER and lower at 88.7% for PR

    OEIS complex: a rare foetal anomaly

    OEIS (Omphalocele, extrophy of bladder/cloaca, imperforate anus, spinal defects) is a rare constellation of malformations involving multiple organs in humans. It is the most serious manifestation of exstrophy-epispadias sequence. The exact aetiology of this condition is not known. It is sporadic but genetic associations have also been hypothesised. It results from defective blastogenesis leading to improper closure of anterior abdominal wall and defective development of cloacae and urogenital septum. Incidence ranges from 1 in 200000 to 1 in 400000. Prenatal diagnosis of OEIS complex can be done by anomaly scan between 18 to 22 weeks of gestation. It carries a poor prognosis. Survival is nil or very less. Even if baby survives it requires multiple surgeries with many potential complications. We had an undiagnosed case of this complex presenting to us in advanced labour, but a timely ultrasound helped in the diagnosis and avoided an unnecessary caesarean section which we would have done for an elderly primigravida with breech presentation

    Psychology students’ perception of and engagement with feedback as a function of year of study

    Undergraduate students’ perception of feedback and level of engagement with the feedback they receive have gained increasing attention in the educational literature recently to identify areas which require educators’ attention. However, research in this area has generally been based on limited self-selecting samples, and has not considered how students’ relationship with feedback may alter depending on their year of study. To address this, a survey measuring students’ views and practices regarding feedback was completed at a higher education institution by 447 first-, second- and third-year psychology students, representing 77% of the cohort. Findings revealed that third years responded more negatively in both areas than their first- and second-year counterparts, whose ratings on these aspects themselves were far from optimal. These findings highlight the need for early interventions to improve students’ perception of and engagement with feedback in the earlier years, and to prevent the recorded deterioration later on in the degree course

    Secondary metabolites of marine-derived Bacillus spizizenii against the enteric redmouth disease in common carp, Cyprinus carpio

    Looking for effective alternatives, such as secondary microbial metabolites, is needed to restrict the use of antibiotics in farmed fish and their detrimental effects on public health and the environment. Thirty-three water and sediments samples were collected from coastal areas in the Basrah Governorate, southern Iraq, to assess their biological activity against bacterial pathogens isolated from Cyprinus carpio, with enteric red mouth disease. 20 spore-forming bacteria were isolated and identified by VITEK BCL cards and amplifying the gyrA gene. Furthermore, the secondary metabolites produced by the strains were extracted and analyzed by GC-MS. Four pathogenic bacteria were isolated from common carp infected with the enteric red mouth disease. The antibacterial activity of the extracts of the isolated marine strains was examined on bacteria causing enteric red mouth disease and Y. ruckeri and P. aeruginosa. Based on the results, the marine isolates were identified as B. spizizenii and GC-MS analysis revealed that these strains' extract contained amino acids and their derivatives and esters and hydrocarbons. Also, biochemical identifications showed that the bacteria isolated from fish belonged to the species of Yersinia ruckeri, Aeromonas hydrophila, Streptococcus agalactiae, and Pseudomonas aeruginosa. According to the antibacterial activity assay, the extracts of B. spizizenii strains were considerably active against bacteria involved in enteric red mouth disease, especially Y. ruckeri. These findings indicate marine B. spizizenii can be replaced with antibiotics in the aquaculture industry to combat infections
