    Changes in Outcome and Complication Rates of Very-low-birth-weight Infants in One Tertiary Center in Southern Taiwan Between 2003 and 2010

    BackgroundNeonatal intensive care has changed dramatically over the past few decades and the survival of infants has generally improved in many countries. The purpose of this study was to explore the recent evolution of mortality and morbidities among very-low-birth-weight (VLBW) infants in southern Taiwan.MethodsWe retrospectively reviewed the medical records of VLBW (birth weight <1500 g) infants who were admitted to a neonatal intensive care unit at a tertiary medical center in southern Taiwan from 2003 to 2010. The study period was divided into two cohorts: the first cohort of 2003–2006 and the second cohort of 2007–2010. Demographic profiles and complications were recorded, including the following information: sex, birth body weight (BBW), gestational age (GA), Apgar score, patent ductus arteriosus (PDA), necrotizing enterocolitis, retinopathy, chronic lung disease (CLD), inguinal hernia, and sepsis. The length of stay (LOS) in hospital was compared between the two cohorts.ResultsA total of 420 (212 male) VLBW infants were enrolled with 52 (12.4%) deaths. Compared to surviving infants, deceased infants had significantly lower GA, Apgar scores, and BBW. The mortality of VLBW infants remained static between the two birth cohorts, but the incidence of major morbidities generally decreased. The LOS for overall surviving infants and the proportion of LOS > 60 days were both reduced in the period of 2007–2010. With further stratification by BBW, the major reduction of long LOS was only found in the group of BBW ≄ 1000 g. The multivariate logistic regression model found PDA, CLD, and BBW < 1000 g were major complications to be associated with long LOS among surviving infants.ConclusionPeriodic evaluation of the mortality and morbidity of preterm infants can help to understand the changes and trends of our neonatal care. Further study using the national dataset to provide more representative information is warranted

    Nitroprusside modulates pulmonary vein arrhythmogenic activity

    <p>Abstract</p> <p>Background</p> <p>Pulmonary veins (PVs) are the most important sources of ectopic beats with the initiation of paroxysmal atrial fibrillation, or the foci of ectopic atrial tachycardia and focal atrial fibrillation. Elimination of nitric oxide (NO) enhances cardiac triggered activity, and NO can decrease PV arrhythmogensis through mechano-electrical feedback. However, it is not clear whether NO may have direct electrophysiological effects on PV cardiomyocytes. This study is aimed to study the effects of nitroprusside (NO donor), on the ionic currents and arrhythmogenic activity of single cardiomyocytes from the PVs.</p> <p>Methods</p> <p>Single PV cardiomyocytes were isolated from the canine PVs. The action potential and ionic currents were investigated in isolated single canine PV cardiomyocytes before and after sodium nitroprusside (80 ΌM,) using the whole-cell patch clamp technique.</p> <p>Results</p> <p>Nitroprusside decreased PV cardiomyocytes spontaneous beating rates from 1.7 ± 0.3 Hz to 0.5 ± 0.4 Hz in 9 cells (P < 0.05); suppressed delayed afterdepolarization in 4 (80%) of 5 PV cardiomyocytes. Nitroprusside inhibited L-type calcium currents, transient outward currents and transient inward current, but increased delayed rectified potassium currents.</p> <p>Conclusion</p> <p>Nitroprusside regulates the electrical activity of PV cardiomyocytes, which suggests that NO may play a role in PV arrhythmogenesis.</p

    The complete primary structure of abrin-a B chain

    AbstractThe complete 267 amino acid sequence of abrin-a B chain was determined by analysis of peptides obtained by digestion with trypsin, chymotrypsin, lysyl endopeptidase, Staphylacaccus aureus V8 protease and thermolysin. The sequence is not identical with that predicted previously by nucleotide sequencing, indicating the presence of isoforms of abrin. Comparison of the amino acid sequence of abrin-a B chain with that of ricin-D B chain reveals a high degree of sequence identity (59%). Abrin-a B chain appears to consist of two domains, each domain with subdomains (α, ÎČ, Îł) of about 40 amino acid residues

    The design of quasi-sliding mode control for a permanent magnet synchronous motor with unmatched uncertainties

    AbstractIn this study, the concept of a quasi-sliding mode control (QSMC) is introduced for the robust control of a permanent magnet synchronous motor (PMSM) system subjected to unmatched uncertainties, and even with input nonlinearity. On the basis of the new concept of QSMC, continuous control is obtained, to avoid the chattering phenomenon. As expected, the system state can be stabilized and driven into a predictable neighborhood of zero. Also, this approach only uses a single controller to achieve chaos control, which reduces the cost and complexity of implementation. The results of numerical simulations demonstrate the validity of the proposed QSMC design method

    Strategic Optimization of Water Reuse in Wafer Fabs via Multi-Constraint Linear Programming Technique

    The risk of water shortage has been posing as a threat to water demanding industries in Taiwan, including the high-tech industries where ultrapure water is needed for the production of microchips. Such risks are especially unpredictable in the age of climate change, where more frequent extreme climate events such as prolonged droughts have sent these industries scrambling for securing water supply at a very high cost. The national policy also mandates strict water recycling standards for these high-tech plants, while the risk of water supply shortage also forces the industry to be water-conscious. However, most plants set their water recycling strategies based on experience or ‘‘rules of thumb” practices, without implementing optimization tools that can help making decisions in a more scientific approach. In this study we applied linear programming technique to optimize the water recovery path for a microchip assembly plant. A water balance diagram was formulated and completed to determine the existing water recycling performance, and the data was converted to a water flow network. The water flow network was then derived with a mathematical model to formulate a linear optimization problem. The proposed linear programming model is composed of mass balance constraints, unit specification constraints, capacity constraints as well as water quality constraints (discharge limits). The linear programming method was effectively applied to improve the efficiency of water reuse. With the installation of the regeneration units, an increase of 40.1% in the volume of reused water was predicted. The results from water cost structure also indicated that, at higher water tariff, water reuses through reclaiming and generating spent effluents can alleviate the overall water consumption costs

    Thermal Effect on Structure of Silver in Ion-Exchanged Soda-Lime Glasses and Aluminum-Doped Zinc Oxide Films

    Heat treatment is commonly used during device processing in order to achieve specific functionalities of the devices. How a series of heat treatment applies to accomplish this goal can be found in the literature. However, specific properties of the devices after the treatment are more emphasized than the details of the structural modifications in the industrial applications. In this paper, it is intended to illustrate the fundamental changes in the structure due to heat treatment which result in the desired physical properties of the devices. Two study cases, Ag ion-exchanged soda-lime glasses and aluminum doped ZnO (AZO) films, were illustrated. The changes in chemical states, the structural modification during and after heat treatment are explored. By understanding how the metallic Ag formed and accumulated during annealing, an optimum heat treatment to grow the proper size and density of silver quantum dots in the films are possible. Post annealing effect on the AZO films shows that the crystallinity, the peak positions shifts, and grain sizes were changed after annealing. Both illustrated cases indicate thermally induced changes in chemical state, the stress release, and rearrangement of atoms in materials during and after annealing

    Absorption Cross Sections of NH_3, NH_2D, NHD_2, and ND_3 in the Spectral Range 140-220 nm and Implications for Planetary Isotopic Fractionation

    Cross sections for photoabsorption of NH_3, NH_2D, NHD_2, and ND_3 in the spectral region 140-220 nm were determined at ~298 K using synchrotron radiation. Absorption spectra of NH_2D and NHD_2 were deduced from spectra of mixtures of NH_3 and ND_3, of which the equilibrium concentrations for all four isotopologues obey statistical distributions. Cross sections of NH_2D, NHD_2, and ND_3 are new. Oscillator strengths, an integration of absorption cross sections over the spectral lines, for both A ← X and B ← X systems of NH_3 agree satisfactorily with previous reports; values for NH_2D, NHD_2, and ND_3 agree with quantum chemical predictions. The photolysis of NH_3 provides a major source of reactive hydrogen in the lower stratosphere and upper troposphere of giant planets such as Jupiter. Incorporating the measured photoabsorption cross sections of NH_3 and NH_2D into the Caltech/JPL photochemical diffusive model for the atmosphere of Jupiter, we find that the photolysis efficiency of NH_2D is lower than that of NH_3 by as much as 30%. The D/H ratio in NH_2D/NH_3 for tracing the microphysics in the troposphere of Jupiter is also discussed
