    Assessment of the reliability and quality of breast cancer related videos on TikTok and Bilibili: cross-sectional study in China

    BackgroundAs the most common malignant tumor in the world, breast cancer also brings a huge disease burden to China. Ordinary people are increasingly inclined to use the Internet, especially video social platforms, as a source of health information. Educating the public to obtain correct information is important to reduce the incidence of breast cancer and improve the prognosis. However, the quality and reliability of breast cancer-related video content have not been fully studied.ObjectiveThis study aims to evaluate the quality of the information of breast cancer-related videos on TikTok and Bilibili video sharing platforms and factors related to video quality.MethodsWe collected the top 100 videos about breast cancer on TikTok and Bilibili, respectively. Categorize videos according to video source and video content. Video quality and reliability were assessed using Global Quality Score (GQS) and modified DISCERN (mDISCERN) tools. We also analyzed the correlation between video quality and video likes, comments, saves, and shares.ResultsAlthough the quality and reliability of Bilibili’s breast cancer videos were higher than TikTok (p = 0.002 and p = 0.001, respectively), the video quality of both video sharing platforms was not satisfactory, with a median GQS scores of 2.00 and 3.00 and mDISCERN scores of 1.00 and 2.00, respectively. In general, the quality and reliability of videos released by medical practitioners were higher than those of non-medical practitioners, and the quality and reliability of videos covering disease-related knowledge were higher than those of news reports (all p < 0.001). Among medical practitioners, the quality of videos uploaded by doctors in breast disease was significantly lower than that of doctors in other areas (p < 0.05). There was a significant positive correlation between video quality and duration (r = 0.240, p < 0.001), a weak negative correlation between video quality and likes (r = 0.191, p < 0.01), video quality and comments (r = 0.256, p < 0.001), video reliability and likes (r = 0.198, p < 0.001), video reliability and comments (r = 0.243, p < 0.01).ConclusionOur study shows that the quality and reliability of breast cancer-related videos on TikTok and Bilibili are poor, and the overall quality is unsatisfactory. But videos uploaded by medical practitioners covering disease knowledge, prevention and treatment are of higher quality. Medical practitioners are encouraged to publish more high-quality videos, while video social platforms should formulate relevant policies to censor and supervise health education videos, so as to enable the public to obtain reliable health information

    Intensified paraglacial slope failures due to accelerating downwasting of a temperate glacier in Mt. Gongga, southeastern Tibetan Plateau

    Topographic development via paraglacial slope failure (PSF) represents a complex interplay between geological structure, climate, and glacial denudation. Southeastern Tibet has experienced amongst the highest rates of ice mass loss in High Mountain Asia in recent decades, but few studies have focused on the implications of this mass loss on the stability of paraglacial slopes. We used repeat satellite- and unpiloted aerial vehicle (UAV)-derived imagery between 1990 and 2020 as the basis for mapping PSFs from slopes adjacent to Hailuogou Glacier (HLG), a 5 km long monsoon temperate valley glacier in the Mt. Gongga region. We observed recent lowering of the glacier tongue surface at rates of up to 0.88 m a−1 in the period 2000 to 2016, whilst overall paraglacial bare ground area (PBGA) on glacier-adjacent slopes increased from 0.31 ± 0.27 km2 in 1990 to 1.38 ± 0.06 km2 in 2020. Decadal PBGA expansion rates were ∼ 0.01 km2 a−1, 0.02 km2 a−1, and 0.08 km2 in the periods 1990–2000, 2000–2011, and 2011–2020 respectively, indicating an increasing rate of expansion of PBGA. Three types of PSFs, including rockfalls, sediment-mantled slope slides, and headward gully erosion, were mapped, with a total area of 0.75 ± 0.03 km2 in 2020. South-facing valley slopes (true left of the glacier) exhibited more destabilization (56 % of the total PSF area) than north-facing (true right) valley slopes (44 % of the total PSF area). Deformation of sediment-mantled moraine slopes (mean 1.65–2.63 ± 0.04 cm d−1) and an increase in erosion activity in ice-marginal tributary valleys caused by a drop in local base level (gully headward erosion rates are 0.76–3.39 cm d−1) have occurred in tandem with recent glacier downwasting. We also observe deformation of glacier ice, possibly driven by destabilization of lateral moraine, as has been reported in other deglaciating mountain glacier catchments. The formation, evolution, and future trajectory of PSFs at HLG (as well as other monsoon-dominated deglaciating mountain areas) are related to glacial history, including recent rapid downwasting leading to the exposure of steep, unstable bedrock and moraine slopes, and climatic conditions that promote slope instability, such as very high seasonal precipitation and seasonal temperature fluctuations that are conducive to freeze–thaw and ice segregation processes

    Long-term outcomes of infantile spasms in children treated with ketogenic diet therapy in combination with anti-seizure medications in a resource-limited region

    ObjectiveDespite numerous guidelines, the overall outcome of infantile spasms is poor, with only a small number of patients being able to attend school. The purpose of this study was to investigate long-term outcomes. Patients had poor access to the recommended first-line anti-seizure medications (ASMs), such as hormones (corticotropin or prednisolone/prednisone) and vigabatrin, and their alternative treatment was other ASMs and a ketogenic diet.MethodsPatients suffering from infantile spasms who had at least 2 years of medical records in the electronic medical record system between January 2014 and August 2022 were included in this study. Patient information was retrospectively reviewed. All patients had received ketogenic diet therapy (mainly classical ketogenic diet therapy). The ketogenic diet therapy was combined with ASMs not used as first-line therapies. The primary endpoint outcome measure was the number of patients with seizure freedom. The secondary measures included the duration of ketogenic diet therapy, choice of ASMs, and patient development at the last visit.ResultsA total of 177 patients with infantile spasms were included, and 152 (86%) of them had seizure freedom. The median duration from the first to the last hospital visit was 53.27 months, and the number of visits was 47.00. The median age at the initial hospital visit was 8.00 months, and the median age at initiation of the ketogenic diet was 17.73 months. At the last visit, the proportions of patients with neurodevelopmental delay, developmental epileptic encephalopathy, drug-resistant epilepsy, and generalized seizures increased significantly. The frequently used ASMs were topiramate, valproic acid, levetiracetam, nitrazepam, and vitamin B6 injection, while the recommended first-line drugs corticotropin and vigabatrin were rarely selected. The study duration of 9.5 years was divided into three periods but the prescription of ASMs did not change significantly between these periods.ConclusionsAlthough the seizure freedom rate was high with ketogenic diet therapy combined with non-standard ASMs, the patients had a significant neurodevelopmental delay at the last visit, which was, however, similar to that of standard treatment. To improve the outcomes of infantile spasms, multicenter clinical trials of the ketogenic diet as a first-line treatment in combination with non-standard ASMs are needed

    Experimental investigation on an ORC twin-screw expander with an emphasis on its suction process

    Organic Rankine Cycle (ORC) is an effective and promising technology to recover low-grade waste heat and output electricity. As a key component in an ORC system, the expander’s performance significantly affects the whole system’s efficiency. Twin-screw expander is widely employed for its compact structure, reliability, two-phase tolerance and high efficiency. To investigate the characteristics of twin-screw expander, a test bench was set up in a laboratory taking 120 waste steam as heat source. R245fa was selected as the working fluid. Pressure sensors with high sensitivity and accuracy were installed at appropriate locations in the expander casing to monitor the p-V indicator diagrams. The expander’s p-V indicator diagrams and operating parameters were recorded under different operating conditions. The effects of rotational speed and suction pressure on the expander’s suction pressure drop, volumetric efficiency and indicated adiabatic efficiency were studied. Results show that rotational speed and suction pressure affect the suction pressure drop altogether, which has a significant effect on the density and mass flow rate of the sucked gas. The expander volumetric efficiency is above 85% and the indicated adiabatic efficiency can also reach 82%

    The effect of screw orientation on internal fixation of Letenneur type II Hoffa fractures: a biomechanics study

    Abstract Background To investigate the biomechanical effects of screw orientation and fracture block size on the internal fixation system for Letenneur type II Hoffa fractures. Methods The fracture models were randomly divided into six groups according to the fracture subtypes and the direction of nail placement, and a plumb line of the posterior condylar tangent was made across the base of the posterior femoral condyle. The fracture blocks of the three types of fracture were calculated and recorded in the sagittal position, and the biomechanical performance of the six groups was evaluated by biomechanical tests. The axial load on the fracture block at a displacement of 2 mm was set as the failure load, a gradually increasing axial load was applied to each fracture model using a customized indenter at a load of 250-750 N, and the displacements and failure loads of the six groups were recorded at different axial loads. Results Biomechanical test results showed that the larger the fracture block, the greater was the stability when nailing from front to back, and the smaller the fracture block, the greater was the strength when nailing from back to front (p  0.001). Therefore, there is a mechanical threshold that ranges from 38.36 to 52.33% between type IIa and type IIb fractures. Conclusions The effect of the nailing direction on the strength of fixation has a fracture-block critical point, which is consistent overall with the trend that the larger the fracture block is, the greater the stability when nailing from anterior to posterior, and the smaller the fracture block is, the greater the strength when nailing from posterior to anterior

    Wear life of PTFE/Kevlar self-lubricating composite under high frequency oscillatory conditions

    In this study, the PTFE/Kevlar self-lubricating composite was prepared for the service conditions of high frequency oscillation. The full-life cycle friction and wear characteristics was analyzed, and the tribological properties and failure mechanism at different life stages were studied. Fatigue life test of PTFE/Kevlar self-lubricating composite was performed by a self-developed heavy load reciprocating test machine. Results show that the self-lubricating performance of the material increases while the wear life decreases with the increase of stress and frequency. The main failure mode of the material consists of abrasive wear and adhesive wear, accompanied by extrusion fracture of the fiber. Tribological performance and surface state show distinct phase characteristics in the life cycle. The change of frequency affects the adhesive wear degree in the fiber fracture area. Differently, the change of stress affects the adhesive wear degree in the fiber fracture area as well as the uniform stability of the transfer film. The comparison of frequency and stress shows that the change of frequency has a greater influence on the material self-lubricating performance and life. This study could provide a research basis for the design and development of self-lubricating composites in high frequency oscillating conditions, and also provide reference data for the calibration of service conditions for PTFE/Kevlar self-lubricating composites
