Exercise-Induced Changes in Exhaled NO Differentiates Asthma With or Without Fixed Airway Obstruction From COPD With Dynamic Hyperinflation.
Asthmatic patients with fixed airway obstruction (FAO) and patients with chronic obstructive pulmonary disease (COPD) share similarities in terms of irreversible pulmonary function impairment. Exhaled nitric oxide (eNO) has been documented as a marker of airway inflammation in asthma, but not in COPD. To examine whether the basal eNO level and the change after exercise may differentiate asthmatics with FAO from COPD, 27 normal subjects, 60 stable asthmatics, and 62 stable COPD patients were studied. Asthmatics with FAO (n = 29) were defined as showing a postbronchodilator FEV(1)/forced vital capacity (FVC) ≤70% and FEV(1) less than 80% predicted after inhaled salbutamol (400 μg). COPD with dynamic hyperinflation (n = 31) was defined as a decrease in inspiratory capacity (ΔIC%) after a 6 minute walk test (6MWT). Basal levels of eNO were significantly higher in asthmatics and COPD patients compared to normal subjects. The changes in eNO after 6MWT were negatively correlated with the percent change in IC (r = −0.380, n = 29, P = 0.042) in asthmatics with FAO. Their levels of basal eNO correlated with the maximum mid-expiratory flow (MMEF % predicted) before and after 6MWT. In COPD patients with air-trapping, the percent change of eNO was positively correlated to ΔIC% (rs = 0.404, n = 31, P = 0.024). We conclude that asthma with FAO may represent residual inflammation in the airways, while dynamic hyperinflation in COPD may retain NO in the distal airspace. eNO changes after 6MWT may differentiate the subgroups of asthma or COPD patients and will help toward delivery of individualized therapy for airflow obstruction
Wavelet analysis of head-related transfer functions
The directional-dependent information in the head-related transfer function (HRTF) is important for the study of human sound localization system and the synthesis of virtual auditory signals. Its time-domain and frequency-domain characteristics have been widely studied by researchers. The purpose of this paper is to explore the ability of discrete wavelet transform to describe the time-scale characteristics of HRTFs. Both the time-domain characteristics and energy distribution of different frequency subbands were studied. Discrete wavelet analysis is found to be a new direction-dependence information showing the relation of the characteristics of the HRTFs to sound source directions.published_or_final_versio
Neural network model of binaural hearing based on spatial feature extraction of the head related transfer function
In spatial hearing, complex valued head-related transfer function (HRTF) can be represented as a real valued head-related impulse response (HRIR). Using Karhunen-Loeve expansion, the spatial features of the normalized HRIRs on measurement space can be extracted as spatial character functions. A neural network model based on Von-Mises function is used to approximate the discrete spatial character function of HRIR. As a result, a time-domain binaural model is established and it fits the measured HRIRs well.published_or_final_versio
Ensemble of Hankel Matrices for Face Emotion Recognition
In this paper, a face emotion is considered as the result of the composition
of multiple concurrent signals, each corresponding to the movements of a
specific facial muscle. These concurrent signals are represented by means of a
set of multi-scale appearance features that might be correlated with one or
more concurrent signals. The extraction of these appearance features from a
sequence of face images yields to a set of time series. This paper proposes to
use the dynamics regulating each appearance feature time series to recognize
among different face emotions. To this purpose, an ensemble of Hankel matrices
corresponding to the extracted time series is used for emotion classification
within a framework that combines nearest neighbor and a majority vote schema.
Experimental results on a public available dataset shows that the adopted
representation is promising and yields state-of-the-art accuracy in emotion

Quantum fluctuations can promote or inhibit glass formation
The very nature of glass is somewhat mysterious: while relaxation times in
glasses are of sufficient magnitude that large-scale motion on the atomic level
is essentially as slow as it is in the crystalline state, the structure of
glass appears barely different than that of the liquid that produced it.
Quantum mechanical systems ranging from electron liquids to superfluid helium
appear to form glasses, but as yet no unifying framework exists connecting
classical and quantum regimes of vitrification. Here we develop new insights
from theory and simulation into the quantum glass transition that surprisingly
reveal distinct regions where quantum fluctuations can either promote or

Transcutaneous immunization as preventative and therapeutic regimens to protect against experimental otitis media due to nontypeable Haemophilus influenzae
We have developed three nontypeable Haemophilus influenzae (NTHI) adhesin-derived immunogens that are significantly efficacious against experimental otitis media (OM) due to NTHI when delivered parenterally. We now expanded our preventative immunization strategies to include transcutaneous immunization (TCI) as a less invasive, but potentially equally efficacious, regimen to prevent OM due to NTHI. Additionally, we examined the potential of TCI as a therapeutic immunization regimen to resolve ongoing experimental OM. Preventative immunization with NTHI outer membrane protein (OMP) P5- and type IV pilus-targeted immunogens, delivered with the adjuvant LT(R192G-L211A), induced significantly earlier clearance of NTHI from the nasopharynges and middle ears of challenged chinchillas compared with receipt of immunogen or adjuvant alone. Moreover, therapeutic immunization resulted in significant resolution of established NTHI biofilms from the middle ear space of animals compared with controls. These data advocate TCI with the adhesin-directed immunogens as an efficacious regimen for prevention and resolution of experimental NTHI-induced OM
Differences in bleeding behavior after endoscopic band ligation: a retrospective analysis
<p>Abstract</p> <p>Background</p> <p>Endoscopic band ligation (EBL) is generally accepted as the treatment of choice for bleeding from esophageal varices. It is also used for secondary prophylaxis of esophageal variceal hemorrhage. However, there is no data or guidelines concerning endoscopic control of ligation ulcers. We conducted a retrospective study of EBL procedures analyzing bleeding complications after EBL.</p> <p>Methods</p> <p>We retrospectively analyzed data from patients who underwent EBL. We analyzed several data points, including indication for the procedure, bleeding events and the time interval between EBL and bleeding.</p> <p>Results</p> <p>255 patients and 387 ligation sessions were included in the analysis. We observed an overall bleeding rate after EBL of 7.8%. Bleeding events after elective treatment (3.9%) were significantly lower than those after treatment for acute variceal hemorrhage (12.1%). The number of bleeding events from ligation ulcers and variceal rebleeding was 14 and 15, respectively. The bleeding rate from the ligation site in the group who underwent emergency ligation was 7.1% and 0.5% in the group who underwent elective ligation. Incidence of variceal rebleeding did not vary significantly. Seventy-five percent of all bleeding episodes after elective treatment occurred within four days after EBL. 20/22 of bleeding events after emergency ligation occured within 11 days after treatment. Elective EBL has a lower risk of bleeding from treatment-induced ulceration than emergency ligation.</p> <p>Conclusions</p> <p>Patients who underwent EBL for treatment of acute variceal bleeding should be kept under medical surveillance for 11 days. After elective EBL, it may be reasonable to restrict the period of surveillance to four days or even perform the procedure in an out-patient setting.</p
Perspectives in Melanoma: meeting report from the Melanoma Bridge (December 3rd-5th, 2020, Italy)
Advances in immune checkpoint therapy and targeted therapy have led to improvement in overall survival for patients with advanced melanoma. Single agent checkpoint PD-1 blockade and combination with BRAF/MEK targeted therapy demonstrated benefit in overall survival (OS). Superior response rates have been demonstrated with combined PD-1/CTLA-4 blockade, with a significant OS benefit compared with single-agent PD-1 blockade. Despite the progress in diagnosis of melanocytic lesions, correct classification of patients, selection of appropriate adjuvant and systemic therapies, and prediction of response to therapy remain real challenges in melanoma. Improved understanding of the tumor microenvironment, tumor immunity and response to therapy has prompted extensive translational and clinical research in melanoma. Development of novel biomarker platforms may help to improve diagnostics and predictive accuracy for selection of patients for specific treatment. There is a growing evidence that genomic and immune features of pre-treatment tumor biopsies may correlate with response in patients with melanoma and other cancers but they have yet to be fully characterized and implemented clinically. Overall, the progress in melanoma therapeutics and translational research will help to optimize treatment regimens to overcome resistance and develop robust biomarkers to guide clinical decision-making.
