An Extreme Solar Event of 20 January 2005: Properties of the Flare and the Origin of Energetic Particles
The extreme solar and SEP event of 20 January 2005 is analyzed from two
perspectives. Firstly, we study features of the main phase of the flare, when
the strongest emissions from microwaves up to 200 MeV gamma-rays were observed.
Secondly, we relate our results to a long-standing controversy on the origin of
SEPs arriving at Earth, i.e., acceleration in flares, or shocks ahead of CMEs.
All emissions from microwaves up to 2.22 MeV line gamma-rays during the main
flare phase originated within a compact structure located just above sunspot
umbrae. A huge radio burst with a frequency maximum at 30 GHz was observed,
indicating the presence of a large number of energetic electrons in strong
magnetic fields. Thus, protons and electrons responsible for flare emissions
during its main phase were accelerated within the magnetic field of the active
region. The leading, impulsive parts of the GLE, and highest-energy gamma-rays
identified with pi^0-decay emission, are similar and correspond in time. The
origin of the pi^0-decay gamma-rays is argued to be the same as that of lower
energy emissions. We estimate the sky-plane speed of the CME to be 2000-2600
km/s, i.e., high, but of the same order as preceding non-GLE-related CMEs from
the same active region. Hence, the flare itself rather than the CME appears to
determine the extreme nature of this event. We conclude that the acceleration,
at least, to sub-relativistic energies, of electrons and protons, responsible
for both the flare emissions and the leading spike of SEP/GLE by 07 UT, are
likely to have occurred simultaneously within the flare region. We do not rule
out a probable contribution from particles accelerated in the CME-driven shock
for the leading GLE spike, which seemed to dominate later on.Comment: 34 pages, 14 Postscript figures. Solar Physics, accepted. A typo

Firearms craniocerebral wounds of peacetime are the one of urgent problems of modern neurosurgery. There are no clear statistics, the approved classification of gunshot wounds to the head, algorithm of the treatment and tactics of managing children patients. The greatest number of cases of injuries is caused by modern pneumatic and gas guns, which, by their capacity, are not inferior to firearms. The purpose of this study is to summarize the accumulated experience in countries where there is a legalization of weapons and there was registered a high percentage of its spread among the civilian population, as well as the use of prognostic scales for choosing the tactics of treating patients with gunshot wounds to the head.ΠΠ³Π½Π΅ΡΡΡΠ΅Π»ΡΠ½ΡΠ΅ ΡΠ΅ΡΠ΅ΠΏΠ½ΠΎ-ΠΌΠΎΠ·Π³ΠΎΠ²ΡΠ΅ ΡΠ°Π½Π΅Π½ΠΈΡ ΠΌΠΈΡΠ½ΠΎΠ³ΠΎ Π²ΡΠ΅ΠΌΠ΅Π½ΠΈ - ΠΎΠ΄Π½Π° ΠΈΠ· Π°ΠΊΡΡΠ°Π»ΡΠ½ΡΡ

In the presented review of the literature, we conducted a meta-analysis of the effectiveness of 7 surgical treatment options for patients with craniosynostosis based on the long-term results of the normalization of cephalic index (CI) depending on the timing of surgical intervention. All methods for the correction of isolated nonsyndromic craniosynostosis, according to numerous authors, demonstrate comparative efficacy, but a statistically significant difference in evaluating the aesthetic result by CI indicates an advantage after performing a total reconstruction of the cranial vault (p < 0,01).Π ΠΏΡΠ΅Π΄ΡΡΠ°Π²Π»Π΅Π½Π½ΠΎΠΌ ΠΎΠ±Π·ΠΎΡΠ΅ Π»ΠΈΡΠ΅ΡΠ°ΡΡΡΡ ΠΏΡΠΎΠ²Π΅Π΄Π΅Π½ ΠΌΠ΅ΡΠ°Π°Π½Π°Π»ΠΈΠ· ΡΡΡΠ΅ΠΊΡΠΈΠ²Π½ΠΎΡΡΠΈ 7 Π²Π°ΡΠΈΠ°Π½ΡΠΎΠ² ΠΎΠΏΠ΅ΡΠ°ΡΠΈΠ²Π½ΠΎΠ³ΠΎ Π»Π΅ΡΠ΅Π½ΠΈΡ Π±ΠΎΠ»ΡΠ½ΡΡ

Sturge-Weber syndrome belongs to the group of phacomotoses and is characterized by a combined lesion of the skin, eyes, nervous system and internal organs. The clinical course of Sturge-Weber syndrome is quite diverse. Of particular interest in the practice of pediatric neurology and neurosurgery is the fact that 72-90% of patients present with epilepsy. Of particular difficulty is the differential diagnosis of epileptic seizures and stroke-like episodes. The article presents clinical cases of patients with Sturge-Weber syndrome of various ages with epileptic seizures and transient ischemic attacks.Π‘ΠΈΠ½Π΄ΡΠΎΠΌ Π¨ΡΡΡΠ³Π΅-ΠΠ΅Π±Π΅ΡΠ° (Π‘Π¨Π) ΠΎΡΠ½ΠΎΡΠΈΡΡΡ ΠΊ Π³ΡΡΠΏΠΏΠ΅ ΡΠ°ΠΊΠΎΠΌΠΎΡΠΎΠ·ΠΎΠ² ΠΈ Ρ

Arterial ischemic stroke in children: The use of thrombolytic therapy
In recent years, the number of diagnosed cases of arterial ischemic stroke (AIS) in children has increased significantly. Intravenous and endovascular thrombolytic therapy of AIS is the most effective treatment for adults. Currently, the literature provides descriptions of a series of clinical observations in children with AIS, that received therapy with thrombolysis. This type of therapy is recommended to be performed on the basis of pediatric stroke primary centers. Since 2018, on the basis of the Center for Cerebrovascular Pathology Treatment, Morozov Childrenβs City Clinical Hospital, 34 children with AIS aged 3,5-14 years received intravenous thrombolytic therapy. The article describes the clinical observations of 4 patients with AIS who underwent treatment with thrombolysis. Β© 2020, Pediatria Ltd. All rights reserved
